Abstract

Dyslipidemia is lipid metabolism disorder as indicated by increase in total cholesterol, triglycerides, Low Density Lipoprotein (LDL) and decrease in High Density Lipoprotein(HDL). Riset Kesehatan Dasar (RISKESDAS) Database in 2018 showed 12.6% Indonesian population aged 55-64 years had high cholesterol and very high LDL and triglycerides. The Mediterranean diet is non-pharmacotherapeutic treatments in the form of nutrition therapy for dyslipidemic patients. This study aimed to determine differences of adherence to the Mediterranean diet on lipid profile and Plasma Atherogenic Index (PAI) in dyslipidemic patients at a private clinic in Bandung. It used an observational analytic method with a prospective cohort of 30 dyslipidemia patients. Sampling was done by systematic random sampling technique. The subjects were divided into 3 categories based on adherence, namely low, medium and high, then lipid profile examination and PAI were assessed at the end of the month. Total cholesterol, LDL, HDL data were analyzed using Anova and Post Hoc Tukeywhile triglycerides and PAI were analyzed using Kruskal Wallis and Mann Whitney. Adherence to the Mediterranean diet showed significant results on total cholesterol (p=0.007), LDL level (p=0.019), HDL level (p=0.006), triglycerides (p=0.005) and PAI (p=0.036). The result wasassociated due to the influence of food content rich in monounsaturated fatty acids (MUFA), polyunsaturated fatty acids (PUFA), phytosterols and polyphenols that affect the levels of lipid profiles and PAI.   DOI : 10.35990/amhs.v1n1.p1-9

Abstract

Noise Induced Hearing Loss (NIHL) is a quite common health problem among factory workers, including furniture manufacturing factories. Noises above the threshold intensity for a long time can conduct to hearing loss, either temporary or permanent. If this is not taken seriously, it conducts to auditory and non-auditory impacts. The purpose of the study was to determine the prevalence of noise-induced hearing loss, worker characteristics, length of work, PPE use habits and auditory/non-auditory symptoms among workers at PT Chitose in Cimahi, West Java. Subjects were 66 workers consisting of 62 men and 4 women who were selected by total sampling, with a cross-sectional descriptive research method. Anamnesis was performed by filling out a questionnaire, physical examination of the ear, and pure tone audiometry. The results showed that the prevalence of NIHL was found in 37 people (56%). Out of patients with NIHL the average age was 37.8 years, male gender 95%, the workplace was from production department 73%, the average length of work was 16.6 years, never used PPE 54%. The most perceived auditory symptoms were tinnitus 54%, and the most non-auditory symptoms were difficulty sleeping 19%. The prevalence of NIHL among PT Chitose Cimahi workers is quite high. This is due to exposure to noise and is influenced by various risk factors. Occupational preventive program is needed from the company and rehabilitation for workers to overcome this. In conclusion, hearing loss found in PT Chitose Cimahi furniture factory workers is hearing loss caused by noise. DOI : 10.35990/amhs.v1n1.p10-18

Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by increased blood glucose levels due to decreased insulin production, insulin action or both. Red guava fruitghurt contains flavonoid phytochemical compound. This research aims to determine the effect of red guava fruitghurt on reducing blood glucose levels in rats. The study was laboratory experiment. The subjects of this study were white rats (Rattus norvegicus) with wistar lines, where each group needed 5 rats, therefore the total was 25 rats. The sampling technique used is simple random sampling, The experimental animals are grouped into 5 groups, negative control, positive control, treatment 1 (administration of fruitghurt), treatment 2 (administration of glibenclamide), and treatment 3 (combination of fruitghurt and glibenclamide). The data analysis was performed with paired t test, one way anova, pairwise comparison test and post hoc duncan. The study found that red guava fruitghurt can reduce blood glucose levels in rats induced by streptozotocin and show a probability value or p value of 0.000 (p value < 0.05),meaning that there is a significant difference between Treatment 1, 2, and 3. The blood glucose level of rats after administration of fruitghurt was 75.4mg/dl, after administration of glibenclamide was 48.4mg/dl and the combination of fruitghurt and glibenclamide was 44.0mg/dl. From these data, it can be seen that the most influential treatment in reducing blood glucose levels is the combination of fruitghurt and glibenclamide. The significant effect in lowering blood glucose levels is the combination of fruitghurt and glibenclamide.   DOI : 10.35990/amhs.v1n1.p19-27

