Abstract

One important reason why diabetic ulcer fails to heal is an increase of collagen degradation by matrix metalloproteinase-9 (MMP-9). Vibration therapy is one of complementary therapies which can reduce inflammation and improve reepithelialization. However, up to present, there is no study that investigate the effect of vibration on reduction of MMP-9 in diabetic ulcer. Therefore, the effect of vibration on reduction of collagen degradation in diabetic ulcer is still unknown. If vibration can reduce collagen degradation in diabetic ulcer, nurse can recommend patients to apply vibration not only during inflammation and proliferation phase, but also during maturation phase since MMP-9 also contributes to collagen degradation during maturation phase.The purpose of this study was to investigate the effect of vibration on reduction of MMP-9 in diabetic ulcer. Diabetic rats were divided into two groups, vibration and control groups. The intensity of polymorphonuclears cells (PMNs) and fibroblast, collagen appearance, and the presence of positive cells for MMP-9 were compared. Fibroblast, PMNs and collagen appearance were stained with H and E staining, and MMP-9 was stained by immunohistochemistry method. The difference in the intensity of fibroblast and PMNs was analyzed by Mann-Whitney U test. The result showed that the intensity of PMNs was significantly lower, and the intensity of fibroblast was higher in vibration group than in control. Collagen alignment in the vibration group was more organized in a more regular fashion than in control group. The intensity of positive cells for MMP-9 was lower in the vibration group than in control group. This study showed that vibration could reduce MMP-9, therefore it could be concluded that vibration could reduce collagen degradation of diabetic ulcer. This study bring implication in the clinical setting that vibration therapy should also be applied during maturation phase since MMP-9 is also released in the maturation phase.

Abstract

Pendahuluan:Banyak pasien Diabetes Mellitus (DM) yang tidak mendapatkan perawatan akibat dari kebijakan rumah sakit yang melakukan pengurangan kunjungan dan konsultasi untuk menghindari penularan Covid-19. ImplementasiTelehealth sudah banyak dilakukan.  Review ini bertujuan untuk mengetahui model implementasiTelehealthpada pasien Diabetes Melitus saat pandemi Covid-19. Metode:Pencarian artikel menggunakan databaseelektronik seperti Pubmed, Science direct, dan Google Scholar.Pencarian artikel yang digunakan yaitu 5 tahun terakhir. Kata kunci yang digunakan diantaranya, “Telehealth” AND “Diabetes Mellitus” AND “Pandemic Era” OR “Covid-19”. Hasil:Berdasarkan 6 artikel yang didapatkan bahwa Telehealthmerupakan implementasi yang dapat diterapkan pada masa Pandemi Covid-19 khususnya pada penderita Diabetes Melitus. Telehealth mampu menggantikan metode konsultasi tatap muka dengan mengurangi ketidakhadiran pasien (dari 21% menjadi 4%), dibandingkan sebelum pandemi, dan mampu memenuhi kebutuhan fisik maupun psikis. Selama pandemi Covid-19, estimasi waktu pelayanan kesehatan yang diberikan 75 menit berbeda saat sebelum pandemi (pasien membutuhkan waktu 175 menit. Simpulan:sistem Telehealth sebagai solusi terbaik untuk merawat pasien DM dalam mencegah tertularnya covid-19.

Abstract

Introduction: Negative Pressure Wound Therapy (NPWT) is considered as the best treatment for accelerating wound healing, including diabetic ulcer. However, this device is still not available in Indonesia. Therefore, the purpose of this study were to make NPWT by using modifi cation of breast pump, and to elucidate the effect of this device on acceleration of wound healing in diabetic ulcer.Methods: NPWT was made by using digital breast pump. Wounds were divided into two groups; wound-treated by NPWT (experimental group) and wound-untreated by NPWT (control). The negative pressure used for this study was 85 mmHg. Rats were sacrifi ced on day 7, and wound samples and surrounding skin were stained with Hematoxylin and Eosyn. Infl ammation, intensity of necrotic tissue, and wound closure were observed.Results: The device could deliver a negative pressure at a range of 85–140 mmHg. Wound size in the experimental group was smaller than in control group. Infl ammation was also less in the experimental group than control group.Conclusion: The modifi cation of breast pump could be used as a negative pressure therapy for wounds, and has effect on reducing infl ammation and necrotic tissue. Further study is needed to elucidate the effect of this device on human subject.

