Abstract

Human Immunodeficiency Virus (HIV) merupakan virus yang menyebabkan menurunnya sistem kekebalan tubuh. Diketahui ada hubungan antara infeksi HIV dengan berbagai manifestasi penyakit kulit. Herpes zoster merupakan penyakit kulit yang disebabkan reaktivasi dan multiplikasi virus varicella zoster yang menetap pada ganglia sensoris setelah varicella. Kasus Laki-laki usia 32 tahun dengan gelembung bergerombol berisi air dengan dasar kulit eritem disertai nyeri pada betis sampai punggung kaki kiri sejak 3 hari. Pasien terinfeksi HIV sejak 7 bulan. Didapatkan lesi vesikel herpetiformis di atas dasar eritem sesuai dermatom Lumbal 5 (L5) sinistra. Tes Tzank menunjukkan adanya sel raksasa berinti banyak. Jumlah sel CD4 212/μL.Human Immunodeficiency Virus (HIV) may suppress the immune system. There is a relationship between HIV infection with various skin manifestations. Herpes zoster is caused by reactivation and multiplication of varicella zoster virus in sensory ganglia after varicella. Case : A 32 year-old male with painful, clustered vesicle on erythematous base along left calf to foot since 3 days. He was diagnosed HIV positive 7 months ago. Physical examination showed herpetiform vesicles on erythematous base in left Lumbal 5 (L5) dermatome. Tzank test showed the presence of multinucleated giant cells. CD4 cell count is 212/μL.

Abstract

Latar Belakang: Psoriasis adalah penyakit autoimun dengan dasar genetik dan dicetuskan oleh berbagai faktor di antaranya stresor fisik, stresor kimia dan paparan bahan-bahan kimia. Pembentukan radikal bebas dalam tubuh dapat dihambat oleh zinc. Tujuan: Untuk mengetahui kadar zinc serum penderita psoriasis. Metode : Penelitian analitik komparatif numerik dengan rancangan penelitian potong lintang. Sampel terdiri dari 10 orang penderita psoriasis dan 10 orang kontrol. Analisis sampel serum darah untuk pemeriksaan kadar zinc menggunakan inductively couple plasma-mass spectrometry (ICP-AES). Analisis statistik menggunakan uji independent sample T-Test dan bermakna jika nilai p<0,05.Hasil: Kadar zinc serum penderita psoriasis lebih rendah dibandingkan kontrol (p = 0,02).Background: Psoriasis is an autoimmune disease with a genetic basis and triggered by several factors such as chemical agent. Abstract

Background: Psoriasis is a chronic skin disease caused by inflammatory response and attacks 2% of population, with the prevalence influenced by age, geographical location, and genetic background. Until this day, there are several studies that discuss about the relationship between blood type with disease. However, discussion about the difference between blood type and the relationship with skin disease remains limited. The objective of this study is to know the relationship between blood type, psoriasis, and onset of psoriasis.Methods: This study adopted cross-sectional design of analytic observational study. The subjects of this study were patients in Department of Dermatology and Venereology at Dr. Moewardi General Hospital and UNS Hopital in Surakarta. The blood types were determined by two methods; by using ABD-Pad measurement tools that able to see the reaction of antigen and antibody in short time and by using the hospital&rsquo;s laboratory facility where sample was taken. Anamnesis were done to collect data about the onset of psoriasis. Chi-square (X2) statistical test was used to analyze the data.Results: Total sample of this study were 62 people with 31 subjects (50%) were psoriasis patients and 31 others (50%) are non-psoriasis patients. Chi-square test (X2) between blood type with psoriasis and blood type with the onset of psoriasis are p = 0.269 and p = 0.188 respectively.Conclusion: The significance level used was p < 0.05, therefore there&rsquo;s no significant correlation between blood type with psoriasis and the onset of psoriasis. Abstract

Background: Epidermoid cyst is one of the most common forms of blockage in the pilosebaceousgland. The surgical procedure for excision of the cyst aims to prevent recurrence, however, severalreports suggest that enucleation can be performed with certain considerations.Case:A 22 year old man who lives in Wonogiri, came to Dermatology and Venereology Outpatientclinicof Dr. Moewardi General Hospital Surakarta, with the chief complaint of a lump in the scrotum. Thelumps appeared since 3 years ago, the size of a pea seed and accompanied by itching. The resultsof the examination of the dermatological status of the scrotal region showed several nodules of theskin color, felt dense and rubbery without any signs of inflammation. Histopathology shows a basketwave type parakeratosis at the stratum corneum, while the epidermal layer shows cysts filled withkeratin and surrounded by squamous cells. Based on the results of the history, physical examinationand histopathology, the diagnosis of the patient was a scrotal epidermoid cyst, then we performed twoenucleation measures and found improvement.Conclusion: Epidermoid cyst is a form of benign epithelial cyst that can occur in the scrotal area. Themain treatment to prevent recurrences is surgery, where the enucleation technique can be performedwith several considerations such as small size of the cyst without signs of inflammation, preventingdystrophic calcification and capsule routine.

Abstract

Chromoblastomycosis is a chronic mycosis infection of the skin and subcutaneous tissue. The lesson begins with a history of trauma characterized by slowly gradual growing nodule lesions, especially in the lower extremities. Management of chromoblastomycosis may be physical and non-physical and combination to achieve the best result. A 70-year-old male farmer came with a rough lump on his left leg in the past six years ago. Lesions were multiple-verrucous, varying sizes nodules on the left limb. Skin scraps examination showed copper penny appearance or Medlar bodies. Histopathological examination showed granulomatous inflammation and Medlar bodies. In fungi culture, we obtained Fonsecaea pedrosoi. Patients were treated with a combination of Itraconazole 400 mg/day for a week for three months (pulse dose) and serial cryosurgery once per week. The combination therapy gave clinical improvement and good results. The diagnosis of chromoblastomycosis is based on history, physical examination, histopathology, and culture. Predisposing factors are working in the fields in this case and being exposed to trauma such as soil and plants. Giving combination therapy with itraconazole and cryosurgery makes good results for this patient.

