Abstract

Cataracts are the cause of blindness which accounts for approximately half of all blindness in the world, especially in developing countries. This is because in developing countries the level of knowledge related to this disease is still low. Awareness of the importance of checking eye health regularly is still low. From this figure, cataract is the highest cause of blindness, about 81 percent. The method used is case report. Primary data were obtained through autoanamnesis and alloanamnesis, physical examination and home visits, to complement family data, psychosocial and environmental data. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study qualitatively. Based on a holistic diagnosis, it was found that an 88-year-old patient complained of wanting to check the condition of his right and left eyes because there was foggy vision in both eyes, the right eye looked more foggy than the left. On eye localis status, eye examination found: The lens of the right eye is cloudy, mature, with a shadow test - with 6/20 vision, and the left eye is immature cloudy with a shadow test + with a vision of 1/60. Internal risk factors in the form of lack of sufficient knowledge of the disease he suffered. The evaluation results showed a strong desire to perform cataract surgery and awareness of the importance of therapy for hypertension. Cases of elderly patients in the form of cataracts and hypertension. Selecting the patient's daughter-in-law as PMO and patient companion. Mrs. M has dared to perform cataract surgery. The patient's management, namely lens extraction, was appropriate. There has been a change in knowledge of Mrs. M, Mr. H, Mrs. U, and Mr. S regarding the disease suffered by the patient and how to respond to it because of the counseling carried out by the researcher.

Abstract

Bacterial vaginosis is a disease that is a public health problem, especially in women, both non-pregnant and pregnant women. Bacterial vaginosis is caused by an excess and overgrowth of anaerobic bacteria such as Gardnerella vaginalis, Ureaplasma urealyticum, Mycoplasma hominis, Mobiluncus sp., Prevotella sp., and other anaerobic bacteria. Bacterial vaginosis is characterized by the presence of an itchy greyish white discharge with a fishy odor, an increase in the pH of the vaginal fluid and is found clue cells on Whiff's test using KOH10%. Management of bacterial vaginosis is in the form of antibiotics with the drug of choice being metronidazole 500 mg orally twice a day for 7 days. In pregnant women, bacterial vaginosis has been reported to increase the incidence of abortion, premature rupture of membranes, preterm labour, low birth weight, and intrauterine infection. Therefore, early diagnosis and appropriate management of bacterial vaginosis can reduce maternal and infant morbidity and mortality.

Abstract

Students have dense activities so they often forget to do exercises that cause declining body health. Jump rope and jogging exercises which are types of aerobic exercise are practical exercises that can improve the functional performance of the body especially the functional capacity of the pulmonary. The purpose of this aerobic exercise research is to determine differences in the functional capacity of the pulmonary of students who did jogging and jump rope. RCT research with 35 students of the Faculty of Medicine, University of Lampung which was divided into a control group and an intervention group. The intervention group had a jogging exercise dose with requency 2 times a week, intensity 40%-90% Hearth Rate Reserve (HRR), time 30 minutes, aerobic type and jump rope exercise dose with frequency 4 times a week, intensity 70%-75% Maximum Hearth Rate (MHR), time 5 minutes, type aerobics. The mean VO2 Max value of the exercise group was higher than the control group and had significant differences. The mean VO2 Max jump rope group was higher than the jogging group but there was no significant difference between them. There was no significant difference between the jump rope and jogging exercise groups but there were significant differences between the exercise training groups and the control group so that jump rope and jogging exercises were considered to increase VO2 Max.

