Abstract

Coronavirus Disease 2019 atau COVID-19 pada akhir tahun 2019 mulai menjadi sorotan masalah kesehatan dunia. Penularan virus ini terjadi dengan sangat cepat melalui droplet dan kontak erat antar manusia hingga menyebabkan tingginya kasus COVID-19 secara global. Kelompok orang yang memiliki komorbid (DM tipe 2, hipertensi, hiperurisemia, penyakit kardiovaskular, asma, PPOK, TB, Hepatitis, dan gagal ginjal kronik) diketahui rentan terinfeksi COVID-19 dan dapat memperberat derajat keparahan penyakit serta memperpanjang durasi perawatan. Penelitian ini bertujuan untuk mengetahui hubungan komorbid dengan durasi perawatan pasien COVID-19 pada dua rumah sakit rujukan COVID-19 di Kota Ambon tahun 2020. Manfaat penelitian ini dapat memberikan gambaran kepada tenaga kesehatan mengenai hubungan komorbid dengan durasi perawatan pasien COVID-19. Penelitian ini merupakan penelitian analitik observasional dengan metode cross sectional dan sampel diambil menggunakan teknik total sampling. Data yang diperoleh dianalisis menggunakan Statistical Package for the Social Science (SPSS). Hasil penelitian menunjukkan bahwa dari 109 sampel, sebanyak 61 orang (56%) memiliki komorbid dan sebanyak 82 orang (75,2%) memiliki durasi perawatan lebih dari 14 hari. Hasil analisis bivariat menggunakan uji chi-square didapatkan nilai p = 0,000 yang berarti terdapat hubungan yang signifikan antara komorbid dengan durasi perawatan pasien COVID-19. Abstract

Pulmonary tuberculosis (TB) is an infection disease caused by Mycobacterium tuberculosis. Pulmonary TB remain a major health problem and the most common cause of death in the world especially in developing countries. Pulmonary TB infection could cause clinical manifestation, which is haematology disorders like anemia. Many studies has reported anemia as a common complication in patients with pulmonary TB. This research aims to determine haemoglobin levels and erythrocytes index of patients with pulmonary TB at RSUD Dr. M. Haulussy Ambon in the periods from January 2017 - April 2018. This research uses a descriptive study by using medical records of patients with pulmonary TB. Among 65 patients with pulmonary TB, number of patients with anemia are 44 cases (67,7%) and 21 cases (32,3%) are not anemia. Pulmonary TB with anemia most suffered by male as much 24 cases (72,7%). The age group suffered most is 18 - 30 years old as much 23 cases (69,7%), but the age group suffered most according to percentage is 51 - 60 years old as much 80%. The most common types of anemia is hipocromic micrositer as much 23 cases (52,3%). Haemoglobin levels in patients with pulmonary TB are found most below the normal value or anemia and the most common types of anemia is hipocromic micrositer. Needs to be done more research on the analysis of the relation between anemia with pulmonary TB.

Abstract

Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis. The last few decades emerged another problem related to tuberculosis namely MDR TB (Tuberculosis with multidrug-resistant) which is very widespread. MDR TB treatment takes 2 years and with drugs that are 100 times more expensive when compared to first-line treatment. The purpose of this research was to determine the characteristics of multidrug-resistant pulmonary tuberculosis patients (MDR TB) in Dr. M. Haulussy Ambon in 2014 - 2018. This research is an observational descriptive research with cross sectional approach. The sampling technique is to use the total sampling method. The sample in this research amounted to 113 people. MDR TB patient medical record data obtained were then analyzed using Microsoft Excel. The results of the research showed that in RSUD Dr. M. Haulussy Ambon, MDR TB sufferers were mostly found in adulthood 26 - 45 years, namely 61 cases (54%), MDR TB patients were male, 69 cases (61%), type of patient relapsed 67 cases (59% ), 39 cases (34.51%) with duration of treatment 0 - 5, rifampicin-resistant patients were 79 cases (69.9%), patients died as many as 32 cases (28.3%) and there were 8 cases (7.1 %) MDR TB p atients are HIV positive.

Abstract

Glucocorticoid represent a kind of stress hormone that suppressed the normal immune response by blockade T-Helper 1 cell program that has function to produce gamma interferon. Glucocorticoid could give some impact like losing weight, immunodeficiency, and change the differentiation of leukocyte. This fruit of nutmeg (Myristica fragrans Houtt) is a traditional plant in Maluku and contains secondary metabolite compounds that can induce the secretion of immune system (immunomodulatory). This research has a target to discover the number of mice leukocyte and its various cell that will be induced by stress itself and will be given ethanol extract of nutmeg seed. This research considers as pure experimental with posttest only control group design. 30 mice divided into 5 different groups, which are normal control (KN), negative control (stress), positive control (stress+0.2 Alprazolam), ethanol extract of nutmeg seed 4% (P1) and 16% (P2). Group control of K-, K+, P1, P2 were stress inducted using FST method for 6 minutes, and then give orally all the treatment for 7 days long and at the 8th days the blood collected intracardially. The counting process held as manual observation. The data analyzed by using ANOVA. The result of this research shows that P1 on the experiment could decrease the presentation of neutrophil, increase the presentation of monocyte and lymphocyte, meanwhile on P2 could decrease the presentation of neutrophil and monocyte, increase the presentation of lymphocyte and has no effect to the percentation of eosinophil and basophil. We can conclude that the giving of ethanol extract of nutmeg seed didn’t give a specific effect to the average mount of neutrophil, monocyte, lymphocyte, eosinophil, and basophil (p>0.05).

