Abstract

Abstract: Hospital management information system (HMIS) processes and integrates the entire flow of hospital service processes to obtain precise and accurate information. This study aimed to analyze the use of HMIS in the management of drugs and consumable medical materials at the Pharmacy Installation and procurement of medical equipment in RSU GMIM Siloam Sonder. This was a qualitative and descriptive study using purposive sampling technique. There were nine informants in this study. Data were obtained from in-depth interviews, document searches, direct surveys, and comparing with theories in the literatures. The results showed that managements of drugs and consumables medical materials at the Pharmacy Installation had used the HMIS which supported the development of service performance in the hospital, improved users’ skills, easy to be understood and learnt, and hospital employees received training in operating the HMIS, however, it was not optimal. Its operation was also supported by a computer that was easy to operate, however, some difficulties used to occur. Moreover, the process of procuring medical equipment did not involve the HMIS. In conclusion, the HMIS at RSU GMIM Siloam Sonder has been used in management of drugs and consumables medical materials at the Pharmacy Installation, albeit, it has not been used in the process of procuring medical equipment. Keywords: HMIS; drugs and consumable medical materials; procurement of medical equipment Abstrak: Sistem informasi manajemen rumah sakit (SIM RS) memroses dan mengintegrasikan seluruh alur proses pelayanan rumah sakit untuk memperoleh informasi secara tepat dan akurat. Penelitian ini bertujuan untuk menganalisis pemanfaatan SIM RS dalam pengelolaan obat dan bahan medis habis pakai (BHP) di Instalasi Farmasi dan pengadaan peralatan medik di RSU GMIM Siloam Sonder. Jenis penelitian ialah deskriptif kualitatif dengan menggunakan teknik purposive sampling. Informan penelitian ini berjumlah sembilan orang. Data diperoleh dari hasil wawancara mendalam, penelusuran dokumen, survei langsung, serta membandingkan dengan teori-teori pada literatur. Hasil penelitian menunjukkan bahwa kegiatan pengelolaan obat dan BHP di Instalasi Farmasi RSU GMIM Siloam Sonder sudah memanfaatkan SIM RS yang mendukung pengembangan kinerja pelayanan di rumah sakit, meningkatkan keterampilan penggunanya, mudah dipahami dan dipelajari, serta karyawan mendapatkan pelatihan dalam mengoperasikan SIM RS, namun belum maksimal. Pengoperasiannya didukung juga oleh komputer yang mudah dioperasikan, namun masih sering terdapat kesulitan. Proses pengadaan peralatan medik belum melibatkan SIM RS. Simpulan penelitian ini ialah SIM RS di RSU GMIM Siloam Sonder sudah dimanfaatkan dalam kegiatan pengelolaan obat dan BHP namun belum dimanfaatkan dalam proses pengadaan peralatan medik. Abstract

