Abstract

ABSTRACTPatients treated at the ward in a hospital can get nosocomial infection through exposure of microbes that contaminate air and medical equipments. This study was conducted to determine the amount of air microbes and to identify bacterium found on a swab equipments at Isolation room on decrease immunity and operating theatre in “Dharmais” Cancer Hospital Jakarta. This study uses secondary data on a periode of 2013-2014. The amount of microbial colonies that meet the threshold value in accordance with a predetermined on Kepmenkes RI No. 1204 / Menkes / SK / X / 2004 on a Hospital Environmental Health Requirements in RIIM is at 100%, while at OK is 13:33%. Types of microbes found in the RIIM were E. gergoviae, A. baumanni, S. liquefaciens, C. freundii, Y. enterocolitica, C. diversus and fungi while the types of microbes found in the OK room were A. haemolyticus, M. morganii, A. baumanni, E. coli, A. Iwoffli, K. oxytoca, C. diversus, K. ozaenae, P. stuartii, S. marcescens, Y. enterocolitica, and fungi. ABSTRAKPasien yang dirawat di ruang rawat inap sebuah rumah sakit bisa mendapat infeksi nosokomial melalui paparan dengan mikroba udara maupun yang mengontaminasi peralatan medis. Penelitian ini dilakukan untuk mengetahui jumlah koloni mikroba udara dan identifikasi bakteri pada swab peralatan di Ruang Isolasi Imunitas Menurun (RIIM) dan ruang operasi (OK) di RS Kanker “Dharmais”, Jakarta. Penelitian ini menggunakan data sekunder periode 2013-2014. Jumlah koloni mikroba yang memenuhi nilai ambang batas sesuai dengan yang telah ditetapkan pada Kepmenkes RI nomor 1204/Menkes/SK/X/2004 tentang Persyaratan Kesehatan Lingkungan Rumah Sakit pada RIIM adalah sebesar 100%, sedangkan pada OK sebesar 13,33%. Jenis mikroba yang ditemukan pada ruang RIIM adalah E. gergoviae, A. baumanni, S. liquefaciens, C. freundii, Y. enterocolitica, C. Diversus, dan jamur, sedangkan jenis mikroba yang ditemukan pada ruang operasi adalah A. haemolyticus, M. morganii, A. baumanni, E.coli, A. Iwoffli, K. oxytoca, C. diversus, K. ozaenae, P. stuartii, S. marcescens, Y. enterocolitica, dan jamur.

Abstract

Routine hematology tests (Hb, Hct, RBC, WBC, PLT) generally done by automation methods using a hematology analyzer. Ideally, the examinations should be done as soon as possible, although, in some circumstances, it can be delayed. Based on the literature, the sample for routine hematology testing should be processed within 4 – 8 hours of venesection. Some studies revealed that the sample could be stored for up to 48 hours, and it can be influenced by the technology applied to the hematology analyzer. Abstract

Indonesian Red Cross (IRC) is an organization that received an assignment to provide blood for transfusion therapy. Blood for transfusion should free from infectious disease. Several studies have shown that viruses, bacteria and protozoa can be transmitted by transfusion. Clinical features due to bacterial contamination varies from asymptomatic, mild fever, acute sepsis, hypotension and can even cause death. Components of platelet concentrates can be a good place for the growth of Gram positive and Gram negative bacteria, because it is stored at a temperature of 20O-24OC with the addition of dextrose that can be used as an energy source for bacteria. Bacteria identification performed by culturing the component of platelet concentrates on BPA tube of Bact / ALERT. Tube with contaminated bacteria is subcultured onto blood agar media. Bacterial colonies from blood agar stained with Gram staining and further identified using API20E, API 20NE, API 20STREP, MSA media or staphilase test using reagents Staphaurex*, according to the type of bacteria detected on Gram stain. The results shown a component of platelet concentrates is contaminated by bacteria., The bacteria is Gram positive cocci clusters. MSA and staphilase test have a negative result, so it can be concluded that the bacteria that had contaminated platelet concentrates is Staphylococcus epidermidis.

Abstract

Thalassemia is a genetic disorder caused by impaired synthesis of globin chains in hemoglobin (Hb) molecule which can be partial or total. Since thalassemia is inherited from parents to their children, screening is compulsory to prevent newborn at risk from major thalassemia. The incidence thalassemia cases can be found around the world but majority cases emerge among Mediterranean, easter, and Asian. Thalassemia minor is diagnosed based on erythrocyte indices and confirmed by molecular analysis of Hb and DNA. Currently, several formulas are developed to identify the existence of thalassemia. This cross sectional study was aimed to know how good the formulas is. Thalassemia screening was done to 138 donors with minimum of Hb 12,5 g/dL. Detection and identification proses was conducted by the use of calculation index formulas for erythrocytes namely Mentzer index, Srivastava, Shine & Lal, and England & Fraser. The results showed that all of four indexes can detect ? thalassemia minor. The highest degree of sensitivity is Mentzer and Shine & Lal indexs (100%), whereas the highest specificity is Srivastava index (100%). In addition, ? thalassemia and Hb variant (HbE) can also be detected, although not 100%. This result shows that the formulas can be used as a screening of thalassemia, especially in areas those have no accessto molecular laboratory.