Abstract

Osteosarcoma is a musculoskeletal malignancy that often occurs in pediatric patients requiring multidisciplinary management. Management in the form of amputation surgery (limb-salvage surgery) with a combination of chemotherapy or radiotherapy. Post-amputation complications that are often experienced by patient is postsurgical pain. Persistent Postsurgical Pain (PPSP) in pediatric patients is influenced by risk factors such as age, gender, preoperative preparation (pain intensity, anxiety, pain efficacy), intraoperative measures (site of surgery, potential for nerve damage, administration of analgesia regimens, technique of anesthesia), and postoperative management. The incidence of PPSP in osteosarcoma postamputation patients is related to Phantom Limb Syndrome with varying incidence and duration. This syndrome is a phenomenon that often occurs after post-amputation procedures in the form of sensation (Phantom Limb Sensation) or pain (Phantom Limb Pain) due to loss of part of the body. Phantom Limb Pain (PLP) is an effect and complication that occurs after amputation surgery in pediatric osteosarcoma patients. The PLP pathomechanism is related to the interaction of the Central Nervous System (Cortical Remapping Theory and Proprioceptive Theory) and the Peripheral Nervous System (Dorsal Root Ganglia and nocireceptor stimulation). Management of PLP consists of neuropathic pain medications such as antidepressants, opioids, anticonvulsants, N-methyl-D-aspartate (NMDA) receptor antagonists and other anesthetic agents.

Abstract

Vertigo is a false perception of a person's movements or the surrounding environment. Dyspepsia is a collection of symptoms of the upper digestive tract disorders. This case report aims to implement a holistic and comprehensive approach of a family doctor and patient management based on evidence based medicine, patient-centered, family oriented and community oriented. Primary data was obtained through autoanamnesis, physical examination and home visits. Secondary data was obtained from the patient's medical record. Assessment based on a a qualitative and quantitative holistic diagnosis. The results obtained are patients suffering from vertigo and dyspepsia with internal risk aspects such as inappropriate diet and eating habits, often late to eat, likes to consume spicy, sour, and fried foods, curative treatment patterns, and lack of knowledge about the disease. The external risk aspects are the lack of family knowledge about the patient's illness and curative family treatment patterns. Holistic management of patients and families through poster about the diseases suffered by patients, exercise that can be done by the patient, regular eating patterns, and foods that can cause the symptoms. At the time of evaluation, the patient had no symptoms anymore. The conclusions of the case report are holistic management can increase knowledge and change the patient’s behaviour and the role and support of the family are needed in the treatment of the patient. Abstract

Diabetes mellitus (DM) remains as major health problem in the world and significantly increase every year. Family approach in the management of diabetes mellitus helps identify factors that influence clinical, personal and psychosocial family. This is a case report study. Primary data were collected by history taking (autoanamnesis and alloanamnesis of the family and patients), physical examination, examinations and home visits to assess the physical environment. Assessment is based on a holistic diagnosis from the start, process, and end of the study in a qualitative and quantitative manner. Patient complaints of tingling in both legs, weakness, frequent urination since one weeks ago, especially at night, besides that, they often feel thirsty and hungry easily. The patient's concern is that he has the same illness as his mother and younger siblings and the complaints will worsen which must be treated in a health facility. The patient's perception of the disease, the patient considers that this disease can be cured with drugs given by doctors without changing the patient's lifestyle Hope the patient can find out the disease and be treated so that complaints disappear so that patients can do activities as usual. Management of diabetes mellitus patients holistically and comprehensively, patient center, family approach with diabetes mellitus treatment according to EBM is needed in order to control blood sugar and change patient behavior to prevent acute and advanced complications.

Abstract

Maternal mortality is maternal deaths during pregnancy, childbirth, and puerperium but not due to other causes such as accidents or falling in every 100,000 live births. One of the important risk factors for maternal mortality is Peripartum Heart Disease. Peripartum Heart Disease (PPHD) is the highest cause of maternal death in 2018 at Abdul Moeloek Hospital in Bandar Lampung. This study aims to determine the relation between PPHD and maternal mortality. This research was a retrospective cohort study design and conducted in October—November 2019 at Abdul Moeloek Hospital in Bandar Lampung. The research sample was 96 respondents obtained using simple random sampling technique. The data used are secondary data from medical records from January 2016 to September 2019. The case group consisted of 48 pregnant women suffering from cardiac disease and the control group consisted of 48 pregnant women who did not suffer from cardiac disease. Data were analyzed by univariate and bivariate tests using the chi-square statistical test with α = 0.05.There were 9 respondents (18.75%) in the case group who experienced maternal death and 1 respondent (2.1%) in the control group who experienced maternal death. Statistical test results obtained there is a relation between Peripartum Heart Disease with maternal mortality in RSUD Abdul Moeloek Bandar Lampung period of 2016—2019.

