Abstract

Background: Changes in heart structure result in a decrease in the ability to pump blood, thereby impacting tissue perfusion. This condition triggers fatigue so that patients suffer from limitations in carrying out physical activities. Purpose: To find out the relationship between mental health and the ability to perform physical activity in heart failure patients. Method: The study employed a quantitative design with a cross-sectional approach on 140 heart failure patients. The researcher applied a purposive technique in determining samples. Data collection lasted for 3 months starting from October to December 2023 at the Cardiac Clinic of Dr. Moewardi Hospital, Surakarta. The researcher administered the depression anxiety stress scale-42 (DASS 42) instrument to assess mental health, and the international physical activity questionnaire (IPAQ) to assess the ability to perform physical activities. For relationship analysis among variables, the researcher used the Spearman test with a significance <0.005. Results: The results showed a relationship between depression and physical activity (p=0.001), anxiety and physical activity (p=0.002), and stress and physical activity (p=0.002). Conclusion: The state of mental health in heart failure patients has a relationship in their ability carrying out activities.

Abstract

Background: Community health center have a strategic role in providing primary services, including family health services and monitoring. Family health is the main foundation for the welfare of society as a whole. Through good service at the family level, various health problems can be identified and treated early, preventing the potential Purpose: To measure and evaluate the success of community health centers in providing family health services. Method: Descriptive research with the main data source coming from community health center performance evaluation documents. Data collection was carried out at the Wanasaba Community Health Center, East Lombok Regency. The data used in this research were obtained from various public health center performance evaluation documents, such as activity reports, health service records, and other related documents. Data analysis in this research will be carried out using a descriptive approach, where data obtained from performance evaluation documents will be described and interpreted in depth. Results: This evaluation shows that several aspects have gone well, but there are also aspects that require further attention. Health promotion, it can be seen that the dissemination of health information through electronic media reached a good achievement level of 98.30%. In terms of environmental health, it reaches a good level. In improving community nutrition, the results showed good success in efforts to prevent and control disease, successfully implementing various disease control activities (96.69%). In terms of public health services, it achieved a good achievement level of 92.2%. When turning to family health, there are several areas that require further attention because the percentage is smaller than other areas. Overall, the family health aspect is included in the poor category (74.74%). Conclusion: There are good achievements in several aspects, but there are still several shortcomings and potential improvements. Overall, community health centers have taken positive steps in improving work programs, one of which is family health, especially in maternal health and family planning. However, continuous efforts are needed to improve achievements in certain aspects, such as the health of children under five, the health of school-aged children, the health of adolescents and the health of the elderly. Suggestion: For future researchers, it is important to deepen their understanding of the local context and involve active community participation in the design and implementation of research.

Abstract

Background: The background of this research arises from the need to understand the overall performance of Community Health Centers (CHCs) in East Lombok Regency. CHCs play a crucial role in providing public health services, and the evaluation of their performance is essential to ensure the effectiveness of these services. Through descriptive analysis of health survey data, valuable insights are expected to contribute to the development of health policies and the improvement of health services in East Lombok Regency. Purpose: To provide a comprehensive overview of the extent to which the performance goals of CHCs in the East Lombok region have been achieved. Method: This research employs a descriptive analysis approach by collecting data through a health survey involving respondents from various layers of the community in East Lombok Regency. Carefully crafted survey instruments are used to gather information related to CHCs performance, covering aspects such as accessibility, service quality, and public satisfaction. Data collection is conducted meticulously and structured to ensure the validity and reliability of the results. Descriptive statistical data analysis is employed to intricately describe the conditions of CHCs performance. This research method is expected to provide a comprehensive understanding of the factors influencing CHCs performance in East Lombok Regency. Results: There are several aspects that still need attention and improvement. In public health efforts, especially in health promotion, there are some indicators that are lacking, such as the number of types of printed health promotion media and the coverage of policies supporting the healthy living community movement. Maternal and child health also show satisfactory achievement levels, but there are still some variables that require further attention, such as the coverage of delivery services for mothers in health facilities and the coverage of services for toddlers. Additionally, in the development of community oral health efforts, there are some aspects that are assessed as less satisfactory, such as the implementation of community-based oral health efforts in integrated health service posts and traditional health practices that have not reached the desired targets. Therefore, to improve the performance of Masbagik CHC, a thorough evaluation of these less satisfactory aspects is needed, along with the design of appropriate improvement strategies to achieve more optimal results. Conclusion: Essential public health efforts, especially in health promotion, show less than optimal achievements. Meanwhile, environmental health and family health efforts demonstrate good achievements. Specific health services, such as vaccination, infectious disease control, and early detection of diseases, show good to excellent achievements. However, certain aspects of traditional health practices, sports, and occupational health show satisfactory achievements and require more attention. The implication is to enhance efforts in health promotion, strengthen traditional health efforts, and improve achievements in hajj health, sports, and occupational health to achieve a more holistic public health.