Abstract

Concentrated platelet components are the components or platelets that are processed through centrifugation from whole blood or a single donor. The components of the concentrate platelets are stored in an agitator at a temperature of 20-24◦C with a maximum shelf-life of five days. During the storage, a change in component metabolism occurs, which is characterized by alteration in platelet morphology as an indicator of decreasing quality of platelet components. The purpose of this study was to determine the quality of the components of platelets based on the shelf-life by measuring the platelet morphological values with the Kunicki method. This study applied an experimental method by conducting inspections of the regional blood transfusion unit (UTDD) of the Indonesian Red Cross of Jakarta Capital Special Region (PMI DKI Jakarta). Platelet morphological values of six components of platelets were examined by the Kunicki method on day 0 to day 5 of shelf-life. Quality assessment was carried out based on a significant difference-dependent t-test and comparison with normal value. The results of the study by performing statistical analysis of the significant difference-dependent t-test showed a significant difference of values between day 0 and day 1 to day 5 of shelf-life. Comparison with normal values ​​depicted that all samples stored on day 4 had a morphological value of more than 200. On day 5 of shelf-life, the platelet morphology values ​​of five samples were below 200. The quality of the concentrated platelet components is said “good” if it is above 200. This study concluded that there was a significant difference in the platelet morphology values between the components of concentrated platelets on day 0 and day 1 to day 5 of shelf-life. All samples were of good quality until day 4 of storage. However, on day 5 of storage, only one sample was good in quality.

Abstract

Saat ini metode otomatisasi menggunakan hematology analyzer banyak dipilih dibandingkan metode manual dengan keutamaan mempunyai presisi yang lebih baik. Kontrol kualitas pada hematology analyzer umumnya hanya menggunakan aturan Westgard, dan belum digunakannya analisis six sigma di laboratorium RSUD Budhi Asih, walaupun analisis six sigma juga diperlukan untuk melihat tingkat akurasi dan presisi suatu metode. Penelitian ini bertujuan untuk menganalisis penggunaan six sigma sebagai evaluasi kontrol kualitas hematology analyzer Sysmex XN-1000 di laboratorium klinik RSUD Budhi Asih pada parameter HGB, RBC, WBC, HCT dan PLT. Penelitian ini merupakan penelitian deskriptif menggunakan data hasil kontrol kualitas internal laboratorium klinik RSUD Budhi Asih periode Januari s/d Februari 2022 dengan bahan kontrol level low, normal dan high. Analisis six sigma melibatkan CV dan bias. Perhitungan CV dibandingkan dengan nilai target kit insert dan nilai bias dibandingkan dengan standar Rico’s biological variation. Hasil menunjukkan, bahan kontrol masuk dalam kategori ‘in control’, nilai CV di bawah batas yang ditetapkan dan nilai bias umumnya berada di bawah standar, kecuali parameter HCT. Nilai sigma berada pada kategori world class performance sampai marginal performance. Kesimpulan penelitian ini analisis kontrol kualitas cukup digunakan dua level bahan kontrol setiap hari, yang disertai dengan analisis kurva Levey Jenning’s melalui aturan Westgard. Abstract

Pemeriksaan Laju Endap Darah (LED) adalah pemeriksaan darah yang menggambarkan kecepatan mengendapnya eritrosit dalam plasma darah yang dinyatakan dalam satuan mm/jam. Pemeriksaan metode manual yang disarankan ICSH adalah metode Westergren, dimana sampel diencerkan dan pembacaan dilakukan setelah satu jam. Prosedur pemeriksaan metode otomatis dilakukan tanpa pengenceran sampel dan hasil dibaca setelah 20 menit. Penelitian ini bertujuan  untuk mengetahui apakah ada perbedaan hasil LED antara metode otomatis dengan metode manual pada pasien anemia. Penelitian ini merupakan penelitian observative menggunakan data primer dengan desain studi potong lintang. Subyek penelitian ini adalah 51 pasien dengan kadar hemoglobin kurang dari 10 g/dL. Bahan pemeriksaan yang digunakan adalah whole blood dengan antikoagulan EDTA. Uji beda bermakna dilakukan setelah uji normalitas data (nilai α 0,05). Data penelitian diuji menggunakan uji statistik paired T-test.Simpulan penelitian ini terdapat perbedaan yang signifikan antara hasil pembacaan LED metode otomatis dan metode manual. Kata kunci: Laju Endap Darah; LED metode otomatis; LED metode Westergren
