Abstract

Malaria is an ancient disease, which is that still survive to this day, which is transmitted by the bite of Anopheles mosquito containing plasmodium, and poses a health problem that causes anemia, lowered work productivity and even death. Cases of clinical malaria in Langkat in the last three years has increased, which in 2010 from 47.18 per 1000 population increased to 63.79 per 1000 population in 2012. This type of research used in this study is quasi-experimental (quasi experimental) with a pretest-posttest design, which aim to analyze the effectiveness of the method of discussion by using the media module and leaflets to increase the knowledge and attitudes of community leaders on the prevention of malaria in Kutambaru Subdistrict, Langkat district. The population in this study are all Community Leaders in Kutambaru Subdistrict, Langkat district totaling 36 people. The sample is the entire population, numbering 36 people were divided into 2 groups: 18 people for the group threat with leaflet and 18 people for the group threat with modules. Analysis of the data using a statistical test Wilcoxon.  The results of this study showed that the average improvement of knowledge and attitude was more effectively obtained through leaflet media compared to using modules media. It was found out that based on the knowledge of respondents, the difference in the mean value of leaflet media was 4.2 1and SD 1.49 and the mean values of modules media was 2,13 and SD 1.38. In terms of the average value of the difference based on the attitude of respondents for leaflet media was 6.34 , SD 1.49 , while for modules media was 3.32 and SD 1.42. Thew result of statistical test showed that the value p ( 0.000 ) < α ( 0,050 ). It is recommended that the health extension workers Puskesmas Maryke more active in providing health education to the community leaders on the prevention of malaria by using media modules and leaflets.

Abstract

Operation  is  in  one  of  the  medical  action  that  needs  mental  and physical preparation. Medical action could cause physiology interference that is worries. Base on preliminary data, there are many patient get big worries before operation  does.  One  of  the  interventions  to  reduce  worries  pre  operation  is progressive muscle relaxation.This research is aim to identify the different rates worry level before and after doing the progressive muscle relaxation to the patient pre operation in Santa Elisabeth Hospital Medan 2018.The research design is using Quasy Experimental by One Group PreTest Post Test without Control Group Design.The result of this research is about pre test 14,33 and post test 12,40 the significancy  value  p  value  =  0,000  that  state  there  is  correlation  between progressive muscle relaxation to worries level in pre operation in Santa Elisabeth hospital 2018. The  conclusion  of  this  progressive  muscle  relaxation  research  is there  is  correlation  between  reduce  worry  to  pre  patient  operation.  Nurse  is expected  to  do  progressive  muscle  relaxation  as  one  of  intervention  nursing  to reduce worry to pre operation patient.

Abstract

To determine the effect of progressive muscle relaxation therapy on quality of sleep in elderly  people at Binjai Senior Recident Social Service in 2017. One group pre-tes and post test, quasi-experimental design  was used in this study, 30 elderly people were selection with purposive sampling.  Quality of sleep in elderly peopole was measured by Pittsburgh Sleep Quality Index (PSQI. Comparative assessment on difference in quality of sleep of  elderly peopole before and after using Pair t- test. The result of this study found that  after receiving progressive muscle relaxation therapy for 4 weeks, every day, 15 minutes duration the quality of sleep in elderly  people at Binjai Senior Recident Social Service significantly  increased. (p  < .05). The progressive muscle relaxation therapy can use  as one of the intervention to improve the quality of sleep in elderly  people at Binjai Senior Recident Social Service  Indonesia.