Abstract: Provision of health services is closely related to human resources in achievement of good implementation of services especially during the covid-19 pandemic. The need for health workers in accordance with this pandemic situation is very necessary to provide qualified health services. This study aimed to analyze the need for number of nurses in terms of workload on the inpatient installations of RSUD Dr. Jhon Piet Wanane, Sorong, due to the increasing need for health services during the Covid-19 pandemic. This was a quantitative and descriptive study. Informant selection was based on the knowledge and understanding of the nurses on duty in the inpatient rooms. Calculation of the number of health personnel needs was carried out using a calculation formula which was number of cases handled multiplied by the number of treatment times then divided by the effective working time of nurses in one year. The results showed that the highest workload was in the inpatient class I and II, which was 1,065 minutes (24%), however, it was still below standard level. In conclusion, there is no high workload in RSUD Kabupaten Sorong. Hospital management could use a real formula adapted to workload situation to calculate the need for nurses and review the application model of nursing care management. Keywords: needs of human resources; health services; workload Abstrak: Pemberian pelayanan kesehatan berkaitan erat dengan penyediaan sumber daya manusia kesehatan (SDMK) agar penyelenggaraan pelayanan kesehatan dapat berjalan dengan baik terlebih pada masa pandemi Covid-19. Kebutuhan tenaga kesehatan yang memadai pada kondisi pandemi saat ini sangat diperlukan untuk memberikan pelayanan kesehatan yang berkualitas. Penelitian ini bertujuan untuk mengetahui kebutuhan jumlah tenaga perawat ditinjau dari beban kerja pada instalasi rawat inap RSUD Dr. Jhon Piet Wanane Kabupaten Sorong akibat meningkatnya kebutuhan akan pelayanan kesehatan pada masa pandemi Covid-19. Jenis penelitian ialah kuantitatif deskriptif. Informan pene-litian dipilih berdasarkan pengetahuan dan pemahaman perawat yang bertugas pada ruang rawat inap. Perhitungan jumlah kebutuhan tenaga kesehatan menggunakan rumus perhitungan banyaknya kasus yang ditangani dikalikan dengan jumlah waktu tindakan kemudian dibagi dengan waktu kerja efektif perawat dalam satu tahun. Hasil penelitian mendapatkan beban kerja tertinggi di ruang rawat inap kelas I, II yaitu 1.065 menit (24%) yang masih di bawah nilai standar. Simpulan penelitian ini ialah tidak ditemukan beban kerja yang tinggi di RSUD Kabupaten Sorong. Manajemen RSUD dapat menggunakan formula riil situasi di lapangan untuk menghitung kebutuhan tenaga perawatan di ruangan dan meninjau kembali model penerapan manajemen asuhan keperawatanKata kunci: kebutuhan sumber daya manusia kesehatan; pelayanan kesehatan; beban kerja

Abstract

Abstract: Currently, a concept of health services by the hospital, Hospital without Walls, is intro-duced.  This concept moves health services from the hospital to outside the hospital building.  This study aimed to determine the application of the concept of a hospital without walls in Dermatovene-reology health services. This was a qualitative research design (grounded research) using informants through in-depth interviews, face-to-face and indirect. This study was carried out at Maria Walanda Maramis General Hospital, North Minahasa. Data were collected manually by making a transcript then were compiled in a matrix form and analyzed using the inductive analysis method. The results showed that the Hospital without Walls could be implemented, however, there were obstacles, internally and externally. The internal obstacles were that the hospital did not have a legal basis, SOP, and supporting facilities such as applications for this activity, payment for services of doctors and paramedics, professional human resources in the field of information technology to manage these activities. The external obstacles were the absence of regulation of BPJS for execution and payment of claims. In conclusion, the hospital without walls concept can be applied to the dermato-venereology health services at Maria Walanda Maramis General Hospital, North Minahasa, North Sulawesi Province.Keywords: Hospital without Walls; dermatovenereology services Abstrak: Saat ini berkembang suatu konsep pelayanan kesehatan oleh Rumah Sakit disebut Hospital without Walls yang memindahkan pelayanan dari Rumah Sakit ke luar bangunan Rumah Sakit. Penelitian ini bertujuan untuk mengetahui penerapan konsep Hospital without Walls pada pelayanan kesehatan kulit dan kelamin di Rumah Sakit Umum Maria Walanda Maramis Kabupaten Minahasa Utara. Desain penelitian ialah kualitatif (grounded research) dengan menggunakan informan (10 orang) melalui wawancara mendalam, secara langsung dengan tatap muka dan tidak langsung. Data yang terkumpul, diolah secara manual dengan membuat transkrip kemudian disusun dalam bentuk matriks dan selanjutnya dianalisis menggunakan metode analisis secara induktif. Hasil penelitian mendapatkan bahwa penerapan Hospital without Walls dapat dilaksanakan walaupun ditemukan hambatan secara internal dan eksternal. Hambatan internal ialah Rumah Sakit belum mempunyai dasar hukum, SOP, dan fasilitas penunjang seperti aplikasi untuk kegiatan ini, serta pembayaran jasa untuk dokter dan paramedis, SDM profesional di bidang informasi teknologi untuk mengelola kegiatan ini. Hambatan eksternal yaitu belum adanya regulasi BPJS untuk pelaksanaan dan pembayaran klaim. Simpulan penelitian ini ialah konsep Hospital without Walls dapat diterapkan pada pelayanan kesehatan kulit dan kelamin di Rumah Sakit Umum Maria Walanda Maramis, Kabupaten Minahasa Utara, Provinsi Sulawesi Utara.Kata kunci: Hospital without Walls; pelayanan kesehatan kulit dan kelamin

