Abstract

Children with cerebral palsy are considered a population at risk for sleep disturbance. Various factors can cause sleep disorders in children with cerebral palsy. This study investigates the relationship between endogenous factors and sleep disorders in children with cerebral palsy. It was a cross-sectional analytical study using randomized sampling on children with cerebral palsy who met the inclusion criteria for the period of May–August 2017. The location of the study was special schools in the Bandung area, Indonesia. All participants were screened with the Sleep Disturbances Scale for Children (SDSC) questionnaire to determine the prevalence of period of sleep disorders. Data analysis was then performed using the unpaired t test to compare the characteristics of two variables with a p value≤0.05 considered statistically significant. Sixty-six subjects aged 8–14 years were recruited. The results showed that the prevalence of sleep disorders was 67% (32 children), with insomnia as the most common type of sleep disorder (39%). There was a significant association between motor disabilities type and sleep disorders (p≤0.05). The most common type of sleep disorder in children with cerebral palsy is insomnia. In conclusion, there is a relationship between motor disability type and sleep disorders in cerebral palsy children.

Abstract

Vitamin B9 (folic acid) and B12 (cobalamin) are essential vitamins that play roles in the process of hematopoiesis and maintaining the function of peripheral nerves. Therefore, these deficiencies may create a risk for peripheral neuropathy in beta-thalassemia major patients. The purpose of this study is to determine the relationship between vitamin B9 level, vitamin B12 level, and peripheral neuropathy in beta-thalassemia major children. It was an observational analytical study with a case-control design has been conducted at Dr. Hasan Sadikin General Hospital Bandung, Indonesia, in May–July 2019. There were 47 beta-thalassemia major children with peripheral neuropathy (case) and 41 healthy children (control). All subjects completed a general demographic questionnaire, underwent neurological examination, and were tested for vitamin B9 and B12 serum levels. Data were then analyzed using the unpaired t test to compare the vitamin levels between both groups and Spearman’s rank correlation test to investigate the correlation between vitamin levels and the number of affected nerves in the case group. Comparison of folic acid levels in the case group (21.52±6.22 ng/mL) and the control group (23.81±7.51 ng/mL) showed no significant difference (p=0.19). In contrast, cobalamin in the case group (288.57±168.61 ng/mL) and the control group (385.95±197.48 ng/mL) showed a significant difference (p=0.01). In addition, there was a moderate correlation (p=0.004, r=0.41) between folic acid level and the number of motoric nerves affected in the case group. In conclusion, cobalamin level correlates with peripheral neuropathy in beta-thalassemia major patients, and folic acid level correlates with the number of affected nerves, especially motoric nerves. HUBUNGAN ANTARA VITAMIN B9 (ASAM FOLAT), VITAMIN B12 (KOBALAMIN), DAN NEUROPATI PERIFER PADA ANAK DENGAN TALASEMIA BETA MAYORVitamin B9 (asam folat) dan B12 (kobalamin) merupakan vitamin esensial yang berperan dalam proses hematopoesis dan menjaga fungsi saraf tepi. Defisiensi vitamin ini dapat menimbulkan risiko neuropati perifer pada pasien talasemia beta mayor. Tujuan penelitian ini mengetahui hubungan antara kadar vitamin B9, vitamin B12, dan neuropati perifer pada anak talasemia beta mayor. Metode penelitian ini adalah analitik observasional dengan rancangan studi kasus kontrol yang dilakukan di RSUP Dr. Hasan Sadikin Bandung, Indonesia pada Mei–Juli 2019. Terdapat 47 anak talasemia beta mayor dengan neuropati perifer (kelompok kasus) dan 41 anak sehat (kelompok kontrol). Seluruh subjek penelitian mengisi kuesioner demografi umum, menjalani pemeriksaan fisis neurologis, serta dilakukan tes kadar vitamin B9 dan B12 serum. Uji t test tidak berpasangan digunakan untuk membandingkan kadar vitamin pada kedua kelompok dan uji korelasi Spearman untuk membandingkan kadar kedua vitamin tersebut dengan jumlah saraf yang terkena pada kelompok kasus. Perbandingan kadar asam folat kelompok kasus (21,52±6,22 ng/mL) dengan kelompok kontrol (23,81±7,51 ng/mL) menunjukkan perbedaan yang tidak bermakna (p=0,19), sedangkan perbandingan kadar kobalamin kelompok kasus (288,57±168,61 ng/mL) dengan kelompok kontrol (385,95±197,48 ng/mL) menunjukkan perbedaan yang bermakna (p=0,01). Selain itu, terdapat korelasi sedang (p=0,004; r=0,41) antara kadar asam folat dan jumlah saraf motorik yang terkena pada kelompok kasus. Kesimpulan, kadar kobalamin berhubungan dengan neuropati perifer pada penderita talasemia beta mayor dan kadar asam folat berhubungan dengan jumlah saraf yang terkena, terutama saraf motorik.