Abstract

Diabetes Mellitus is a degenerative disease with an increasing number of patients characterized by an increase in blood sugar (hyperglycemia) resulting from impaired insulin secretion, insulin performance, or both. Diabetes mellitus can result in acute and vascular metabolic disorders that can cause damage to the nervous system (neuropathy). Neuropathy is characterized by reduced loss of sensation in the leg area. Neuropathy requires pharmacologicaltreatment with the provision of drugs  and non pharmacological treatment is by exercise such as foot exercise. Foot exercise aim to smooth the blood circulation in the legs, strengthen the leg muscles, and prevent further complications.The purpose of this study was to analyze the effect of foot exercise on the reduction of neuropathy in patients with diabetic foot ulcer in Asri Wound Care Medan. Neuropathy was assessed using an observation sheet of neuropathy disabilithy score (NDS).The research used one group pre-post test design with sampling technique using purposive sampling. The population in this study were diabetic foot ulcer patients who had neuropathy in ASRI Wound Care Clinic amounting to 41 people. The number of samples in this study were 10 people. The results show the mean difference before and after intervention where the mean pre-intervention is 6.70 and post intervention 1.40. To test the effect of foot exercise on the decrease of neuropathy in patients with diabetic foot ulcer was used  Wilcoxon Sign Rank Test where obtained p value = 0.004 where p<0.05 this means that there is influence of foot exercise on decreasing neuropathy in patients with diabetic foot ulcer. The results of this study can be used as input for ASRI Wound Care Clinic, especially to nurses in order to apply foot exercises to patients with diabetes mellitus to prevent neuropathy, besides foot exercises can also be applied to patients with diabetic foot ulcer that have neuropathy to help reduce or even eliminate neuropathy.

Abstract

Intensive Care Unit (ICU) is the last gate of hospital patients, especially patients who use ventilator and ICU need 24 hour monitoring. Nurses working in the Intensive Care Unit must be fast and precise, continuous work, high workload, less modern tools and family conditions can stress the consequences of nurse work and other health teams are not fluent and empathy for patients is decreasing, so that nursing care is given partially implemented and addressed on physical issues. This study aims to explore the work of nurses in caring for patients in the ICU. This research was conducted at ICU Pirngadi General Medan. The type of research used was qualitative with phenomenology approach, the participants in this study were nurses ICU which amounted to 4 people. Technique of taking participant in this study by way of purposive sampling. The results in this study have six themes, namely physical, nursing care, psychology, environment, facilities and expectations. The conclusion in this research is nurses tend to experience various problems in work so that nurses become less maximal in work it will affect service in ICU less maximal. Suggestion in this research is expected to nurse to maximal and discipline in work and to hospital service to pay more attention nurses work load by increasing knowledge and held training, addition of award to create maximum service.

Abstract

The occurrence of the Sinabung eruption resulted in damage to houses and land, even some residents had to lose their homes due to heavy damage and were not fit to be occupied. This makes people very sad, feeling confused, anxious because the eruption still continues. Even so, this does not discourage people from staying in Gurukinayan Village. This high fighting spirit makes people able to survive even not wanting to move house. They tried to make adjustments after natural disasters, in order to survive. Adaptation by the community aims to enable them to re-adapt adaptively. The purpose of this study was to analyze the adaptation strategies of Gurukinayan Village residents after the eruption of Mount Sinabung. This type of research is qualitative with a descriptive phenomenological approach. Data collection was done through participant observation and in-depth interviews with key informants who were victims of Mount Sinabung eruption. The analysis was carried out using a description of content analysis and life history with the number of participants 6. The results obtained were survival strategies conducted by Gurukinayan Village residents: On the health aspect: participants stated that they went to health services for treatment when experiencing health problems such as coughing, shortness of breath, flu, and fever, but there were some participants who made their own efforts by concocting traditional medicine, but there were also those who let the complaint heal themselves without having to go for treatment. In the psychological aspect: participants revealed a lot of the losses suffered due to eruptions, of course this made people experience deep sadness, but they still tried to stay motivated despite the many impacts experienced, this high enthusiasm made citizens become motivated to maintain their survival. Socio-economic aspects: Participants express the strong attachment and social interaction of citizens so that this is what makes citizens stay afloat. Infrastructure Aspect: participants said they were trying to improve infrastructure such as repairing damaged houses and land.

Abstract

Universal Precaution is an infection control action carried out by all health workers to reduce the risk of spreading infection. In the act of infusion there are 3 universal precautions, namely washing hands before the procedure of infusion, wearing gloves, and washing hands after infusion. The research design was descriptive correlation which aimed to find out whether there was a relationship between nurses’ knowledge and the application of Universal Precaution in the infusion procedure in the inpatient ward of Pringadi General Hospital, Medan City. The population in this study were all nurses in the inpatient ward of Pringadi General Hospital in Medan City as many as 52 nurses, taken by means of total sampling. Data analysis used bivariate analysis with Chi-Square test (P <0.05). The instrument used in this study is the nurse knowledge questionnaire about universal precaution and observation sheets for the application of universal precaution in the infusion procedure. The results showed that 51.9% of nurses in Pringadi Hospital Medan City applied Universal Precaution well in the infusion procedure. In the Chi-Square test it was found that there was a significant relationship between the knowledge variables on the application of Universal Precaution in the infusion procedure with a value of P = 0,000. The conclusion is that there is a relationship between nurses’ knowledge and the application of universal precaution in the act of infusion in RSUD. Dr. Pirngadi Medan City. Further research is recommended on the different application of Universal Precaution nursing in hospitals by adding other variables related to Universal Percaution.

