Abstract

Breast cancer is the main cause of women death after cervical cancer. In 2015, the Ministry of Health called for a socialization regarding the importance of early detection of breast cancer to reduce mortality. The 2014, Health Profile Report of Kabupaten Lamongan stated that there were 53 of 3,712 people indicated by breast tumor from 3,712 people in the age range of 30-50 year old. The high prevalence of breast cancer each year makes this issue need to be considered. Moreover, there is still a lack of supporting equipment for breast cancer screening in Kabupaten Lamongan. Early detection of breast cancer with the BSE method (Breast Self-Examination) needs to be socialized to the wider community because it is quite effective in increasing awareness and knowledge regarding early breast cancer examination. From this Community Service Program, which is to carry out counseling on early detection of breast cancer using a tutorial method to health surveyors regarding BSE and examination using ultrasonography (USG). It adopted action research steps which consist of 4 (four) stages, following are planning, action, observation and evaluation, and reflection. This program showed that there werethe simple cyst in 15 of 35 participants and no solid mass revealed. There was also one participant who had underwent Fibroid Mass of breast surgery. The target outcome of the activity was exceeded with an increase in knowledge about early detection of breast cancer, 83% (very good categories). The tutorial method for health surveyors regarding BSE and examination using ultrasonography successfully improved the knowledge of early detection.

Abstract

Background : Ultrasonography (US) is the cheaper and non invasive modality to determine Achilles tendon. Prone position is the standart position of Achilles tendon US. However, it is a discomfort for an uncooperative patient and a difficult technique too. The erect position is an alternative technique of Achilles tendon US. The goal of this study is to compare the erection as an alternative position with prone as a standart position.Material and Methode: The patient who had an injury or any inflamation process of Achilles tendon were excluded. The patient underwent two positions of Achilles tendon US, 900 and dorsoflexi. Longitudinal axis measured tendon thickness and a transversal axis which covered a cross-sectional area of the tendon.Result: From all the 21 patients coming, 13 patients were males (61,9%), and eight patients were females (38,1%). The mean of tendon thickness and cross-sectional area in 900 prone positions were 4,24±0,24 mm, 30,08±2,86 mm, respectively. The mean of tendon thickness and cross-sectional area in 900 erect positions were 4,27±0,23 mm, 31,36±2,19 mm, respectively. There was no anisotropy effect during longitudinal axis examination. Conclusion: We found that there were no significant differences between a prone and erect position (p<0.05). The erect postion could be an alternative position, uncooperative patient in particular, without reducing the diagnostic value. Keywords: Achilles tendon the US, erect position, prone position, tendon thickness, cross-sectional area

Abstract

Background: The use of MRI as a non-invasive diagnostic radiological examination modality is painless. However, patients may experience claustrophobia and anxiety. Sensory intervention is a method of providing sensory stimulation to an individual, which is expected to reduce the anxiety. Purpose: To determine the effect of the sensory intervention on the anxiety level of patients using an intervention scent by inhalation and intervention sound based on heart rate and MRI-AQ in patients with MRI lumbal examination. Method: 24 samples were divided into three groups: intervention scent, combination intervention scent-sound, and control, with eight samples each. Intervention scent is given using lavender essential oil as aromatherapy, drop three drops on the mask, then put the mask on for 5 minutes before the examination until the examination is completed. Intervention sound is given by playing music (nature-sound) during the examination. Result: Sensory intervention, specifically the use of combination intervention scent-sound, significantly reduces anxiety in patients during MRI lumbal examination significantly p = 0.004 (p<0.05). Conclusion: The use of a combination between lavender and nature-sound as media for sensory intervention has a positive effect on reducing anxiety during MRI lumbal examination based on heart rate. Due to the easy administration of this aromatherapy and the minimal risk for this treatment, it is recommended that it is applied to reduce anxiety during MRI.

Abstract

Background: The description of Degenerative Disc Disease in MRI lumbar FSE sequence T2WI is seen as a decrease in signal intensity. Patients with cases of Degenerative Disc Disease experience severe low back pain and cannot lie supine for a long time, while MRI is very sensitive to movement. GRAPPA is a parallel imaging technique that can produce images with a fast scan time but is followed by a decreased Signal to Noise Ratio SNR value. This technique needs to be followed by setting other parameters to produce an optimal image, namely by applying Magnetization Transfer Contrast (MTC). Purpose: To compare the quality of image results on lumbar MRI examination of the sagittal T2 qTSE sequence in the case of Degenerative Disc Disease with and without MTC activation. Method: This research was conducted at the dr. Soedono Madiun from August to September 2020. A sample of 16 patients who met the inclusion criteria was taken during the study. The GRAPPA and GRAPPA+MTC technique imagery results on each sample were assessed for the image quality quantitatively based on the SNR and CNR values. Result: Based on the SNR value, the GRAPPA technique and MTC activation have a higher mean than the GRAPPA technique alone. Likewise, with the CNR value, the GRAPPA technique and MTC activation have a higher average than the GRAPPA technique alone. Conclusion: The GRAPPA technique and MTC activation can be applied in Lumbar MRI examination with cases of Degenerative Disc Disease, especially in uncooperative patients.
