Abstract

Stroke merupakan penyebab kecacatan utama di Indonesia. Berbagai kelainan paska stroke dapat mempengaruhi kualitas hidup penderita. Gangguan psikiatri paska stroke mencapai 30% dan sering diabaikan sehingga memberikan tantangan untuk mengatasinya. Gangguan psikiatri paska stroke yang sering terjadi adalah depresi, kecemasan, fatigue dan apatis sedangkan yang jarang terjadi adalah psikosis, mania, dan perubahan kepribadian. Penyebab gangguan psikiatri paska stroke belum diketahui dan hubungannya dengan letak lesi tidak jelas. Diperlukan penelitian untuk mengidentifikasi penyebab dan uji klinis untuk pencegahan dan pengobatan gangguan tersebut Kata Kunci: Paska Stroke, Gangguan Psikiatrik

Abstract

HIV (Human Immunodeficiency Virus) merupakan suatu jenis retrovirus yang berasal dari famili lentivirus. Virus ini memiliki kemampuan khusus yaitu merubah komponen RNA (Ribonucleic Acid) menjadi DNA (Deoxyribonucleic Acid). Virus HIV merupakan jenis virus yang menyerang sel limfosit CD (Cluster Differentiation) 4+ . Negara Indonesia merupakan negara kepulauan yang masih memiliki keterbatasan dalam melakukan pemeriksaan CD4+. Hitung jumlah limfosit total dapat dijadikan alternatif menggantikan pemeriksaan CD4+ dalam menentukan waktu terapi HAART (Highly Active Antiretroviral Therapy) atau melakukan monitoring. Secara umum tujuan penelitian ini adalah untuk mengetahui korelasi antara jumlah limfosit total dan limfosit CD4+ pada pasien HIV/AIDS di RSUD Provinsi NTB. Penelitian ini menggunakan metode kuantitatif dengan metode pengambilan data secara cross-sectional. Sampel penelitian yang telah memenuhi kriteria inklusi dan eksklusi dipilih berdasarkan teknik consecutive sampling. Pengambilan data menggunakan data rekam medis pasien HIV/AIDS pada klinik VCT (Voluntary Conseling and Testing). Analisis statistik yang digunakan yaitu analisis deskriptif dan bivariat dengan uji korelasi pearson. Sebanyak 52 sampel yang masuk kedalam kriteria inklusi didapatkan nilai korelasi positif lemah antara jumlah limfosit total dan CD4+ pada sampel pre-HAART (r = 0.396) dengan nilai sig 0.004 sedangkan pada sampel post-HAART didapatkan korelasi positif kuat (r = 0.665) dengan nilai sig 0.000. Terdapat korelasi yang signifikan antara TLC dan CD4+ pre-HAART dan post-HAART

Abstract

Dukungan sosial merupakan dukungan hidup berupa bantuan dan interaksi sosial yang diberikan oleh keluarga dan lingkungan kepada seseorang. Dukungan sosial dapat meningkatkan kualitas hidup. Pasien pasca stroke dapat mengalami suatu depresi dan penurunan kualitas hidup akibat kondisi yang dialaminya sehingga akan menjadi kurang responsif dalam proses rehabilitasi. Hal tersebut menyebabkan pasien pasca stroke membutuhkan dukungan sosial terutama dari keluarga dan orang-orang terdekat sehingga dapat menerima kondisinya setelah mengalami serangan stroke. Kemampuan seseorang menerima kondisi hidupnya disebut penerimaan diri. Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara dukungan sosial dengan penerimaan diri pada pasien pasca stroke. Penelitian ini menggunakan metode kuantitatif dengan teknik pengambilan data secara cross-sectional. Pengambilan data dilakukan dengan menggunakan kuesioner yang terdiri dari kuesioner dukungan sosial dan kuesioner penerimaan diri. Responden yang telah memenuhi kriteria inklusi dan eksklusi dipilih dengan tehnik consecutive sampling. Analisis statistik yang digunakan adalah analisis deskriptif dan analisis bivariat dengan menggunakan uji korelasi Spearman. Jumlah responden pada penelitian ini adalah 55 orang. Sebanyak 43 orang (78,2%) responden memiliki dukungan sosial baik dan sebanyak 12 orang (21,8%) responden memiliki dukungan sosial cukup. Sebanyak 13 orang (23,6%) responden memiliki penerimaan diri baik, sebanyak 39 orang (70,9%) memiliki penerimaan diri cukup, dan sebanyak 3 orang (5,5%) memiliki penerimaan diri kurang baik. Hasil uji korelasi Spearman menunjukkan nilai p=0,008 dengan r=0,356. Terdapat hubungan yang signifikan antara dukungan sosial dengan penerimaan diri pada pasien pasca stroke

