Abstract

Background : There were many studies proved that precordial ST depression in Inferior STEMI show LAD disease from angiografi, but there were also many who unproved . Altought LAD disease was not the main cause of precordial ST depression in Inferior STEMI, but there were many patients with Inferior STEMI who had high grade LAD disease. Sum of precordial ST depression is one of ECG criteria that can help us to prove it . The aim of this study in to know if there is corelation between sum of precordial ST depression with LAD disease and the severity of LAD disease in Inferior STEMI patients with precordial ST depression . Methods : We analized 60 patients inferior STEMI with precordial ST depression onset less than 24 hours that hospitalized in Haji Adam Malik General Hospital since December 2013-June 2017. Patients were divided in to 4 groups based on sum of precordial ST depression. Bivariate analysis were made to see the corelation between sum of precordial ST depression with LAD disease and severity of LAD, p value < 0.05 is statistically significant. Result : Bivariate analysis show that there is strong corelation between sum of precordial ST depression with LAD disease (p=0.01). Sum of precordial ST depression also corelate with severity of LAD (p=0.01). There are also corelation with location of the lesion in proximal LAD and the complexity of the lesion but not statistically significant (p=0.233 and p=0.102). Analysis ROC curve sum of precordial ST depression to LAD disease give the cut off ≥5.15 mm with sensitivity 70% and spesificity 76.2%(AUC 0,752 ,95% CI: 0,620 – 0,883 p<0,001) Conclusion : There is strong corelation between sum of precordial ST depression with high grade LAD disease. The more the sum of precordial ST depression the more posibility high degree LAD disease ( p=0.01).

Abstract

Ketuban pecah dini merupakan salah satu penyebab terjadinya asfiksia neonatorum dan infeksi yang dapat meningkatkan mortalitas dan morbiditas perinatal. Penelitian ini bertujuan untuk menilai hubungan antara lama ketuban pecah dini dengan nilai Apgar pada kehamilan aterm yang dirawat inap di Rumah Sakit Umum Mitra Sejati Medan. Jenis penelitian yang digunakan adalah penelitian analitik dengan desain penelitian cross sectional dengan metode pengambilan sampel accidental sampling. Dari sampel yang memenuhi kriteria restriksi didapat 68 ibu dengan kasus KPD. Hasil penelitian menunjukkan hasil lama KPD < 12 jam dengan Apgar baik adalah sebesar 22 kasus (73,3%) dan dengan Apgar buruk sebanyak 8 kasus (26,7%) sedangkan KPD ≥ 12 jam dengan Apgar baik sebesar 10 kasus (26,3%) dan nilai Apgar buruk sebesar 28 kasus (73,7%). Dari uji statistik dengan tes Chi Square didapatkan nilai X2 = 14,876 dan probabilitasnya (ρ) = 0,001. Dapat disimpulkan terdapat hubungan antara lama ketuban pecah dini dengan nilai Apgar.

Abstract

Introduction: Heart failure (HF) is a complex clinal manifestation that cause abnormality of structural and/or functional of the heart. HF is due to diastolic dysfunction and systolic dysfunction of the left ventricle (LV). The aim of this study is to assess the relationship between grading of pre-discharge LV diastolic dysfunction and readmission at 30 days post discharge and 6 months post discharge. Method: This is a cohort retrospective study in HFrEF patients which are treated at Pusat Jantung Terpadu RSUP H. Adam Malik Medan on January 2021 until December 2021. Pre discharge echocardiography has been done to assess LV diastolic dysfunction and the we do the follow up of readmission at 30 days and 6 months by phone. Results: From 93 patients, there are 40 patients (43%) with grade I LV diastolic dysfunction, 22 patients (23,7%) with grade II LV diastolic dysfunction and 31 patients (33,3%) with grade III LV diastolic dysfunction. There are 34 patients (26,6%) with readmission at 30 days and 21 patients (26,6%) readmission at 6 months. There is a significant relationship between grading of pre-discharge LV diastolic dysfunction and readmission at 30 days post discharge (p value 0,000), but not at 6 months post discharge (p value 1,000). Conclusion: Grading of pre-discharge LV diastolic dysfunction on HFrEF patient is correlated with readmission at 30 days post discharge.

Abstract

Introduction: TID ratio derived from ratio of LV volume in stress and rest phase of MPS. It is said that this phenomenon has been a useful marker of severe CAD, that can be present with atypical angina. This occurs due to global myocardial hypoperfusion caused by severe and extensive CAD. Method: This study was an analytic observational study with a retrospective cohort design in CCS patients who underwent Myocardial Perfusion SPECT at Haji Adam Malik General Hospital from January 2022 to April 2023.  All participants underwent MPS with Tc99m sestamibi with pharmacology Adenosine stress test and coronary angiography. The MPS could be performed before or after coronary angiography without any revascularization procedure between the two examination preocedures. An unpaired t-test analysis was performed to find the mean difference in TID values in the mild CAD and moderate-severe CAD groups. Results: The study subjects totaled 93 people with an average age of 55.87±7.44. It was found that the TID value was significantly different between the two groups of mild and moderate-severe CAD based on Syntax score, 0.906±0.13 vs 1.03±0.11 in the mild vs moderate-severe CAD group (p<0.001). Bivariate analysis showed that in this study, the ratio of TID was only associate with LV ESV and LV EDV in stress phase of MPS (p = 0.001). Conclusion: There is a relationship between TID and CAD severity based on Syntax score with higher TID values in patients with moderate-severe CAD compared to patients with mild CAD. TID ratio was only depends on severity od CAD and directly proportional to the volume of LV in the stress phase, suitable to the theory that said TID ratio comes from ratio of LV volume in stress and rest phase of MPS.

