Abstract

Introduction: Osteoarthritis(OA) is a musculoskeletal disorder that causes changes in the joints progressively slowly and intermittently. Usually, in the field, muscle strengthening exercises are often found that only focus on the agonist's muscle. Whereas antagonistic muscles also play a role and are even more dominant. The most recommended intervention to be able to activate the two muscles is in the form of taskspecific exercises. The basic concept of task-specific exercise itself is muscle coactivation. To be able to see muscle activity being activated, task-specific exercises are combined with visual surface electromyography (SEMG) biofeedback. Case Presentation: the patient is 57-years old woman and works as a housewife. Patient from Solo, Central Java. Examination of the patient was found to have 3 types of causes such as age more than 45 years or more, morning stiffness for less than 30 minutes, and pain when doing activities. Management and Outcomes: Given treatment in the form of task-specific training combined with visual surface electromyography (SEMG) biofeedback to increase the walking speed and functional activity of the patient. Furthermore, the measuring instrument used for walking speed is the 10-meter walking test and WOMAC Discussion: A study revealed that the addition of a visual surface EMG biofeedback component in task-specific functional training had a positive effect on increasing muscle function and strength. The authors assume that the addition of visual biofeedback can have a positive impact on the rehabilitation process. Conclusion: Giving this intervention can provide a statistically significant improvement and can be implemented clinically to increase walking speed in individual patients and this can provide a significant improvement in functional ability in patients with osteoarthritis of the knee.

Abstract

Introduction: Osteoarthritis Knee is a symptom that often comes in elderly people, a chronic progressive condition that causes movement disorders and interferes with activities. The intervention is task specific training. Task specific training is an effective rehabilitation program for balance, walking speed and reducing pain in OA. External Clue is the therapist providing information about the performance of motor skills so that the patient can get feedback from the patient himself. Case Presentation: Mrs. S is 68 years old. The patient has a height of 155 cm and a weight of 70 kg. Mrs. S works as a housewife. The patient had this complaint several years ago. There is no history of illness in the family. Supporting data in the form of X-ray with the interpretation of knee osteoarthritis grade 2. The patient also suffered from mild deformity of the foot towards varus, the presence of crepitus, but the patient did not have local oedema, no pes anserinus bursitis and no joint tendeness. Management and Outcome: The patient underwent treatment consisting of Task Specific Training based on Co-Contraction with External Clue. The patient is instructed to contract the muscles while walking. Coactivation occurs when the flexor muscles are active during the moment of extension. (External Clue) is the therapist providing additional information about the performance of motor skills so that the patient can get feedback from the patient himself. The osteoarthritis instrument used is WOMAC (Western Ontario and McMaster Universities) pain T1: 8 to T7: 3, stiffness T1:2 to T7: 2, physical function T1: 24 to T7: 7. Discussion: The subjects of this study have typical symptoms of osteoarthritis of the knee, so there is no need for additional examinations to diagnose knee osteoarthritis in these subjects. This is supported by international guidelines which state that individuals with signs such as individuals aged >45 years, pain when doing activities, and morning stiffness <30 minutes can be directly diagnosed with knee osteoarthritis. Task specific has the concept of muscle cocontraction where (simultaneous coordinated activity of agonist and antagonist muscles) is considered as the main mechanism for joint stabilization, load distribution, and control of movement during gait. Muscle co-contraction for activities of daily living (ADL) can be combined with different muscles. It is therefore important to understand muscle co-activation strategies across different ADLs and across different muscle combinations. Conclusion: Obtained an increase in functional ability in patients with knee osteoarthritis.

Abstract

The purpose of this article is to show the benefits of physiotherapy modalities in the form of Trancutaneus Electrical Nerve Stimulation (TENS) and Neuro Mobilization in patients with Cervical Root Syndrome (CRS). It begins with the presence of auctivity will cause excessive effect on a person, such as a complaint on the muscular system (musculoskeletal) in the form of complaints of pain, pain, fatigue and other. This research using this type of research is a case study. The implementation of the research made in In RSUP Dr. Sardjito. After getting physiotherapy treatment for 3 times using TENS and Neuro Mobilization obtained positive results such as a decrease in pain silent, press, and motion, the presence of reelkatan LGS, and increased aktivity. In addition, it was found that the presence of a decrease in pain in cervical, i.e. there is the addition of ROM on some field of motion. Another thing is there is a drop points NDI which signifies that the patient is gradually able to release its dependence with other people in the activity diary.

