Abstract

Medical records storage is an activity to protect the medical records from the physical damage and the contentof the medical records. Medical records storage process has risks that can threatening the occupational safetyand health of medical records staff. One of the efforts in occupational safety and health is by giving protectionfor the medical records staff by using personal protective equipment. At filing RSUP dr. Soeradji TirtonegoroKlaten found a problem that employees do not use personal protective equipment such as masks andhandscoon, the roll o’pack high is not ergonomic and it does not have Standart Prosedure Operational. Thenurse has risk to experience musculosveletal disorders, hands are itchy, sneeze, scraped paper or sharpmedical record map, and has strangulated the roll o’pack when they do the job. The purpose of this researchwas to analyzed the Health Management risk and Occupational Safety in ifiling RSUP dr. Soeradji TirtonegoroKlaten. Type of qualitative research. Some employees in filing used as Respondents. The results of thisresearch were 16,6% low risk, 50 % moderat risk, and 33,4% in high risk. Based on this results the controlefforts was redesign the map folders material, regulated the high of roll o pack, provided the vacuum cleaner,had encouraged the officers to always washed their hands after touched the medical records, made theStandart Prosedure Operational for occupational safety and health in filing, and gave a punishment ifemployees did not used personal protective equipment.

Abstract

Medical record controlling is the supervision of medical record borrowing and medical record returning to thestorage rack. Based on observation at Airlangga University Hospital in October 2019 there were 3 out of 5borrowed files were returned by exceeding the deadline. From December 2019 until February 2020 therewere 11 of the 16 medical record files borrowed by other units, but the expedition book did not contain anyinformation regarding the date of returning file. Delay in the medical record returning file results in misplacedor miss file. From March 9th, 2020 to March 13th, 2020 there were 237 of 3053 medical record files thatwere misplaced or miss file. This study aimed to analyze the medical record file control system at AirlanggaUniversity Hospital using the 5M method (man, method, machine, material, and money). This research wasqualitative. Data collection techniques used were observation, interviews, and documentation. The subjectof study was medical record officer. The results show that the factors causing problems in controlling medicalrecord files are high workload of officers, late return of medical record files, officer discipline in usingexpedition books, absence of tracers as a substitute for medical record files, and the number of storageshelves that were under the needs. Suggestions for the problems are to add more officers in the fillingsection, conduct socialization and evaluate officers in using expedition books, make electronic tracerapplications, and adding storage shelves.

Abstract

Medical records are said as good if there is a complete information to support orderly administration inimproving health services. The quality of hospital service is seen not only in the completeness of medicalrecords but also in the accuracy, speed, and accuracy of hospital management information systems. Basedon a preliminary study taken at Sidoarjo district medical records installation, it was known that in theapplication of hospital management information systems there was a problem complained about by officials.One of the efforts to minimize these problems is to evaluate the system using the PIECES method(Performance, Information, Economics, Control, Efficiency, Service). This study aimed to analyze theperformance of BPJS outpatient registration information systems in RSUD Sidoarjo based on 6 aspects ofPIECES namely performance, information, economics, control, efficiency, and service. This type of researchwas qualitative. The method of data collection used was the interview and observation of the two enlistmentofficers. From the results of study obtained information that Information systems performance based onperformance aspects of Information Systems, it already runs fit with the user needs even though there wasan incomplete menu; information aspects, the information generated was quite accurate; economicsaspects, it had not produced an economic value, the hospital management information could not bridgingwith other information systems; control aspects, it already runs with the control of each user access andsecurity system provide passwords and usernames; efficiency aspects, it already provides efficiencies in itsapplication; service aspects, it already gives ease of use for the users. Based on the results of this evaluationit is expected that the Regional Hospital may complete the system information menus and conduct an MOUwith the BPJS Health for integrating the hospital management information with the BPJS Health’s V-Claim.

