Abstract

ABSTRACT Background: Diabetic ulcer is a major complication of diabetes and is the main cause of infection. One of the efforts or interventions to reduce infection by doing wound cleansing, but research on the effects of wound cleansing on bacterial colonization is still limited. The purpose of this literature study was to determine the effectiveness of using a type of wound cleansing on changes in bacterial colonization in DM wounds. Methods: The data base used in making this review literature is Pubmed, Science Direct, Google Scholar and Cochrane. there are 144 articles identified and published from 2010-2018. Of the 144 articles 8 articles that met the inclusion criteria. Results: Various wound cleansing solutions are used to optimize the wound healing process and the effect is quite good in decreasing bacterial colonies in DM wounds. Conclusion: wound cleansing type ESWA has a bactericidal effect, is effective in reducing bacterial colonization, and economically it is expected to be a concern in the treatment process injury, research is needed as to whether ESWA washing types can be used to reduce bacterial colonization in DM wounds.

Abstract

Wound healing is a complex physiological response from living systems for physical, chemical, mechanical or thermal injury, where cells and matrix components act in rhythm to facilitate wound regeneration and restore tissue integrity. The skin is a complex organ consisting of the epidermis, dermis, and complementary skin, including hair follicles and sebaceous glands besides that the skin is also known as the largest organ in the body and plays several important roles ranging from regulating temperature, preventing infection and also a very dynamic organ which continually replaces cells, the depth of damage to the skin, both superficial and partial, can affect the length and amount of use of wound dressing material so that wound healing requires an appropriate environment and skill ability in selecting topical dressings to improve the healing process that is suitable for each type wounds, the right and innovative healing stage is estimated to heal within 2-8 weeks. This literature study aims to determine the benefits of topical cream extract of hylocereus polyrhizus 7.5% on wound healing. Search was done using pubMed, cochrane, google scholar and secondary search data published from 1990 to 2017. 7.5% red dragon fruit (EBNM) contains antioxidants, flavonoid phlyphenols which can increase collagen synthesis in wound healing besides seeds dragon fruit is able to relieve flasks and keep the skin moisture smooth due to the content of linoleic acid. Every 1 gram of Red Dragon Fruit contains 8.3 mg of total flavonoids and 10.8 mg of total phenol. Red Dragon Fruit (BNM) with high fiber, low in calories, and rich in antioxidants. Red Dragon Fruit Extract (EBNM) 7.5% is better at repairing granulation tissue and epithelial tissue, so it has the potential to be used as a wound healing therapy.

Abstract

Latar belakang : Kandungan ekstrak tumbuhan telah digunakan sudah sejak lama sebagai salah satu metode alternatif/komplementer terutama dalam penyembuhan luka, hanya saja belum terinci sehingga perlu pengujian baik secara in vitro maupun in vivo. Tujuan : Untuk mengetahui sejauhmana efektifitas tanaman Jatropha Curcas L. terhadap penyembuhan luka dengan pendekatan dari beberapa penelitian yang telah ada sebelumnya. Metode : Metode dari tinjauan literature ini menggunakan pedoman Prisma dengan penelusuran artikel ilmiah menggunakan data based Pubmed, Google Scholar dan Science Direct. Melakukan pencarian pada Pubmed dengan kata kunci Extract Jatropha Curcas L. [MeSH Terms] AND wound healing [MeSH Terms], google Scholar dengan kata kunci Extract Jatropha Curcas L AND wound healing, sedangkan Science Direct dengan kata kunci Ekstrak Jatropha Curcas L. AND wound healing Hasil : Dari beberapa artikel penelitian yang telah diulas menunjukkan bahwa efektifitas dari ekstrak tanaman jarak pagar (Jatropha Curcas L.) memiliki pengaruh terhadap penyembuhan luka pada hewan coba tikus Wistar.

