Abstract

Introduction: Post-stroke depression often causes problems. Depression can slow motor recovery. Giving antidepressants takes one month, and thus Transcranial Magnetic Stimulation (TMS) was developed, especially repetitive Transcranial Magnetic Stimulation (rTMS). This study aims to assess the effectiveness of rTMS in improving depressive symptoms and motor strength in ischemic stroke patients. Methods:  An experimental study with a randomized pretest-posttest control group design was conducted at Wahidin Sudirohusodo Hospital and a network hospital in Makassar from August to October 2022. The treatment group received standard therapy for ischemic stroke, antidepressants, and rTMS; the control group received standard therapy for ischemic stroke and antidepressants for ten days. In this study, the Hamilton Depression Rating Scale (HDRS) and the Medical Research Council (MRC) scale were used and measured in the treatment and control groups on the first and tenth days. Results: A total of 40 subjects met the criteria and were divided into a treatment group (n = 20) and a control group (n = 20). The HDRS score in the treatment group was smaller than in the control group. The value of left extremity motor strength in the treatment group was greater than that of the control group using the Wilcoxon Test. The correlation of the HDRS score to motor strength in the treatment and control groups showed a negative correlation of the HDRS score to the left extremity using the Spearman Test.   Conclusions:  rTMS effectively improves depressive symptoms and motor strength in ischemic stroke, given standard ischemic stroke therapy, antidepressants, and rTMS.

Abstract

Introduction and importance: One of the worst complications that affects people with stroke is dysphagia. Dysphagia causes mortality through dehydration, malnutrition, aspiration pneumonia, and suffocation. For stroke survivors to have better results, post-stroke dysphagia must be adequately addressed using proven treatment methods. Presentation of case: A 64-year-old man complaints of difficulty swallowing with Gugging Swallowing Screen scoring (GUSS)= 7 (severe dysphagia) with left hemiparesis and slurred speech due to ischemic stroke on August 14th 2022. Head CT Scan found bilateral lacunar basal ganglia infarcts with brain atrophy. Patients were treated with standard ischemic stroke therapy, medical rehabilitation and repeated Transcranial Magnetic Stimulation (rTMS) procedures for 10 sessions. GUSS was carried out after the 10th session of rTMS and increased to 14 (moderate dysphagia). Discussion: By stimulating the esophagus cortex bilaterally in post-stroke dysphagia, rTMS is known to be beneficial in modulating cortical excitability and minimizing the imbalance between the hemispheres. Furthermore, it appears to be safe and well-accepted by patients. Conclusions: Our case study shows that rTMS in bilateral esophageal cortex is safe and has therapeutic potential in patients with post-stroke dysphagia.

Abstract

Introduction: Atherosclerosis is currently recognized as part of the inflammatory disease. In the acute phase, inflammation causes brain edema and expansion of ischaemic area. Vascular cell adhesion molecule 1 (VCAM-1) plays a crucial role in the inflammation mechanism immediately after cerebral damage in stroke, facilitating the migration of leukocytes across the endhothelial cells. Objective: This study aimed to establish the correlation between VCAM-1 serum levels and severity and outcomes in stroke patients. Research Methodology: It was a cross-sectional study conducted from September to November 2023 on 51 patients with acute ischemic stroke at RSUP Wahidin Sudirohusodo Makassar and other affiliated hospitals. VCAM-1 serum levels were examined using the ELISA principle. Stroke severity was assessed using the National Institutes of Health Stroke Scale (NIHSS) upon admission, and clinical outcomes were evaluated using the modified Rankin Scale (mRS) on the 30th day of onset. Results: Mann- Whitney test results showed no difference in serum VCAM-1 levels between severity groups (mild and moderate) and clinical outcome groups (good and poor). Spearman correlation test results indicated a correlation between serum VCAM-1 levels and severity (p=0.635, r=-0.068) and clinical outcomes (p=0.459, r=-0.106). Conclusion: There is no association between serum VCAM-1 levels and the severity and clinical outcomes of acute ischemic stroke patients. Recommendation: Further research is needed to explore the relationship between serum VCAM-1 levels and severity and clinical outcomes in acute ischemic stroke patients at various centers in Indonesia.

Abstract

Introduction Stroke is an acute disorder of cerebral perfusion or vascularization. Stroke patients commonly encounter numerous functional disorders, such as motor, psychological or behavioral disorders, where the most typical symptom is hemiparesis which greatly affects their ability to carry out daily life activities. Repetitive Transcranial Magnetic Stimulation (rTMS) is one of the interventions proposed to improve stroke recovery. Constraint-Induced Movement Therapy (CIMT) is a neurological rehabilitation treatment designed to improve upper extremity motor function after stroke Objective: This study aims to compare the effectiveness of rTMS with CIMT in improving upper extremity motor function in ischemic stroke patients. Research Methodology: This is an experimental clinical trial study with a randomized pre-test post-test control group design on 30 ischemic stroke patients at Wahidin Sudirohusodo General Hospital Makassar and other network hospitals. Repetitive Transcranial Magnetic Stimulation was carried out for 10 stimulations, CIMT was carried out for 30 days. Motor improvement was evaluated by measuring the Fugl Mayer Assessment Upper Extremity (FMA-UE) score Results: The Friedmann test showed that rTMS and CIMT provided significant changes in FMA-UE scores (p: 0.000). Mann Whitney test showed there was a significant difference in FMA-UE scores in the rTMS group compared to CIMT (p: 0.000) Conclusion: There was a significant difference in the FMA-UE score in the rTMS group compared to CIMT. Recommendation: Further research is needed with longer observation and stimulation time and looks at the effectiveness of these two therapeutic modalities through a biomarker so that more objective results can be obtained.

Abstract

LATAR BELAKANG : Selama pandemi, pemerintah menetapkan Skala Besar Pembatasan Sosial (PSBB) peraturan untuk mengurangi kasus infeksi COVID-19, sehingga intensitas kejadian LBP meningkat lebih dari sebelumnya karantina, mencerminkan ed the negative effect of the regulation. This is due to significant lifestyle changes such as increased time spent sitting, reduced exercise time, and the percentage of individuals reporting more stress during quarantine than before the PSBB was established. The current study reports the impact of low back pain which results in disruption of the patient's functional status due to the need for annual medical expenses and the limited visits of patients to health services, so this indicates that low back pain sufferers need to evaluate how much functional disability occurs and the factors that influence it. OBJECTIVE : To determine the relationship between pain intensity and functional status of LBP patients at the Neurology Polyclinic, RSUP Dr. Wahidin Sudirohosudo. METODE : Observasi analitik dengan pendekatan cross sectional dan teknik pengambilan sampel adalah simple random sampling. Data yang digunakan adalah kuesioner Numerical Rating Scale dan Barthel Index. Penelitian ini dilaksanakan di RSUP Dr. Wahidin Sudirohusodo selama dua bulan dimulai dari bulan Maret 2021 sampai dengan April 2021. HASIL : Pada hasil analisis data sampel menggunakan uji korelasi tau-b Kendall, nilai korelasi (-0,644) dengan nilai signifikansi hubungan (p.sig 0,000) menunjukkan adanya hubungan yang signifikan antara intensitas nyeri dengan status fungsional. KESIMPULAN : Intensitas nyeri berhubungan bermakna dengan status fungsional pasien Low Back Pain (LBP) di RSUP Dr. Wahidin Sudirohusodo Kata kunci : Low Back Pain, NRS, Barthel Index
