Abstract

Latar Belakang: Pengukuran sudut Tibiofemoral (TF) dan jarak Intermaleolar/Interkondilar (IM/IC) secara klinis merupakan cara yang cepat, murah, simpel, reliabel, dan bebas radiasi untuk melakukan skrining genu varum atau valgum patologis. Sudut TF dan jarak IM/IC normal berbeda antar negara. Sedangkan penelitian mengenai sudut TF dan jarak IM/IC normal di Indonesia belum pernah dilaporkan sebelumnya. Tujuan: Mengetahui rata-rata sudut TF dan jarak IM/IC pada mahasiswa pendidikan dokter di Fakultas Kedokteran Universitas Airlangga angkatan 2013.Metode: Penelitian ini menggunakan metode deskriptif epidemiologis. Data yang digunakan diambil langsung dari sampel.Hasil: Peneliti mengukur 168 mahasiswa yang memenuhi kriteria inklusi. 65% dari sampel berumur 20 tahun dan 61% dari sampel adalah wanita. Diantara seluruh sampel, 64% memiliki BMI yang normal. Underweight sebanyak 11% dan total overweight dan obese adalah 26%. Rata-rata BMI adalah 22,9 ± 4,1. Rata-rata sudut TF adalah -7,50o ± 3,350o valgum sedangkan rata-rata jarak IM/IC adalah -4,4 ± 33,09 mm intermaleolar. Sudut TF lebih valgus dan jarak IM/IC lebih varus dengan sedikit perbedaan dibandingkan studi sebelumnya.Kesimpulan: Dari studi ini, peneliti menyimpulkan bahwa diantara pendidikan dokter di Fakultas Kedokteran Universitas Airlangga angkatan 2013, sudut TF dan jarak IM/IC memiliki perbedaan yang sedikit dibanding studi sebelumnya dengan dominasi memiliki genu valgus.

Abstract

Background: Management of Gartland type III supracondylar humerus fractures is conduc­ted by open and closed repositioning. An ade­quate re­p­o­sition and a stable and accurate fixation are des­perately needed to prevent fixation failure, defor­mi­ty, and complication. The study aims to com­pa­re the clinical and radiological results bet­ween crossed and lateral fixation techniques.Subjects and Method: The study was a retro­s­pective study toward Gartland type III SCHF children in Dr. Soetomo Hospital, Surabaya, Indonesia from 2013–2016. The dependent vari­able is Supracondylar hu­me­rus fracture. Independent variables were the type of fixation option, clini­cal functional test, degrees of satisfactory, and radiology evalua­tion. The radiology para­me­ter used was Skaggs criteria. An observation was conducted for the occurrence of complications in the form of infection and peripheral nerve injury. All data were analyzed using Kolmogorov Smirnov and Fischer exact test.Results: The study discovered 28 patients consisted of 20 males and 8 females with an age range from 3 – 13 years old with an average age in crossed fixation group was 7.6 years and in lateral fi­x­ation was 4.7 years. The injury sides were 46.4% right elbow and 53.5 % left elbow. Among the cros­sed fixation group, there were 54.5 % left elbow and 45.5 % right elbow. Among lateral fix­a­tion group, there were 50% left side and 50% right side. There was no significant difference in cli­nical fun­ctions, radiology as well as com­pli­ca­tion in the form of infection and peripheral ner­ves injury.Conclusion: There is no difference of functional clinical, radiology result as well as post-surgery com­plication in the form of infection and peri­phe­ral nerves injury between crossed fixation tech­nique and lateral fixation technique.Correspondence: Komang Agung Irianto. Abstract

Introduction: Marfan syndrome is an autosomal dominant disorder of connective tissue, and skeletal system involvements are the cardinal features.  Triple rigid large major curve in conjunction with developmental dysplasia of the hip in Marfan syndrome is rare then sagittal and coronal imbalance correction on this case is challengingCase Presentation: We reported a case of 12 years old female with Marfan syndrome presented triple rigid large Major curve scoliosis on left cervico-thoracal, right main  thoracal and left thorakolumbal, also developmental dysplasia of the left hip has been done correction surgery consist of open reduction following cast immobilization of the left hip, multiple ponte osteotomy and asymetrical pedicle substraction osteotomy to correct sagittal and coronal imbalance.Discussion: the treatment of scoliosis in patients with Marfan syndrome parallels that in patients with idiopathic scoliosis and spinal fusion and instrumentation should be considered for curves greater than 45 to 50 degrees. Posterior technique, Ponte osteotomy correct 10 to 20 degrees and pedicle substraction osteotomy correct 30 to 40 degrees at each segment are effective for managing sagittal and coronal imbalance in scoliosis. Open reduction developmental dysplasia of the hip to overcome complicated coronal balance correction.Conclusion : Detailed planning scoliosis surgery is needed to correct a rare the sagittal and coronal imbalance due to scoliosis complicated with developmental dysplasia of the hip in Marfan syndrome.