Abstract

Introduction: Diabetic ulcers accompanied by ischemia is difficult to treat. Such ulcers require therapy that can improve the blood flow. Previous studies have revealed that two therapies could improve blood flow and accelerate the healing of diabetic ulcers; vibration and electrical stimulation (ES). However, it is unknown which of these two therapies is best at accelerating wound healing in diabetic ulcers. The purpose of this study was to compare both therapies in relation to accelerating the wound healing of diabetic ulcers.Methods: This study was an experimental study involving diabetic rats. The rats were divided into two groups: vibration and ES. Vibration and ES were applied for 10 minutes per day for 7 days. Wound size, inflammation, intensity of fibroblast infiltration, area of necrosis and degree of re-epithelialisation were compared. The difference in wound size was analysed using an independent t-test, while the histological data were analysed using a Mann-Whitney U-test.Results: On day 5 onwards, there was a thin slough in the ES group which was not present in the vibration group. Day 4 onwards and the wound size was significantly smaller in the vibration group than in the ES group. The intensity of inflammation was significantly less, and the degree of fibroblast infiltration was significantly higher in the vibration group compared with the ES group. Re-epithelialisation was more advanced in the vibration group than the ES group.Conclusion: Our study revealed that wound healing in diabetic ulcers following vibration was better than after ES. We suggest that nurses should use vibration rather than ES in clinical settings.

Abstract

EMPOWERMENT OF ELDERLY POSYANDU CADRES FOR PREVENTING HYPERTENSION AND ITS COMPLICATION IN PURWOKERTO. The highest prevalence of degenerative diseases in Banyumas regency is hypertension. The prevention of hypertension in elderly could be done by Elderly Posyandu. However, there are many problems which occur in Elderly Posyandu of Soka Nirmala III and Sejahtera II in preventing hypertension and its complication, such as the limitation of materials for blood pressure measurement, lack of knowledge for hypertension management, lack of knowledge about self-treatment for reducing blood pressure. The purposes of this programme were to give kits for hypertension measurement to elderly posyandu, to conduct training of hypertension prevention, training about diet management, training about elderly exercise, training about self-treatment for reducing blood pressure. This community service was done through health education, training, and simulation. This community service could improve the knowledge of cadres and elderly in managing hypertension, improve knowledge in conducting exercise for reducing hypertension, improve knowledge in diet management, and improve the knowledge about self-treatment for reducing hypertension. It is expected that the prevalence of hypertension and its complication could be reduced and the quality of life of elderly could be improved.

Abstract

Abstract Hypertension is a state of increased blood pressure of more than 140/90 mmHg. Hypertension can be caused by individual anxiety and stress factors, and the state of hypertension can trigger anxiety in patients. Efforts that can be made to prevent and overcome anxiety in hypertension can be achieved with independent treatments or interventions such as Spiritual Emotional Freedom Tehnique (SEFT) spiritual therapy that is effective in maintaining blood pressure stability in the normal range. The purposed of this study was to determine of effectiveness health cadres and hypertension sufferers will increase their knowledge and skills on how to reduce anxiety through SEFT therapy. This program was carried out through health education through information transfer, discussion, demonstration, and ongoing assistance to cadres. Participants in this activity were 36 elderly Posyandu health cadres in Mersi Sub-District, Purwokerto Timur District. Knowledge scores were measured before and after education. The mean value of the pre test was known to be 52.78 while the mean value of the post test was 83.33. The results of paired t test analysis revealed that the value of p = 0,000 showed that there were significant differences between the scores of health cadres' knowledge scores on the understanding of SEFT therapy to reduce anxiety in patients with hypertension between before and after education. SEFT complementary therapy training can effectively improve the knowledge and skills of health cadres and hypertension sufferers in reducing anxiety.

Abstract

Latar Belakang : Hipertensi merupakan suatu keadaan terjadinya peningkatan tekanan darah lebih dari 140/ 90 mmHg. Hipertensi dapat disebabkan oleh faktor kecemasan dan stress individu, dan keadaan hipertensi dapat memicu terjadinya kecemasan pada penderita. Upaya yang dapat dilakukan untuk mencegah dan mengatasi kecemasan pada hipertensi dapat dicapai dengan pengobatan maupun intervensi mandiri seperti terapi spiritual Spiritual Emotional Freedom Tehnique (SEFT) yang efektif. SEFT telah banyak dibuktikan dalam menurunkan tingkat kecemasan, sehingga dapat diharapkan mampu menurunkan kecemasan pada pasien hipertensi dan menjaga kestabilan tekanan darah dalam rentang normal. Tujuan: Diharapkan para kader kesehatan dan penderita hipertensi mengalami peningkatan pengetahuan dan keterampilan tentang cara menurunkan kecemasan melalui terapi SEFT. Metode: Program ini dilakukan melalui pendidikan kesehatan melalui transfer informasi, diskusi, demonstrasi, dan pendampingan secara berkelanjutan kepada para kader. Partisipan kegiatan ini adalah adalah 36 orang kader kesehatan posyandu lansia di Kelurahan Mersi Kecamatan Purwokerto Timur. Hasil: Skor pengetahuan di ukur sebelum dan sesudah pemberian edukasi. Nilai rata- rata pre test diketahui 52,78 sedangkan nilai rata- rata post test 83.33. Hasil uji analisis paired t test diketahui nilai p=0,000 yang menunjukkan ada perbedaan yang bermakna antara nilai skor pengetahuan kader kesehatan terhadap pemahaman tentang terapi SEFT untuk menurunkan kecemasan pada penderita hipertensi antara sebelum dan sesudah dilakukan edukasi. Kesimpulan: Pelatihan terapi komplementer SEFT efektif dapat meningkatkan pengetahuan dan keterampilan kader kesehatan serta para penderita hipertensi dalam menurunkan kecemasan.