Abstract

Abstract: Level of patient satisfaction in hospitals must be evaluated periodically, accurately, and continuously since it describes the quality of services provided by the hospital. Patient satisfaction can be achieved if the patient receives services according to what is needed and expected. This study aimed to analyze factors related to patient satisfaction with services at Rumah Sakit (RS) Bhayangkara TK III Manado. This was a quantitative study with a cross sectional design conducted at RS Bhayangkara TK III Manado. The population in this study were 3,055 patients with total samples of 231 patients taken by using simple random sampling. Data were obtained by using questionnaires, and were analyzed univariately, bivariately with the chi-square test, and multivariately with the logistic regression test. Statistical calculation used SPSS program. The results of the chi square test were, as follows: reliability p=0.000; OR=77.407; responsiveness p=0.000; OR=234.3; assurance p=0.000; OR=120.375; empathy p=0.000; OR=194,33; and tangibles p=0.000; OR=238,889. The multivariate analysis using the logistic regression test showed that the highest Exp (B) value of 128.412 was on the empathy aspect. In conclusion, reliability, responsiveness, assurance, empathy, and tangibles are related to patient satisfaction with the services at RS Bhayangkara TK III Manado. The most related factor to patient satisfaction is empathy. Keywords: patient satisfaction; service quality; hospital   Abstrak: Pengukuran tingkat kepuasan pasien di rumah sakit wajib dilakukan seara berkala, akurat dan berkesinambungan oleh karena tingkat kepuasan menggambarkan kualitas pelayanan yang diberikan oleh rumah sakit. Kepuasan pasien dapat tercapai apabila pasien menerima pelayanan sesuai dengan yang dibutuhkan dan diharapkan. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan kepuasan pasien terhadap pelayanan di RS Bhayangkara TK III Manado. Jenis penelitian ialah kuantitatif dengan desain potong lintang. Populasi dalam penelitian ini yaitu 3.055 dengan jumlah sampel 231 pasien yang diambil secara simple random sampling. Data diperoleh dengan menggunakan kuesioner, kemudian dianalisis secara univariat, bivariat dengan menggunakan uji chi- square, dan multivariat menggunakan uji regresi logistik. Perhitungan statistik menggunakan program SPSS. Hasil uji chi-square mendapatkan untuk aspek reliability p=0,000; OR=77,407; responsiveness p=0,000; OR=234,3; assurance p=0,000; OR=120,375; empathy p=0,000; OR=194,33; dan tangibles p=0,000; OR=238,889. Hasil uji regresi logistik mendapatkan nilai Exp (B) tertinggi yaitu 128,412 pada aspek empathy. Simpulan penellitian ini ialah tangibles, responsiveness, reliability, assurance dan empathy berhubungan dengan kepuasan pasien terhadap pelayanan di RS Bhayangkara TK III Manado. Abstract