Abstract

Everyone has experienced feelings of stress or stress. Stress can arise from a variety of sources, including demands. Work demands can be a potential source of stress. Potential sources of stress trigger the onset of stress related to performance and psychological events of the employee. This research aims to determine whether there is an effect of stress on the performance of members of the police at the Buru island. This research is a correlative analytic study with cross sectional approach. The research subject is 85 people who were selected using simple random sampling. The analysis used in this study was the chi square test, with the variables studied were stress and the performance of the police. The results of the research showed that the stress level of the police was most in the category of frequent stress by 54.1% and the category of stress rarely experienced as much as 45.9%, while the performance of police with the largest percentage was in the quite good category of 49.4%, followed good performance by 28.2%, and poor performance by 22.4%. The results of this research indicate that there is an influence between stress on the performance of members of the police in the Buru island (p = 0.014).

Abstract

Dengue infection is an infection caused by the dengue virus and is transmitted through the bite of aedes aegypti female mosquito. Platelets and hematocrit are important parameters in the treatment of dengue infection patients. Proper diagnosis of the stage and condition of sufferers of dengue infection is important to determine the prognosis. Platelet and hematocrit examinations for each degree of dengue infection clinic are expected to help in grouping and managing patients based on their clinical degree. This research aims to see the relationship between platelet and hematocrit examination results with the clinical degree of DHF based on WHO criteria. The purpose of this study was to determine the relationship of hematocrit and platelet counts with the severity of dengue infection patients in RSUD Dr. M. Haulussy ambon period 2019. This research is an analytic study using secondary data. Data collection using total sampling techniques in all patients with dengue infection period 2019. Samples in this study were 92 people. Hypothesis testing uses bivariate analysis with Spearman's nonparametric hypothesis, testing with SPSS software. The results found 51 women (55.4%) more than 41 men (44.6%). The results of the analysis by the Spearman correlation test on a combined sample and after being separated for a female sample obtained platelets associated with the clinical degree of dengue infection. Hematocrit associated with the clinical degree of dengue infection. In a male sample, platelets were found to be unrelated to the clinical degree of dengue infection. Hematocrit is not related to the clinical degree of dengue infection

Abstract

Stress is a non-specific response in the body to conditions or conditions that occur as a result of exposure to stressors. High stress usually has a negative impact on the body, one of which is increasing blood sugar levels. Brown algae is one of the traditional marine plants and contains secondary metabolite compounds such as alkaloids, flavonoids, and steroids which are antidepressants so that they can prevent the increase in blood sugar levels. This study aims to determine the effect of acute stress on blood sugar levels of mice by giving ethanol extract of brown algae. This research is a true experimental research design with pre test and post test control group design. 30 mice were divided into normal control (KN), negative control (K-), positive control (K +) groups, ethanol extract of brown algae with a concentration of 2.5% (P1) and 5% (P2). The study was started by measuring the initial blood sugar levels in all groups, then groups K-, K +, P1, and P2 were given stress treatment with the FST method for 6 minutes for 7 days. Group K + was given alprazolam at a dose of 0.2 ml at 1 hour before treatment, while P1 and P2 were given brown algae at 30 minutes before treatment. After the treatment, it was followed by measurement of the final blood sugar level. The data from the observations were analyzed using the dependent T-test with a confidence level of 95%. The results showed that the post-test blood sugar levels in the P1 and P2 groups experienced an increase, but the increase was not significant. The results also showed that the significance values (2-tailed) in the P1 and P2 groups were 0.772 and 0.392 (sig> 0.05), meaning that it could be concluded that there was no effect of giving ethanol extract of brown algae on the prevention of increasing blood sugar levels after acute stress.

Abstract

Glucocorticoids are stress hormones that suppress normal immune responses by blocking Th1 cells. Glucocorticoids have an impact on decreasing the body's immune system and changes in leukocyte differentiation. Sargassum sp. is one type of brown algae that lives in the sea. Sargassum sp. contains metabolite compounds that are immunomodulatory. This study aims to determine the number of types of leukocytes in mice that are induced by stress and given ethanol extract of Sargassum sp. Pure experimental research design with post test only control group design. Thirty mice were divided into five treatment groups, namely the normal control group (KN), negative control (K-), positive control (K +), ethanol extract of Sargassum sp. 35% (P1) and 70% (P2). At K-, K +, P1 and P2 were given stress treatment with the FST (Forced Swim Test) method to see immobility time in mice. The treatment was given for seven days and on the 8th day the intracardial mice were taken. Observation of leukocytes from the blood smear of mice was carried out using a microscope with 1000x magnification. Observation data were analyzed by ANOVA. The results showed that the P1 group could reduce the percentage of neutrophils and monocytes but increase the percentage of lymphocytes, while the P2 group showed a decrease in the percentage of neutrophils, an increase in the percentage of lymphocytes, but could not decrease the percentage of monocytes compared to P1. The mean percentage of eosinophils and basophils did not change significantly, both in the treatment group and in the control group. However, statistical data shows no significant change between groups. So it can be concluded that stress treatment given to mice does not have a significant effect on the average percentage of monocytes, lymphocytes, neutrophils, eosinophils and basophils after being induced by stress.