Abstract

Al-Quran sound as auditory stimulation may influence the development of systems related to memory. This study aimed to investigate the effect of Al-Quran acoustic stimulation to novel object recognition (NOR) memory and amount of hippocampal formation cells at postnatal day (PND) 21 Wistar rats. This study was conducted in September 2016 to January 2017. Adult Wistar rats divided into intervention and control groups were bred at Faculty of Medicine Universitas Islam Bandung. Rat pups in the intervention group were exposed to Quranic sound from postcoital day 0 to the PND 20. Rats of PND 20 was involved in the NOR test by documenting the value of the duration of exploration of the familiar and novel object. The rats’ brains were extracted and processed at Faculty of Medicine Universitas Padjadjaran for cell counting of hippocampal formation stained with hematoxylin-eosin. The results displayed a higher value of D1 (exploration time difference) and a total number of hippocampal formation cells in the Al-Quran groups. These results can be related to the role of the Quranic voice in suggesting higher learning aspects, activating neurogenesis or cell survival transcription factors. However, there was no difference in discrimination index (DI) value between groups which could be indicating inadequate habituation period, interval, testing age, or stress factors. Numerous limitations from this field of research suggest that the biological role of sound stimulation is still in its early stages of development. In conclusion, exposure to perinatal Al-Quran sound may serve as stimulation which enhances learning, memory, neurogenesis or cell survival of hippocampal formation. SUARA AL-QURAN PERINATAL PADA MEMORI NOVEL OBJECT RECOGNITION DAN JUMLAH SEL HIPOKAMPUSSuara Al-Quran sebagai stimulasi pendengaran diperkirakan dapat memengaruhi perkembangan sistem tubuh terkait memori. Penelitian ini bertujuan mengamati pengaruh stimulasi suara Al-Quran terhadap memori novel object recognition (NOR) dan jumlah sel formasi hipokampus pada tikus Wistar 21 hari setelah lahir (postnatal day/PND). Penelitian ini dilaksanakan pada September 2016 sampai Januari 2017. Tikus Wistar dewasa yang dibagi dalam kelompok perlakuan dan kontrol dibiakkan di Fakultas Kedokteran Universitas Islam Bandung. Anak tikus Wistar dipaparkan dengan suara Al-Quran sejak hari postcoital 0 sampai anak tikus lahir dan berumur 20 hari setelah lahir (PND 20). Tikus PND 20 dilibatkan dalam tes NOR dengan dokumentasi nilai durasi eksplorasi objek lama dan baru. Otak tikus diproses di Fakultas Kedokteran Universitas Padjadjaran untuk penghitungan jumlah sel formasi hipokampus dengan pewarnaan hematoxylin-eosin. Hasil penelitian ini menunjukkan nilai D1 (perbedaan durasi eksplorasi objek lama–baru) dan jumlah sel formasi hipokampus lebih tinggi pada kelompok perlakuan dengan Al-Quran. Hasil tersebut dapat terkait dengan peran suara Al-Quran dalam aktivasi faktor pertumbuhan atau transkripsi. Namun, tidak terdapat perbedaan bermakna pada nilai indeks diskriminasi antarkelompok yang dapat terkait dengan faktor stres atau kurangnya periode habituasi atau periode uji. Berbagai keterbatasan penelitian ini serta riset di bidang stimulasi embriologi mengindikasikan bahwa peran biologis suara Al-Quran masih harus diteliti lebih lanjut. Simpulan, paparan suara Al-Quran perinatal dapat berlaku sebagai stimulasi yang meningkatkan pembelajaran, memori, neurogenesis atau ketahanan sel formasi hipokampus.