Abstract: Telemedicine can expand access to services for patients in urban to rural areas with various benefits and obstacles in its impelementation. This study aimed to analyze the community perception and knowledge about telemedicine. This was a qualitative and descriptive study using primary, secondary, and tertiary data in the form of in-depth interviews with informants and direct observation. The informants in this study consisted of five key informants and five triangulation informants. Data analysis was carried out through the stages of data reduction, data presentation, data verification or conclusion stages, and triangulation stages. The results showed that telemedicine use in the community services was carried out to minimize face-to-face meetings between patients and doctors by using technology. The results of interviews with triangulation informants showed that the community knowledge about telemedicine was still lacking, therefore, telemedicine use in the community was still limited to those who were used to technology. There were several obstacles of telemedicine use inter alia technical barriers related to unstable internet access and low digital literacy in the community, for that reason, the telemedicine services was not optimally used. In conclusion, the community perception and knowledge about telemedicine use as a consultation media is good enough. Keywords: telemedicine; health care; community perception and knowledge   Abstrak: Telemedicine dapat memperluas akses pelayanan, baik bagi pasien yang berada di wilayah perkotaan hingga pedesaan dengan berbagai manfaat dan hambatan dalam pelaksanaannya. Penelitian ini bertujuan untuk mengetahui persepsi dan pengetahuan masyarakat mengenai telemedicine. Jenis penelitian ialah kualitatif-deskriptif dengan menggunakan data primer, data sekunder dan data tersier berupa wawancara mendalam terhadap informan dan obervasi langsung. Informan penelitian terdiri dari lima informan kunci dan lima informan triangulasi. Analisis data penelitian dilakukan melalui tahapan reduksi data, penyajian data, tahapan verifikasi data atau simpulan, dan tahapan triangulasi. Hasil penelitian menunjukkan bahwa penggunaan telemedicine dalam layanan masyarakat dilakukan untuk meminimalkan pertemuan tatap muka antara pasien dan dokter dengan menggunakan teknologi. Hasil wawancara dengan informan triangluasi menunjukkan bahwa pengetahuan masyarakat tentang telemedicine masih kurang, dan penggunaan telemedicine di masyarakat masih terbatas pada yang terbiasa dengan teknologi. Beberapa hambatan dalam penggunaan telemedicine, seperti hambatan teknis terkait akses internet yang belum stabil dan rendahnya literasi digital di masyarakat, sehingga masyarakat belum sepenuhnya menggunakan layanan telemedicine. Simpulan penelitian ini ialah persepsi dan pengetahuan masyarakat mengenai penggunaan telemedicine sebagai media konsultasi sudah cukup memadai. Abstract

Abstract: Condition of mental health problems is reflected in its impact including a high enough number of people with mental disorders associated with a high economic burden. This study aimed to analyze the community mental health program at Tombulu Health Center and its obstacles in implementing the program. This was a qualitative and descriptive study. Data were obtained through interviews with informants. The results showed that the mental health program was implemented through a home visit program with health examination, monitoring patient progress, and counseling about family involvement in the treatment. Obstacles experienced in implementing this program included the community's stigma against the patients, therefore, the community and families were less involved in the healing process. Efforts to develop the mental health program included increasing family and community involvement in the treatment process, number of health cadres, and mental health promotions. In conclusion, implementation of the mental health program at Tombulu Health Center in the treatment processs of patients is carried out through home visits, early detection program, and screening to identify mental health problems for the community and health workers. Obstacles include the absence of family support, negative stigma from the community, lack of supporting facilities and infrastructure for the patients, and the hospital location which is quite far away. Keywords: mental health; development of community mental health program   Abstrak: Kondisi masalah kesehatan jiwa tercermin dari dampak yang ditimbulkannya, antara lain angka yang cukup tinggi dari orang dengan gangguan jiwa (ODGJ) diikuti beban ekonomi yang tinggi. Penelitian ini bertujuan untuk menganalisis program kesehatan jiwa masyarakat di Puskesmas Tombulu serta hambatan yang dihadapi dalam  pelaksanaan program tersebut. Jenis penelitian ialah deksriptif kualitatif. Data penelitian diperoleh melalui wawancara dengan informan. Hasil penelitian menunjukkan program kesehatan jiwa dilaksanakan melalui program kunjungan rumah (home visit), disertai kegiatan pemeriksaan kesehatan, pemantauan perkembangan pasien serta penyuluhan tentang keterlibatan keluarga dalam proses pengobatan dan penyembuhan ODGJ. Hambatan yang dialami antara lain stigma dari masyarakat terhadap ODGJ sehingga masyarakat dan keluarga kurang terlibat dalam proses penyembuhan ODGJ. Upaya pengembangan program kesehatan jiwa antara lain dengan meningkatkan keterlibatan keluarga dan masyarakat dalam proses perawatan ODGJ, menambah jumlah kader kesehatan, dan meningkatkan upaya promosi kesehatan jiwa. Simpulan penelitian ini ialah pelaksanaan program kesehatan jiwa di Puskesmas Tombulu dilakukan melalui kegiatan kunjungan rumah (home visit), program deteksi dini, serta skrining untuk mengenali masalah kesehatan jiwa baik bagi masyarakat maupun tenaga kesehatan. Hambatan meliputi tidak adanya dukungan keluarga, stigma negatif dari masyarakat, kurangnya sarana dan prasarana pendukung untuk pasien ODGJ, dan letak rumah sakit yang cukup jauh. Abstract

