Abstract

Background: Pneumonia is the leading killer in under-five children. Therefore, by identifying the danger signs, we are able to predict children who are at higher risk of mortality. The objective of the study is to identify the relationship between the clinical profile and mortality in children with pneumonia at Dr. Hasan Sadikin General Hospital, Bandung.Methods: This was an analytical study with a retrospective case control approach using medical records with patient’s age limitation of 1–60 month. The study period started on January 1st 2010 and ended on December 31st 2011. All types of pneumonia included whereas congenital anomalies, immunocompromised and Down’s syndrome patients were excluded in this study. Data were presented as frequency distribution.Results: During the study period, there were 653 children under age of 5 with pneumonia. Only 56 subjects met the inclusion and exclusion criteria. Twenty-eight patients with pneumonia were cured and 28 died. Based on the phi’s coefficient, tachycardia (α-value = 0.019) and hepatomegaly (α-value = 0.001) were significant predictors of death and based on the Mantel-Haenszel analysis, hepatomegaly (OR=9.62, CI 95% 2.349–39.35) was significant as a risk for mortality. Inability to drink, cyanosis, tachypnea, grunting, vomiting, convulsion, and unconsciousness were not related to mortality.Conclusion: Tachycardia and hepatomegaly have a significant relationship with mortality in under-five children with pneumonia. Abstract

Background: Pneumonia is one of the causes of death in infants in developing countries especially Indonesia. Appropriate treatment is needed to decrease mortality rate in children due to pneumonia. Ampicillin is one of first choices empirical antibiotic to children with severe pneumonia. The study was aimed to determine the failure rate of ampicillin in children with severe pneumonia.Methods: This study was a descriptive study which used medical records as source of data. Subjects were children aged 2–59 months with World Health Organization (WHO) defined with severe pneumonia and treated with intravenous ampicillin during January 2014 to July 2015 at Dr. Hasan Sadikin General Hospital Bandung. Samples were obtained using total sampling method while variables were analyzed using statistics software.Results: This study acquired 107 patients who met the inclusion criteria with 23.36% of them aged 2–11 months and 62.21% aged 12–23 months. Majority of subjects were male amounted as 63.55% while female occupied 36.45%. Treatment failure on the third day was 45.8% while 16.7% on the sixth day of therapy with majority failure due to existence of lower chest indrawing.Conclusions: Treatment failure on the third and sixth day of therapy still high that is characterized by the existence of lower chest indrawing as its main factor. Abstract

Pendidikan yang berkualitas adalah pendidikan yang tidak hanya mempersiapkan para peserta didiknya untuk suatu profesi atau jabatan saja, tetapi untuk menyelesaikan masalah-masalah yang dihadapinya dalam kehidupan sehari. Tujuan penelitian tindakan kelas ini adalah untuk meningkatkan hasil belajar peserta didik kelas VI pada mata pelajaran matematika materi Luas dan Volume di SD Negeri 3 Nambuhan pada Semester 1  Tahun Pelajaran 2016/2017. Penelitian ini dilaksanakan bulan Agustus sampai November 2016 bertempat di SD Negeri 3 Nambuhan, Kecamatan Purwodadi, Kabupaten Grobogan. Sumber data berasal dari guru dan Peserta Didik  kelas VI yang berjumlah 22 orang. Validasi data dilakukan melalui triangulasi data dengan analisis data dilakukan terhadap hasil data observasi, analisis data angket/soal, dan analisis hasil wawancara. Penelitian ini terdiri dari dua siklus yang masing-masing terdiri dari perencanaan, pelaksanaan, pengamatan dan refleksi. Hasil penelitian menunjukan bahwa nilai rata-rata hasil belajar peserta didik Kelas VI  di SD Negeri 3 Nambuhan, Kecamatan Purwodadi, Kabupaten Grobogan Semester 1 Tahun Pelajaran 2016/2017 pada mata pelajaran Matematika materi Luas dan Volume, hal ini dibuktikan dari peningkatan hasil belajar peserta didik pada pra pembelajaran rata-rata hasil belajar peserta didik sebesar 63,86, meningkat pada siklus I setelah menggunakan media gambar menjadi 76,13 dan akhirnya bisa di tingkatkan lagi pada siklus II menjadi 83,86. Adapun saran yang dapat diajukan antara lain : (1)Guru sebaiknya menggunakan media atau alat peraga dalam menyampaikan materi pelajaran, agar peserta didik lebih mudah dalam menerima materi pelajaran yang disampaikan; (2) Hendaknya peserta didik mengembangkan inisiatif dan keberanian dalam menyampaikan pendapat dan bekerjasama dalam proses pembelajaran untuk menambah pengetahuan sehingga dapat meningkatkan hasil belajar; dan (3) Sekolah dapat memfasilitasi adanya media pembelajaran dalam proses kegiatan mengajar.

Abstract

The diagnosis process of TB in children is complicated. Bacteriological exam is needed to establish diagnosis, but it is not possible to be done in children. Clinical symptoms are the main criteria for TB diagnosis in children. This study aimed to provide clinical profile of TB in children and was conducted using descriptive design. The data was taken from medical record of children with TB at RSHS pediatric outpatient clinic from January 2016 to December 2016. We collected 104 subjects using simple random sampling method. We found out that 64 subjects (61.54%) was diagnosed as lung TB and 40 (38.46%) with extra-pulmonary TB. Contact with the source of transmission was found in 33 patients (60%). We found that clinical symptoms of the subjects were fever 2 weeks (62.5%), weight increase difficulty (49%), lymphnode enlargement (36.5%), cough >= 2 weeks (14.4%), chest tightness (7.7%), bone swelling (4%), and night sweats (2%). We concluded that clinical symptoms that are found mostly in children with TB are fever >= 2 weeks, weight increase difficulty, lymphnode enlargement, cough >= 2 week, chest tightness, bony swelling, and night sweats.