Abstract

Hypertension is a silent killer disease which commonly found but not to give symptoms as a trigger for non-communicable disease, when it occurs continuously and uncontrolled it willcause death. Increase blood pressure requires control and important to anticipate the heart's workload. Uncontrolled blood pressure is associated with poor self-care behavior. Changes behavior in patient are needed based on an understanding of knowledge and self efficacy independently or assisted by the family. Achieving changes behavior in patients is requireself efficacy and as a follow-upeducation is important regarding blood pressure control. The purpose of this study was to determine the effect of blood pressure control education on self efficacy in hypertensive patients in the hemodialysis unit.This type of research is quasi-experimental with nonequivalent design pre-post test with control group. Sampling uses a purposive sampling technique with 64 respondents consisting of 32 respondents in the intervention group and 32 respondents in the control group. Data were analyzed using the Wilcoxon Sign Rank test and Mann Whitney test. The results obtained by the average self-efficacy of patients in the intervention group in controlling blood pressure before education mean 42,19 (SD 2,23) and after education the mean value was 46,03 (SD 1,82). The average self efficacy of the control group patients in controlling blood pressure before education was a mean of 41.03 (SD 41.03) and after education the mean value was 43.69 (SD 3.17). There is an effect of self efficacy in controlling blood pressure before and after education in the intervention group (p = 0.00) and controls (p = 0.00). There is an effect of self efficacy in controlling blood pressure after education between the intervention and control groups (p = 0.00). In conclusion, there is an effect of self efficacy in controlling blood pressure in hypertensive patients in the hemodialysis unit. It is recommended for patients and hospitals or health workers to motivate self efficacy in improving their quality of life.

Abstract

Menopause is a condition when physiological menstruation cycle concerning the human growing older. The increase in the number of growing old women will have its own problem with complaints about menopause. Although it does not cause death, it brings about uncomfortable condition and sometimes disturbs daily activities so that it can influence the life quality of menopause women. The research used analytic descriptive method with cross sectional design. It was conducted at Kabanjahe Puskesmas, Karo Regency, from February until June, 2018. The population was 103 menopause women who regularly visited Kabanjahe Puskesmas, taken by using purposive sampling technique. The result of univariate analysis showed that 71 respondents (68.9%) had good life quality. The result of bivariate analysis, using chi square test, showed that the factors which influenced life quality of menopause women were education (p=0.003), occupation (p=0.001), the duration of menopause (p= <0.001), but parity did not have any influence on the life quality of menopause women.The conclusion was that there was the correlation of education, occupation, duration of menopause, with the life quality of menopause women. It is recommended that menopause clinic and health care board which is related to health care providers be established.

Abstract

Exposure to HIV patients in North Sumatra until March 2016 the cumulative number of HIV cases was 11,295 people. HIV spread is more vulnerable to women, HIV can be transmitted through sex, this can be caused by employment status such as female sex workers (WPS). The purpose of this study was to determine the factors associated with HIV incidence in WPS. The study used a descriptive analitic research design with a cross sectional approach. The location of the study in the VCT clinic in the Padang Bulan Medan Health Center working area. The population of this study were all WPS who conducted examinations at the Padang Bulan Health Center. The sample of this study amounted to 140 people. Data analysis by Chi Square and multiple logistic regression. The results showed that there was a relationship between age (p = 0.004), education (p = 0.035), marital status (p = 0.001), age at first time having sex (p = 0.001), consistency in condom use (p = 0.0001) and knowledge (p = 0,0001) with HIV incidence in WPS. The conclusion was that there was the between age, education, marital status, age at first having sex, consistency of condom use, knowledge of HIV incidence in WPS. It is recommended that health workers at the Padang Bulan health center should approach WPS routinely to be localized to conduct HIV examinations in VCT clinics and provide information on prevention of HIV transmission by using condoms.
