Abstract

Abstrak- Rekam medis adalah kegiatan yang berisikan catatan medis setiap pasien yang dihasilkan dari setiap pasien berobat jalan atau rawat inap. Sistem Informasi Retensi merupakan kegiatan mengelompokan rekam medis menjadi rekam medis aktif dan inaktif yang akan dilanjutkan kedalam proses pemusnahan berkas rekam medis yang tidak berguna. Rumah Sakit Islam Arafah Jambi masih ada kelemahan dalam mengelola sistem retensi serta penyimpanan berkas rekam medis masih belum tertata dengan rapi berdasarkan hal itu diperlukan pengembangan sistem informasi untuk membantu permasalahan tersebut. Metode pengembangan perangkat lunak menggunak teknik Software Development Life Cycle (SDLC) yaitu metode waterfall, metode ini memiliki keunggulan dalam melakukan kegiatan analisis dan perancangan sistem secara deta.il. Implementasi dalam kegiatan penerapan sistem ini dengan menggunakan Microsoft visual studio. Dengan adanya kegiatan merancang sistem informasi retensi memberikan harapan dalam pegelolaan rekam medis agar sesuai dengan standarisasi kegiatan rekam medis didalam rumah sakit.

Abstract

Tujuan dari penelitian ini adalah merancang sistem informasi kunjungan pasien rawat jalan BPJS dengan menggunakan pemrograman berorientasi objek. Metode survei yang digunakan dalam survei ini adalah metode kualitatif dengan menggunakan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah observasi langsung, wawancara dengan pasien BPJS, dan studi pustaka. Metodologi pengembangan perangkat lunak yang digunakan adalah v-model variasi dari waterfall dan pemodelan menggunakan bahasa pemodelan Unified Modeling Language. Investigasi menemukan masalah berikut: (1) Proses pembuatan laporan pemeriksaan pasien BPJS masih belum optimal. (2) Penyampaian laporan akhir pemeriksaan pasien BPJS sering terlambat. Saran yang disampaikan adalah: (1) menjadikan pengembangan sistem informasi lebih efisien dan efektif; (2) Menambah sumber daya manusia dan dukungan sistem, laporan dibuat dengan cepat dan laporan akhir kunjungan pasien BPJS tidak tertunda.

Abstract

In fact there are still some of factors that for the referral late, as the occurence of, obstructed labor the difficulty of the decision-making process related to the economic aspect and transportation costs, and the aspect of geographic and availability of infrastructure hospital. This study attempts to analyze the supporters and the barrier covering input and process in the referral system patients BPJS at UPT Puskesmas Gumuruh. A method of made in this study is the method field, a method of literature and method of development systems which is waterfall. The result showed that the lack of facilities and services at UPT Puskesmas Gumuruh still experienced limited supplies and unsatisfactory.The referral online at health facilities first degree is expected to ease patients to obtain, referral letter there is one problem in registration and making. Therefore, the authors designed a web-based BPJS patient referral letter information system to make it easier for patients to obtain hospital services tailored to competence, distance, and capacity according to patient needs. The puskesmas, health offices and BPJS Health need to work together to disseminate information regarding the referral system and pay attention to the provision of drugs and health facilities to maximize the implementation of the referral system.

Abstract

Posyandu (Pos Pelayanan Terpadu) merupakan bentuk upaya kesehatan yang pengelolaan dan penyelenggaraan dilakukan dari, oleh, untuk masyarakat sebagai bentuk pembangunan kesehatan di tingkat dasar. Posyandu Kelurahan Cigugur Tengah merupakan mitra posyandu yang memiliki tingkat pelayanan yang kurang maksimal. Metode pengabdian yang dilakukan adalah 1) pengumpulan data permasalahan yang dihadapi; 2) Penyuluhan pelayanan posyandu yang efektif dan maksimal dari narasumber; 3) Penyuluhan pelayanan posyandu dengan bantuan aplikasi E-Posyandu. Aplikasi E-Posyandu yang dirancang merupakan Aplikasi berbasis smartphone (Android) yang digunakan untuk memonitoring pelayanan yang terdiri dari Informasi Waktu Pelaksanaan Posyandu, Informasi Pelayanan Posyandu, Informasi mengenai penyuluhan Kesehatan Ibu dan Anak, Informasi Perkembangan Dan Pertumbuhan (gizi) bayi dan balita, Informasi data Imunisasi serta informasi ibu hamil dan KB. Dengan adanya aplikasi e-posyandu ini dapat memberikan kontribusi dan berdampak positif yang baik bagi Mitra Posyandu sehingga tingkat pelayanan menjadi maksimal serta dapat meningkatkan strata posyandu yang bersangkutan di wilayah kelurahan Cigugur Tengah.