Abstract

Introduction: Ulcus Decubitus is a condition that is serious with figures of morbidity an mortality in the age of advanced and would be a burden families with the cost of high care. Ulcer sores can occur at any stage of the age, but the case is a matter which specific ally on advanced age due to immobilization. Specificity lies in the incidence of even that are closely related to immobilization which is a problem major in patients geriatric1. The patient slipped about 8 months ago, since then the patient has been unable to walk and move. The left leg hurts to move, and until now it's getting stiffer and can't be straightened, then a wound appears on the top of the left leg. At first it was just red and like a blister, but as time went on the wound became wet, Case Presentation: The method used in this study is a case report of one patient at RSU ‘Aisyiyah Ponorogo Hospital with identity Mrs.P with the age of 98 years. Management and outcome: The provision of positioning, active passive exercise and TENS is one of the effective methods to reduce complaints of pain due to ulcus decubitus. Discussion: One of the actions to reduce the incidence of pressure sores is by giving the side position. The reason for giving the tilted position is because this position is able to prevent the skin from rubbing and tearing the tissue, thereby reducing the incidence of pressure sores2. According to the researcher, the tilted position also has advantages, namely it does not take much time and is easy to implement, tools and materials are easy to obtain and the patient's waiting family can carry out their own in reducing the patient's pressure sores3. Conclusion: Handling physiotherapy in the form of positioning, active passive exercise and TENS can reduce pain complaints that Mrs.P.

Abstract

Introduction: Work from Home during the pandemic covid-19 made almost all Indonesian students online learning activities at home. The use of laptops in a sitting position for too long causes Myalgia Upper Trapezius in students. A study says complaints of 86.4% in the neck and 76.2% in the shoulders during WFH. The purpose of writing is to find out the benefits of Short- Wave Diathermy, Myofascial Release and Stretching to reduce pain and increase joint range of motion. Case Presentation: The patient used was a female student with complaints of pain and limitation of motion in the neck area and was chronic. Pain is caused by a static position sitting in front of the laptop for too long, causing limited neck movement during WFH. Management and Outcome: Short Wave Diathermy, Myofascial Release and Stretching can reduce pain intensity and increase joint range of motion in women with Myalgia Upper Trapezius. Discussion: After 3 weeks physiotherapy, the results were evaluated 3 times. Get the results of reducing silent pain with VAS evaluation 1: 4.5 evaluation 3: 1.1 tenderness evaluation 1: 5.1 evaluation 3: 2.1 and motion pain evaluation 1: 5.3 evaluation 3: 2.5. An increase in the range of motion neck of the joint also occurs in movement with Goniometer, Flexion evaluation 1: 260 evaluation 3: 380, Extension evaluation 1: 300 evaluation 3: 450, Side right flexion evaluation 1: 380 evaluation 3: 440, Side left flexion evaluation 1: 200 evaluation 3: 440, Right rotation evaluation 1: 400 evaluation 3: 480, Left rotation evaluation 1: 300 evaluation 3: 470. Application of Short-Wave Diathermy, Myofascial Release and Stretching can reduce pain and increase the range of motion of joints in patients with Upper Trapezius Myalgia. Conclusion: Physiotherapy management in the Myalgia Upper Trapezius after carrying out the program for 3 weeks with the intensity of 3 times the threat in 1 week reduce pain and increase ring of motion. Ergonomic position and consistency patient during physiotherapy with SWD, Myofascial release and stretching is very helpful in-patient recovery.