Abstract

Background: This evaluation research is essential as community health plays a crucial role in ensuring the well-being and productivity of a community. Assessing the performance of community health centers in community health development efforts is vital to understand the extent to which various programs and initiatives have achieved their goals and to identify areas that require improvement. Evaluation data provide a clear picture of the effectiveness and efficiency of various activity components, aiding stakeholders, including the government, healthcare institutions, and the community, in making informed decisions when designing, implementing, and enhancing community health programs. Purpose: To assess and analyze the performance of community health development efforts in various activity components, such as community dental health, traditional health practices, sports health, occupational health, and pilgrimage health. Method: This research adopts a qualitative approach with a focus on evaluating the performance of community health efforts at public health centers Keruak, East Lombok Regency. Data collection involves analyzing the performance assessment results, coverage data and performance levels of activity components like community dental health, traditional health practices, sports health, occupational health, and pilgrimage health. Research methods include observing the implementation process of programs, conducting interviews with healthcare personnel, public health centers managers, and the local community. Data analysis is conducted qualitatively by detailing the achievements and challenges faced in each activity component. Results: The results of community health development efforts reflect diverse achievements in several activity components. Community dental health efforts exhibit good performance with a coverage rate of 95.0%, while traditional health practices show less satisfactory performance, reaching only 40.0%. Sports health demonstrates good performance (98.3%), while occupational health exhibits less satisfactory performance (73.1%). Pilgrimage health efforts achieve 100% coverage, indicating good performance. However, the overall average performance of these activity components falls into the 'sufficient' category (81.28%). Evaluation and improvement are crucial, especially for components with less satisfactory performance, to enhance the overall effectiveness and efficiency of community health development efforts. Conclusion: Development efforts indicates significant variation among activity components. While community dental health and pilgrimage health efforts achieve good performance, challenges exist in traditional health practices and occupational health, displaying less satisfactory performance. Sports health efforts demonstrate good performance, and the overall average performance falls within the 'sufficient' level. Therefore, in-depth evaluation of components with less satisfactory performance is necessary, focusing on increasing community participation and improving infrastructure. Recommendations include enhancing information and education regarding traditional health practices and creating a supportive working environment for health. These efforts are expected to enhance the overall effectiveness and efficiency of community health development initiatives. Suggestion: Based on the performance results of community health development efforts, several recommendations can be proposed to enhance the effectiveness and efficiency of health programs. Concrete steps, such as improving information and education regarding traditional health practices, can increase community acceptance of these efforts. In addition to improving recommendations for community health programs, further research should aim to focus on aspects that support sustainable development.

Abstract

Background: Pain can be relieved through medical and nonmedical approaches. The provision of analgesic drug therapy is a collaborative nursing practice that can significantly improve patient care through the use of opioids, nonopioids, and nonsteroidal anti-inflammatory drugs. Benson relaxation can reduce the problems experienced by patients which have an impact on reducing the cost of care. Purpose: to prove that the application of Benson relaxation therapy can reduce acute pain in postoperative patients. Method: This study uses a case report method with a nursing care approach by applying evidence-base nursing practice to 5 managed patients, namely patients with postoperative treated in the lotus surgical room at RSUD dr. Soehadi Prijonegoro Sragen. The instrument used in this nursing care report is a numeric scale to measure the intensity of patient pain with an interval of 0-10. Value 0 does not feel pain while 1-3 mild pain, 4-6 moderate pain and 7-10 shows severe pain. Results: Showed that there were significant changes in the patient's pain scale after performing benson relaxation therapy. Conclusion: it was concluded that interventions carried out using Benson therapy in postoperative patients can reduce pain intensity.

Abstract

Background: Pneumonia is an acute respiratory infection, commonly caused by viruses or bacteria. It can cause mild illness or even be life-threatening. When a person suffers from pneumonia, the alveolus will be filled with pus and fluid, which makes breathing painful and limits oxygen intake so that the patient will feel short of breath. PLB therapy will help develop the alveolus in the lung lobes so that it helps push the secretions in the respiratory tract during expiration and increase alveolus pressure. Purpose: To determine the respiratory rate (RR) using pursed lip breathing. Method: Study with evidence-based practice. The subjects used were 5 patients with pneumonia, data analysis using descriptive analysis. Results: The application carried out for 15 minutes in a span of 3 days, there was a decrease in respiratory rate in pneumonia patients. Conclusion: The Pursed Lips Breathing (PLB) technique is effective in reducing or reducing the rate of respiratory in patients with pneumonia.

Abstract

Background: One of the efforts made by the government to realize health development is to create public health services that are managed by, for and with the community with the aim of empowering the community and making it easier for people to obtain or access health services. Purpose: To determine the relationship between the performance of community health center cadres and the level of satisfaction of mothers of toddlers. Method: Correlation analytical research with quantitative methods uses a cross sectional approach. This research was conducted at the Flamboyan integrated service post, Tanggan Village, the working area of the Gesi Health Center. Data collection was carried out in November 2023. Sampling used a total sampling technique so that a total of 32 people were obtained with the inclusion criteria including mothers of toddlers who lived around the research location, came to the integrated service post, and were willing to become respondents. The independent variable in this research is the performance of integrated service post cadres for toddlers, while the dependent variable is the level of satisfaction of mothers of toddlers. Results: The majority of respondents were very satisfied with the performance of cadres with results of 13 (65.0%). The results of the Spearman Rank correlation test obtained a p-value of 0.002 (< 0.005) and a correlation coefficient value of 0.528, so that H0 was rejected and H1 was accepted, meaning that there is a relationship between cadre performance and the satisfaction of mothers of toddlers using integrated service posts. Conclusion: There is a relationship between cadre performance and the level of satisfaction of mothers of toddlers using integrated service posts. Suggestion: Cadres are expected to be the driving force of community health center activities to improve service quality and optimize the implementation of planned activities.