Tourist safety and security play an important role in the image of a destination to increase tourist visits. North Sulawesi Province as one of the regions that is currently intensively promoting its regional tourist destinations and cooperation between the local government, tourism office, health facilities, and the Police. This study aimed to analyze the contribution of the North Sulawesi Regional Police to the management of tourist events according to the factors that hindered the North Sulawesi Regional Police, and the efforts and strategies that had been carried out by the North Sulawesi Regional Police in developing health tourism. This was a descriptive and qualitative study. Primary data were obtained through in-depth interviews and using interview guidelines. Data analysis was carried out using the interactive Miles and Hubberman model. The results showed that the role of the police in developing health tourism was carried out quite well, albeit it was not optimal. Obstacles to the police in carrying out their role, namely the absence of a tiered system regarding inter-regional security arrangements and a system for reporting the security of tourists and the absence of coordination between relevant stakeholders, and the regional police in carrying out the main duties of Keskamtibmas. The health tourism program must be known by all parties, a digital system that clearly regulated the security duties of the regional police, and coordination of all relevant stakeholders. In conclusion, North Sulawesi has the potential to advance the tourism industry, especially health tourism, but the contribution of the regional police in the development of health tourism is still not optimal. Keywords: health tourism; tourist safety; Regional Police   Abstrak: Keselamatan dan keamanan wisatawan berperan penting dalam membangun citra destinasi untuk meningkatkan kunjungan wisatawan. Provinsi Sulawesi Utara tengah gencar memromosikan destinasi wisata daerahnya dan membangun kerjasama antara pemerintah daerah, Dinas Pariwisata, fasilitas kesehatan, dan Kepolisian Daerah (Polda). Penelitian ini bertujuan untuk menganalisis kontribusi Polda Sulawesi Utara (Sulut) dengan pengelola tempat wisata sesuai faktor-faktor yang menghambat Polda Sulut, dan upaya serta strategi yang telah dilakukan Polda Sulut dalam pengembangan health tourism. Jenis penelitian ialah kualitatif deksriptif. Pengambilan data primer melalui in-depth interview dan pedoman wawancara. Analisis data menggunakan model interaktif Miles dan Hubberman. Hasil penelitian menunjukkan bahwa peran Kepolisian dalam pengembangan health tourism telah dilaksanakan cukup baik namun masih kurang maksimal. Hambatan Kepolisian dalam menjalankan perannya yaitu belum adanya sistem berjenjang mengenai pengaturan pengamanan antar wilayah dan sistem pelaporan pengamanan wisatawan, serta belum adanya koordinasi stakeholder terkait dengan polda dalam menjalankan tugas pokok Keskamtibmas. Program health tourism harus diketahui oleh semua pihak, adanya sistem secara digital yang mengatur secara jelas tugas pengamanan Polda serta adanya koordinasi setiap stakeholder terkait. Simpulan penelitian ini ialah Sulawesi Utara telah memiliki potensi untuk memajukan industri pariwisata khususnya health tourism, namun kontribusi Kepolisian Daerah dalam pengembangan health tourism masih kurang maksimal.