Abstract

Introduction: Red cell distribution width (RDW), a measure of erythrocyte size variability, is associated with acute inflammation. Elevated RDW predicts adverse outcomes in cardiovascular diseases like acute myocardial infarction. This study investigated the relationship between RDW and coronary lesion severity, assessed by the SYNTAX Score, in patients with non-ST-elevation acute coronary syndrome (NSTE-ACS) undergoing coronary angiography. Methods: This retrospective analytical study included NSTE-ACS patients who underwent coronary angiography at H. Adam Malik General Hospital Medan. Baseline characteristics, laboratory parameters (including RDW), and angiography results were collected. The association between RDW and SYNTAX Score was analyzed. ROC/AUC curve analysis assessed RDW's predictive ability for coronary lesion severity, determining sensitivity and specificity. Results: In 160 patients, RDW was significantly associated with SYNTAX Score (p<0.001). TIMI score, GRACE score, and number of involved arteries also correlated with lesion severity. Conclusion: RDW is a promising predictor of coronary lesion severity in NSTE-ACS patients undergoing coronary angiography.

Abstract

Introduction: Mitral stenosis and regurgitation, are common throughout the world. Peak Left Atrial Strain (PALS) is a parameter for assessing left atrial deformation in the evaluation of atrial function and a predictor of long-term outcome of various heart diseases. In this study, we assessed the relationship between PALS and the severity of mitral stenosis and mitral regurgitation Methods: This is a cross-sectional study on 119 subjects with mitral stenosis and 103 mitral regurgitation who met the inclusion criteria at Haji Adam Malik General Hospital. PALS measurements were taken. Data were analyzed univariate, bivariate, correlate to assess the relationship between PALS and the severity of mitral valve disease Results: 119 patients with severe mitral stenosis. In mitral stenosis, the mean PALS is 8.2 (4.9-22.8). PALS was significantly higher in patients with sinus rhythm than in the group with AF (10.29 ± 3.89 vs 8.63 ± 7.8%; P = 0.002). PALS had a significant correlation with MVA, pressure gradient, and PHT (r = 0.676, P = <0.001; r=-0.594, P= 0.001 and r=-0.594, P= 0.001). Whereas in mitral regurgitation, it has an average PALS of 15.2 (7.8-19.2). PALS was also significantly higher in patients with sinus rhythm than in the group with AF (16.36 ± 2.43 vs 11.64 ± 2.89%, P = 0.001). PALS has a correlation with VC, PISA, EROA and RVol (r = -0.533, P=0.001; r=-0.618; r=-0.563, P=0.001; r= -0.528, P=0.001). Conclusion: PALS has a significant correlation with the assessment of the severity of mitral stenosis and regurgitation.

Abstract

Coronary artery disease (CAD) remains a significant global health concern with considerable high morbidity and mortality and its development is influenced by various genetic and environmental factors. Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a vital regulator of low-density lipoprotein receptor (LDLR) metabolism, directly impacting serum cholesterol levels. However, its role in development of CAD is not fully understood. The aim of this study was to assess the association between the level of PCSK9 and coronary lesion severity in patients with CAD. A case-control study using consecutive sampling was conducted among CAD patients at H. Adam Malik General Hospital and Murni Teguh Memorial Hospital, Medan, Indonesia. A total of 200 CAD patients were divided into two groups based on the SYNTAX score: control (score ≤22, n=100) and case (score >22, n=100). Plasma PCSK9 levels were measured from venous blood using quantitative sandwich enzyme immunoassay. The Chi-squared test was used to analyze the data. Our data suggested that PCSK9 level was associated with coronary lesion severity (p<0.001) of which high PCSK9 level was associated with severe coronary lesion. We also found that hypertension (p<0.001), smoking (p=0.072), diabetes (p<0.001), dyslipidemia (p<0.001), obesity (p=0.023), and family history (p=0.001) were associated with lesion severity. Using the receiver operating characteristic (ROC) curve analysis, the cut-off 70.35 ng/mL of PCSK9 had sensitivity 75% and specificity 78% to predict severe coronary lesion. This study highlights that PCSK9 level has moderate sensitivity and specificity to predict the coronary lesion severity among CAD patients.

Abstract

Natural food ingredients are becoming increasingly popular as a treatment. According to studies and investigations, experts increasingly turn to natural remedies to treat disease, including date fruit extract (Phoenix dactylifera). This study tested the effectiveness of oral date palm fruit extract (Phoenix dactylifera) in reducing blood malondialdehyde levels in white rats (Rattus norvegicus) induced by excessive physical activity and increasing blood levels. Intense physical exercise increases white rat blood superoxide dismutase. This research is a laboratory experimental research or an actual experiment. The research design used a pre-test and post-test with a control group design to analyze the effectiveness of oral administration of date palm (Phoenix dactylifera) extract on the MDA and SOD levels of white rat (Rattus norvegicus) samples induced by excessive physical activity. The results of date fruit extract (Phoenix dactylifera) at 5 ml were more effective at reducing MDA and increasing SOD levels in white mice induced by excessive physical activity than at 3 and 4 ml. The experimental group treated with date fruit extract (Phoenix dactylifera) had lower MDA and higher SOD levels than the group given distilled water. Date fruit extract (Phoenix dactylifera) contains flavonoids and tannins. Some bioactive substances are antioxidants. Antioxidants can assist the body fight free radicals from excessive physical exertion.