Abstract

Pendahuluan: Sistem rujukan dibuat untuk mengintegrasikan pelayanan dan memudahkan pelayanan ke pasien. Di lain sisi pertimbangan biaya menjadi hal yang sangat penting dalam pelayanan kesehatan. Masih rendahnya pemahaman tentang sistem rujukan sedikit banyak menyulitkan pasien itu sendiri dalam mendapatkan pelayanan kesehatan.Tujuan dari penulisan Literature review ini adalah untuk mengevaluasi pelaksanaan sistem rujukan berbasis pemerintah dan hambatan yang dihadapi dalam layanan kesehatan. Metode penyusunan literatur review ini mengikuti panduan PRISMA. Pubmed, Science Direct, Wiley Online, Proquest, dan Google Scholar adalah database yang digunakan dalam mengumpulkan artikel dengan kata kunci yang relevan. Hasil diperoleh lima artikel penelitian yang sesuai dengan kriteria inklusi. Artikel penelitian mengemukakan proses pelaksanaan, hambatan dan tantangan dalam system rujukan berjanjang di layanan kesehatan. Kesimpulan Sistem rujukan berjenjang merupakan sistem pelayanan kesehatan bagi masyarakat yang bertujuan untuk mengendalikan mutu dan biaya pelayanan dalam sistem JKN. Sistem ini juga dirancang agar pelayanan kesehatan yang diterima oleh pasien bisa optimal dan pasien dapat puas dengan pelayanan tersebut. Meski demikian, hasil penelitian ini menemukan masih banyak persoalan dalam pelaksanaan sistem rujukan berjenjang di FKTP. Pelayanan yang diberikan oleh FKTP belum optimal karena tidak seimbangnya antara jumlah pasien yang dilayani dan petugas dan infrastruktur pelayanan kesehatan.

Abstract

Background: Chemotherapy is highly recommended for cancer treatment, however can cause some side effects such as nausea and vomiting. This will affect food intake and nutritional status in cancer patients who undergo chemotherapy. Aim: To describe nutritional status based on anthropometry Body Mass Index (BMI), hemoglobin (Hb), Patient-Generated Subjective Global Assessment (PG-SGA) and Mini Nutritional Assessment (MNA) in cancer patients undergoing chemotherapy in RSUP. Dr. Wahidin sudirohusodo Makassar. Methode: This experiment uses a quantitative non-experimental research method with cross sectional approach on 1010 patients, nonprobability sampling with purposive sampling technique on 70 respondents, data collection with interview and observation techniques. Results: Nutritional status based on Body Mass Index (BMI) shows that 39 people (55.7%) had normal BMI values. About 37 people (52.9%) had good / normal nutritional status based on Patient-Generated Subjective Global Assessment (PG-SGA) while Mini Nutritional Assessment (MNA) reveals 100% of respondents experienced nutritional status problems. Conclusion: Mini Nutritional Assessment (MNA) is the best tool to identify nutritional status of cancer patients that undergo chemotherapy since this instrument is very sensitive and practical. Patient-Generated Subjective Global Assessment (PG-SGA) is good to assess nutritional status in subject who have lost weight drastically and shows signs of subcutaneous fat loss and muscle mass loss. Abstract

Tujuan dari penelitian ini adalah untuk membuat analisis sistematis literatur yang berkaitan dengan burnout perawat di perawatan intensif serta melihat faktor apa saja yang berkaitan. Tinjauan ini mencakup penelitian yang dilakukan antara tahun 2011-2021 yang : Ditulis dalam bahasa Inggris, penelitian asli, dapat diakses secara terbuka dan tersedia dalam teks lengkap, mengeksplorasi isu yang berkaitan dengan (stres terkait pekerjaan, kelelahan), berfokus pada perawat yang bekerja di ruangan Intensive Care Unit. Kriteria Eksklusi adalah perawat yang tidak berkerja di ruangan Intensive Care Unit, artikel berisi tentang review dan tidak berbahasa Inggris. Instrumen kritisi jurnal Cross Sectional Study dari CEBM (Center for evidence based management). Panduan rekomendasi dari Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). Pencarian dilakukan dalam database PubMed, Pro Quest, Science Direct, dan Wiley. Sepuluh Artikel teridentifikasi, dengan hasil artikel tersebut membahas factor terkait burnout perawat yaitu faktor organisasi, hubungan kerja, dan karakteristik pribadi yang akan mempengaruhi terjadinya burnout pada perawat yang berkerja khusunya di ruangan Intensive Care Unit atau Critical care. Burnout lebih sering terjadi pada lingkungan perawatan kritis yang terkait erat dengan perkembangan sindrom ini. Dalam penelitian ini menjelaskan factor terkait burnout perawat yang bekerja di intensive care unit.Kata Kunci: Kelelahan perawat, sindrom kelelahan, perawatan intensif

