Abstract

Background: Oral lesion is one the clinical signs found in chronic myeloid leukemia (CML). CML is a myeloid cell neoplasm that have oral manifestations such as gingival hyperplasia, spontaneous haemorrhage, petechiae, oral infection, mucosal pallor, and ulceration. The purpose is to find out how to diagnose and oral lesions associated CML to occure, so the dentist could provide the right therapyCase Management: 46 years old male was referred from Internal Medicine Department with diagnosis CML. The patient complained of pain at the right upper gum. Cheilitis exfoliative was found at the extra oral examination, while intra oral examination found ulcer at the palate. Blood test result showed decreases of Haemoglobin, Hematocrite, erythrocytes which showed iron deficiency anemia causes disruption of oral mucosal integrity, thereby facilitating oral ulcerations. Based on history, clinical examination and laboratory investigation, intra-oral abnormality was diagnosed as oral lesion associated CML. Triamcinolone acetonide 0.1% in orabase as theraphy oral lesion, vaseline album for management exfoliative cheilitis, and Surbex Z® as multivitamin. Oral ulcer improved within 6 weeks.Conclusion: The diagnosis oral lesions associated CML can be diagnosed based on history, clinical examination and laboratory investigation. Oral ulcers in this patient caused by iron deficiency anemia.

Abstract

Background: Candida albicans is an opportunistic microorganism found in the oral cavity. High numbers of C. Albicans were often detected in dental plaque from children with caries. Aloe vera contains active compounds such as anthraquinone and flavanoid that have an antifungal effect against C. Albicans. The study aimed to compare the effect between Aloe vera juice and 0.06% chlorhexidine mouth rinse on C. Albicans colonies in plaqueMethod: Experimental study consisted of 30 children with carious teeth >4. Subjects were divided into two groups; Aloe vera juice and chlorhexidine 0,06%.Subjects were instructed to use 10 ml of each mouthwash twice daily for 7 days. At baseline and 7thdays, dental plaque samples were collected and then inoculated on saboroud dextrose agar. C. Albicans colonies were counted and expressed in CFU/ml.Result: The data were analyzed using a paired T-test and Mann-Whitney U test. A significant reduction on C. Albicans colonies in plaque was observed with Aloe vera juice (p<0,05), which was not significantly different from the control group (p>0,05).Conclusion: The present study suggested that Aloe vera juice can be used as amouthwash due to its ability in reducing C. Albicans in plaque.

Abstract

Background: A cross infection is the displacement of the infection agents between patients and health ofcers. Knowledge and behaviors of using personal protective equipment (PPE) are needed to prevent and control the infections. The aim of this study is to fnd out the behaviors of the dentists in using PPE at RSIGM Sultan Agung Semarang.Method: This research is descriptive study using cross sectional approach. There were 19 dentists as the respondents. It was using sampling technique. The data collection was done by providing questionnaire concerning the knowledge and observing the behaviors in using PPE. Statistical analysis used Spearman rho.Result: The Spearman rho test obtained the sig value 0,043 (P<0,05) with a correlation coefcient of 0,469. It can be concluded there is signifcant relationship between the dentist knowledge on the use of PPE at RSIGM Sultan Agung Semarang, with the strength of moderate relationships. Observation results of dentist behavior showed: 42.2% had a moderate category.Conclusion: Thus, it can be concluded that the knowledge is related to dentist behaviors in using PPE is in moderate category. Moreover, the dentist behaviors in using PPE can be influenced by several factors such as the lack of isolation awareness, the surveillance of Prevention and Control of Infection (PPI), the PPI education and training.

Abstract

Background: Odontogram is one part of the dental record. The odontogram form isfilled with diagnoses and treatment done by the dentist. Odontogram can be used for the further treatment plan and help the forensic department to do the identification. The purpose of this study was to identify the completeness of odontogram filling out by the dentist in Semarang.Method: The research was descriptive using a cross-sectional study design. The subject of the research was graduated dentist in year 2015 until 2017. Subjects were chosen through information from PDGI.Result: The result of this study showed that none of the subjects filled odontogramcompletely, 5 subjects filled less complete and 39 others were categorized incomplete according to medical record guidance book published in 2015.Conclusion: Based on the study, concluded that the need to pay attention aboutthe importance of filling odontogram by the dentist was still low.

Abstract

Background: Primary tooth enamel are susceptible to damage because it has a low thickness so it requires preventing treatments to maintain. Theobromine, an alkaloid of the methylxanthine group, has been shown to improve crystallinity and resistance of enamel apatite solubility to acids. The enamel resistance to acid requires a series of test of physical properties, by measuring at microhardness. This study analyzes the effect of theobromine gel consentration on the hardness resistance of primary incisors enamel surfaces.Method: Twenty-fve specimens were divided into 5 groups of topical application of theobromine gel with concentrations of 1%, 2%, 3%, 4%, and 5%, and then immersed in acidic solution. Results were analyzed using one-way ANOVAResult: One-way ANOVA test showed signifcant differences in the increase and decrease in enamel microhardness after the application of theobromine gel and acid immersion (p <0.05). The highest values of increase and decrease in enamel microhardness (764,74±187,08 VHN and 457,18±54,37 VHN, respectively) was shown in 5% group of theobromine gel.Conclusion: The higher theobromine gel concentration, the higher the microhardness value but unable to maintain the enamel resistance against acid. The 5% theobromine gel concentration was the most effective one to increase the microhardness of primary teeth enamel.