Abstract

Background: Fracture is a continuous disruption of bone structure, either completely or partially. The incidence of closed fractures increases annually by around 15 million per year with a prevalence rate of 3.2%. In people with fractures, one of the common clinical symptoms is pain. Inability to cope with pain can cause anxiety and increased blood pressure. Purpose: To determine the relationship between pain intensity and blood pressure in patients following near lower extremity fracture surgery. Method: Quantitative research with a cross sectional approach. Data collection uses documentation studies in the form of medical records. Sample data was taken using a quota sampling technique of 200 data from the category of patients with post-operative fractures near the femur at the Prof. Dr. R. Soeharso Orthopedic Hospital, Surakarta in January-December 2023. Results: Analysis of 200 data showed that the highest pain intensity was mild, 112 people (56%) and the highest blood pressure data was pre-hypertension, 81 people (40.5%). The results of the Chi-Square statistical test produced a p-value of 0.01 (p<0.05), meaning that there was a significant relationship between pain intensity and blood pressure. Conclusion: There is a significant relationship between pain intensity and blood pressure in patients after femur fracture surgery.

Abstract

Background: Cardiovascular disease, particularly Coronary Artery Disease (CAD), is strongly linked to pneumonia. Patients suffering from both pneumonia and CAD frequently experience dyspnea as a symptom. Dyspnea, caused by both conditions, can significantly impact the patient's quality of life. Consequently, it is crucial to address and manage dyspnea in individuals with pneumonia and CAD. Purpose: To provide a thorough overview of managing dyspnea in patients with pneumonia and CAD in public hospitals in West Java, Indonesia. Method: Employing a case study methodology using a nursing care approach, encompassing nursing assessment, data analysis, diagnosis, intervention, implementation, evaluation, and documentation. The assessment indicated that the patient experienced shortness of breath, with a blood pressure of 145/86 mmHg, a respiratory rate of 24 breaths per minute, a pulse rate of 95 beats per minute, and an oxygen saturation level of 88%. The patient received 5 liters per minute of oxygen via a nasal cannula, and their body temperature measured 36.4ÂșC. Results: In this case study, dyspnea management involved positioning the patient, administering oxygen therapy in stages, and providing pharmacological treatments, including diuretics and antibiotics. Additional therapies included respiratory muscle training and effective cough exercises. These interventions successfully reduced the patient's respiratory rate to 18 breaths per minute and increased their oxygen saturation to 97% with a nasal cannula. Additionally, the patient's Medical Research Council (MRC) dyspnea scale score decreased to 1 out of 5. Conclusion: Nursing care management that includes positioning the patient in a semi-Fowler's position while resting and a Fowler's position when awake, administering oxygen therapy in stages, providing pharmacological treatments such as diuretics and antibiotics, and conducting respiratory muscle training, can lower the breathing rate and alleviate symptoms of shortness of breath in patients with pneumonia and coronary artery disease.

Abstract

Background: Diabetes mellitus (DM) is a significant global health issue that has escalated, leading to serious implications for public health. This disease not only increases the risk of long-term health complications but also has a considerable economic impact due to treatment costs and lost productivity. The research findings can inform the development of targeted health policy recommendations and more effective prevention programs. Purpose: To describe gender, age, and body weight of diabetes mellitus among patients visiting community health centers in West Nusa Tenggara. Method: A descriptive study design conducted at several community health centers across the province to reflect geographic and demographic diversity and ensure the results represent actual community conditions. The study population included all patients diagnosed with diabetes mellitus at these centers. Data collection involved patient medical records, which provided demographic information such as age and gender, medical history, and laboratory data related to diabetes mellitus. Results: The age group 57-66 is identified as the most vulnerable, making up 38.19% of the total diabetes patient population. The 47-56 age group also shows a significant risk, with lower risks observed in the 37-46 and 67-76 age groups. Notably, there are no diabetes patients in the 77-86 and 87-96 age groups, indicating a lower risk for these ages. Gender analysis reveals that females are at a higher risk, comprising 75.88% of the diabetes patient population, while males account for only 24.12%. In terms of body weight, individuals in the 50-59 and 60-69 kg weight groups are at high risk, representing 64.82% and 35.18%, respectively, of the total diabetes patients. These findings suggest that individuals weighing over 50 kg have a higher risk of developing diabetes. Conclusion: The age range of 57-66 is the most vulnerable group to diabetes, with the highest number of patients. Among this group, females tend to be more susceptible to diabetes, especially those with a body weight above 50 kg.
