Abstract

Background: the realization management health and occupational very important to a company. In the CV Cipta Mandiri still occured job accident that can damage employee although facilities that available. As long as CV Cipta Mandiri has perform management system safety and health occupational until necessary does evaluation of realization system. Method: this research constitute of descriptive research with planning cross sectional. The collect of data with does analysis direct on field and interview going deep to researce person. Result : the facilities of work is good, but it need completed wirh maintanance for good function. The company have comitment for does management system safety and job safety program for prevent occupational accident, such as training and counseling. Generally, realization of apply safety and system can run well. Conclusion: the management system of safety and healthy job includes having good facilities. Comitment of the company and the program of preventing accident in a job has been run well, but there must be luck in other aspect that must be improved. Abstract

Background: The growth and developed hospital should be effectively and efficiency managed. This fact should be carried out in order to face strict competitions in the health world. The hospital can survive in facing the existing competitions by counting the previous hospital fee and using fee account model competitively. In PKU Muhammadiyah Unit I Yogyakarta, Set of Essential Services (SES) of caesarea operation fee, class III, and cost sharing between the hospital and the goverment are different. In this study, the researcher only counts caesarea operation fee for re-use in order to create low cost sharing. Up till now, the hospital party never counts those existing differences. This case will lead them to get a financial loss. By having this fact, the researcher was eager to know about cost sharing analysis of caesarea operation fee account, class III, in PKU Muhammadiyah Unit I Yogyakarta.Methode: This was qualitative descriptive research which analyses the exact fee by using real cost method to know the difference of patient?s fee toward the real fee in the hospital. Bisides that, interview method was applied to know the head of finance and Central Surgeon Installation (CSI) workers opinion.Resulth: The result of Set of Essential Services (SES) fee of health insurance? toward caesarea operation was Rp 1.230.429, caesarea operation fee by using real cost method was Rp 3.764.388, in fact, the exact fee from RS PKU Muhammadiyah Unit I Yogyakarta was Rp 4.705.388. In conclusion, the final results were the difference of Set of Essential Services (SES) fee of health insurance between real cost was Rp 2.533.959 and the difference of Set of Essential Services (SES) fee of health insurance? between the exact hospital costs was Rp 3.475.056.Conclusion: The researcher has concluded that set of costs which is based on unit cost was higher that hospital cost recently. The highest cost was given for buying medicines and temporary materials. The hospital party has to decide the best strategy to reduce those differences.Key words: cost sharing, real cost method, Set of Essential Services (SES) of caesarea operation fee, class III.

Abstract

Background: Wonosobo station lies in Wonosobo Regency, Central Java, Association of Driver of Wonosobo-Sawangan City Transportation (PERKASA) is an organization which serve the transportation needs of Wonosobo people. It has 80 buses with 60 drivers. Method: this study is an analytic observational study with a cross sectional design on drivers of city transportation in Wonosobo. The study aims to identifying the relationship between the age, working load, and sitting position when having a job and complain on back. The applied instrument is a checklist based on the adoption from Nilamsari?s study with 33 respondents who meet the inclusive criteria. Result: most the age of drivers in the risk category of positive backache, 17 people (51.5%). While the age of drivers without the risk of negative backache is 5 people (15.3%). The respondents? working load in the light category with negative backache is 14 people (42.4) and with positive backache are 17 people (51.5%. The sitting position of the respondent in the unsuitable category with the negative backache is 12 people (36%) and with the positive backache are 18 people (45.5%). While the sitting position of respondents is less suitable with negative backache is 3 people (9.1%) and nobody feel positive backache. Conclusion: There is a relationship in the year between age and sitting position when having a job with complain of backache in driver relationship city transportation in the year 2010. Keyword: age, working load, sitting position and load back.

