Abstract

Dinas Kesehatan Tentara (DKT) Tuban is a Army Health is hospital provide by the Indonesian Army that also serve to public people. There are many  problems on DKT Tuban such as validation on medical record that happen effect by unintegrated medical record from registration until a month of patiens, patiens visit list, financial reports. Refers on that problem,this research offer design and build an outpatient administration application on DKT Tuban. This application discusses about patient registration, medical action, recording the disease, prescription drugs, payment, a month of patiens, patiens visit list, and financial reports that integration in one system. This application can be create some reports: reports outpatient, patiens visit list, financial reports, statical and graphical monthly reports, a mount of  visit list each policlinic dayly. Keywords: medical record, administration, outpatient, policlinic, registration

Abstract

Depo Galvalum is a company that moves in galvalum distributor .The process of business on purchase system done by Depo Galvalum still based on Information stock availability . The buying system goods less could be handled well because there is no recording. Sales system just do recording of daily transactions . Recording producing such the total revenues and the amount of goods sold out. The results of recording have not been able to provide information the to conduct an evaluation on a system sales. Leader have only information to the number of goods that have been sold and the rest of the goods. From the report leaders will check repeated that takes time long enough, This leader find it difficult to analyze reports sales still in the form of rekap sales.With the this problem then required inventory information system of goods on Depo Galvalum web-based equipped with reorder point to buy goods next. Of the analysis and design of which is done to provide information on sourcing , an inventory of goods , sale of goods , and preparing reports. The design process produce information system an inventory of goods to suit the needs of Depo Galvalum. These information systems help the Depo Galvalum in the process of purchase of goods , an inventory of goods , sale of goods , and preparing reports.Kata Kunci: information system, inventory , Depo Galvalum , reorder point.

Abstract

Adi Husada Nursing Academy is an educational institution in the field of nursing that conducts Diploma lecture 3. Adi Husada Academy Surabaya was established in 1983 and located in Kapasari street Surabaya. During this time, academic difficulties in the recapitulation of attendance of students and lecturers as well as the academic value of the students. This is because the recapitulation is done one by one from the report of 183 lectures. While the recapitulation of academic value of students should be done by selecting one by one with the name of the students with the subject followed. From the existing problems of administrative applications required courses that can assist in the process of recapitulation of attendance of students and lecturers as well as the academic value of students. Recapitulation of student attendance is used to know the status of students in the examination. Based on the results of trials that have been done, the application successfully assist in the process of recapitulation. This journal discusses the outline of the academic application design lecture at Adi Husada Nursing Academy Surabaya.

Abstract

SMK Kartika 2 Surabaya with a total of 664 students divided into 4 majors, namely majoring in multimedia, light vehicle engineering, audio video techniques, and banking. Light vehicle engineering and audio engineering video have difficulties in asset management, such as borrowing, returns, and maintenance of goods. The staff still uses the logbook that must be filled by the students. The tool man also does not provide receipts for the items borrowed by the students, therefore, making it difficult to track the availability and viability of the goods. In the process of generating the report, the tool man has difficulties in reporting quantities, specifications, clarifications, and feasibility of certain asset items.Based on the above description of the problem, an asset management application was built, which can assist the head of facilities and infrastructure, the tool man, and students in lending, return and maintenance of goods. This application can help Tool man, in the process of creating reports, reduce the loss of goods and simplify the control of goods.From the asset management test results can generate lending reports, return reports, best-selling assets, as well as a list of assets, employees, and students. Keywords: Website, SMK Kartika 2 Surabaya, Asset Management.

Abstract

PT Multi Kadera Sejati is a manufacturing company located at Jl Tropodo, Krian. Sidoarjo RT06 RW03. The company is engaged in manufacturing producing various wicker chairs. The company is engaged in manufacturing producing various wicker chairs. Of all the types of seats almost all products for export. There are several problems, namely, the PPIC officer to make raw material procurement based on intuition / estimate only so that there are often shortages and excess of raw materials, if the shortage resulted in slowing down the production process, resulting in inappropriate delivery time targets to customers, and if the excess will have high impact storage costs and risk of loss and damage to raw materials. And for information from the warehouse provided PPIC. So with the way the information is delivered less quickly, and PPIC late in ordering materials to cover the existing shortages in the warehouse. Based on the availability of raw material stock and problems related to data warehouse data updating and data consistency can be solved by MRP method in the form of web-based computer application. Expectations of the use of these applications is an increase in effectiveness and efficiency, and timeliness in delivery to consumers can be met that led to increased profits of the company. Abstract

BMT Amal DD cooperative as a syari’ah financial cooperative working a credit unit activity. The process of writing and transaction of the members still uses manual way that is by writing transaction of the members in a big book transaction. This causes many problems, such as the difficulty in finding the transaction of the members because the coorperation clerk has to find the transaction data one by one in every transaction book. Besides that, the calculation process of the monthly instalment and the division of profit can cause new problems. Based on the existing problem, it is needed saving and loan cooperative application which can help solve the problems in BMT Amal DD cooperative. The saving and loan cooperative application will assist the cooperative clerk in finding the data of members transaction, calculating montly instalment, calculating the division of the profit as well as helping to make financial report. The application  of saving and loan cooperative can manage the transaction data in BMT Amal DD cooperative resulting information in the form of members saving transaction, cost transaction report, instalment payment report, operational cost transaction report, profit division report, general journal report, general ledger, balance report, and lost and profit report.

Abstract

Rasyid english courses is english  course in the Denpasar city that provide score ofevaluation daily test and the final exam. The score obtained an evaluation recorded by teacher written in score card, then submitted to administration officer using desktopapplication. There are several problems cause losing data when collecting them. First, teachers must come to Rasyid English and then the second one is have to queue to entry data because the entry computer only one so it must be done one by one sequencely. This condition makes the teacher to give the score card to the administration officer. It will takes along time to provide information that will help decision maker for Rasyid english course managers. In the other hand student must come to Rasyid English Course just to know test result eventhought there is nothing else to do. Thus situation both teacher, administration officer and students makes unefficient activity and invalid data to the score management system.Based on the problem above, it will set up an application that can help manager to Rasyid english course to make decision and reduce data loss. Another feature, that can make user entry data anywhere is because use web based application.This research produce web application that guide by SDLC method. This application generate student progress report, academic student report using computer web base application. With web base application allow each the teacher to entry student score anywhere and anytime as long as connected to the internet. Abstract

PT. SETRA SARI Branch of Surabaya is a company operates in cleaning service, pest control, Man Power Supply services. PT SETRA SARI branch of Surabaya have some problems in the payroll system. The problem is low validation according to data recording employee present. Because generate manually. Another problem is calculation and management of employees payroll. To solve these problems, then this research offer an information system based on web application that can be used to record employee present data, and to calculated also manage employees payroll in integrated system.  This information system can help PT.SETRA SARI Surabaya Branch can solve record employee present data, and to calculated also manage employees salary in integrated system.Keywords: payroll, information systems, information systems payroll
