Abstract

Pendahuluan: Periodontitis apikalis adalah peradangan dan kerusakan jaringan pada daerah apikal jaringan periodonsium. Terjadi akibat pulpa nekrosis mengalami perluasan infeksi dari bakteri saluran akar menuju apeks gigi. Penelitian terdahulu membuktikan bahwa periodontitis apikalis dapat dikaitkan dengan peningkatan kadar interleukin-6. Interleukin-6 adalah salah satu sitokin pro-inflamatori yang mempunyai peran penting dalam respon inflamasi. Tujuan penelitian menganalisis perbedaan kadar interleukin-6 dalam darah vena antara pasien dengan periodontitis apikalis dan pasien tanpa periodontitis apikalis. Metode: Jenis penelitian observasional analitik dengan dua puluh sampel stok darah vena yang terdiri atas 10 sampel darah vena pasien dengan periodontitis apikalis dan 10 sampel darah vena pasien tanpa periodontitis apikalis. Serum darah yang telah dipisahkan dengan metode sentrifugasi dipakai sebagai sampel uji enzyme-linked immunosorbent assay (ELISA) sandwich untuk mengukur kadar IL-6 dengan membaca nilai absorbansi dan kurva standar. Data yang diperoleh dianalisis menggunakan uji Shapiro-Wilk, dilanjutkan dengan uji T tidak berpasangan. Hasil: Kadar IL-6 teridentifikasi pada semua sampel. Sampel darah vena pasien tanpa periodontitis apikalis memiliki kadar IL-6 berkisar antara 4,7-18,74 mg/L, sedangkan kadar IL-6 pada pasien dengan periodontitis apikalis 4,0-90,75 mg/L. Uji T tidak berpasangan menunjukan bahwa terdapat perbedaan yang bermakna (p=0.02). Simpulan:  Kadar IL-6 pada darah vena dengan periodontitis apikalis lebih tinggi dibandingkan kadar IL-6 pada tanpa periodontitis apikalis.Kata kunci: periodontitis apikalis; interleukin-6; Enzim-Linked Immunosorbent Assay SandwichABSTRACTIntroduction: Apical periodontitis is inflammation and tissue damage in the apical area of the periodontium. Occurs due to pulp necrosis experiencing an expansion of infection from root canal bacteria to the apex of the tooth. Previous studies have shown that apical periodontitis can be associated with elevated levels of interleukin-6. Interleukin-6 is a pro-inflammatory cytokine that has a vital role in the inflammatory response. The study aimed to analyze differences in interleukin-6 levels in venous blood between patients with apical periodontitis and patients without apical periodontitis. Methods: This research was an analytic observational study with twenty venous blood samples consisting of 10 venous blood samples from patients with apical periodontitis and ten venous blood samples from patients without apical periodontitis. Blood serum that has been separated by centrifugation method was used as a sandwich enzyme-linked immunosorbent assay (ELISA) test sample to measure IL-6 levels by reading absorbance values and standard curves. The data obtained were analyzed using the Shapiro-Wilk test, followed by an unpaired T-test. Results: IL-6 levels were identified in all samples. Venous blood samples from patients without apical periodontitis had IL-6 levels ranging from 4.7-18.74 mg/L, while IL-6 levels in patients with apical periodontitis were 4.0-90.75 mg/L. The unpaired t-test showed that there was a significant difference (p=0.02). Conclusion: IL-6 levels in venous blood with apical periodontitis were higher than IL-6 levels in those without apical periodontitis. Abstract