Abstract

ABSTRACT  Demands of orthopedic implants are increasing in Indonesia, but its production are limited and still imported. Stainless steel, especially 316L is one of metal that is used commonly. This research evaluates biocompatibility of stainless steel 316L implant prototype, local implant manufactured using imported stainless steel, and imported stainless steel implant. Cytotoxicity test used mesenchymal human cell with MTT assay. Irritation test, acute toxicity/pyrogenicity test, and implantation test used 24 Wistar rats respectively which were assigned to 4 groups. We evaluate Draize score, weight and rectal temperature changes, and fibrous capsule ring thickness. Cytotoxicity test shows viable cell percentage in stainless steel 316L implant prototype, local implant, and imported implant are 83.7%, 87.5%, and 83.7% respectively. Comparison test using One Way ANOVA shows no significant difference between 3 groups (p>0.05). Iritation test results in 3 groups are grade 0 (no edema and no erythema). Acute toxicity/pyrogenicity test shows no statistically significant difference (p>0.05) among all groups. There is also no significant difference (p>0.05) of fibrous capsule ring thickness on 1 and 4 weeks after implantation. Stainless steel, utama 316L adalah salah satu metal yang sering digunakan. Penelitian ini mengevaluasi biokompatibilitas dari prototipe implan stainless steel 316L, implan lokal yang diproduksi menggunakan stainless steel impor, dan implan stainless steel impor. Uji sitotoksisitas menggunakan sel mesenkim human dengan MTT assay. Uji iritasi, uji pirogenositas, dan uji implantasi menggunakan 24 tikus Wistar yang terbagi dalam 4 grup. Kami mengevaluasi skor Draize, perubahan berat dan temperatur rektal, dan ketebalan fibrous capsule ring. Uji sitotoksisitas menunjukkan persentase viabilitas sel pada prototipe stainless steel 316L, implan lokal, dan implan impor berturut-turut adalah 83,7%; 87,5%; dan 83,7%. Uji komparasi dengan One Way Anova tidak menunjukkan perbedaan signifikan diantara 3 grup (p>0,05). Hasil uji iritasi 3 grup menunjukkan grade 0 (tidak ada edema dan eritema). Uji pirogenositas tidak menunjukkan perbedaan signifikan pada semua grup (p>0,05). Tidak ada perbedaan signifikan (p>0,05) pula pada ketebalan fibrous capsule ring di minggu ke 1 dan 4 setelah implantasi. Abstract

Introduction: Clinical measurement of Tibiofemoral (TF) angle and Intermalleolar/Intercondylar (IM/IC) distance are quick, low cost, reliable, and radiation-free way to screen for pathological genu varus/valgum. TF Angle and IM/IC distance varied between countries and races. Meanwhile, research regarding normal TF Angle and IM/IC distance in Indonesia has not been reported before.Aim: To find the average TF angle and IM/IC distance on medical students of Medical Faculty of Airlangga University batch 2013.Methods: Descriptive epidemiologic study were conducted. The data is collected by direct measurement on the sample.Results: We measured 168 students which fulfilled the inclusion criteria. 65% of the samples were 20 years old and 61% of the samples were females. Amongst the samples, 64% had normal BMI, 11% were underweight, and 26% were overweight and obese. Average BMI is 22,9 ± 4,1. The average TF angle were -7,50 ± 3,350 valgum while the average IM/IC distance were -4,4 ± 33,09 mm intermalleolar. Compared to previous studies, there is slight differences in TF angle and IM/IC distance but were comparable.Conclusion: Amongst medical students of Faculty of Medicine Airlangga University, the TF angle and IM/IC distance were comparable to other studies and mostly had valgus angle.

