Abstract

Terapi periodontitis kronis poket 4-6 mm biasanya dilakukan kuretase atau operasi flep. Skeling dan penghalusan akar(SPA) dapat mengubah kondisi klinis. Tujuan penelitian untuk mengetahui efek kllinis setelah SPA pada periodontitiskronis poket 4-6 mm. Empat puluh tiga penderita periodontitis kronis dilakukan SPA pada kunjungan awal, hari ketujuh,hari ke-14, hari ke-21, bulan kedua, bulan ketiga, dan bulan keenam. Pada kunjungan awal, bulan kedua, ketiga, dankeenam, sebelum dan setelah SPA dilakukan pemeriksaan kedalaman poket dan Indeks perdarahan gingiva (PBI). Hasilpenelitian menunjukkan terdapat penurunan kedalaman poket dan indeks perdarahan gingival ( p < 0,05) antara sebelumdan sesudah SPA. Sebagai kesimpulan, SPA dapat meningkatkan kondisi klinis pada terapi periodontitis kronis poket 4-6mm.

Abstract

Pada kerusakan tulang alveolar, pembentukan tulang baru dapat dirangsang oleh bahan yang bersifat merangsangosteogenesis. Kitosandapat sebagai interfase darah dan jaringan serta memiliki efek regeneratif sebagai scaffoldpada pertumbuhan tulang. Tujuannya mengamati efek kitosan terhadap sel punca PDL pada proses osteogenesis,diamati melalui viabilitas sel punca PDL. Sel punca dipanen dari apikal gigi molar 3 sehat dan gigi periodontitis,didentifikasi dengan Antibodi Stro-1. Kelompok eksperimental terdiri atas sel punca PDL yang dipapar kitosan0,025, 0,5, 0,1, 0,15, 0,2 dan 0,25% dan kelompok kontrol tanpa dipapar kitosan.Viabilitasnya diamati dengantehnik MTT. Sejumlah 10.000 sel punca PDL ditebar dalam plat kultur jaringan yang berisi 10% FBS, 10% NCS,penstrep, and 250µg/ml fungizone/amphoterisin B. Hasil penelitian menunjukkan viabilitas sel tertinggi terdapatpada kelompok sel punca PDL sehat yang dipapar kitosan 0,15%, artinya pada konsentrasi tersebut kitosanmemiliki efek tertinggi dalam menstimuli proliferasi sel. Kesimpulannya, kelompok sel punca PDL sehat yangdipapar kitosan menunjukkan viabilitas sel yang tertinggal dalam menstimulasi proliferasi sel.

Abstract

Reduction of the amount of tooth roots which are embedded in their periodontium could cause tooth mobility. Splinting a weaker tooth with a more stable one, and using the principle of the multiple-root stabilization is one way to overcome tooth mobility. Temporary splinting aims to prevent pathological migration, restore masticatory function, stabilize teeth before/after surgery, and evaluate the prognosis of periodontal treatment. The use of intracoronal splint is still controversial because there are only a few studies that have evaluated the effect of splinting on periodontal health. We report two cases to evaluate the effect of intracoronal splint on periodontal treatment. Two periodontal cases that use intracoronal splint before, during, and after periodontal regenerative therapy using bone graft. Causes of tooth mobility were removed and the splinting principles, terms and guidelines were mastered to get the maximum results of periodontal treatment. Both cases were evaluated radiographically 10 months after treatment. In these cases, intracoronal splint has supported the therapy before, during, or after surgery. Splinting is only for adjunctive therapy, and does not serve as the sole method in getting occlusal stability.

Abstract

Background: Dental implant is one of an alternative solutions reconstruction therapy for missing teeth. Complication of dental implant could occurs and leading to implant failure. In order to restore the complication, surgical treatment with guided bone regeneration (GBR) is indicated. The potential use of bone substitutes is widely known to be able to regenerate the bone surrounding the implant and maintain bone volume. Purpose: The study aimed to demonstrate the effectiveness of implant-bone fully coverage by using sandwich technique of biphasic calcium phosphate (BCP) and demineralized freeze-dried bone allografts (DFDBA) bone substitutes combined with collagen resorbable membrane. Case: A 24-year-old male came with diagnosis of peri-implantitis on implant #11. Clinical finding indicated that implant thread was exposed on the labial aspect. Case management: After initial therapy including oral hygiene improvement performed, an operator did a contemporary GBR to correct the defect. Bone graft materials used were 40% β-tri calcium phosphate (β-TCP)-60% hydroxyapatite (HA) on the outer layer and DFDBA on the inner layer of the defect. Resorbable collagen membrane was used to cover the graft. Conclusion: GBR with sandwich technique could serve as one of the treatment choices for correcting an exposed anterior implant that would enhance the successful aesthetic outcome.

Abstract

Background: Acute myeloid leukemia (AML) is a bone marrow cancer, a malignant disease that triggering the cells develops into different types of blood cells. It is widely recognized that the main manifestation of AML could be gingival hyperplasia and bleeding. Occasionally, an initial diagnosis of leukemia is made after a dental examination. In relation to systemic diseases, gingival enlargement could constitute the intensification of an existing inflammation initiated by dental plaque, or a manifestation of the systemic disease independent of the inflammatory condition of the gingiva. Gingival enlargement negatively affects the quality of life, especially nutritional intake. Purpose: This study aimed to report on gingival enlargement in AML patients, dental management of this condition and considerations when treating patients. Case: A 46 year-old woman diagnosed with AML who chiefly complained of gingival enlargement in all parts of the mouth which restricted her nutritional intake. Case management: The subject attended the clinic twice where nonsurgical treatment for the gingival enlargement, supragingival scaling and dental health education to maintain her oral hygiene was carried out. Unfortunately, she did not return for follow-up appointments due to having already passed away. Information about AML and its relation to gingival enlargement contained in the literature is also reviewed. Conclusion: In conclusion, gingival enlargement represents one oral manifestation of AML. This condition is related to and affects the nutritional intake of the patient.

Abstract

Objective: Periodontitis is an inflammatory disease of the supporting tissues of the teeth caused by specific microorganisms, resulting in decay of the teeth and damaging of supporting tissues. Systemic conditions and increasing age will affect the periodontal tissue due to amongst others changes in the immune defense system. The classification for periodontal diseases used today is the 1999 American Academy of Periodontology (AAP) classification.  The latest development was in 2017, when the AAP published a new classification that classifies periodontitis based on stage and grade and is the classification that is widely used today. Research on determining the stage and grade of periodontitis using the 2017 AAP classification is still very rare in Indonesia.Material and Methods: In this study, a cross sectional analytical descriptive approach was used to determine the stage and grade of periodontitis according to the classification of periodontal disease based on the 2017 AAP in relation to the age of patients with systemic conditions and diseases. A total of 331 medical records from patients treated at the RSKGM FKG UI in the 2017 – 2019 visit were researched.Results: The results have shown that the highest stage and grade in all research subjects was stage III grade C, , this stage and grade was found in subjects who smoke. Different results were found in research subjects with Diabetes Mellitus or Hypertension namely stage III grade B.Conclusion: Patients with hypertension without any systemic disease and non smoking should be advised to request a test for diabetes mellitus.
