Abstract

Erythrocyte which is infected by Plasmodium falciparum will have various changes on its architecture, affinity, and biomolecular. Beside that, the infected erythrocyte also forms a knob at its surface. This knob are contained with various parasite proteins, one of them is Plasmodium falciparum erythrocyte membrane protein-1 (PfEMP-1). Our previous study had been identified a protein with molecular weight 270 kDa at P. falciparum infected erythrocyte from Malang isolate that was playing a role in cytoadherence process. The aim of this study was to detect the possibility of 270 kDa protein expression at complicated malaria falciparum patient erythrocyte membrane. The method that used was immunocytochemistry with polyclonal antibody to 270 kDa protein. The results showed that two (2) erythrocyte samples from healthy people as control had negative reaction, and so did with five (5) erythrocyte samples of uncomplicated malaria patient, but there was positive reaction that shown at two (2) samples of complicated malaria patient erythrocyte. It can be concluded from the results that 270 kDa membrane protein of P. falciparum infected erythrocyte in complicated malaria patient might be a PfEMP-1. Abstract

Background: The epidemic of SARS-CoV-2 has spread worldwide for about one and half years, including in Indonesia, with several peaks in cases and mortality rates. A descriptive pattern of Covid 19 inpatients in Saiful Anwar General Hospital Malang needed to improve health service quality and reduce mortality rates. Aim: Determine the descriptive pattern of inpatients Covid 19 in the internal medicine ward in Saiful Anwar General Hospital from March 2020 until February 2022. Methods: This descriptive study was conducted from March 2020 to July 2021 and used convenience sampling due to sample availability. A total of 137 hospitalized patients in December 2020, 387 in July 2021, and 113 in February 2022 patients were included with information about age, sex, medical diagnosis, comorbid, and outcome, which was obtained from medical records. Descriptive analysis was used to explore frequency distribution and analysis. Results: We found several factors associated with the frequency of SARS-CoV-2 cases. Overall, these results emphasize the role of comorbidity as a possible driver of the COVID‑19 case fatality rate. Conclusion: In the three waves of Covid 19 in Indonesia, the highest case fatality rate was during the Delta variant in July 2021, where more factors significantly affected case mortality.

Abstract

Idiopathic CD4 lymphocytopenia (ICL) is a rare condition characterized by an unexplained deficit of circulating CD4 T cells leading to increased risk of serious opportunistic infections. Reactivation and severe symptomatic CMV is also a rare case, except in immunocompromised patient. Patient concerns are 36-year-old male patient complained seizure, fever, cough, and dyspnea. History of chronic urticaria and prolonged Mebhydrolin consumption. HIV was negative. CD4 was low. Diagnosis in this case is Viral encephalitis, Viral pneumonitis, CMV reactivation, hemophagocytic lymphohistiocytosis immunodeficiency state. The results in this study indicate that the patients were treated with conventional therapies for Severe CMV infection with Acyclovir. Steroid was given for the last five days. The first day after administration of therapy showed clinical and laboratory improvement. Laboratory values returned to normal values on the fifth day of therapy. Since the second day after the therapy was given, the patient had no complaints. In cases of CMV reactivation, an immunodeficiency condition should be suspected. A comprehensive history, physical examination, laboratory and radiological examination, adequate therapy and and policy support are required to establish a definite diagnosis and reduce the risk of mortality.

Abstract

Family is a basic social unit of senior citizen. Relationship among family members considered as source of emotional comfort. Meanwhile, economic, political, and social changes during pandemic has an impact on the family, the support provided can be manifested among vulnerable groups such as the elderly. Prolonged stress due to isolation care of Covid-19 infection increase psychological distress by reducing family support. This study aimed to explore perceived family function among elderly patients after Covid-19 infection. A cross sectional study was conducted among 33 elder patients with history of Covid-19 infection in Saiful Anwar General Hospital, Indonesia. Perceived family functionality was evaluated using APGAR family score. Data was analyzed using descriptive analyses, independent T test, correlation and regression. Mean total APGAR score was 9.610.70 with joint pain had significant association with APGAR score (p 0.021) indicating full family support in elder person with symptoms. Gender female was associated with 'Resolve' (p 0.038) which might correlate with bigger role of elder female in Indonesian family. Length of stay was significantly associated with 'Partnership' (p 0.012) and 'Growth' (0.043) with inverse correlation. It showed how prolonged hospital stay affected family relationship especially in terms of communication. Perceived family supports remained high in elderly during Covid-19 isolation in Indonesia. Specific symptoms such as joint pain, even strengthen the family support for elder member of family. Prolonged stay in the hospital during Covid-19 infection, might become a communication barrier between elder person and the family which affect decision sharing and emotional expression.

Abstract

COVID-19 menyebabkan banyaknya kematian pada populasi lanjut usia (lansia). Di Indonesia, hingga akhir Mei 2021 terdapat 49% kematian akibat COVID-19 dan pada pasien di atas 60 tahun (lansia) hanya didapatkan 10% kasus sembuh. Lansia dikategorikan sebagai kelompok berisiko terinfeksi virus Corona.  Wabah penyakit ini tidak hanya berdampak pada kesehatan fisik tetapi juga kesehatan mental. Pada saat pandemi, memahami kualitas hidup lansia yang sembuh dari COVID-19 pasca perawatan di rumah sakit adalah tantangan global yang baru. Penelitian ini dilakukan dengan mengambil data lansia yang selamat dari COVID-19 pasca rawat inap untuk dinilai kualitas hidupnya dan kesehatan diri. Penilaian dengan wawancara melalui telepon. Data demografi, sosial ekonomi, status kesehatan diambil dari rekam medis pasien. Responden memperoleh rerata skor kualitas hidup (EQ-5D) 0,911 dan rerata skor VAS 89,6. Nyeri sendi memiliki korelasi yang signifikan secara statistik dengan ketidaknyamanan (p < 0,000). Lama rawat inap kurang dari 14 hari memiliki hubungan yang bermakna dengan ketidaknyamanan (p < 0,05).  Sedangkan dimensi lain seperti mobilitas, perawatan diri, aktivitas sehari-hari dan kecemasan/depresi tidak berkorelasi dengan ketidaknyamanan secara signifikan.  Kesimpulannya, kualitas hidup dan kesehatan diri yang dilaporkan oleh lansia penyintas COVID-19 relatif  bagus. Adanya keluhan nyeri sendi dikaitkan dengan kualitas hidup yang lebih buruk (dimensi ketidaknyamanan), sementara risiko ketidaknyamanan meningkat secara signifikan dengan lama rawat yang lebih singkat.