Abstract

Background: Recurrent Aphthous Stomatitis (RAS) is inflammation in oral mucosa characterized by recurrent single or multiple ulcers that usually affected in non keratinized mucosa. Etiology RAS is unknown but psychological stress, allergy, and gastrointestinal disease can be predisposing factors Case Management: A 23rd years old complained recurrent oral ulcer with free ulcer period for 3-5 days. The patient had a history of food allergy, GERD and psychological stress. Intraoral examination showed recurrent multiple ulcers in variation site of the mouth. DASS 42 screening showed high stress and high anxiety. Skin Prick Test showed positive allergy to kapok, beef, chicken, cow milk, white egg, duck egg, shrimp, cob fish, milkfish, chocolate, and peanut. Ulcers treated with nonsteroid antiinflammation Aloe Vera gel and stress management by reading assignment method.Discussion: Psychological stress altered the immune system so oral mucosa prone to inflammation, and make the history of GERD getting worse. Stress causes cortisol secretion that changes the imbalance of proinflammatory and antiinflammatory cytokines. Oral mucosa becomes more susceptible to hypersensitivity. In addition, stress decreased oral and esophageal mucosa resistance to GERDConclusion: RAS triggered by psychological stress, allergy, and GERD. Treatment of RAS is by elimination predisposing factors to prevent recurrence.

Abstract

Background: Arsenic paste was used to devitalize dental pulp in some problems before surgical, but it has been proven to have many hazardous effects. So, other alternative drugs are needed that are equally effective but safer. Jatropha (Jatropha curcas L.) can cause lysis blood vessels in the pulp, whereas the sidaguri (Sidarhombifolia L.) are effective in relieving inflammation. The purpose of this study was to determine the devitalization effect of jatropha and sidaguri on rabbit animal model.Method: Sidaguri roots were extracted using reflux method while jatropha sap was dried using lyophilization method. For easy application, the extract made into a paste (1; 1) and inserted into the cavity. After 7 days of exposure, the teeth were removed, calcified and followed to histopathological staining and COX-2 expression (IHC).Result: Histopathological examination showed that the paste was able to cause necrosis of pulp nerve. Higher dose of the drug increased the necrosis area, but the paste is not able to reduce COX-2 expression.Conclusion: It can be concluded that the paste has a potential to developed as devitalization agent although it is necessary to study to the reduce the inflammation process.

Abstract

Background: Chronic osteomyelitis mandibula is one of the complications from dental extraction. Inadequate wound handling can have an impact on the spread of infection in the surrounding tissue like nerve which results in facial nerve paralysis. The purpose is to present a rare case that facilitative nerve paralysis as a result of the spread of osteomyelitis infectionCase Management: A 69 years old woman with chief complains numbness onher lips accompanied by pus out beside the lower teeth. No sistemic disease. Panoramic radiograph showed abnormal bone-like sequester. Extraoral examination appeared the bluish color on the right cheek and there was right facial muscle paralysis. Debridement, sequesterectomy by general anesthesia and medication using ceftriaxone intravenous, ketorolac injection, multivitamin, and corticosteroid, physiotherapy for facial nerve paralyze, also.Discussion: Pathogenesis mandibular osteomyelitis involves contiguous spreadfrom an odontogenic focus infection. The bacteria produce an exotoxin, which, while unable to cross the blood-brain barrier, can have deleterious effects on thePeripheral Nerve System (Fasialis Nerve) in up to 75% of cases, with the severity of presentation correlating with the severity of the infection.Conclusion: Chronic mandibular osteomyelitis can spread the infection to around another anatomy oral cavity like facials nerves.

Abstract

Background: Garlic (Allium sativum L) is one of the most important Allium species consumed worldwide and has been used for decades as a cure for various diseases. The aim of this research was to compare the efficacy of single-clove garlic’s n-hexane and ethyl acetate fractions in inhibiting the growth of Streptococcus mutans biofilmMethod: This was a true experimental research. The biofilm mass was determined by absorbance value at 590 nm wavelength with ELISA reader in a microplate using safranin. Data was analyzed using ANOVA followed by post hoc analysis with ρ value <0.05.Result: The result showed that average decrease of biofilm mass after 1-30 minutes of ethyl acetate fraction administration was 33.4 ± 8.03 mg/mL, n-hexane 23.6 ± 0.97 mg/mL and chlorhexidine 35.5 ± 0.98 mg/mL. The average decrease of biofilm mass after 30-60 minutes of ethyl acetate fraction administration was 11.94 ± 7.44 mg/mL, n-hexane 43.87 ± 41.6 mg/mL and chlorhexidine 16.35 ± 4.6 mg/ mL. Statistic analysis showed that there was a notable difference in the decrease of Streptococcus mutans biofilm mass (ρ value= 0.00). Conclusion: The conclusion of this research was that administrating either n-hexane or ethyl acetate fractions of a single-clove garlic has the ability to decreaseS. mutans biofilm mass.

Abstract

Background: Gingival recession is an exposure of the root surface due to migration of the marginal periodontal tissues apical to the cemento enamel junction. There are so many root coverage procedures, among these techniques, VISTA technique combined with ADM and PRF offers minimally invasive approach.Case Management: A-40-year-old male visited Periodontics Clinic of DentalHospital Airlangga University with the chief complaint of poor aesthetic resulting from exposed root surfaces in upper right front region of the jaw. Root coverage by VISTA technique using ADM combined with PRF was planned to this case.Discussion: The minimal invasive VISTA technique approach, combined with ADMand PRF has a number of advantages for successful treatment of multiple recession defects. VISTA technique was used because it can maintain better blood supply and maintain critical papillary integrity. ADM works like an autogenous graft by providing a bioactive matrix consisting of collagen, elastin, blood vessel channels, and bioactive protein that support natural revascularization, cell repopulation, and tissue remodeling. Growth factors present in PRF plays a crucial role in hard and soft tissue repair.Conclusion: VISTA technique together with ADM and PRF membrane can besuccessfully used as a treatment method for multiple gingival recessions of Miller’s class I defects
