Abstract

Background: Cataract is a serious problem in the world because it is the biggest cause of blindness in the world. Cataract healing can only be done with operative measures, one of them is phacoemulsification. The phacoemulsification technique is the latest operative technique for cataract healing but both conventional techniques or phacoemulsification techniques can cause a decrease in tear secretion after cataract surgery. The reduction in tear secretion can be evaluated using the Schirmer I test. In this study an analysis of differences in tear secretion before and after phacoemulsification surgery.Aims: Knowing, comparing, and analyzing the differences in tear secretion before and after phacoemulsification surgery using the Schirmer I test.Methods: 22 cataract eyes consisting of 15 patients who will undergo phacoemulsification surgery were collected by consecutive sampling at the Sultan Agung Eye Center, Sultan Agung Hospital, Semarang from August to September 2019, examined using the Schirmer I test. After the data was collected, data analyzed with Wilcoxon rank test.Results: A total of 15 cataract patients consisting of 10 female patients and 5 male patients with a total of 22 eye samples. The average tear secretion before and after phacoemulsification surgery was 12.77 ± 2.74 mm and 11.00 ± 1.95 mm. Data were tested with the Wilcoxon rank test, p = 0,000 (p <0.05) which showed a difference in average tear secretion before and after phacoemulsification.Conclusion: There are differences in tear secretion before and after phacoemulsification surgery.

Abstract

Background: Myopia is one of the visual disorders that has a high prevalence in the world. Based on the severity degree, myopia is divided into three criteria, namely mild, moderate and severe. Severe-degree myopia patients are more likely to suffer from certain eye diseases, especially those associated with the retina, such as retinal detachment.Objective: To analyze the relationship between the degree of myopia and the incidence of retinal detachment.Methods: This research is an observational analytical with cross-sectional approach. The research data was obtained using consecutive sampling from the medical records of myopia patients in Diponegoro National Hospital during 2015-2020. Research subjects used in this research were 70 patients who fulfill the inclusion and not the exclusion criteria. The data were analyzed using Chi Square test.  Results: The prevalence of retinal detachment is dominated by the age group of 51-60 years old (18.6%), male (35.7%) and a severe degree of myopia (34.3%). The results of the Chi Square test show that there is a significant relationship between the severity degree of myopia and the incidence of retinal detachment.Conclusion: There is a significant relationship between the degree of myopia and the incidence of retinal detachment.

Abstract

Background: Diabetic retinopathy is one of the second highest complications of microangiopathy in Indonesia due to diabetes mellitus in the form of damage to retina of the eye and can cause blindness in adults. The management of diabetic retinopathy is done by operative measure, one of which vitrectomy. However, this procedure can lead to an increase in the value of intraocular pressure after surgery. In this study, analysis of differences in intraocular pressure values before and after vitrectomy was carried out in patients with diabetic retinopathy.Research Purpose: To know, compare, and analyze the intraocular pressure values before and after vitrectomy in patients with diabetic retinopathy.Method: The study design was an analytic observation with a study design cross-sectional. Sampling using method consecutive sampling obtained from medical record of 45 diabetic retinopathy patients who underwent vitrectomy at the RSND for the period July-December 2019. After the data was collected, the data were analyzed using the difference Wilcoxon rank test.Result: A total of 45 diabetic retinopathy patients underwent vitrectomy from medical records were dominated by 24 people aged >69 years and 26 female patients. The mean values of intraocular pressure before and one week after vitrectomy were 20.32 + 2.57 mmHg and 23.89 + 9.24 mmHg. The results showed that there was a higher intraocular pressure value after vitrectomy than before vitrectomy in diabetic retinopathy patients (p=0.021) or p,0.05

Abstract

ABSTRACT DIFFERENCE OF OSDI SCORE BEFORE AND AFTER ‘SENAM PERMATA-KU’ IN COMPUTER VISION SYNDROME Background: Computer Vision Syndrome (CVS) is a group of symptoms that affects the eye and vision due to prolonged use of computer, cellphone, tablet, and e-reader. Dry eye is one of the symptoms that appear in CVS. In this study we did ‘senam PERMATA-KU’ (Pelihara Mata–Kendorkan Bahu) as additional exercise to reduce dry eye in CVS. Aim: Observe there is any improvement of OSDI (Ocular Surface Disease Index) score before and after doing “senam PERMATA-KU” in CVS. Methods: This was a Pre-Post with Control Design quasi experimental study. The subjects were students of Faculty of Medicine of Diponegoro University who had CVS and met the inclusion and exclusion criteria that were divided randomly into control group and experimental group. The experimental group were given an education about using computer ergonomically and ‘senam PERMATA-KU’ for 14 days. The control group were given an education about using computer ergonomically. OSDI score were measured before and after intervention was given using Ocular Surface Disease Index questionnaire. Result: The subjects were 32 students with 16 students in the experimental group and 16 students in the control group. One student in the control group and one student in the experimental group dropped out because they could not complete the procedure so that only 30 subjects were analyzed. OSDI score after intervention in experimental group was decreased significantly (p<0.05). OSDI score after intervention in experimental group was better compared to control group (p<0.05). OSDI score improvement in experimental group was higher compared to control group but statistically is not significant (p=0.595). Conclusion: ‘senam PERMATA-KU’ that was done for 14 days can improve OSDI score in Computer Vision Syndrome. Abstract