Abstract

Patients with transfusion-dependent thalassemia (TDT) may experience an increase in ferritin due to shorter erythrocyte lifespan and lysis, as well as side effects of transfusion. Increasing ferritin can cause various complications, including pain, which can develop into chronic pain and interfere with life quality. This study aims to determine the relationship between pain and serum ferritin levels in adults with TDT. This study was an analytical observational study using a cross-sectional design on adult TDT patients with pain who came to the Hemato-Oncology Clinic of Dr. Hasan Sadikin General Hospital Bandung. This research was conducted from March to June 2021. All subjects were assisted to fill out the Indonesian version of the Brief Pain Inventory Short Form (BPI-SF) questionnaire before transfusion. Ferritin levels in the last three months were obtained from medical records. If more than three months, serum ferritin levels were examined. Ferritin levels and BPI-SF scores were then correlated using the Pearson test. The study was conducted on 51 adult TDT patients with pain, and the average value of ferritin levels in research subjects is 5081±2929 g/L. There was a relationship between pain (the dimensions of pain interfere with life on the BPI-SF score) and an increase in ferritin levels (p=0.042, r=0.29). The results showed there is a relationship between pain and serum ferritin levels. Regular consumption of iron chelation tablets can reduce ferritin levels and improve the quality of life for adults with TDT. HUBUNGAN NYERI DENGAN KADAR FERITIN SERUM PADA PENYANDANG TRANSFUSION-DEPENDENT THALASSEMIA DEWASAPenyandang transfusion-dependent thalassemia (TDT) dapat mengalami peningkatan feritin akibat umur eritrosit yang lebih pendek dan mudah lisis, serta efek samping terhadap pemberian transfusi. Peningkatan feritin tersebut dapat menyebabkan berbagai komplikasi di antaranya nyeri yang dapat berkembang menjadi nyeri kronik dan mengganggu kualitas hidup. Penelitian ini bertujuan mengetahui hubungan nyeri dengan kadar feritin serum pada penyandang TDT dewasa. Penelitian dengan observasional analitik menggunakan rancangan potong lintang pada penyandang TDT dewasa dengan nyeri yang datang ke Klinik Hemato-Onkologi RSUP Dr. Hasan Sadikin Bandung. Penelitian dilakukan dari Maret hingga Juni 2021. Seluruh subjek dilakukan pendampingan untuk mengisi kuesioner Brief Pain Inventory Short Form (BPI-SF) versi Indonesia sebelum transfusi. Kadar feritin dalam tiga bulan terakhir didapatkan dari rekam medis dan bila lebih dari tiga bulan dilakukan pemeriksaan kadar feritin serum. Kadar feritin dan skor BPI-SF kemudian dikorelasikan menggunakan Uji Pearson. Penelitian dilakukan kepada 51 penyandang TDT dewasa dengan gejala nyeri dan didapatkan nilai rerata kadar feritin pada subjek penelitian adalah 5081±2929 μg/L. Hasil penelitian memperlihatkan terdapat hubungan antara nyeri (dimensi efek nyeri terhadap kehidupan) dan peningkatan kadar feritin (p=0,042; r=0,29). Hasil penelitian menunjukkan terdapat hubungan antara nyeri dan kadar feritin serum. Konsumsi tablet kelasi besi secara rutin dapat menurunkan kadar feritin dan memperbaiki kualitas hidup penyandang TDT dewasa.
