Abstract

ABSTRACTChronic Obstructive Pulmonary Disease (COPD) is one of pulmonary disease that becomes health problem in world, including Indonesia. Nutritional status is the condition of health of a person that is influenced by the intake and utilisation of nutrients. COPD may affect the nutritional status and cause malnutrition, otherwise malnutrition may increase risk of complications, morbidity and mortality rate of COPD’s patient. Nutritional status and malnutrition also affected the quality of life COPD’s patient. This study was using descriptive methods with crossectional approached. The sampling technique used consecutive sampling. The study was held on February-March 2016 with number of sample is 60 patients with COPD in Pulmonary Polyclinic of Arifin Achmad Pekanbaru general hospital. The result of this study is characteristic based age showed 32 (53,3%) persons were >65 years old, 55 (91,7%) persons were male. Characteristic based on occupation showed 33 (55,0%) persons were retired, 48 (80,0%) persons were ex-smoker, 31 (51,7%) persons had COPD for 1-5 years, and 35 (58,3%) persons with good appetite. Nutritonal status based on Body Mass Index (BMI) showed 28 (47,0%) persons had normal category. Nutritional status based on Mid-upper Arm Circumference (MUAC) measurement showed 48 (80,0%) persons had ideal nutrition category. The risk of malnutrition based on Malnutrition Universal Screening Tool (MUST) showed 26 (43,0%) persons were low risk category.Keywords: COPD outpatients, nutritional status, Malnutrition Universal Screening Tool (MUST).

Abstract

Chronic Obstructive Pulmonary Disease is characterized by persistent airflow limitation, progressive, associated with chronic inflammatory response caused by noxious particles and gases. The severity of COPD depends on its exacerbations and comorbidities of each individual. One of the most factors cause exacerbation of COPD is bacterial infection. The purpose of this study was to know the percentage of bacterial infection cause the exacerbation and the pattern of its etiologic bacteria. The samples were sputum of hospitalized patients with exacerbation of COPD in the Lung room RSUD Arifin Achmad Riau Province. The sputum specimen was collected using sterile containers and subjected to Gram’s stain, culture and biochemical reactions. This study obtained 100% of sputum cultur for pathogenic bacteria was possitive in all 23 cases. The most etiologic bacteria were gram negative 83% which is Klebsiella sp. as the leading bacteria 48%, followed by Acinetobacter sp. 22% and Enterobacter sp. 13%. Gram possitive Staphylococcus aureus were found about 17% in exacerbation of COPD. Keywords : COPD, exacerbation, bacterial identification

Abstract

AbstractChronic Obstructive Pulmonary Disease (COPD) is chronic inflammatorydisease of the lungs marked by breath and obstruction of air flow is persistent andprogressive as a response to the particles or gases dangerous. The severity of COPDdepends on its exacerbations and comorbidities of each individual. The purpose ofthis study to determine the profile of COPD patients who are hospitalized at ArifinAchmad Hospital in 2013 by age, sex, occupation, smoking history, chief complaint,additional complaints and a history of exacerbations. The study was conducted inNovember-December 2014. The study was conducted in November-December 2014.The study sample as many as 80 people. The data collected is secondary dataobtained from medical records. The results showed that COPD patients who arehospitalized at Arifin Achmad Hospital in 2013 based on age are more common inthe age of 61-70 years as many as 29 people (36.3%), based on gender men are oftensuffer ppok 87,5 people ( 70 % ), based on the work of people with COPD who foundwork as self-employed as many as 33 people (41.25%),based on a history of smoking,COPD patients who had a positive smoking history as many as 50 people (62.5%),the main complaint is shortness of breath are found as many as 69 people (86.25%),additional complaints were most common are chest pain and headaches as many as20 people (21.3%) and COPD patients who are hospitalized with a history ofexacerbations most 0-1 times per year as many as 55 people (68.75%).Keywords: COPD, patient profile

Abstract

World Health Organization (WHO) classified tuberculosis (TB) as global emergency at the end of 1993. Directly Observed Treatment Short-course (DOTS) is a direct supervision for short-term treatment, recommended by the World Health Organization for the world TB prevention. The Millennium Development Goals (MDGS) in 2015 is to reduce the number of deaths caused by tuberculosis for 50%. To achieve the target, program policy against tuberculosis improvement and evaluation will be needed. Evaluation in this research done by using two nasional indicators, which are success rate (national targets should be at least 85%) and conversion rate (national targets should be at least 80%). This research was using mixed methods with quantitative and qualitative approachment. Samples was taken by total sampling method, where 304 people have obtained as quantitative research samples and 3 people as qualitative research samples. Research results showed that the success rate at the end of 2009 and 2012 was 94%, in 2010 was 93%, in 2011 was 86%, and in 2008 was 77%. While the conversion rate at the end of 2011 was 96%, in 2012 was 94%, in 2009 was 91%, in 2008 was 89%, and in 2010 was 82%. It can be concluded that success rate in General Hospital Arifin Achmad Riau Province in 2008 have not reached national targets yet, but in 2009, 2010, 2011, and 2012 have exceeded national targets. The conversion in General Hospital Arifin Achmad Riau Province at the end of 2008, 2009, 2010, 2011 and in 2012 have exceeded national targets.Key words : Tuberculosis (TB), Directly Observed Treatment Short-course (DOTS), success rate, conversion rate