Abstract

Background: Genu varum is the condition of the legs that bend inward, leading to walking disruptions. However, data on genu varum are still lacking to prevent the disease. This study aimed to identify the epidemiology of patients with genu varum in preventing, managing and determining the prognosis and as a source for future research.Methods: This research is a descriptive study using a retrospective research design. The sampling technique used is a total sampling of genu varum patients gained from the Department of Orthopedics and Traumatology database, medical records from Dr. Soetomo General Academic Hospital Surabaya, and patients' home visits. The target population is all genu varum patients from 2010-2018.Results: The total sample was 31 patients, 21 patients were male (67%), and 10 patients were female (32%). The average age of patients was 4.3 years. The first complaint of the disease was, on average, realized at 1.8 years. The birth weight data obtained an average of 3.49 kg. The average body mass index is 26.3. Langenskiold stage I,II,III,IV,V,VI type; 2 (3%), 43 (70%), 2 (3%), 5 (8%), 2 (3%), 8 (13%). Eighteen patients (58%) had bilateral Blount disease and 7 patients (22%) had unilateral Blount disease, and only six patients (20%) had physiologic genu varum. Eighteen patients were carried out conservative methods, and 13 underwent operative methods.Conclusion: Blount disease is the commonest cause of genu varum in Dr. Soetomo General Academic Hospital, specifically infantile. The highest number of patients are male and those who underwent operative treatment.

Abstract

Background: Zoledronic acid as bisphosphonates could increase bone mineral density (BMD), which in osteogenesis imperfecta will reduce clinical manifestations. Pediatric patients with osteogenesis imperfecta given zoledronic acid therapy should improve their quality of life.Methods: A retrospective analytic study was conducted on 16 pediatric osteogenesis imperfecta patients who had received intravenous zoledronic acid. Interview using PedsQL 4.0 regarding the condition before and after therapy assessed the quality of life. A paired t-test was used to assess the contrast in each aspect of PedsQL 4.0. Results were expressed as an adjusted odds ratio with a 95% confidence interval. P-value <0.05 is considered statistically significant.Results: A significant increase was found in the child report of social performance (+12.083, p=0.023), and proxy report of physical performance, social performance, and total score of the proxy report (+14.844, p=0.006; +10.625, p=0.010; +10.364, p=0.006). An insignificant increase was found in the child report of physical performance, school performance, and total score of the child report (+8.833, p=0.148; +5.000, p=0.359; +7.065, p=0.115), and proxy report of emotional performance and school performance (+2.500, p=0.669; +6.250, p=0.167). An insignificant decrease was found in child reports of emotional performance (-2.500, p=0.669).Conclusion: After receiving bisphosphonate therapy, pediatric patients with osteogenesis imperfecta experienced an increased quality of life in most aspects within every perception. Physical and social aspects experienced the greatest improvement. Emotional is the aspect that experienced the lowest increase in the child's perception and decreased parent perception of the value of quality of life after bisphosphonate therapy.

Abstract

Penelitian komplikasi pasca operasi di RSUD berfokus pada pasien anak dengan tujuan utama memahami karakteristik demografi dan kejadian komplikasi pasca operasi tertentu. Dari segi demografi, mayoritas responden adalah laki-laki yaitu sebesar 61,8% sampel dengan rata-rata usia pasien 8 tahun. Rentang usia responden bervariasi, mulai dari usia minimal 2 tahun hingga maksimal 17 tahun. Selain itu, aspek riwayat kesehatan juga dievaluasi. Tidak ada satu pun responden yang memiliki riwayat infeksi saluran pernapasan atas (ISPA) atau penyakit refluks gastroesofagus (GERD), dua kondisi yang dapat mempengaruhi hasil pembedahan. Dalam konteks komplikasi pasca operasi, hasil penelitian menunjukkan bahwa tidak ada kejadian croup, bradikardia, atau desaturasi pada responden. Hal ini menunjukkan tingkat keberhasilan yang baik dalam manajemen bedah dan pemulihan pasca operasi. Namun ada catatan khusus terkait hipotermia. Sebanyak 34,7% sampel mengalami hipotermia pasca operasi, sedangkan hanya 0,8% yang tidak mengalaminya. Komplikasi ini menyoroti pentingnya memantau suhu pasien setelah operasi. Secara keseluruhan temuan ini menyoroti bahwa meskipun keberhasilan dalam mencegah beberapa komplikasi serius, masih ada aspek tertentu dari pemulihan pasca operasi yang memerlukan perhatian lebih, seperti penanganan hipotermia.