Background: Pulp diseases can be affected by bacterial invasion such as Porphyromonas endodontalis. This bacterial invasion can lead to pulp diseases which include irreversible pulpitis and necrotic pulp. Purpose: To determine Porphyromonas endodontalis difference between teeth with irreversible pulpitis and necrotic pulp. Method: Samples were taken from root canals fluid of 18 teeth, 9 teeth with irreversible pulpitis, 9 teeth with necrotic pulp, using paper point. The paper point was placed in sterile Eppendorf tube, then DNA were extracted using Heat-Shock DNA Extraction method. Real-time PCR was used to collect the data. The conversion of microbiological data were used standard curve of Porphyromonas endodontalis. Result: The average of Real-time PCR on irreversible pulpitis was 6.9 x 10-12 ng/mL and on necrotic pulp was 1.3 x 10-12 ng/mL. However, t independent test showed there was no significant difference of Porphyromonas endodontalis between teeth with irreversible pulpitis and necrotic pulp (p>0,05) Conclusion: There was no difference in the total number of Porphyromonas endodontalis between teeth with irreversible pulpitis and necrotic pulp.

Abstract

Background: Porphyromonas gingivalis is one of the bacteria causes primary root canal infections. Bacterial elimination using root canal irrigation agents is important in successful root canal treatment. NaOCl as the gold standard is known to have disadvantages such as irritating periapical tissue. Active compounds in Pluchea indicaleaves such as flavonoids, tannins and terpenoids posses antibacterial effects. Objectives: To determine the effect of Pluchea indica leaves extract on P. gingivalis growth inhibition. Method: This study is an in vitro experimental laboratory of  agar well difusion method used Pluchea indica leaves extract in several concentrations (6,25%, 12,5%, 25%, 50%, and 100%) and NaOCl 5,25% as a positive control. The sample in this study was P. gingivalis. Result: This study showed that the diameter of inhibitor zone against P. gingivalis was exists on concentration of 50% and 100%, their values are 10,216 mm and 13,023 mm. The positive control showed an average inhibition zone of 12.803mm. Conclusion: Pluchea indica leaves extract showed antibacterial effect against P.gingivalis at the concentration of 50% and 100%. The 100% concentration had the largest inhibition zone and was equivalent to positive control in inhibiting P.gingivalis.

Abstract

Background: Porphyromonas gingivalis is one of the bacteria that cause pulp necrosis, NaOCl is an irrigation solution that has become the gold standard in endodontic treatment, however NaOCl is toxic, so other alternative materials are needed. One usable material is papaya plant (Carica papaya L.) which has antibacterial properties. Objective: to know the differences in the activity of papaya leaves extract, papaya seed extract, and papaya fruit juice in inhibiting P.gingivalis. Methods: This study is an in vitro experimental laboratory of agar well diffusion method used papaya seeds extract, papaya leaves extract and papaya fruit juice in several consentrations (6.25%,12.5%,25%,50%, and 100%) and NaOCl 5,25% as positive control. Results: This study showed that the diameter of inhibitory zone against P.gingivalis was found on 2 concentrations of papaya seed extract, a concentration of 100% with an average 23,43 mm and concentration of 50% with an avarege 16,43 mm. The inhibition zone test of papaya leaves extract and papaya fruit juice against P.gingivalis was not found around the well. Conclusion: Papaya seed extract was able to inhibit P.gingivalis in certain concentration, while papaya leave extract and papaya fruit juice were not able to inhibit the growth of P.gingivalis.

Abstract

Background: Health is the most important factor in human survival. Dental and oral health is part of overall health. Caries is a disease most commonly found in the oral cavity so that is the main problem of oral health. Dental caries should be taken care of by filling. Nowadays have been found filling materials for caries for example glass ionomer cement (GIC). At Karang Pule Health Center located in Mataram City, GIC is the preferred material often chosen by dentists for caries. Objective: To know about the use of GIC as restoration material in Karang Pule Health Center located in Mataram, NTB in 2016-2020. Method: This research is a descriptive retrospective research using the cross sectional approach by taking the medical records of patients in Karang Pule Health Center whom were given GIC as the filling material. Result: There were 628 patients using GIC and categorized based on the year of fillng, age,sex, region of feeling and the period of teeth in each patient. Conclusion: Based on the year of filled, it shows that 2017 was the highest frequency of fillings with GIC, which was 193 patients (30.7%), while in 2020 it was the least frequency, which was 17 patients (2.7%). GIC is most widely used in patients aged 26-35 years (adults), as many as 202 patients (32.2%) and the least is done in patients aged 56-65 years and over. GIC treatment is more often accepted by female patients than male patients. The mandibular posterior region of the teeth is the region most filled by GIC.