Abstract

Introduction : ACL injuries represented more than 50% of injuries in the knee and affected more than 200,000 peoples in the United States each year. The ratio of the incidence of ACL injuries in men and women was 66.7% versus 33.3%. The first phase is a key in post operative rehabilitation of cruciate ligament injury. Rehabilitation in the first phase aims to control inflammation, assist the process of tissue formation, protect injury, reduced pain, and muscle activation with isometric. The Problem at the inflammatory stage (phase 1) are usually in the form of uncontrolled inflammation characterized by redness, heat, pain, swelling, and impaired knee function. Case Presentation: Based on the ACL Reconstruction Guidelines Rehabilitation protocol in Skeletally Mature Athletes. A 30-years-old man is a footballer with a weight of 55 kg and a height of 175 cm, complaining of pain and stiffness in his right knee when performing daily activities. On March 25, 2021 the patient experienced a wrong movement in a pivoting position and a collision occurred with another player on the lateral side of the right knee. Management and Outcome :The Patient received physiotherapy program with exercises focusing on swelling and pain management, increasing the range of motion and muscle strength of the right knee and providing a home exercise program according to phase 1 (0-4 weeks). Measurement of swelling with midline, measurement of pain with Numeric Rating Scale and measurement of Range of Motion with Goneometer. Discussion : The patient did physiotherapy sessions twice a week for four weeks. The measurement results showed an increased in the range of motion, a decreased swelling and pain in the right knee. Factors that affected tissue healing including the location of the graft used, management of the preparation of the rehabilitation program, awareness of the patient doing home exercise programs and avoiding contraindications of therapy. Conclusion : The first phase in management of the ligament injury rehabilitation program is very important to accelerate the tissue healing process as well as the requirements for the next phase. If the first phase is not managed properly, there will be a risk of various problems occurring in the subsequent phases and will slow down the tissue healing process and functional activity.

Abstract

Aim : this research aim to know therapeutic exercise appplication with New Bobath Concept approach combining with Core Stability Exercise to improve balance. Quasi experimental design A-B-A use 2 samples measuring every series at a control grup ar RSPAL Dr Ramelan Surabaya. Data analysis with simple descriptive statistic to get obvious image about intervention outcome, using line graphic as an interpretation of treatment and outcome. Based on the grapic shows there is improvement after Core Stability Exercise and New Bobath Concept approach get improvement in 2 items in first week and 2 items in third week. In second week only 1 item get improvement. Conclusion is there is improvement effect in balance by treatment Core Stability Exercise and New Bobath Concept approach.

Abstract

Osteoarthritis (OA) is a degenerative and chronic disease of the knee joint. The recommended intervention is Task-Specific Training with Co-Contraction, namely muscle co-activation (coordinated agonist and antagonist muscle activity simultaneously) for joint stability, even in OA knee individuals who function dynamically as a stabilizer, controlling movement during walking. Augmented feedback (External Clue) commonly used in clinical practice and can play an important role in motor learning. Case Presentation: Mrs. S is 68 years old. The patient has a height of 155 cm and a weight of 70 kg. Mrs. S works as a housewife. The patient had this complaint several years ago. There is no history of illness in the family. Supporting data in the form of X-ray with the interpretation of knee osteoarthritis grade 2. Management and Outcome: The patient underwent treatment consisting of Task-Specific Training based on Co-Contraction/ Co-contraction with External Clue. The data analysis used in this study is a single case report. The design for this study used the ABA design. The sampling technique is the consecutive method with 1 person who fits the inclusion criteria. The research measurement instrument related to the quality of walking used the 6 Minute Walking Test (6MWT). The data analysis technique used descriptive analytic. This exercise program uses Task- Specific Muscle Co-contraction Training with External Clue for one week starting from January 7 to January 14, 2020, with Ethical Clearance Letter no. 2682/B.2/KEPKFKUMS/ XI/2019 conducted by the Health Research Ethics Commission (KEPK) FK UMS. Discussion: The subjects of this study have typical symptoms of osteoarthritis of the knee, so there is no need for additional examinations to diagnose knee osteoarthritis. This is supported by the existence of pain, stiffness, and locomotor which are common symptoms of knee OA that is focused on a functional approach, namely component important in locomotor retriction is running. Task- specific training is understanding how distraction-based exercise can affect certain functional tasks which helps improve exercise protocols for people with knee OA. The concept of muscle coactivation from a specific task is muscle co-activation (coordinated agonist and antagonist muscle activity simultaneously) for joint stability, even in OA knee individuals who function dynamically as a stabilizer, controlling movement during walking. Muscle co-activation to help stabilize the joints in the ligaments and distribute pressure on the joint surfaces. Conclusion: There was a significant increase in individual walking distance of osteoarthritis knee patients.