Abstract

Hiperglikemik adalah suatu kondisi medik dimana terjadi peningkatan kadar glukosa darah yang melebihi batas normal. Hiperglikemik yang berlangsung kronik mengakibatkan glukotoksisitas pada sel-sel β pankreas yang menyebabkan disfungsi dan perubahan massa sel β pankreas, sehingga terjadi penurunan sekresi insulin. Diabetes melitus merupakan salah satu penyakit metabolik dengan tanda khas hiperglikemik yang terjadi akibat gangguan sekresi, kerja insulin atau keduanya. Tanaman kelor (Moringa Oleifera) merupakan tanaman berkayu lunak yang digunakan sebagai bahan pengobatan tradisonal dan mudah tumbuh pada daerah tropis, daun tanaman kelor mengandung senyawa metabolit yang mampu menurunkan kadar gula darah. Penelitian ini bertujuan untuk mengetahui efek dari ekstrak daun tanaman kelor (Moringa oleifera) terhadap kadar gula darah mencit. Penelitian ini merupakan penelitian eksperimental dengan desain post test only group design. Mencit sebanyak 25 ekor di bagi menjadi lima kelompok perlakuan yaitu kontrol normal (KN), kontrol negatif (K-), kontrol positif (K+), ekstrak daun kelor konsentrasi 20% (P1), dan konsentrasi 40% (P2) diberi induksi streptozotocin selama 3 hari. Kemudian, pada kelompok K+ diberi obat metformin dosis 0,2 ml, P1 diberi ekstrak daun kelor konsentrasi 20% dan P2 diberi daun kelor konsentrasi 40% dengan dosis 0,2 ml, dengan pemberian dilakukan selama 7 hari. Data hasil pengamatan dianalisis menggunakan SPSS dengan uji ANOVA. Hasil penelitian terlihat bahwa pemberian ekstrak daun kelor berpengaruh signifikan dalam menurunkan kadar gula darah mencit (p>0,05). Rata-rata penurunan kadar gula darah mencit pada kelompok P1 sebesar 115,80 mg/dl dan P2 sebesar 109 mg/dl. Penurunan kadar gula darah disebabkan karena adanya senyawa bioaktif yang diduga memiliki efek antihiperglikmik.

Abstract

Penyakit ginjal kronik (PGK) adalah suatu proses patofisiologi dengan etiologi beragam selama lebih dari 3 bulan yang mengakibatkan penurunan fungsi ginjal yang progresif, dan pada umumnya berakhir dengan gagal ginjal. Pada pasien PGK ekskresi asam urat menurun seiring dengan memburuk fungsi ginjal. Penelitian ini bertujuan untuk mengetahui hubungan nilai estimasi laju filtrasi glomerulus (eLFG) dengan kadar asam urat serum pada pasien PGK non dialisis di RSUD Dr. M. Haulussy, Ambon. Penelitian ini adalah penelitian analitik dengan desain cross sectional. Terdapat 64 sampel yang memenuhi kriteria inklusi. Sampel diambil dari data rekam medis pasien PGK non dialisis di RSUD Dr. M. Haulussy Ambon dari Januari 2019 sampai Mei 2020. Data di analisis dengan menggunakan uji Spearmen. Hasil penelitian menunjukan bahwa jenis kelamin penderita PGK non dialisis yang paling banyak adalah laki-laki (60,9%). Kelompok usia mayoritas adalah 56-65 tahun yaitu sebanyak 27 orang (42,2%). Pada penelitian ini di dapatkan distribusi kadar asam urat serum berdasarkan eLFGCKD-EPI yang paling banyak mengalami hiperurisemia berada pada derajat V (<15 ml/min/1,73m2) dengan total 44 sampel (68,8%) dan nilai median kadar asam urat yaitu 5,8 mg/dl. Hasil analisis menunjukan adanya yang bermakna antara nilai estimasi laju filtrasi glomerulus dengan asam urat pada pasien PGK non dialisis (p=<0,001) dengan arah korelasi negatif dan kekuatan korelasi kuat (r=-0,750). Jadi, dapat disimpulkan adanya hubungan nilai estimasi laju filtrasi glomerulus dengan kadar asam urat serum pada pasien penyakit ginjal kronik non dialisis di RSUD Dr. M. Haulussy Ambon dari Januari 2019 sampai mei 2020.