Abstract

The purpose of this study was to find out the role of Medical Recorders and Health Information in the Implementation of COVID-19 Vaccination at Hospital X. This type of research used a qualitative descriptive method, with a Cross Sectional approach. Data collection techniques using observation, interviews and documentation studies. Vaccination is carried out by health personnel, including medical recorders. Medical recorders has a role that is in accordance with its competence, namely as a data processor, the results of the study show that medical recorders in the implementation of vaccinations are in charge of registering and matching participant data, verifying participant data, recording and reporting service results and in addition to processing medical recorder data, they also carry out other competencies, namely communication. effective. So it can be concluded that in the implementation of this vaccination medical recorders plays a role based on its competence.

Abstract

Hospitals are a complex institution of health care, because hospital service involves various functions of service, education, and research, and covers a wide range of degrees and kinds of discipline. To enable hospitals to perform professional functions both in the field of medical engineering and in health administration. Hospital maintenance and improvement requires that it have a measure of quality at all levels. The purpose of this study is to know the incompletion review of discharge summary and the long retrieval of medical record files. The method the writer is a quantitative descriptive,50 files of medical records were taken, Analysts used were observation and interview, The subjects in this study are doctors and medical recorders. The results of this study amounted to 79% complete medical records and 21% incomplete files. 75% of files returned on time, and 25% of files returned intime. It seems that hospital x has filled the SOP and the technical instructions are in place. The incompleteness of medical record files greatly affects the quality of hospital health care. It is therefore expected that all officers will be able to perform their duties according to the SOP in the hospital and increase the number for the medical record file. Hospitals can maintain the quality of service for inpatients and outpatients. Word key : discharge summary, hospital, medical record.

Abstract

The medical record file in a hospital is one that affect the quality of reports in the medical record unit. The return of medical record files that are not timely calculated from the timet the patient returns home results in delays in data processing, reporting, insurance claims and delays in service to patients. The purpose of this study was to determine the return of medical record files in supporting the quality of reports and the level of delay in medical record files.The method used is descriptive method. Method of data collection is done by observasion, interviews and documentation studies. The result of the study show that there are still frequent delays in returning inpatient medical record files. The delay is caused by the doctor’s indiscipline in filling out the form which causes data proccessing and report quality to be ineffective.

Abstract

The hospital is a health service facility that organizes public health efforts. In carrying out its duties, the hospital records and reports patient health. Recording and reporting of patients can be seen from the medical record file. After the storage time limit is exceeded, the medical record can be retained or destroyed. Medical record retention is an activity to separate active and inactive medical records which aims to reduce the burden of storage racks. This study aims to design an outpatient medical record retention information system using Microsoft Visual Studio 2010 at the Proklamasi Hospital Karawang. Based on the research conducted, a problem was found, namely the retention system is still manual and not fully computerized, by sorting the medical record files one by one to separate between active and inactive files resulting in a long process. So it is necessary to have a retention information system to accelerate the destruction of medical record files. The design method uses the waterfall method. The method stages are Planning, Analysis, Design, Implementation, maintenance. The medical record retention information system that has been created, it can be concluded that the system can facilitate officers in carrying out medical record file destruction activities to be more effective and efficient.

Abstract

Tujuan dari penelitian ini adalah untuk mengetauhi hubungan kelengkapan berkas rekam medis dalam pengklaiman perawatan pasien Covid-19 di rumah sakit salah satu di Bandung. Metode penelitian yang di gunakan yaitu metode penelitian observasi analitik dengan pendekatan cross sectional. Penelitian ini dilakukan pada bulan April- Juni 2021 di RSU Bandung. Populasi pada penelitian ini adalah semua berkas rekam medis pasien Covid-19 dengan sample sebanyak 125 berkas. Hasil pada penelitian ini adalah berkas rekam medis yang lengkap adalah, serta tidak lengkapnya sebanyak sedangkan persetujuan klaim perawatan pasien Covid-19 sebanyak dan yang menyebabkan dispute sebanyak. Kesimpulan adalah ada hubungan kelengkapan berkas rekam medis dengan klaim pelayanan pasien Covid-19 untuk kemenkes (p-value ≤ 0,05, p = 0,019). Adapun saran adalah (1) Melakukan evaluasi terkaid kesalahan dalam penginputan berkas klaim, (2) Diberlakukannya sistem reward dan punishment, (3) Sosialisasi  tentang Standar prosedure Oprasional SPO pengklaiman perawatan pasien Covid-19 untuk Kementrian Kesehatan

Abstract

This research aims to find out about the picture, relationship, problems and efforts regarding planning in the medical record file storage hospital X. In the research, the authors use quantitative methods with descriptive approach and data collection techniques are observation, interview and literature who have close relevance to the subject matter. The problem and analisis of (1). Lack of attention to aspects room order aspects in the planning execution in the storage room; (2). Lack of planning shelf is needed, is calculated from the number of visits inpatients and outpatients; (3). Lack of personnel labor efectivity resulting from the storage of poor room order. These are following suggestion (1). For more attention to aspects room order aspects that exist in the planning execution in the storage room; (2). Should the amount of planning required rack is calculated from the number of visits inpatients an outpatients; (3). Layout should be improved in order to increase achievenert efectivity of storage workers.
