Abstract

Mandibular fracture is a discontinuity of mandible bone that usually leads to trauma. The fractured area is mostly the mandibular angle located in the third molar area. Therefore, this study aims to examine the incidence of mandibular angle fractures accompanied by impacted teeth in the oral surgery clinic of Hasan Sadikin Hospital. This is a retrospective observational study and the data used were collected from the medical records of patients with mandibular angle fractures accompanied by impacted third molars at the Oral Surgery Clinic of Dr. Hasan Sadikin Bandung from January 2017 to December 2019. Panoramic radiographs were obtained for confirmation and the data collected were age, gender, fracture aetiology and location, impacted tooth type and classification, as well as treatment. The number of mandibular angle fractures with impacted third molars in male patients (92.8%) was more than in females (7.2%). Fractures caused by traffic accidents and fights were 85.8% and 14.2%, respectively, while all patients were treated with ORIF. The percentage of cases according to the classification of third molars in angle fractures are classes IA (20%), IB (6.7%), IIA (20%), IIB (20%), IIC (13.3%), IIIC (6.7%), and unerupted tooth seeds (13.3%).The incidence of mandibular angle fracture with impacted third molars at the Oral Surgery Clinic, Dr. Hasan Sadikin over the last three years has increased. This case is more common in males due to traffic accidents and is treated with surgery (ORIF).

Abstract

ABSTRAKPendahuluan: Fraktur palatum pada trauma maksilofasial jarang terjadi dan seringkali disertai fraktur dentoalveolar. Fraktur palatum yang disertai fraktur dentoalveolar seringkali menimbulkan kesulitan dalam perawatan dan dapat menimbulkan deformitas lengkung rahang. Penelitian ini bertujuan untuk   mengetahui karakteristik fraktur palatum disertai fraktur dentoalveolar pada pasien trauma maksilofasial di Rumah Sakit Hasan Sadikin Bandung. Metode: Jenis penelitian observasional deskriptif dengan pendekatan cross-sectional dari rekam medis sejak Januari 2019 sampai Desember 2020. Penelitian dilakukan di Klinik Bedah Mulut dan Maksilofasial di RS Hasan Sadikin Bandung tahun 2021. Variabel yang dikumpulkan meliputi usia, jenis kelamin, lokasi fraktur, klasifikasi fraktur palatum dan klasifikasi fraktur dentoalveolar. Data disajikan dalam bentuk distribusi frekuensi menggunakan distribusi frekuensi numerik dengan perhitungan menggunakan software SPSS V 26. Hasil: Sampel penelitian yang memenuhi kriteria inklusi dan eksklusi yaitu sebanyak 12 pasien yang meliputi fraktur palatum disertai fraktur dentoalveolar. Usia terbanyak remaja akhir 17-25 tahun berjumlah 8 sampel. Jenis kelamin paling banyak terjadi pada laki laki 10 sampel. Lokasi fraktur palatum terbanyak pada sisi kiri tipe II dan tipe III sebanyak 50%. Klasifikasi fraktur dentoalveolar yang terbanyak adalah tipe V = 7 sampel. Lokasi fraktur dentoalveolar di anterior pada semua sampel sebanyak 12 sampel. Klasifikasi fraktur palatum yang terbanyak adalah tipe II = 5 sampel. Jenis perawatan terbanyak adalah closed reduction = 11 sampel. Etiologi fraktur palatum disertai fraktur dentoalveolar semuanya dikarenakan kecelakaan bermotor. Simpulan: Karakteristik fraktur palatum yang disertai fraktur dentoalveolar pada pasien trauma maksilofasial di Rumah Sakit Hasan Sadikin Bandung yang terbanyak usia remaja akhir, dengan jenis kelamin laki-laki, fraktur palatumnya tipe II dan III disertai fraktur dentoalveolar tipe V.Kata kunci: fraktur palatum; dentoalveolar; close reduction; interdental wiringABSTRACTIntroduction: Fracture of the palate in maxillofacial trauma is rare and often accompanied by dentoalveolar fractures. It is difficult to treat and can lead to arch deformity. This study aimed to identify characteristics of palatal fractures with dentoalveolar fractures in maxillofacial trauma patients at Hasan Sadikin Hospital, Bandung. Methods: This research is descriptive observational with a cross-sectional approach, medical records from January 2019 to December 2020. The study conducted at the Oral and Maxillofacial Surgery Clinic, Hasan Sadikin Hospital, Bandung, 2021. The variables collected included age, gender, fracture location, classification of palatal fractures, classification of dentoalveolar fractures. The data is presented in the form of a frequency distribution using a numerical frequency distribution with calculations using SPSS V 26 software. Results: 12 patients met the study sample criteria, including palatal fractures accompanied by dentoalveolar fractures. Most of the late adolescents aged 17-25 amounted to 8 samples. The most common sex is male with 10 samples. Location of most palate fractures on the left side of type II, and type III is as much as 50%. The most common classification of dentoalveolar fractures is type V = 7 samples. The location of the dentoalveolar fracture was anterior in all 12 samples. The most common classification of palatal fractures is type II = 5 samples. The type of treatment is closed reduction = 11 samples. The etiology of palatal fractures accompanied by dentoalveolar fractures is all due to motor accidents. Conclusion: Characteristics of palatal fractures accompanied by dentoalveolar fractures in maxillofacial trauma patients at Hasan Sadikin Hospital Bandung, mostly in late teens, with male sex, type II and III palatal fractures accompanied by type V dentoalveolar fractures.Keywords: palatal fracture; dentoalveolar; close reduction; interdental wiring

