Abstract

Delivery assistance by midwives is one strategy regarding maternal and child health problems. In Indonesia, the use of delivery assistance by midwives in the community is still low compared to the expected indicators. The purpose of this study was to analyze the factors related to the selection of birth attendants in Gema Village, Kampar Kiri Hulu I Public Health Center in Kampar Regency in 2020. The design of this study was descriptive analytic with a cross sectional approach. The population in this study were all mothers who gave birth in the Work Area of ​​the Kampar Kiri Hulu I Health Center from January to December 2020 totaling 43 people. All populations were sampled in this study. The measuring tools used are questionnaires and checklist sheets. The analysis used is univariate and bivariate. The results of the study concluded that some mothers chose health workers as birth attendants, with less knowledge, negative attitudes in the selection of birth attendants, family income according to the minimum wage in the selection of birth attendants, and long distances from the reach of health services. The results of the chi-square test obtained a relationship between knowledge (0.002), attitude (0.001), family income (0.020) and reach of health services (0.034) with the selection of birth attendants. It is suggested that the Kampar Kiri Hulu I Health Center in Kampar Regency needs to increase access to health services, increase health education, and provide guidance to traditional birth attendants.

Abstract

ABSTRACT Chronic kidney disease is a long-lasting, gradual, and progressive decrease of kidney function. The therapy that can be given to patients with chronic kidney disease is hemodialysis. The continuous dan long process of hemodialysis makes patients tired, bored, anxious and even depressed. The aim of this literature study is to conduct a systematic review of research articles that evaluate the effectiveness of Benson Relaxation and Guided Imagery Techniques on Anxiety in Chronic Kidney Failure Patients Underwent Hemodialysis. The method used is literature review in the form of secondary analysis through scientific database portals such as Google Scholar and Science Direct published in 2015-2021 both national and international articles. The article search process uses keywords in English and Indonesian such as chronic kidney diseases, hemodialysis, anxiety, Benson relaxation and guided imagery technique. Based on the search results, 4.489 articles were found and then selected using inclusion and exclusion criteria and the PICOS format to become 10 articles from 2 countries, Indonesia and Iran. Benson’s relaxation and guided imagery made respondents feel relaxed and calm. Benson relaxation and guided imagery as non-pharmacological therapies need to be applied by hemodialysis nurses to reduce anxiety levels in chronic kidney diseases underwent hemodialysis because both therapies do not cause side effect in patients.

Abstract

Background: Osteoarthritis (OA) is a degenerative disease of the joints characterized by damage to joint cartilage and subchondral bone that causes joint pain. In Indonesia, OA is the most common rheumatic disease in the community. Dyspepsia is a digestive disorder that refers to a collection of symptoms such as a painful or uncomfortable sensation in the stomach, nausea, vomiting, fullness and bloating. Purpose: Applying evidence-based medicine (EBM)-based family doctor services based on a patient problem solving framework with a family approached and patient centered approach. Method: Primary data were obtained through history taking, physical examination and home visits to complete family data, psychosocial and environmental data. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. Results: Patient Mrs. S, 76 years old, complained of pain in both knees and a burning feeling in the stomach.The patient was diagnosed as osteoarthritis and functional dyspepsia. In this case, the diagnosis and treatment have been carried out according to the theory and related journals. After the intervention, there was a decrease in clinical symptoms and an increase in patient and family knowledge. Conclusion: Holistic management can improve knowledge, attitudes, and behavior in patients. In addition, the role of the family is very important in the care and treatment of patients and support is also needed

Abstract

Background: The prevalence of hypercholesterolemia in Indonesia in the 55-64 year age group increases with age up to 15.5%. Hypercholesterolemia can also cause cardiovascular and metabolic diseases. The incidence of hypercholesterolemia is associated with risk factors due to unhealthy lifestyle. Objective: Application of evidence-based medicine for family doctors by identifying risk factors, clinical problems, and patient management based on the framework for solving patient problems with a patient centered and family approach. Method: This study is a case report. Primary data were obtained through history taking, physical examination, supporting examination and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Results: The patient complained of soreness in the neck since 2 months ago and gotten worse in the last 1 weeks so that it interferes with patients to carry out daily activities. Clinically the patient was diagnosed with hypercholesterolemia (ICD 10:E78.00). In the knowledge assessment, the score before the intervention was 6 (low) and after the intervention it was 9 (high). In the aspect of diet there is a change in the number of calories and the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity at least 3 times a week. Conclusion: After the holistic and comprehensive management, patient and families gained an increase in knowledge about the disease that suffered by patients by 30 points and changes in diet and physical activity

Abstract

Background: Cataract is a cause of blindness that covers approximately half of all blindness in developing countries. There is an increase in new cataract patients about 2 million every year. Objective: Implementation of family doctor services based on evidence based medicine by identifying risk factors, clinical problems, management and prevention in patients based on a patient problem solving framework with a patient centered, family approach and community oriented approach. Methods: The analysis of this study is a case report. Primary data were obtained through autoanamnesis and alloanamnesis, physical examination and home visits. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study qualitatively. Results: A 70-year-old patient came because of cloudy vision in both eyes. Eye localis status: The right eye lens is cloudy shadow test(+) with 6/20 vision, and the left eye lens is cloudy shadow test(+) with 6/15 vision. The patient's knowledge about the disease is lacking, does not know the risk factors and does not know that surgery is the right choice for cataracts. In the knowledge assessment, the score before the intervention was 4 (low) and after the intervention was 7 (high). In the behavioral aspect, the patient has registered for cataract surgery. Conclusion: After a holistic and comprehensive management was carried out, the patient and family experienced an increase in knowledge about the disease suffered by the patient and had registered for cataract surgery
