Abstract

The implementation of medical records has a very broad scope of cooperation between work units, starting from hospital leaders, doctors, nurses to other health workers. One of the units involved in recording patient medical records is the Inpatient Unit. This study aims to determine the timeliness of returning inpatient medical record files to the medical record unit and the delay factor in returning inpatient medical records. This research was conducted on April 26 – June 26, 2021 descriptively with a qualitative approach with data collection methods using observations, interviews, and documentation studies. The results were obtained in the May – June period from a total of 751 inpatient medical record files that returned on time to the medical record unit as many as 594 files (61%) and 157 files (39%). The factor of delay in returning inpatient medical records is that there are still some nurse officers who do not follow the applicable SOP rules in returning inpatient files, namely 2x24 hours after the patient returns, the lack of special care personnel for the management of inpatient medical records and incomplete filling of the record form. medical treatment by doctors and room nurses. So the researcher gives suggestions to be further improved in the timeliness of returning inpatient medical record files so that there is no delay in returning inpatient medical record files to the medical record unit.

Abstract

Medical record is one of the written evidence about the service process which contains clinical data of patients during the process of diagnosis and treatment. Management of medical records in hospitals to support the achievement of orderly administration to achieve hospital goals, namely improving the quality of health services in hospitals. The purpose of this study was to determine the performance of Medical Record Officers at Salak Hospital, Bogor. This study uses a descriptive method with a quantitative approach that aims to describe the performance review of medical record officers in accordance with the facts in the field. The results of the research on the performance of the medical record officer based on the quality of work illustrate that the medical record officer has a fairly good performance, based on the quantity of work indicated that the medical record officer has a good performance, based on the supervision of the medical record officer shows that the medical record officer has a good performance, based on the presence of the medical record officer shows that the medical record officer has a good performance, and based on the conservation of the medical record officer shows that the medical record officer has a good performance.

Abstract

The hospital has a mission to provide quality and affordable health services to the community in order to improve the health status of the community, one of which is through fast patient waiting times. This study aims to analyze the length of waiting time (arrival process, service, human resources) for outpatient services at the Internal Medicine Polyclinic at Salak Hospital, Bogor. During the service process, there are several obstacles that occur, including the patient does not bring a complete file/guarantee, inadequate room, and limited human resources who are experts in refraction and medical records. The waiting time at the Salak Hospital in Bogor is still quite long(> 60 minutes) due to the large number of patients, interference with the insurance network which makes the waiting time longer, the distribution of medical record files is often late. It can be concluded that a person or group of people has the opportunity to make people aware of their own abilities, while at the same time implementing the actions of certain people or groups. In addition, it can be interpreted as the ability to control, dominate, or manipulate the actions of others (Rollo May 1972), thus it can be concluded that a patient can affect the length of waiting time in Health Service Agencies.

Abstract

Medical Record is a file containing records and documents about patient identity, examination, treatment, other service actions that have been given to patients. Medical resume is a summary of the entire period of care and treatment of patients carried out by doctors to patients. The completeness of the medical resume is a reflection of the quality of medical records and services provided by the hospital. This study aims to describe the completeness of the medical resume of outpatients at Meilia Hospital. The research method used is by using observation techniques. The population of the data is medical records of inpatients of 81 medical records obtained from the results of calculations using the Slovin formula from the research conducted, the following results were found: that the population of incomplete medical resumes at Meilia Cibubur Hospital reached 27% and completeness of medical resumes at the Hospital Meilia Cibubur reached 73%. The conclusion that can be drawn from this research is that the analysis of the completeness of the inpatient medical resume has gone well, suggestions need to be re-socialized for the importance of authentication in writing and completing the medical resume because it is related to legal aspects, and sanctions are given if the doctor is not disciplined in filling out the medical resume