ABSTRACT Background:Intra ocular pressure (IOP) elevationis one of topical steroids’ side effects. Corticosteroid will initiate matrix metalloproteinase cascade that leads to extra cellular matrix (ECM)of trabecular meshwork (TM) turnover(remodeling) and its acumulationin TM. Objective: To analyze the reversibility of ECM TM in Wistar rat after discontinuation oftopical prednisolone1% at different periode(14 days and 28 days). Methods:This was an experimental study with post test only controlgroup design. Total samples of 28 rats were divided into 4 groups: Treatment groups 1 was treated with  topical prednisolone for 28 days, and was terminated 14 days after discontinuation of the drug.Treatment goups2 was treated topical prednisolone for 28 days and was terminated28 days after discontinuation of the drug. Control 1 was treated withtopical prednisolone for 28 days, and Control 2 was treated with topical saline for 28 days and terminated without periode of discontinuation. Histopathological grading score was used to evaluate ECM TM deposition. Mann-Whitney test and Kruskal-Wallis test were used to analyze the data. Result: Deposition of ECM in TM was not statistically different intreamentgroup 1 and treatment group 2 (p>0.05). Deposition of ECM in TM were statistically differentbetween treatment group 2 and control group 1 (p<0.05). Comparative test showed p<0.001,which means that there was a change in the  thickness of ECM after discontinuation of instillation. Conclusion: ECM TM was thinner in the experimental animals with a longer duration of topical prednisolondiscontinuation, which demonstrate that maintenance remodeling of ECM was happen.

Abstract

Introduction: to compare the levels of Placenta Growth Factor in the vitreous on patients with proliferative diabetic retinopathy at Aflibercept administration with control. Method: This is a prospective experimental study with post-test with control design. Subjects were taken from 24 eyes from 24 patients, who came to dr. Kariadi General Hospital and National Diponegoro Hospital between March until September 2016. Subjects were divided into the patient with Aflibercept administration and control. Study variables in this study are Placenta Growth Factor levels in the vitreous in the patient with diabetic proliferative retinopathy. Measurements of PlGF levels with ELISA methods are conducted at GAKI Laboratory, Medical Faculty, Diponegoro University. This data tested for normality used with Saphiro Wilk, and homogeneity variance with Lavene and test hypothesis used with Mann Whitney. Results: Sample of this study are 24 patients proliferative diabetic retinopathy, twelve are the control group, and twelve are Aflibercept group. Average levels of PlGF in the control group are 263,97 pg/ml + 354,98 SD, with minimum value 12,19 pg/ml and maximum value 1002,00 pg/ml. Average levels of PlGF in the treatment group are 92,84 pg/ml + 9,54 SD, with minimum value 76,45 pg/ml and maximum value 112,30 pg/ml. Hypothesis test results obtained significance value 0,488. Conclusion: There was no significant difference between PlGF levels in the control group and the treatment group.

Abstract

Background: The extracellular matrix (ECM) in the trabecular meshwork (TM) may play a role in the steroid-induced aqueous humor outflow resistance pathophysiology. Topical steroids such as fluorometholone can cause elevated intraocular pressure. Increased synthesis and accumulation of TM ECM proteins due to steroids result in TM dysfunction and elevated intraocular pressure.Purpose: This study aims to analyze the effect of duration of administration and discontinuation of fluorometholone eye drops on the thickness of the TM ECM of Wistar rats.Methods: This study was an experimental study with a posttest-only control group design. The total sample of 25 rats was divided into 5 groups: Treatment 1 was given topical fluorometholone for 4 weeks and stopped for 4 weeks. Treatment 2 was given fluorometholone for 6 weeks and stopped for 4 weeks. Control 1 was given topical fluorometholone for 4 weeks, Control 2 was given topical fluorometholone for 6 weeks, and the negative control was used as a baseline. TM ECM examination was assessed using histopathological score grading. This study used the Mann-Whitney test and the Kruskal-Wallis comparative hypothesis test.Results: The difference in ECM thickness was not statistically significant either between treatment group 1 and treatment group 2 (p=1.000) or control group 1 (p=0,1000). There was a significant change in the thickness of the TM ECM in treatment group 1, treatment 2, control 1, control 2 compared to the negative control (p=0.003, p=0.003, p=0.003, p=0.004).Conclusion: TM ECM thickness did not return to normal thickness after administration of fluorometholone eye drops one drop 4 times a day for 4 weeks and 6 weeks followed by discontinuation of the drug for 4 weeks. The thickness of the TM ECM was thicker in experimental rats with a longer duration of administration of fluorometholone eye drops.

Abstract

Background: Ethambutol-induced optic neuropathy (E-TON) is one of the most common optic neuropathies, especially in developing countries with an increasing rate of Tuberculosis (TB). Various outcomes of visual impairment that may lead to permanent visual loss makes it imperative to prescribe supplementation to prevent E-TON. Zinc supplementation is suggested as one of many to prevent E-TON given that ethambutol is a Zn chelator.Purpose: To analyze the apoptosis rate of retinal ganglion cell (RGC) in Wistar rats given ethambutol and Zinc supplementation.Methods: Experimental analysis research. True – experimental design and post-test only using Wistar rats. Subjects were divided into 2 groups, treatment grup was given Ethambutol and Zn and control group was given ethambutol without Zn for 30 days. Assessment of apoptosis rate was concluded by an anatomical pathologist with histopathological scoring using Hematoxylin Eosin staining. Differences between two grups were statistically analyzed using Mann Whitney U test, (Signifikan p <0,05).Results: From Distribution of rate apoptosis in treatment group was found 28.6% without apoptosis, 71.4% score 1 and no score > 1. There was a significantly different in apoptosis retinal ganglion cell between two grups (p= 0.015).Conclusion: The rate of retinal ganglion cell apoptosis was lower in rats treated ethambutol with Zn than without Zn.