Abstract

Dental trauma is an injury involving the teeth and/or involving the supporting structures of the teeth. Dental trauma can cause tooth fracture, tooth displacement, tooth loss (avulsion), or damage to supporting tissues (gums and bones). During the Covid-19 Pandemic Period, mothers have more dominant time with their children, so they are expected to provide quick and effective action in dental trauma cases. This community Service Counseling aims to increase knowledge and skills in dealing with dental trauma problems. Counseling was carried out by the Trisakti University Faculty of Dentistry Team to PKK organization in Cideng, Gambir, Central Jakarta on January 5, 2022. Counseling includes the knowledge on handling emergency dental trauma using webinar methode with power point and animated videos. There is a significant difference in the mean percentage of correct answers between the pre-test (54,23 + 16,47) and post-test (68.58 + 11.45) with a significance level of p = 0.013. Thus, it can be concluded that after the Counseling activity, there is an increase in the level of knowledge regarding the handling of emergency dental trauma. Training is needed to improve skills so that trauma emergencies can be handled better

Abstract

ABSTRAKPendahuluan: Periodontitis Apikalis (PA) adalah peradangan jaringan periapikal yang berawal dari lesi karies. Tubuli dentin yang terbuka akibat adanya karies menjadi jalan masuk bakteri dan produknya. Proses ini akan terus berlanjut hingga mengakibatkan nekrosis pulpa dan peradangan berat sampai ke periapikal gigi. Infeksi pada gigi ini akan memediasi peradangan dan menginduksi sel untuk mengeluarkan sitokin proinflamasi. Tumor Necrosis Factor-Alpha (TNF-α) merupakan salah satu sitokin pada respon inflamasi akut bakteri Gram negatif dan protein pada fase akut. Tujuan penelitian menganalisis perbedaan jumlah kadar TNF-α di dalam sirkulasi darah vena antara pasien dengan periodontitis apikalis dan pasien dengan pulpa normal. Metode: Penelitian ini merupakan observasional analitik dengan rancangan cross sectional study. Sampel diambil dari darah vena 10 orang pasien dengan periodontitis apikalis dan 10 pasien normal (tanpa adanya gigi yang terinfeksi PA). Sampel ditentukan berdasarkan hasil anamnesis, pemeriksaan klinis yang dilakukan seperti tes perkusi atau palpasi dan terdapatnya gambaran radiolusen di apikal gigi. Kadar TNF-α, diperiksa menggunakan metode Sandwich ELISA, kemudian data dianalisis menggunakan uji statistik T tidak berpasangan. Hasil: Kadar TNF-α pada sampel darah vena pasien dengan periodontitis apikalis berkisar antara 0,068-0,121 mg/L, sedangkan kadar TNF-α pada pasien dengan pulpa normal berkisar 0,061- 0,143 mg/L. Uji T-test independent menunjukan nilai p=0,389 (p<0,05). Simpulan: Kadar TNF-α pada pasien dengan PA dan pulpa normal tidak memiliki perbedaan yang signifikan secara statistik.Kata kunci: periodontitis apikalis; porphyromonas endodontalis; tumor necrosis factor alpha (TNF-α); pulpa normalABSTRACTIntroduction: Apical periodontitis (AP) is an inflammation of periapical tissue initiated by caries lesions. The exposed dentin tubules due to caries are an entrance for bacteria and its products. This process will continue to result in pulp necrosis and severe inflammation extending to the periapical area of the tooth. The infection of this tooth will mediate inflammation and induce cells to release proinflammatory cytokines. Tumor Necrosis Factor-Alpha (TNF-α) is one of the acute inflammatory response cytokines in Gram-negative bacteria and protein in the acute phase. The purpose of this study is to analyze different levels of TNF-α in venous blood circulation between patients with AP and normal pulp. Methods: The type of study was an observational analytic with cross sectional study design. The blood samples were collected from 10 patients with AP and 10 with normal pulp. Samples were determined from medical history, percussion and palpation clinical examination, and apical radiolucency. TNF-α levels were then examined using the Sandwich ELISA method, and data were analyzed using an unpaired T-test. Results: TNF-α levels in venous blood samples from patients with AP ranged from 0,068-0,121 mg/L, and TNF-α levels in patients with normal pulp were  0,061- 0,143 mg/L. Unpaired t-test showed no significant difference p=0,389 (p<0.02). Abstract