Abstract

The number of outpatient medical records at RSUP dr. Soeradji Tirtonegoro has the highest late provisionthat occurred in June 2019 by 44.66%. The purpose of this report was to analyze the factors causing lateprovision of outpatient medical record files at RSUP dr. Soeradji Tirtonegoro Klaten. This type of researchwas qualitative. Data collection techniques used in this report were interviews and observation. The resultsshowed that the factors causing late provision of outpatient medical record files was man factors, whichare the indiscipline of filing staff in retrieving medical record files and the poly / ward officers' indiscipline inreturning medical records in a timely manner. Material factors were damage to the medical record file onthe map, missfile, emergency patient form without a folder, and the form has not been assembled. Themachine factor were the sudden power failure and the lack of borrowing and returning medical records onSINERGIS. The method factor was the unavailability of regular outpatients in the registration room. Themoney factor is that there is no financial support to give awards to officers. Improvement efforts that can bedone are filing officers not waiting for some tracers to pile up when retrieving files, evaluating the processof returning outpatient or inpatient medical record files to the filing room, always sweeping againstdamaged or non-bound documents. IT added features or menus for borrowing and returning medicalrecord files to SINERGIS. The registration officer installs a regular outpatient registration flow in theregistration room.

Abstract

The incompatibility of SPO (Standart Operational Procedure) for General Consent completion was one ofmany problems that exist in the implementation of medical record activities at Airlangga UniversityHospital. SPO incompatibility in General Consent Completion could be seen from incorrect answer inGeneral Consent form for each indicator. From 21 samples of General Consent forms, there wasunsuitable answers in such as the identity of the person in charge (62%), the identity of the patient (71%),the approval for information release, privacy, personal values and patient confidence as well as thesignature and name of the witness (100%), the signature and the name of the patient person in charge(24%) and the date (43%). The purpose of this study was to analyze the cause of the SPO incompatibilityfor General Consent completion in Airlangga University Hospital. This type of research was qualitativeresearch and data collection obtained through interview. The subjects in this study were 5 registrationofficers at Airlangga University Hospital. The result of this study showed the causes of the GeneralConsent Completion SPO incompatibility were no specific training about medical records, the GeneralConsent Completion SPO did not explain the procedures to complete the form thoroughly, and there wasno socialization related to the General Consent Completion at Airlangga University Hospital. The solutionsfor this problem are, there should be an effort to improve the General Consent Completion SPO, providetraining for registration officers and conduct socialization on General Consent Completion.

Abstract

Medical records are files that contain records and documents regarding patient identity, examination results,treatment, actions and other services that have been provided to patients. Management of medical records is oneof medical support services. Based on the results of a preliminary study conducted at Airlangga UniversityHospital Surabaya it is known that in February 2020 amount 104 files were damaged so that the forms containedin them could be torn. This research was aimed to analyze the factors that cause damage to medical record filesat Airlangga University Hospital Surabaya. This type of research uses qualitative and data collection withinterviews and observations by using the analysis of 5M management elements is Man, Machine, Method,Material, Money. The population in this study is the medical records officer filing room section of 5 officers. Theresults obtained are that there are still many medical records officers who have a background not DIII medicalrecords, the lack of socialization of SOPs related to the maintenance of medical record files, limited number ofrack filing, and the less used folder material. So the suggestion made by the researchers is to redesign themedical record file folder by using a color code, and calculate the rack filing needs.

Abstract

Based on the results of preliminary studies conducted at the RSUP dr. Soeradji Tirtonegoro is known thehighest delay count is the Anggrek of 95% room and 4th Dahlia room of 80%. The delay has an impactwhich daily census returned is not timely, and be it can increase the work of officers for the next day andsending the recapitulation results to the reporting department is delayed. This study aims to observe theimplementation of the daily census and the factors causing the inaccuracy in returning the daily census ofhospitalization, the determination of the main causes of the problem are using a fishbone diagram. Thistype of research uses qualitative and are collected a data with observation, interview and documentation.The results of the main problem are Man (the work of the recapitulation officer is delayed and ineffective,the employee's indiscipline). Method (there is not written policies of Standard Operating Procedure, theofficer does not know the Standard Operating Procedure). Minutes (The deadline for retuning the dailycensus of hospitalization are not consistent). Machine (there are no features that support the daily censusof hospitalization in SINERGIS, daily inpatient census is manually). Money (there is no rewards).