Abstract

Uncontrolled use of aspirin can cause several complications and various damaged to organs, including kidney injury. Mesenchymal stem cells as an alternative therapy is an important source for repair and regeneration of damaged tissue. The purpose of this study is to determine the effect of intramuscular injection of human umbilical chord on kidney tissue impairment of rats (Rattus norvegicus) with aspirin induction. This study was a laboratory experimental study with a post test only control group design with 27 white rats (Rattus norvegicus) Sprague dawley strain which divided into three groups. The control group (KK) was given regular drink and food, P1 was given 200 mg/kgBW aspirin orally for 14 days, P2 was given 200 mg/kgBW aspirin orally for 14 days and treated with intramuscular injection of 0.75 mL extracted human cord mesenchymal stem cells. Data were observed by Kruskal-Wallis Non-Parametric Test, obtained p=0,001 (p<0,05), then get a meaningful result which means Ho is rejected. The statistical results with Mann-Whitney test obtained are also find significant differences in each groups, but P1 with P2 (p=0,497) has no significant differences.

Abstract

Premature rupture of membranes is a rupture of the amniotic membrane before delivery. It occurs in 3% of pregnancies and carries a risk of preterm delivery. Each year there are 15 million babies born and more than 1 in 10 babies born prematurely (gestation less than 37 weeks). Obtained patient Mrs. IR, 36 years old, came with complaints of preterm pregnancy accompanied by water-discharge. This complaint was felt since two days before entered the hospital, clear color, odor less and changed sanitary napkins three times a day. The patient appeared to be moderately ill, consciousness compos mentis, blood pressure 118/87 mmHg, pulse 83x/minute, breathing 23x/minute and, temperature 36 ° C. The generalist status of the patient was within normal limits. On obstetric examination, there was a TFU of 3 fingers above the umbilicus (24 cm). The lower part of the mother's fetus was round, bouncy, hard with the impression of the head, not yet in PAP. His (-), fetal heart rate 148x/minute. Inspeculo examination found closed OUE, fluorine -, fluxus: +, amniotic (+) inactive, erosion / laceration / poly (-), red litmus (+) to blue. The patient was diagnosed as 28 weeks pregnant G1P0A0 with promiscuity of single live fetal head presentation and oligohydramnios. To prevent preterm labor in this case, conservative therapy is given in the form of antibiotics (prevent in increase dinfection), corticosteroids (fetal lung maturation), and tocolytics (suppress in uterine contractions). To assess the success of tocolytic therapy, the tocolytic index was used.

Abstract

Globally, anemia occurs in 1.62 billion individuals or around 24.8% of the population. In Indonesia, anemia occurs in about 15.4% of the population. Anemia is often a problem with misdiagnosis and inaccurate management. The male patient presents with weakness since the last month. The patient began to feel symptoms of hypertension (220/110 mmHg) at 3 months ago accompanied by tightness, itching in the body, weakness and pale days, swelling almost all over the body, and decreased urine. On physical examination, there was hypertension (160/90 mmHg), the conjunctival was anemic and on the palpebrae, superior and inferior limbs there was pitting edema. On supporting examination, it was obtained patient with normocytic and normochromic anemia Hb 5 g/dL, erythrocyte 1.8 million/μL, hematocrit 15%, LED 90 mm/hour, ureum 486 mg/dL, creatinine 39.28 mg/dL, calcium 4.7 mg/dL, chloride 92 U/L, sodium 124 mmol/L, and potassium 6.6 mmol/L. USG of the abdomen shows the results of bilateral chronic kidney disease. Management of anemia in patients needs to be holistic considered starting from the causes and the choice of therapy used in order to reduce morbidity and mortality and improve the patient's quality of life. This journal will explain the approach to anemia through illustrations on case of patient with anemia in chronic kidney disease.