Abstract

Kidney disease is a disease that is feared by many people because the kidneys fail to function to remove metabolic waste. There are 15 health cadres in Sokanegara sub-district, some of which are new cadres who are being trained to replace the old cadres. Through this science and technology application program, it is hoped that health cadres will gain knowledge and skills about early detection and efforts to prevent kidney failure. This science and technology application program is carried out through needs-based health education through extension methods, discussions, and demonstrations on preventing kidney damage. Extension activities increase the knowledge of health posyandu cadres, the knowledge score is measured before and after the provision of education, the average pre-test score for the knowledge of health cadres obtained is 63.3, while the average post-test score for the knowledge of health cadres is 82. The results of the paired t test analysis showed that the value (p = 0.00). The results of the analysis indicate that there is a significant relationship or difference between the scores of knowledge scores of health cadres in Sokanegara sub-district, East Purwokerto sub-district on the understanding of early detection and prevention of kidney failure before and after education. The PKM program for implementing science and technology has been running smoothly and has produced services in the form of providing counseling by experts in the health sector on early detection and prevention of kidney failure, modules, and published texts.

Abstract

Penanganan pandemi covid-19 ini, salah satunya adalah penerapan protokol kesehatan seperti penggunaan masker, cuci tangan dengan sabun dan jaga jarak. Protokol kesehatan ini diterapkan di semua lini masyarakat, termasuk pula di pondok pesantren. Pola kehidupan pondok pesantren yang sifatnya komunal dan berkerumun, memiliki potensi dalam penularan Covid-19. Melemahnya penerapan protokol kesehatan Covid-19 di Pondok Pesantren dapat meningkatkan potensi penyebaran Covid-19, seperti penggunaan masker yang tidak benar, tidak menjaga jarak, tidak cuci tangan dengan sabun, dan lainnya. Untuk itu perlu dilakukan upaya untuk mengedukasi santri supaya tetap patuh pada protokol kesehatan Covid-19. Sebagai bentuk salah satu tri dharma perguruan tinggi, dilakukan kegiatan pengabdian kepada masyarakat seperti melatih kader santri tanggap Covid-19 dan memasang media visual terkait upaya pencegahan Covid-19 di lingkungan pondok pesantren. Sebagai target / tujuan dari kegiatan ini adalah santri di Pesantren Baitul Quran, Desa Karangsalam Kidul, Kecamatan Kedungbanteng, Kab. Banyumas. Kegiatan pengabdian ini sesuai dengan visi UNSOED, “diakui dunia sebagai pusat pengembangan sumber daya perdesaan dan kearifan lokal”, dimana pesantren juga merupakan salah satu wujud kearifan lokal. Metode pelatihan kader santri ini dengan diskusi dan role-play. Hasil menunjukkan terjadi peningkatan pengetahuan dan ketrampilan dari kader santri tentang covid-19. Lebih lanjut, tim pengabdi juga menghasilkan video dan poster tentang covid-19.

Abstract

Background. Incidence of pressure injury (PI) in Indonesia is high. Therefore, Indonesian nurses should take measures to prevent PI from developing. Considering the high incidence of PI in Indonesia, there could be some barriers faced by Indonesian nurses in performing prevention of PI. However, to date, no study has assessed the perceived barriers and factors associated with perceived barrier of prevention of PI in Indonesian nurses. Therefore, this study aimed to assess perceived barriers to prevention of PI and associated factors in Indonesian nurses. Methods. A total of 521 nurses participated in the study. A pressure injury prevention barriers questionnaire was used to assess perceived barriers of prevention of PI. A multivariate logistic regression model was used to analyse factors associated with the perceived barriers. Results. The two most commonly perceived barriers in nurses were lack of preventive devices such as special mattresses, cushions, and skin care products, and a lack of training courses related to prevention of PI. Associated factors emerging from the results were working experience (AOR =1.74) and working unit (AOR =2.73).