Abstract

Filling room has an important role in maintaining medical records, especially in terms of storage and protection of medical records. The creation of officers ease in taking and storing medical records in supported by the existence of an ergonomic storage space that is adapted to the anthropometric aspects. The design of the storage rack in accordance with the anthropometric data of the officer can facilitate the work of the officers. Storage racks at Airlangga University Hospital are not in accordance with the anthropometry of the officers, resulting in work discomfort and fatigue. The purpose of this study was to analyze the ergonomics aspects of the storage rack based on the anthropometry of the filling officer. This type of research is descriptive and data collection is done by observation. The population of this study was 5 filing officers. The results of this study indicate that the condition of the storage rack in the filling room of the Airlangga University Hospital is not in accordance of the filling officer. The anthropometry of the filling officer used in designing the rack, namely the reach of the hand up to 202 cm, is used to measure the height of the medical record file storage rack, data fathom length 162 cm is used to reference the length of the medical records file storage rack and shoulder width data used to reference the distance between medical record file storage rack.

Abstract

Puskesmas Patrang merupakan salah satu puskesmas yang sudah menggunakan aplikasi Primary Care (P-Care). Aplikasi ini digunakan untuk mengentrikan data sosial, data pemeriksaan pasien dan pembuatan surat rujukan untuk pasien BPJS. Dalam pelaksanaannya masih ditemui berbagai kendala yang dialami oleh pengguna,yaitu  seperti aplikasi p-care yang mengalami error,  pembuatan surat rujukan hanya dilakukan oleh 1 petugas serta pengguna harus melakukan double entry data guna untuk kebutuhan laporan yang dibutuhkan oleh puskesmas. Tujuan dari penelitian ini adalah mengevaluasi implementasi aplikasi  Primary   Care dengan   menggunakan    metode   TTF   (Tasks  Technology  Fit) . Jenis penelitian ini adalah penelitian kualitatif dengan pendekatan deskriptif. Hasil evaluasi implementasi aplikasi Primary care   di Puskesmas Patrang menunjukkan bahwa dari 5 responden menyatakan karakteristik  tugas sudah sangat baik, semua tugas yang dilakukan sudah terfasilitasi disetiap menu aplikasi. Kriteria karakteristik dalam penerapan teknologi yang digunakan sudah baik, meskipun ketersediaannya belum merata. Kriteria kesesuaian tugas yang dirasakan oleh pengguna sudah sesuai dengan teknologi yang digunakan. Kriteria kebermanfaatan diperoleh bahwa pengguna merasakan butuh dengan aplikasi ini. Serta kriteria dampak kinerja dalam penerapan aplikasi ini yaitu ini sudah dirasakan dengan baik. Upaya penambahan kebutuhan pendukung seperti komputer dan scanner, penggunaan SIMPUS serta pembuatan SOP perlu diterapkan agar implementasi aplikasi primary care bisa berjalan dengan baik.

Abstract

Medical records according to Permenkes No. 269 of 2008 is a file containing notes and documents about patient identity. The problem that often arises in Klaten General Hospital is the length of the file search process due to the absence of applications that control the entry and exit of files, when officers borrow and return only to write in the control book. Supporting all the problems and objectives needed by the data, so the data collection methods used are observation and interviews. Analysis of the data obtained from the interview and observation process is to draw conclusions and identify the most urgent problems. The goal to solve this problem is to make an application that controls the borrowing and return of the RM file so that the officer knows quickly where the file will be sought. The application is made using the waterfall method in the workflow process and uses several programming languages such as PHP, HTML and CSS. The results of the application have been tested by several users using blackbok testing, but still do not know how effective and efficient when the application is tested in actual service.