Abstract

Nevus comedonicus is an extremely rare adnexal hamartoma of pilosebaceous apparatus, with approximately 200 cases reported in the literature so far. It appears as cluster of adjacent honeycomb-like dillated follicular openings with firm pigmented keratin plugs resembling comedones.The comedones oftentimes arranged in linear pattern paralled to Blaschko’s lines. We report a case of a 5-year-old boy with open brown to black comedones in a linear pattern localized on the back of the left thigh that appearing since birth. Pain, itch and discharge were not obtain. There were some episodes of infection, due to manual removal done by his mother which left some hypertrophic scars. Dermoscopic examination revealed the distinctive pattern consisting of pigmented, sharply demarcated keratin plugs of 1-3 mm in diameter, some open pores, multiple structurless, various shades of brown homogenous circular areas surrounding the plugs. Histopathological examination showed an aggregation of dilated follicular infundibulum with laminated keratinous material plugging. This case report prove that dermoscopy examination, a simple non-invasive diagnostic tool is very helpful in diagnosing nevus comedonicus. We recommend this tool to differentiate the diagnosis of other rare epidermal nevi while histopathological examination should be performed only in uncertain cases.

Abstract

Melasma is a hypermelanosis disorder that arises typical of areas of the skin that are often exposed to sunlight. Melasma therapy is relatively difficult because there is often chronicity in melasma. Until now therapy with Kligman formula is still an option, but kligman formula has some side effects, especially for long-term use. Cysteamine works by inhibiting the enzyme tyrosinase. Traneksamat acid works by inhibiting the activity of plasmin in keratinocytes known as stimulators of tyrosinase activity The purpose of the study proved and analyzed the effectiveness of cysteamine cream layering therapy 5% and tranexilic acid 3% compared to kligman formula modification in melasma sufferers to improve MASI score. Experimental research method, pre and post control group design double-blind randomied controlled trial with 28 samples of melasma patients namely group A using layering cysteamine cream 5% and traneksamat acid 3% compared to the group of patients melasma i.e. group A using layering cysteamine cream 5% and traneksamat acid 3% compared to the group of patients. Assessment and measurement of the effectiveness of therapy is carried out during weeks 0, 2, 4 and 8, with an assessment of MASI score. The analysis used is the t-pair test and the Wilcoxon test. Statistical tests are considered meaningful if p<0.05. The results of the study decreased the difference in MASI score in group A obtained in week 4 and week 8, while group B in week 8. The difference in the difference in MASI score change between group A and group B in weeks 4 and 8 indicates a significant difference (p=0.025 and p=0.003). Conclusion of 5% layering cysteamine therapy and 3% tranex acid indicates a decrease in the difference in MASI score earlier than modified Kligman cream therapy.

Abstract

Latar Belakang: Karsinoma sel basal (KSB) merupakan kanker kulit non melanoma berasal dari lapisan basal epidermis sel yang tidak mengalami keratinisasi. Penyakit ini dapat bersifat agresif (A) dan non agresif (NA) berdasarkan histopatologi. Tujuan: Analisis jenis KSB berdasarkan jenis kelamin, usia dan lokasi anatomi, diameter tumor. Metode: Penelitian observasional analitik dengan pendekatan cross sectional, pada bulan April – Agustus 2018. Sampel berjumlah 19 preparat pasien karsinoma sel basal di Poli Kulit dan Kelamin RSUD dr. Moewardi yang memenuhi kriteria inklusi dan eksklusi. Sampel dibagi dalam kelompok KSB A dan KSB NA berdasarkan histopatologi. Hasil: Sebagian besar kasus wanita (68,4%) dengan KSB A; rata-rata usia 57,5 ± 17,5 tahun dan sebagian besar petani 25,2%. Lokasi terbanyak di mid face pada KSB A (57,9%) dan KSB NA (10,5%); diameter > 2 cm pada KSB A (52,6%) dan KSB NA (47,4%). KSB A 5.3 kali lebih banyak daripada KSB NA. Simpulan: Kasus KSB terutama pada wanita, usia >50 tahun, petani, lokasi di mid face, diameter >2cm. Background: Basal cell carcinoma (BCC) is a non-melanoma skin cancer originating from the basic epidermal layer of non keratinized cells. Histopathologically it can be aggressive (A) or not (NA). Objective: To analyze histopathological BCC subtype based on gender, age, anatomic location, tumor diameter. Methods: An analytical cross sectional observational study. The study was conducted on 19 basal cell carcinoma patients in Dermatovenereology Clinic dr. Moewardi General Hospital in April - August 2018 who fulfilled the criteria. The samples were divided histopathologically into A BCC and NA BCC groups. Results: Most patients were women (68.4%) with A BCC; the average age is 57.5 ± 17.5 and the occupation were mostly farmers (25.2%). The location were mostly in midface in A BCC (57.9%) and NA BCC (10.5%); diameter > 2 cm in A BCC (52.6%) and NA BCC (47.4%). A BCC was 5.3 times more prevalent than NA BCC. Conclusion: BCC cases were mostly women, age > 50 years, farmer, location in mid face, diameter > 2 cm.
