Abstract

Chronic Venous Leg Ulcers (CVLUs) are vascular diseases and require complex wound care from a patient, wound, health personnel and dressing factors. Cadexomer Iodine (CI) and Polyhexamethylene Biguanide (PHMB) are antimicrobial and broad-spectrum wound washing fluids. The uncontrolled infection will complicate wound healing due to bacteria or plankton from the biofilm stage to infection, which causes a long healing time. Purpose: to determine the effectiveness of the combination of Polyhexamethylene biguanide and Cadexomer Iodine on venous leg ulcer wound healing. Methods: This study is a case study using CVLUs as a sample, with pre-test and post-test designs, patients with CVLUs received topical antimicrobial CI and PHMB interventions on wounds for 7 weeks. The sample in this study used one sample with a pre-post treatment design for seven days.. The instrument uses the Bates Jensen Wound Assessment Tool. Results: The combination of CI and PHMB was shown to improve the healing of CVLUs by controlling bacterial infection in wounds. In the 2nd week, the wound was 26cm x 15cm, the slough was reduced, and the red base wound increased. In the 3rd week, the wound was stage 4, and the granulation increase was 75%. In the 7th treatment, the wound size scale increased 7cmx4cm, the wound became stage 2, and a significant increase in epithelialization was 75%. Conclusion: The combination of PHMB and CI has been shown to control infection at the biofilm stage to critical colonization, reduce odour and significantly control infection, and increase granulation and epithelialization in CVLUs

Abstract

Diabetes mellitus is a metabolic disorder characterized by high sugar levels associated with abnormal carbohydrate, fat, and protein metabolism caused by decreased insulin secretion or decreased insulin sensitivity effectively. One of the nursing problems that always arise in people with Diabetes Mellitus is the risk of unstable blood glucose levels. Ignorance of diet is one of the efforts to achieve treatment goals in type 2 diabetes mellitus patients. Therefore, health education is needed for type 2 DM patients. The author's aim is to provide nursing care for blood glucose instability in Mr D with Type II Diabetes Mellitus. Actions by providing implementation The implementation of nursing care actions carried out is conducting a diet therapy program for clients which is carried out for 3 days, accompanied by daily GDS measurements. Education is education or training regarding knowledge and skills in the management of diabetes mellitus given to every patient with diabetes mellitus. Providing education includes basic knowledge about diabetes mellitus, meal planning, physical activities and foot care

Abstract

Benign Postat Hyperlasia (BPH) is the use of the prostate gland that is benign only in men, usually in middle or old age. The incidence of benign prostatic hyperplasia (BPH) in Indonesia has placed BPH as the number two cause of urinary tract stones. This paper uses a descriptive case study in the form of a nursing care case study which includes assessment, nursing diagnosis, nursing plan, nursing implementation, and nursing evaluation. Data collection techniques were carried out by interview, observation, and documentation. The results of the case study Mr. A with postoperative TURP obtained one nursing diagnosis, namely acute pain. After nursing actions, the patient shows a change in condition for the better, namely the client can adapt to postoperative pain. conclusion of the case study on Mr. A with acute postoperative pain TURP is a problem resolved according to the goals and expected outcome criteria.

Abstract

Pain conditions are very often complained of in hypertensive patients. Pain often occurs in the nape of the neck due to increased pressure on the walls of blood vessels around the neck, increased vascular pressure to the brain so that there is an emphasis on the neck muscle fibers. This study aims to describe the nursing care process and the use of a warm compress intervention combined with deep breathing relaxation techniques. The results showed that the patient was diagnosed with hypertension. The patient complained of dizziness and neck pain like being crushed by a heavy object, a scale of 5 due to stress and a lot of pressure. Pain occurs approximately 1 hour and always occurs on every wake. Blood pressure reached 190/100 mmHg, pulse 91 beats per minute, RR 20 beats per minute, and temperature 36.30C. The warm compress intervention combined with deep breathing was carried out for 3 days and was proven to be able to reduce the pain scale felt by hypertensive clients.

Abstract

Latar belakang: Hipertensi adalah terjadinya peningkatan tekanan darah diatas normal. Prevalensi hipertensi di kota Yogyakarta sebesar 25%. Tujuan: Untuk mengetahui prinsip-prinsip dokumentasi asuhan keperawatan pada pasien hipertensi. Metode penelitian: Desain penelitian yang digunakan dalam penelitian ini dengan pendekatan kualitatif dengan model multikasus terpancang. penelitian dilakukan di ruang Elisabeth Gruyter’s 3 Rumah Sakit Panti Rapih Yogyakarta pada bulan Januari- September tahun 2017 sebanyak 161 pasien hipertensi. Jumlah sampel yang di ambil pada penelitian ini adalah dua Rekam Medis pasien yang menderita hipertensi disertai komplikasi dan hipertensi tanpa komplikasi. Kesimpulan dan Saran: Pasien kasus 1 menderita hipertensi tanpa komplikasi dan terkontrol dengan riwayat CA Cerebri, sedangkan pasien kasus 2 menderita hipertensi disertai dengan komplikasi CKD (chronic kidney disease) dan tidak terkontrol. Prinsip-prinsip dokumentasi sudah sesuai dengan data yang ada pada pasien. Diharapkan hasil penelitian ini dapat menjadi referensi untuk meningkatkan pelayanan optimal tentang penatalaksanaan untuk penderita hipertensi.