Abstract

ABSTRAKPendahuluan: Fraktur dentoalveolar merupakan trauma yang paling umum terjadi pada wajah dibandingkan dengan semua trauma yang terjadi pada wajah. Fraktur dentoalveolar memiliki dampak buruk bagi pasien jika tidak diberikan perawatan yang memadai. Perawatan fraktur dentoalveolar merupakan prosedur kompleks yang butuh pengetahuan, diagnosis, dan rencana perawatan yang akurat dari seorang dokter gigi. Tujuan penelitian ini untuk mengetahui pengetahuan dan pengalaman dokter gigi dalam penanganan darurat fraktur dentoalveolar. Metode: Jenis penelitian cross-sectional  dengan teknik pengambilan sampel proportionate stratified random sampling. Penelitian dilakukan terhadap 306 responden yaitu dokter gigi umum, dokter gigi residen, dan dokter gigi spesialis di Kota Bandung. Instrumen penelitian menggunakan kuesioner dengan 15 pertanyaan untuk mengukur pengetahuan dan 3 pertanyaan untuk mengetahui pengalaman dokter gigi. Analisis data menggunakan Microsoft excel dan disajikan dalam bentuk tabel, tingkat pengetahuan menggunakan kategori Arikunto. Hasil: Untuk tingkat pengetahuan, sebanyak 3 responden (1%) memiliki tingkat pengetahuan kurang, 78 responden (25%) pada kategori cukup, dan 225 responden (75%) pada kategori baik. Pengalaman dokter gigi dalam menemukan kasus fraktur dentoalveolar 1-5 kali selama praktik, pengalaman tindakan yang dilakukan adalah perdarahan dihentikan lalu segera dirujuk, dan mayoritas dokter gigi tidak pernah mengikuti pelatihan darurat fraktur dentoalveolar. Simpulan: Tingkat pengetahuan dokter gigi di Kota Bandung dalam penanganan darurat fraktur dentoalveolar secara keseluruhan berada dalam kategori baik. Pengalaman dokter gigi dalam menjumpai kasus fraktur dentoalveolar cukup sedikit selama praktik dengan kasus terbanyak menghentikan perdarahan dan segera dirujuk serta tanpa adanya pengalaman dalam mengikuti pelatihan perawatan fraktur dentoalveolar.Kata kunci: fraktur dentoalveolar; pengetahuan dokter gigi; pengalaman dokter gigi; cedera gigi traumatis ABSTRACTIntroduction: Dentoalveolar fractures are the most common trauma compared to all traumas to the face. It harms the patient if they do not have adequate treatment. Dentoalveolar fractures are a complex procedure requiring a dentist knowledge, diagnosis, and an accurate treatment plan. This study aimed to determine dentist knowledge and experience level in emergency management of dentoalveolar fractures. Methods: Cross-sectional using a proportionate stratified random sampling technique. The study was conducted on 306 respondents, namely general dentists, resident dentists, and specialist dentists in Bandung. The research used a questionnaire with 15 questions to measure knowledge and 3 questions to determine the experience of dentists. Data analysis was performed using Microsoft Excel and presented as a frequency distribution table. the level knowledge categorized using arikunto. Results: For the level of knowledge, as many as 3 respondents (1%) had a low level of knowledge, 78 respondents (25%) in the good category, and 225 respondents (75%) in the good category. When dentist found cases of dentoalveolar fracture 1-5 times during practice, dentists stopped bleeding and referred immediately, but most dentists never participated in emergency dentoalveolar fracture training. Conclusion: Bandung Dentists’ knowledge in handling dentoalveolar fractures is good. The experience of dentists in cases of dentoalveolar fractures is relatively small during practice, with most cases stopping bleeding and referred immediately without any experience participating in dentoalveolar fracture treatment training.Keywords: Dentoalveolar fracture; dentist knowledge; dentist experience; traumatic dental injury

Abstract

AbstractIntroduction: Oro-maxillofacial traumas comprise injuries to the face, jaws, or both; they include injury to any of the bony structures, skin, and soft tissues around the face. During the COVID-19 pandemic, human activity was mostly at home, with movement restrictions. The purpose of this study is to find out the types of characteristics and management of oro-maxillofacial traumas during the COVID-19 pandemic at the emergency room. Methods: A cross-sectional study was conducted to evaluate every maxillofacial trauma in patients treated by the oral surgeon in the emergency room at RSUP Dr. Hasan Sadikin Bandung Hospital. Incidence records were taken directly from case reports from January 2020 to December 2021. The variables considered were patient age, gender, etiologies, sites, classifications, and management of the trauma. Then integrated into a table to be analyzed epidemiologically. Results: There were 177 maxillofacial trauma patients registered in the Department of Oral and Maxillofacial Surgery. It was found that the gender ratio, female to male, is equal to 2.3:7.6. The 18-36 age group accounted for the largest subgroup in both sexes. Motorcycle accidents accounted for as much as 80.22% of the etiology of the most common traumas. While dentoalveolar (28.22%) was the most common site of the fracture, followed by the mandible (23.31%). Injuries around the mouth made up the largest portion (62.15%) of the areas that were directly impacted by the injuries, followed by injuries to the face (18.64%) and injuries to the mouth and nose (6.21%). Among the 177 patients mentioned above, 10.17% were hospitalized. Conclusion: Male’s are more likely to be involved in traffic accidents, mainly motorcycle crashes, which are the primary cause of traumas. Dentoalveolar fractures were the most common type of fracture, and the management of the oro-maxillofacial trauma is done according to the standard procedure.
