Abstract

Background: The discoloration of teeth, especially anterior teeth, is one of aesthetic problems. The use of tooth bleaching agents for discolored natural teeth is becoming increasingly popular. Many dentists, however, get many problems when they conduct bleaching process since there is much composite filling on patient’s anterior teeth. Although many research have focused on the discoloration of composite resin after bleaching process, the problem still becomes debatable. Purpose: The purpose of this study was to investigate the difference of the discoloration between hybrid composite and nano composite before and after the application of tooth bleaching agent, 38% hydrogen peroxide. Methods: Eighteen disk-shaped specimens (5 mm) of each of two composite resins, hybrid and nano filler, were prepared. The each group was treated 3 times and the specimens were divided into two groups consisted of 9 specimens for each, and then immersed in black tea solutions for 72 hours. Next, after having staining and bleaching processes, the color of the specimens was measured with a optic spectrophotometer by using photo with type BPY-47 and digital microvolt. The differences of the light intensity among three measurements were then calculated. Afterwards, GLM MANOVA Repeated Measure and parametric analysis (Independent t-test and Paired t-test) were then used to analyze the data. Results: After staining process, it is then known that the nano composite had more discoloration and more affected by the black tea solution than the hybrid one. Conclusion: After bleaching, the discoloration was finally removed completely from both hybride and nano filler composite resins and became brighter from the baseline color.Latar belakang: Salah satu problem estetik adalah adanya perubahan warna pada gigi anterior. Peningkatan pemakaian bahan bleaching semakin popular. Banyak dokter gigi mempunyai problem ketika mereka akan melakukan proses bleaching dan ditemukan banyak tumpatan komposit pada gigi anteriornya. Meskipun telah banyak penelitiahan yang dilakukan tetapi masalah ini masih menjadi pro dan kontra. Tujuan: Melihat perbedaan perubahan warna antara komposit hybrid dan komposit nano sebelum dan sesudah terpapar bahan pemutih gigi hidrogen peroksida 38%. Metode: Tiap kelompok mendapatkan perlakuan 3 kali. Dipersiapka 18 spesimen berbentuk tabung (5 mm) dan dibagi menjadi 2 kelompok masing-masing 9 untuk resin komposite hybrid dan komposit nano. Direndam larutan teh hitam selama 72 jam. Perubahan warna diukur dengan spektrofotometer tipe BPY-47 dan mikrovolt digital, baik sebelum, setelah perendaman larutan teh hitam dan setelah proses bleaching. Perbedaan intensitas cahaya dihitung dengan GLM MANOVA Repeated Measuse dan analisa parametrik. Hasil: Setelah perendaman teh hitam, komposit nano mempunyai perubahan warna yang lebih gelap dibandingkan komposit hybrid. Setelah bleaching, baik komposite hybrid dan komposit nano kembali ke warna aslinya. Tetapi perubahan warna komposit nano lebih besar dari pada komposit hybrid. Kesimpulan: Setelah bleaching, baik resin komposit hybrid dan nano menjadi lebih terang dari warna aslinya.

Abstract

Background: A diastema is the distance or space between two or more adjacent teeth. This abnormality can interfere with the aesthetics of a patient, and 97% of diastemas occur in the maxilla. Various treatments can be performed for diastema closure in patients, one of which is composite resin restoration. Purpose: To explain the aesthetic procedure for diastema closure. Case: A 20-year-old female patient presented with complaints of the distance between her anterior teeth (Class I Angle occlusion with normal overjet and overbite). The labial frenum associated with the diastema was normal in size and position. The patient was not amenable to invasive procedures. Case Management: Management of midline diastema closure using the direct composite technique with DSD, the putty index method and button shade technique. Conclusion: The closure of a midline diastema with direct composite using DSD, the putty index method and button shade technique provides aesthetic results with less cost and time due to the absence of laboratory procedures.

Abstract

Background: A successful endodontic treatment is inseparable from the right choice of root canal dressing. The right choice of medicaments would result in patient satisfaction. Enterococcus faecalis (E. faecalis) and Porphyromonas gingivalis (P. gingivalis) are usually found in failed root canal treatments. Calcium hydroxide is a gold standard dressing that creates an alkaline environment in the root canal and has a bactericidal effect. Commercially, there are calcium hydroxide dressings with supporting additions, including calcium hydroxide–iodophors (CH–iodophors) and Calcium hydroxide–barium sulfate (CH–barium sulfate). Purpose: This study aimed to compare the antibacterial efficacy between CH–iodophors and CH–barium sulfate root canal dressings on E. faecalis and P. gingivalis. Methods: CH–iodophors and CH–barium sulfate were obtained commercially. E. faecalis and P. gingivalis were obtained from stock culture taken from the root canal of failed endodontic treatment. E. faecalis and P. gingivalis were cultured in Petri dishes, and for each bacterium, 12 wells were made in the media. Six wells were used for the CH–iodophors group, and six wells were used for the CH–barium sulfate group. CH–iodophors and CH–barium sulfate were deployed in the wells in E. faecalis and P. gingivalis cultured media in the Petri dishes. After incubation, the inhibition zone diameters were measured. An independent t-test was used for analysis, and the significance level was set at 5%. Results: There is a significant difference in the antibacterial efficacy of CH–iodophors and that of CH–barium sulfate on E. faecalis and P. gingivalis (p = 0.00001). Conclusion: CH–iodophors have a higher antibacterial efficacy than CH–barium sulfate on both E. faecalis and P. gingivalis.

