Abstract

Penyebab kematian medis perlu dicantumkan pada setiap rekam medis pasien yang meninggal. Dokter di Rumah Sakit berperan penting dalam menentukan sebab kematian medis (medical cause of death). Tujuan penelitian adalah untuk menilai pengetahuan dan keterampilan dokter tentang cara penulisan penyebab kematian medis (medical cause of death) pada rekam medis di RS Tersier di Bandung yang sesuai dengan standar WHO International Classification of Disease (ICD) 10. Penelitian ini berupa studi deskriptif analitik dengan memberikan kuesioner pada dokter klinis di suatu RS Tersier di Bandung untuk menilai pengetahuan dan keterampilan dokter tentang cara penulisan penyebab kematian medis (medical cause of death) dan kesesuaiannya dengan standar WHO ICD-10. Dari total 928 orang dokter klinis, didapatkan 90 sampel. Sampel mewakili setiap departemen/bagian yang terlibat langsung pembuatan penyebab kematian medis di suatu RS Tersier di Bandung. Hasil penelitian ini menunjukkan 58,1% dokter yang memahami tentang cara penulisan penyebab kematian medis yang benar, sedangkan 41,9% belum memahami secara teori. Keterampilan dokter dalam mengisi penyebab kematian medis yang benar dan lengkap sebesar 20,7%, sedangkan 23,3% mengisi penyebab kematian medis dengan benar namun tidak lengkap. Sebanyak 75,4% mengisi penyebab kematian yang salah, terutama karena menuliskan kondisi akhir sebelum kematian (terminal events). Masih separuh dokter (58,1%) mengetahui cara penulisan penyebab kematian medis yang benar sesuai standar WHO ICD-10. Tingkat keterampilan dalam mengisi penyebab kematian medis yang lengkap, masih sangat rendah (20,7%).

Abstract

Introduction: Sexual assault in women and children are very prominent among other abuse. Knowledge related to the characteristics of sexual assault is vital for optimal handling of sexual assault cases. Aims: This research aimed to evaluate the characteristics of sexual assault cases in referral hospital (type A hospital) in West Java and West Sumatera provinces. Method: This research is a retrospective descriptive research. The samples were patients suspected of being victims of sexual assault, examined from 1 January 2014 to 31 December 2018. Results: The alleged sexual assault victims are dominated by women, unmarried, with the most age groups of 10-19 years. Most victims come to the hospital within a week after the case. The primary type of sexual assault experienced by the victims is vaginal penetration. Most perpetrators were someone who was well known by the victims. The perpetrators prefer to act at noon in a private place. Medical records revealed that in most cases only signs of violence on the anogenital region, and no trace on other body parts. Conclusion: The characteristic of sexual assault from both provinces are almost similar. The difference that stands out lies in the level of education of victims. Knowledge about the aspects of sexual assault cases is essential for health service to provide optimal health service to sexual violence victims as well as the prevention efforts.

Abstract

Transmission of the coronavirus that causes COVID-19 from corpses to people around, namely mortuary officers and the families of the bodies, can occur through direct contact with body fluids, objects around the body that are contaminated with the coronavirus, and aerosols formed when the body is moved or turned over. The curing of the COVID-19 corpse requires special treatment so that the custodian of the corpse avoids the transmission of this coronavirus. The purpose of this activity is to increase the knowledge and awareness of medical and mortuary staff about the importance of organizing COVID-19 bodies by taking into account the standards that the WHO has issued, professional organizations while still paying attention to the ethical, religious, and customs aspects of the local community. This capacity-building method is in the form of an online seminar on the role of medical personnel in curing COVID-19 corpses through the provision of 4 topics by four presenters. The material provided was the ethical and legal aspects of the burial of COVID-19 bodies, the management of the burial of COVID-19 corpses, religious views on the handling of COVID-19 corpses, and filling out the COVID-19 death certificate. The results of the activities obtained are the pretest and post-test results to assess the participants' knowledge. Ninety-one participants attended the pretest, and 94 participants from 150 seminar participants attended the post-test. According to standard operating procedures adopted from WHO, the funeral of the corpse is carried out by taking into account ethical, cultural, and religious aspects. The province of West Sumatra itself is primarily Muslim, so this protocol is also based on the fatwa of the Indonesian Ulema Council. The preparation of SOPs for the healing of COVID-19 patients and socialization needs to be continued with other health workers and the community to reduce the turbidity in the community regarding the handling of the bodies of COVID-19 patients.

Abstract

Forensic medicine is a branch of medicine that examines victims for the benefit of justice. Doctors play an important role in documenting injuries or abnormalities on the victim's body in the patient's medical record. This study aimed to determine the characteristics of cases and injuries of patients at M Djamil General Hospital during the period of 2010–2020. This was a descriptive retrospective study on secondary data from the medical records of patients who were examined and consulted to the Forensic Department of Dr. M. Djamil General Hospital, Padang during 2010–2020. During this period, 11.729 patients (victims) were examined by and consulted to the Department. Most of the victims were male (70.6%) aged 10–19 years old (29.2%). Most of the victims were involved in a traffic accident, followed by persecution. Victims of traffic accident dominated the forensic pathology cases after sudden death.  Abrasions and open wound injury were the most frequent types of injury, with head and neck as the body parts mostly affected. In terms of the type of wound, most victims experienced injuries caused by blunt force (87%). This study concluded that the forensic cases managed at the Dr. M. Djamil General Hospital vary with blunt forces by a traffic accident as the most frequent cause of injuries while the head and neck become the body parts that are  most susceptible to injury, both for survivors and those who died.

Abstract

Incidents of child abuse increase every year, including cases in West Sumatra. Violence experienced by children can cause physical disorders, impairment in cognitive, emotional, and social development, and discrimination in the social environment. The government has set rules regarding the protection of children against violence. Still, not all people know the forms of child abuse, their impact on children, and how to report child abuse cases. Therefore, we held a webinar, "Knowing more about child abuse," to educate people. One hundred three participants from various backgrounds attended this activity, but only 90 completed the pre-test and post-test questionnaires. Based on paired T-test, there was a significant increase in the mean score of participants' knowledge about emotional violence, sexual violence, and neglect/abandonment after the webinar (p<0.05). In addition, there was also an increase in the mean score of participants' knowledge about physical violence, although it was not statistically significant (p=0.103). In conclusion, education in webinars can increase public knowledge about child abuse.

Abstract

Introduction: The lack of experts in forensic medicine will become a serious problem globally in the future. In Indonesia, the unique position of forensic medicine at the intersection between medicine and law itself means that the role of forensic doctors can still expand widely. The future development of forensic medicine in Indonesia is determined mainly by forensic doctors and prospective forensic doctors themselves. Therefore, this study aims to determine the motivation of doctors in choosing the specialty of forensic medicine and their views on work as forensic doctors. Methods: This is a cross-sectional descriptive observational research using mixed methods, with forensic doctors and residents as respondents. Result: The results show that 17.8% chose forensic medicine because of their interests and 11.1% because they considered forensic medicine to be a challenging specialty. The absence of scholarships, the lack of institutional and family support, the lack of number and variety of cases in various educational centers, and the limited supporting facilities and financing of forensic medical services are the main obstacles for forensic doctors. Conclusion: Regeneration is needed to meet the need of forensic doctors in various regions in Indonesia by spreading information about the importance of forensic medicine. Good coordination between forensic experts, the Indonesian Forensic Doctors Association (PDFI), other professional organizations, and relevant stakeholders is needed to overcome various obstacles and ensure the growth of forensic medicine in Indonesia.