Abstract

Background: Universal Precaution is common caution which is nursing must be obeyed by nurse or medical labor in doing every nursing action towards the patient to protect from some of infection material. The nurses knowledge about the universal precaution is needed to be high on guard towards spreading disease. This research purposes to know the relation between nurses knowledge in application universal precaution in PKU Muhammadiyah Yogyakarta hospital. Method: This was an observational analytic with research using cross sectional approach. The population in this research is the nurse in patient II and III classes amounted to 34 nurses. The sample taken by total sampling technique is number of 34 nurses. The data of research is taken by observation and spreading question and processed by using the chi square test. Result: The degree of nurses knowledge in hospital categorize high amounted to be 26 nurses by percentage of 76,47 %. (1) the degree of nurses knowledge in application wash hand to the nurse categorize good amounted by percentage of 79,41%, gotten the score p= 0,019, it means that there is a significant relation between the degree of nurses knowledge with the application of wash hand to the nurse in hospitalized ward of PKU Muhammadiyah Yogyakarta hospital (2) the degree of nurses knowledge with application of antiseptic use and the disinfectant to the nurse categorize good amounted by percentage of 82,35%, gotten the score p= 0,006, it means that there is a significant relation between the degree of nurses knowledge with the application of antiseptic use and the disinfectant to the nurse in PKU Muhammadiyah Yogyakarta hospital. (3) The degree of nurses knowledge with the application the use of self protector to the nurse categorize good amounted by percentage of 85,29%, gotten the score p= 0,037, means that there is a significant relation between the degree of nurses knowledge with application of self protector use to the nurse in the PKU Muhammadiyah Yogyakarta hospital. (4) the degree of nurses knowledge with the application the discharge of medical rubbish to the nurse categorize good amounted by percentage of 85,29%, gotten the score p= 0,001, means there is a significant relation between the degree of nurses knowledge with the application of medical rubbish discharge to the nurse in PKU Muhammadiyah Yogyakarta hospital. Conclusion: There was a significant relationship between the degree of nurses knowledge with application universal precaution to the nurse in patient in PKU Muhammadiyah Yogyakarta hospital. Abstract

Background: Cervical cancer ranks the top cause of cancer deaths in developing countries. At Dr. Moewardi Hospital Surakarta cervical cancer was found in most cancers. Risk factors associated with health and cleanliness of the female reproductive tract can increase the incidence of cervical cancer. Purpose this study aims to determine factors associated with female reproductive health tract and the occurrence of cervical cancer. Method: The study was a quantitative analytical research used cross sectional design. Data used primary data from interviews and secondary data from medical records. Bivariate analysis using chi square test used to prove the hypothesis. Results: Statistical analysis by chi square test for sex at an early age got p value=0,382 and the prevalence ratio= 1,375, for promiscuity with incidence cervical cancer got p value=0,481 and prevalence ratio=1,314, for vaginal douching with incidence cervical cancer got p value=0,121 and prevalence ratio=0,571. Conclusion: There was no significant correlation between sex on early age with incidence of cervical cancer, there was no significant correlation between promiscuity with incidence of cervical cancer, there was no significant correlation between vaginal douching with incidence of cervical cancer at Dr. Moewardi Hospital Surakarta on Mey until Juny 2010. Abstract

Background: Policy about manage medical record had managed the system for reach order administration and improvement health services to show quality of hospital. New policy from Permenkes No.269/MENKES/PER/III/2008 make influence in this hospital policy because some policy using old government policy. At previously research, implementation that policy had many problem with some factor problem to fill in the main diagnose and the accurate code diagnose. The purpose of research is to know the policy to fill in the main diagnose and the accurate diagnose code at medical record in PKU Muhammadiyah hospital Yogyakarta. Method: This was non experiment research, this is deskriptif kualitatif and kuantitatif data as supporting to explain. The subject of research was internis, head of medical record, coder and documents medical record patient treatment internal disease at November 2008. Technique sampling for doctor internal disease, head of medical record, and coder was purposive sampling. But Technique sampling for documents medical record was simple random sampling. Kualitatif data tested validity with triangulasi. While for kuantitatif data the accurate diagnose code check using ICD-10. Result: The result of triangulasi identificated some factor problem to fill in the main diagnose and the accurate code diagnose. There is completeness documents medical record, busy, forget, lazy, not discipline, over burden work, patient APS, new terminology, difference perception, tools not support. The other policy most supporting that activity policy. Percentace to fill diagnose in the main diagnose from analyze 161 documents medical record is 71 (43,48%) to fill in and 91 (56,52%) not fill in. While the accurate code diagnose, from analyze 161 documents medical record have 237 code diagnose and 192 (81,01%) is accurate and 44 (18,99%) is not accurate. Conclusion: The policy most important for manage activity and communication between staff most supporting to result of policy implementation. Some factor problem can be overcome if all staff aware that policy is important and put into effect. Key word: Policy, Diagnose, Diagnose Code, Medical Record