Abstract

Latar Belakang: Fraktur adalah penyebab kematian ketiga di Indonesia setelah penyakit jantung koroner dan tuberculosis. Fraktur disebabkan oleh trauma atau tenaga fisik, kecelakaan, baik kecelakaan kerja maupun kecelakaan lalu lintas. Tujuan: Mampu melaksanakan asuhan keperawatan pada post operasi open reduction internal fixation dengan masalah gangguan mobilitas fisik di Ruang Dahlia RSUD dr. R. Goeteng Taroenadibrata Purbalingga. Metode: Desain penelitian deskriptif dalam bentuk studi kasus dengan pendekatan proses keperawatan yang terdiri dari pengkajian, diagnosa keperawatan, intervensi, implementasi, dan evaluasi. Teknik pengumpulan data menggunakan observasi, wawancara, pemeriksaan fisik, dan dokumentasi. Hasil: penulis merumuskan diagnosa keperawatan yang muncul yaitu gangguan mobilitas fisik berhubungan dengan gangguan musculoskeletal. Intervensi keperawatan yang dilakukan pada Ny. R sesuai dengan diagnosa yang ditegakkan dan disesuaikan dengan teori yang ada. Intervensi keperawatan yang dilakukan yaitu latihan rentang gerak (I.05177) disusun sesuai dengan masalah yang ditemukan berdasarkan hasil pengkajian yang dilakukan secara mandiri maupun berkolaborasi. Penulis melakukan implementasi keperawatan sesuai dengan intervensi yang telah ditetapkan sebelumnya dengan menggunakan Standar Intervensi Keperawatan Indonesia (SIKI) yang sudah ditetapkan sebelumnya. Kesimpulan: Evaluasi keperawatan yang dilakukan pada tanggal 11-13 Januari 2022 dilakukan dengan SOAP antara lain yaitu subjektif pasien mengatakan pergerakannya mulai tidak sulit dan nyeri mulai hilang. Objektif TD 160/85 mmHg, N: 85x/menit, S: 36,3 C, dan RR: 20x/menit, pasien sudah bisa mengikuti gerakan ROM aktif walaupun sangat terbatas, pasien dan keluarga memahami tentang materi yang disampaikan saat penkes. Analisis masalah teratasi sebagian dan lanjutkan intervensi.

Abstract

Purpose: to determine the description of changes in Mean Arterial Pressure (MAP) of patients after general anesthesia at Juanda Kuningan Hospital, West Java. Methods: This research is descriptive quantitative with a cross sectional approach. The sample in this study were all surgery patients with general anesthesia who met the criteria, while the sampling technique used was total sampling resulting in 100 respondents. Data analysis using univariate test. Findings: The results of the analysis showed that the most dominant age of respondents was early elderly (34%), with female gender (58%) and the most dominant Body Mass Index (BMI) was normal (91%). Normal MAP (70-100 mmHg) was obtained before induction of anesthesia as much as (77%), normal MAP (70-100 mmHg) after general anesthesia in the recovery room as much as (93%%). Conclusions: These results describe that changes in MAP values after general anesthesia can occur and the majority of pre-induction anesthesia MAP values of respondents will experience a decrease in MAP values during post-general anesthesia in the recovery room. So it is very important to maintain normal MAP values (70-100 mmHg) so as to reduce the risk of postoperative complications that can occur in patients.

Abstract

Background: Every profession and employee in both developed and developing countries is affected by occupational stress, which is a global problem. Due to the great burden and responsibility of safeguarding human lives, the anesthesia care profession is particularly vulnerable to stress. And high levels of occupational stress can cause health problems, such as mental and behavioral disorders such as fatigue, anxiety, and depression, as well as other physical disorders such as cardiovascular disease and musculoskeletal disorders that affect service functions. The role of expert anesthesiologist has a function in pre, intra, post anesthesia care services and emergency management. Objective: To identify the characteristics of anesthesiologist's work stress level based on age, gender, working period, working hours and latest education in the surgical room. Methods: This research is descriptive quantitative with a cross sectional approach. The study population was 39 respondents, the focus of observation in this study was a description of the stress level of anesthesia administrators in the surgical room of the Banyumas Regency Hospital. Results: This study shows the level of stress with mild stress category with a total of 19 respondents with a percentage (48.7%) and moderate work stress as many as 4 respondents with a percentage (10.3%). Conclusion: The results of this study indicate the presence of mild and moderate work stress in surgical room anesthesiologists at the Banyumas Regency Regional Hospital.

Abstract

Background: The performance of health workers is a very crucial issue to be studied in order to maintain and improve health services. The impact that occurs if the performance of health workers in the hospital is not good can cause patients and families to feel dissatisfied, patients and families will not use health services again. One of the factors that affect performance is motivation. Objective: Knowing the relationship between motivation and the performance of anesthesiologists in the Banyumas Regency area. Research: The research method is correlation analytics research with a cross sectional approach. The sampling technique used total sampling, a total of thirty-nine respondents. Data collection using work motivation and performance questionnaires. Bivariate analysis using spearman-rank. Results: The results of the study obtained a percentage of 39 respondents of anesthesiologists in the Banyumas Regency area with sufficient motivation as many as 28 respondents (71.79%) and respondents who had sufficient performance as many as 31 respondents (79.98%). The results of spearman-rank analysis showed a significant relationship, work motivation with the performance of anesthesiologists with a correlation value of 0.733 and a 2-tailed sig of 0.000. Conclusion: Advice for the anesthesiologist profession who is especially incorporated in the organization at the DPC IPAI Banyumas Branch Secretariat which researchers recommend is expected to improve the quality and quantity of health services to patients and families, namely by carrying out predetermined procedures and friendly, so that patients and families feel comfortable and served with satisfaction for the services provided.