Microleakage is one of the major problems in the successful of a composite resin restoration. The application of coating materials using surface sealant or re-bonding using bonding agent to the surface of the restoration is a method to reduce the occurrence of microleakage. The aim of this study is to determine the differences in microleakage between the application of surface sealant and one-step self-etch adhesive after composite resin restoration on class V cavities. Twenty-one lower premolar teeth were randomly selected and then divided into two treatment groups that consist ten samples for each group and one sample as control. Group I: Surface sealant (G-Coat Plus), group II: One-step self-adhesive (OptiBondTM All-In-OneTM Kerr), and the control group was not given any coating materials. All samples were prepared in the buccal surface with a size of 3 mm occlusal-gingival, 3 mm mesial-distal, and 1.5 mm cavity depth. After applied coating materials for all treatment groups, all samples including control group are smeared with nail coloring on the crown and apex, except the surface of the restoration until 1 mm around the restoration. Samples were then immersed in artificial saliva for 24 hours, followed by immersion in methylene blue for the next 24 hours. The teeth are cut in a buccal-lingual direction and then observed the penetration depth of methylene blue under a stereo microscope using 20x magnification. Statistical analysis using Mann-Whitney test between two treatment groups (p<0.05). The results of this study indicate that group I has less microleakage than group II. The conclusion of this study is that there are differences in microleakage between the application of surface sealants and one-step self-etch adhesive after composite resin restoration on class V cavities.

Abstract

It is important for everyone, regardless of age, to take into account their dental and oral health to enhance their overall well-being. Dentists have the responsibility of educating the public on proper dental and oral care in order to attain the best possible dental and oral health. Nevertheless, there are still numerous individuals who do not prioritize their dental and oral health. With the aim of fulfilling the Tridharma mission in Community Service, the Faculty of Dentistry at Trisakti University has implemented Public Health Services tha provide counseling on oral and dental health for families, staring with the elderly, general dental health, childrens dental health, and safe medications for families. Through the use of a questionnaire, an increase in knowledge was observed among individuals of different age groups regarding oral health care.

Abstract

Background : Fractures are the most undesirable thing in endodontically treated teeth. The main reason in endodontically treated teeth has a considerable risk for fractures is the loss of tooth structure. Objective : This study aims to determine the effect of access cavity preparation and instrument taper percentage on upper premolar fractures. Material and Methods : Twenty-four upper premolar samples were stored in sterile saline solution. The sample was divided into 2 large groups which were then divided into two groups for each group by random allocation. Twelve contracted endodontic cavity (CEC) preparations and the remaining twelve samples using traditional endodontic cavity (TEC) techniques. Each group was prepared with 6 samples 4% taper instrumentation and 6 samples 8% taper instrumentation. Each sample was irrigated with 5 ml of NaOCl. After that, the sample is tested using a universal testing machine. Result : The CEC group showed a significant reduction in fracture rates compared to the TEC group (p <0.05), but there was no significant difference in the fracture in the tapering instrumentation (p> 0.05). Conclusion : Preparation with the concept of minimally invasive CEC provides better resistance to fractures in the upper premolar while tapering instrumentation does not affect the fracture of the upper premolars.