Abstract

The high incidence of Diabetes Mellitus (DM) causes increased complications associated with DM. One is the Diabetic Foot Ulcer (DFU). Delayed healing in DFU is usually caused by infection. The infection of DFU contains many gram negative (-) and gram positive (+) bacteria, so it is important. To identify the type of bacteria present on the DFU based on the cause. This was an experimental cross sectional study conducted at ETN Center, Griya Afiat, Avaro, Isam Cahaya, RSUP DR. Wahidin Sudirohusodo Makassar and Microbiology Laboratory Hospital Unhas Makassar. There were 29 respondents who met the inclusion criteria. Specimens of wound fluid were taken by swabbing technique. The specimen was inoculated on NA (Nutrient Agar) and MCA (MacConcey Agar) then incubated at 37 ° C for ± 18-24 hours to see bacterial morphology. If a gram negative bacteria (-) is tested biochemically by bacteria is grown on TSI (Tripple Sugar Iron), SIM (Indole Motility Sulfide), citrate urea, kovacks. As for gram positive bacteria (+) in MSA medium (Manitol Salt Agar), catalase (H2O2), incubated for 18-24 hours at 37 ° C, record the results of identification and match the bacterial identification table based on gram properties. Based on morphological test, 29,5% gram negative (-) and Gram positive (+) bacteria were 8.8% in traumatic LKD, with the dominant gram negative (-) species being proteus vulgaris (8.8%) and staphylococcus aureus 8.8%) in the gram bacteria (+). While for non trauma LKD found gram negative bacteria (-) as much as 58.8% and gram positive bacteria (+) 2.9% with the dominant type of bacteria are S.Aureus (2.9%) and P. Mirabilis (17.6%) for gram negative bacteria (-). Identification of bacteria in DFU caused by trauma and non trauma is dominant gram negative bacteria (-). Further research is expected to identify the type of bacteria based on the Wagner and sensitivity of the bacteria.

Abstract

Treatments of diabetic ulcer are very essential to prevent further ulceration damage. Insufficient treatment of the wound can consequence in an infection that will have an impact upon the amputation. One of the antimicrobial wound dressing in the treatment of ulcus diabetic is chitosan. Therefore, this report aims to assess the effectivity of the usage of chitosan gel as a primary dressing for epithelial progression in diabetic ulcer at the wound care clinic of ETN Center, Makassar. Methods, this study was conducted on one patient, a female, 38 years old with random blood glucose 239 mg/dl. The wound occurred in April 2019, the wound has been experienced about three months and then followed prospectively starting 8th July until 1st August 2019 with 8 treatments. Treating the wound uses chitosan gel as a primary dressing and looks at the development process of the wound, especially the progress of the epithelium using the modified Bates-Jansen Wound Assessment Tool. Results, the comparison of treatment results using chitosan in the first and eighth treatments (25 days) respectively slough 10% and 5%, granulation 75% and 60%, and epithelialization 15% and 35%. While the size of the wound 41 x 17 cm to 32 x 13 cm. Conclusion, chitosan gel as a primary dressing plays an important role in helping the wound healing process as indicated by an increase in epithelium and reduction in wound size.