Abstract

Obesity is a multifactorial disease that occurs when there is an abnormal accumulation of body fat due to an imbalance between energy intake and energy expenditure. The prevalence of Riset Kesehatan Dasar in 2018 showed that 21.8% of the Indonesian population over 18 years is obese. One of the diets recommended by European Association for the Study of Obesity Community for obesity is the Mediterranean diet. This study aims to determine the difference of adherence level in the Mediterranean diet to nutritional status, and muscle mass in obese patients. The method used in this study is analytic observational with prospective cohort observations. Subjects in this study were 37 obese patients who were monitored for diet four times in one month with food frequency questionnaire and checked for BMI, waist circumference, body fat percentage, and muscle mass at the end of the month. BMI and body fat percentage were analyzed using the Anova test while waist circumference and muscle mass data were analyzed using the Kruskal Wallis test. The level of adherence to the Mediterranean diet has been shown to significantly affect BMI (p=0.008), waist circumference (p=0.042), body fat percentage (p=0.014), and muscle mass (p=0.009) in obese patients. This associated with the composition of Mediterranean diet which affects increase in thermogenesis, fat oxidation, anti-inflammatory, and increases satiety for a longer period. The results of this study support the hypothesis that there is the difference of adherence to the Mediterranean diet to nutritional status and muscle mass in obese patients.   DOI : 10.35990/amhs.v1n1.p28-37

Abstract

Inflammation is a defense that exists in the body when an infection occurs. Excessive inflammatory reactions can be induced by lipopolysaccharides (LPS) which are the most immunogenic components of a bacterial capsule or cell wall, which will result in damaging effects on the body. Black rice (Oryza sativa L.indica) is one of the natural ingredients that contains phenols, flavonoids, and antioxidant anthocyanin as immunomodulator. This study aims to find out the effect of giving black rice ethanol extract as an antiinflammatory to the proliferation of polymorphonuclear cells (PMN) in the spleen of ratsinduced on lipopolysaccharides. This study was conducted using 27 mice who were given black rice ethanol extract as much as 400 mg/kg BW in 14 days and then rats induced by LPS 0.3 mg/kg BW at 24 hours intraperitoneally before being CO2 euthanized and taken the spleen to be checked for measuring the size of the germinal center diameter. The observations showed the results of the germinal center diameter size in the treatment group with a value of 1,311 ± 0.226 μm (p<0.05 compared to positive control group which showed that LPS administration led to the disorganization of the white pulp and the reduction of lymphocyte cells and PMN in the germinal center. In conclusion, black rice ethanol extract at a dose of 400 mg/kgBB can affect the proliferation of PMN indicated by changes in the morphology of the white pulp in the germinal center in the spleen of rats.   DOI : 10.35990/amhs.v1n1.p38-44

Abstract

Major depressive disorder (MDD) with psychotic is a complicated affective diseasecharacterized by abnormal clinical sympotms, including neurovegetative disfunction (appetiteor sleep disturbances), cognitive dissonance (inappropriate guilt, feelings of worthlessness),aberrant psychomotor activities (agitation or retardation), and elevated suicide risk withpsychotic features such as delusions or nihilistic, non-bizarre delusions, somatic, poverty,worthlessness, or delusional beliefs about guilt and being punished, and sometimes appearhallucination. The prevalence of MDD with psychotic features increases with age. In generalpopulation, the point prevalence of MDD is about 2% to 4% and it is increasing about 20%lifetime risk. Depression is a leading cause of burden of disease among young people. Currenttreatments are not uniformly effective, in part due to the heterogeneous nature of MDD. MDDis caused by many factors. A 22-year-old male presented to the inpatient psychiatric unit withsuicide attempt. The patient reported experiencing a lack of energy, difficulty falling asleep,lack of motivation, and feeling overwhelmed about his work and experiencing auditorichallucination. The insight level was 5. Multiaxial diagnosis are axis I: major depressive episodewith psychotic symptoms; axis II and III: currently not found; axis IV: problems withworkplace; axis V: GAF 20-11. Patient treated with pharmacotherapy group SelectiveSerotonin Reuptake Inhibitor (SSRI) and second-generation antipsychotics which combinedwith supportive psychotherapy such as mindfullness.   DOI : 10.35990/amhs.v1n1.p45-53