Abstract

Background: Caries is the world's number one chronic disease and the prevalence increases in modern times. But most people ignore it and choice dental care only when it feels pain. It will worsen the condition of the tooth until require multivisit treatments. The number of visits required for multivisit treatments results incomplete treatment due to patient’s non-obedience. One of the factors that affect obedience is attitude. One's attitude towards something will determine someone's behavior towards the same thing. Purpose: To determine the relationship of patient’s dental and oral health attitude towards patient’s obedience during multivisit treatment. Method: Analytical observational research with cross-sectional study conducted on 30 patients who came to RSGMP UPF Conservative Dentistry FKG UNAIR Surabaya period September-November 2011. The data about the attitude is obtained from questionnaire, while collecting cards of patient’s status and direct interviews of the operators conducted to obtain data about patient’s obedience. Result: Based on the results of Spearman Correlation with α=0,05 obtained p-value=0,364 (p>0,05). It suggests that there is no significant relationship between patient’s dental and oral health attitude towards patient’s obedience during multivisit treatment. In addition, test results of the relationship between unexamined variables with obedience obtained p-value=0,010 (motivation), p=0,017 (desire), p=0,004 (perception), and p=0,009 (financing). This suggests that there is a significant relationship between motivation, desires, perceptions, and financing with obedience. Conclusion: There is no relationship between patient’s dental and oral health attitude towards patient’s obedience during multivisit treatment. However, other factors found associated, namely motivation, desires, perceptions, and financing.

Abstract

Background: Dental caries is one of oral and dental disease that has high prevalence in Indonesia. The fact showed that there are many dental caries were in advanced condition, resulting in root canal treatment needed. The failure of root canal treatment usually caused by patient non compliance in multivisit treatment. It was probably because of the lack of public knowledge about oral and dental health. Purpose: This study was to investigate the correlation between oral and dental health knowledge with patient compliance in multivisit treatment. Methods: An analytic survey using cross sectional design was carry out on 35 patients in UPF Konservasi Gigi RSGMP FKG UNAIR around September-November 2011. The data gathering was done using both questionnaire and focused interview. The test of Spearman correlation was used to measure the correlation between the oral and dental health knowledge to patient compliance in multivisit treatment. Result: This study showed that there was no significant correlation between oral and dental health knowledge to patient compliance (p=0,837). But there was significant correlation between patient compliance with patient motivation  (p=0,006), needs (p=0,020), perception (p=0,018) and dental treatment cost (p=0,034) with patien compliance in multivisist dental treatment. Conclusion: There was no significant correlation between oral and dental health knowledge to patient compliance. There are other factors that give significance contribution to the increasement of complianced beside knowledge, such as motivation, needs, patient’s perception in continousy treatment and and cost.

Abstract

Background: Before restoration, it is necessary to clean the cavity from the smear layer and residual bacteria such as Sreptococcus mutans using a 'gold standard' cavity cleanser, namely 2% Chlorhexidine digluconate (CHX), however CHX 2% has a disadvantage of having a toxic effect on fibroblasts, osteoblasts, myoblasts, odontoblast-like cells, Chinese hamster ovary cells, and buccal epithelial cells. The shortcomings of the 2% CHX triggered researchers to look for alternative cavity cleansers that are more biocompatible, namely cocoa peel extract because it contains of antibacterial compounds including alkaloids, flavonoids, tannins, saponins, and terponoids with a non-toxic 6% concentration. Purpose: To analyze the difference of antibacterial activity between cocoa peel extract with a concentration of 6% compared to chlorhexidine digluconate 2% against Streptococcus mutans. Methods: This research was an in vitro laboratory experimental study with the posttest only control group design which included two treatment groups, namely 6% cocoa peel extract and 2% CHX. This research was conducted using the inhibition zone diffusion method against S. mutans to see the antibacterial power of each sample. Results: There was a significant difference (p <0.05) in the mean diameter of the inhibition zone between 6% cacao peel extract, namely 11.5406 mm and CHX 2%, namely 13.2156 mm.  Conclusion: Chlorhexidine digluconate 2% has a greater antibacterial power than 6% cocoa peel extract (Theobroma cacao L.) against Streptococcus mutans.

Abstract

Background: Composite resins restoration is a treatment for tooth structure loss due to pathological conditions. Longevity of composite resins restoration can be affected by surface hardness restoration. Glycerin can increase surface hardness restoration with inhibit bond oxygen and free radicals on polymerization composite resins. Purpose: Analyze the increase surface hardness composite resins restoration after glycerin application before light-curing composite resins. Review(s): Of the six journals included in this literature review, five journals reported significant differences because of the obstacles in the polymerization process of the composite resin when composite contact with light-curing will activate the photoinitiator to produce highly reactive free radicals, free radicals will break the double chain carbon bonds of monomers and form single bonds of free radicals with monomers. Bonding of free radicals with monomers will produce polymeric bonds (degree of conversion) which affects the level of surface hardness of the filling. While one journal noted no significant difference in the surface hardness of composite resin after glycerin application. Conclusion: The use of glycerin before light-curing can increase the surface hardness composite resin restorations.

Abstract

Background: Tooth discoloration can be treated with dental bleaching using Hydrogen peroxide (H2O2). Dental bleaching may interfere with the shear bond strength of composite resins because the remaining free radicals can affect bonding polymerization. Epigallocatcehin-3-gallate (EGCG) as an antioxidant can neutralize the free radicals produced during bleaching process. Purpose: Analyze the role of EGCG antioxidants in increasing the shear bond strength of composite resin after bleaching.. Reviews: Of the seven journals included in this literature review, six journals reported significant difference, and one journal noted no significant difference in the shear bond strength of composite resin following the EGCG application. Conclusion: The use of EGCG can increase the shear bond strength value of post-bleaching composite resin restorations.