Abstract

Presbycusis is an age-related hearing loss caused by a degeneration process which is acumulative effect of various risk factors. Hearing Handicap Inventory for the Elderly-Screeningversion (HHIE-S) questionnaire is a questionnaire that can be used to measure the quality oflife in patients with hearing loss. This study aimed to determine the relationship betweenpresbycusis with the quality of life. The research uses cross sectional analytical researchmethods. The data used in form of primary data is the result of filling in the guided questionnaireand the results of the audiogram, while secondary data is in the form of medical records ofpresbycusis patients at Dustira Hospital in the period January 2017-December 2018. Data wereanalyzed by Chi-Square and Spearman Correlation Test. The incidence of presbycusis in theperiod January 2017-December 2018 at Dustira Hospital was 2,6%. The result showed most ofthe patients were 65-74 years old (61,8%), male (52,7%), married (58,2%), economic weresimilar between moderate and low economic status (49.1%), had a history of chronic diseases(76, 4%), had a moderate degree of hearing loss (61,8%) and had a mild-moderate degree ofquality of life (52,7%). Based on HHIE-S questionnaire the results showed that there was asignificant and strong relationship between presbycusis and patients’ quality of life (p <0.001)and r=0,509. In conclusion, degrees of hearing loss in presbycusis patients will correspond todegrees of decrease in quality of life.   DOI : 10.35990/amhs.v1n2.p54-63

Abstract

Dyslipidemia is a lipid metabolism disorder that indicates abnormal levels of lipid profile inplasma. The most notable lipid profile abnormalities include increased levels of total cholesterol, low-density lipoprotein cholesterol (LDL), triglycerides, and a decrease in high-density lipoprotein cholesterol (HDL). One of the non-pharmacological management for dyslipidemia is diet modification with the Mediterranean diet. This study aims to determine therelationship between the duration of the Mediterranean diet and lipid profile levels in Prolanispatients who experience dyslipidemia in a clinic in Bandung. The research method wasanalytical with cohort observations. Sampling was done by consecutive sampling. Subjects inthis study were 30 dyslipidemic patients who followed a Mediterranean diet and were examinedfor lipid profile levels in the first and third months. Data on total cholesterol, LDL, HDL, andtriglycerides were analyzed using the T-dependent test. Total cholesterol and Triglyceridesdecreased significantly, and HDL increased significantly in the first and third month. Theduration of the Mediterranean diet shows a reduction effect of total cholesterol, LDL,triglycerides and significantly increase HDL in patients with dyslipidemia because therecommended foods are high in monounsaturated fatty acids (MUFA) and polyunsaturated fattyacids (PUFA), which can correct abnormalities in lipid profile levels.   DOI : 10.35990/amhs.v1n2.p64-70

Abstract

Formative assessment is a continuum process of evaluating students’ performance to enhance students’ learning quality. The shift to full-online learning in pandemics reduces student-lecturer attachments during formative assessment. Therefore, we developed the Online Formative Assessment system (OFAs) based on a self-regulatory learning framework. Thisstudy aimed to explore the learning impact of OFAs, such as learning preparation, feedbackperception, and self-reflection. We used the mixed method with an exploratory approach. Wecollected quantitative data from 150 students who experienced OFAs, using 26 questionsadapted from AEQ (Assessment Experience Questionnaire) and analyzed by pre-post design(paired-T test). To explore a deeper understanding, we conducted 6 FGDs (N=28). Finally, weanalyzed the quantitative and qualitative data sequentially. We found the difference in students’preparation (p<0.5), feedback perception (p<0.5), and no difference in self-reflection (p>0.5).In Qualitative data, we found three themes as follows: (1) OFAs improve student preparationin learning; 2) Students are aware of being assessed during tutorials; 3) Require more specificand individual written feedback; 4) Lack of self-reflection due to unspecific feedback. Thesefindings represent that learning impact is a complex pathway that online assessment systems can drive. It needs externally driven feedback quality and internally driven factors such as self-reflection skills. However, we found the challenges to improving students' quality feedback and self-reflection skills. Abstract

COVID-19 is a respiratory disease that has currently become a pandemic in the world. WestJava is one of the provinces with the most cases of COVID-19 in Indonesia. Health workers including young doctors are a group that has a very high risk of getting infected with COVID-19. The purpose of this study was to determine the characteristics of health protocols and COVID-19 cases in young doctors at Faculty of Medicine, General Achmad Yani University(FK UNJANI) who were fully vaccinated against COVID-19. This study was designed as adescriptive study with a cross-sectional approach. The characteristics assesses in this studywere: age, gender, and a confirmed history of COVID-19. Data was collected in June 2021-September 2022 using google form to fill out a questionnaire. The questionnaire was filled outby 105 eligible respondents. Respondents have received at least 2 doses of the COVID-19vaccine and are aged between 22-23 years. The characteristics of respondents were 71 females(68.57%) and 33 males (31.43%). A total of 78.1% had confirmed COVID-19 and 21.9% hadnever been infected. A total of 52,4% respondents had good health protocols and 47.6% hadpoor health protocols.   DOI : 10.35990/amhs.v1n2.p79-87