Abstract

Hemichorea / hemibalismus (HCHB) and other hyperkinetic movement disorders are a rare presentation of stroke, usually secondary to deep infarctions affecting the basal ganglia and the thalamus. This case report aims to describe patients who experience HCHB as early symptoms of acute stroke infarction accompanied by hyperglycemia. In this case the complaint was accompanied by facial and hypoglossus nerve paresis, and hemiplegic of the contralateral limb of the lesion. Serum glucose and HBA1C levels are elevated. CT scan image shows an infarction in the basal ganglia. The combination of new striatal lesions and hyperglycemia can induce HCHB because both of these can lead to increased inhibition of the subtalamic nucleus and disruption of thalamus GABAergic inhibition. Abstract

Approximately, 10-20% of stroke cases in a year caused by a hemorrhagic strokes. The most dangerous and deadly of hemorrhagic stroke’s is subarachnoid hemorrhage (SAH). SAH have higher impact on economic, morbidity, and mortality than other types of stroke even though the prevalence is quite low. About 80% of non-trauma subarachnoid hemorrhage (SAH) cases are caused by rupture of intracranial aneurysm, which encourages blood to fill the subarachnoid space and stimulates the surrounding structures, it’s cause pathognomonic symptoms of stiff neck and thunderclap headache. Even though there are typical symptoms, misdiagnose of this stroke is still happens. Therefore, a comprehension of all aspects that are related to subarachnoid hemorrhage is very important in optimizing patient ability and service. Abstract

Background: Diabetes Melitus (DM) is a degenerative diseases characterized by hyperglycemia condition. Basic Health Research data for 2018, shows an increase in prevalence of DM in Indonesia over last 5 years with 6,9% in 2013 to 8,5% in 2018. Pancreatic insulin receptor expression can be a marker for assessing presence of ? cells, production and secretion of insulin. Insulin could cause GLUT4 to be translocated to cell membrane of hippocampal neurons. In DM conditions, there may be changes in expression of pancreatic insulin receptors accompanied by changes in expression of GLUT4 hippocampal neurons. Methods: This experimental research used 16 white rats (Rattus Novergicus) which divided randomly into 4 groups, namely 2 control grups (A0 and A1) and 2 treatment groups (B0 and B1). Treatment group were induced by diabetes with a single dose of nicotinamide 110 mg/kgBB and streptozotocin 70 mg/kgBB intraperitoneally. The research data were obtained from immunohistochemical analysis of pancreatic and hippocampal tissue. Result: The mean expression of pancreatic insulin receptor in diabetic rat group was higher but not significant on the 14th day observation compared to the 0th day (p>0,05), while the mean GLUT4 expression of hippocampal neurons was significantly higher on 14th day of observation compared to the 0th day (p<0,05). The result of correlation test did not show a significant results in each group both at the 0th and 14th day observation (p>0,05). Conclusion: There was no significant effect of pancreatic insulin receptor expression on hippocampal neuron GLUT4 expression in diabetic model rats.

Abstract

Background: Epilepsy is the highest prevalence diseases in neurology. The decreased of cognitive function is one of the complications which is important. The purpose of this study is to determine the correlation between Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment in Indonesian Version (MoCA-Ina) score on patient with epilepsy in Mataram. Method: This is an observational cross-sectional study with 56 subjects with epilepsy from West Nusa Tenggara 2 center hospital. Data that collected from the samples were MMSE and MoCA-Ina score. Beside of that patients’ characteristics data were also collected which is age, gender, education, seizure control, etiology of epilepsy, seizure type, antiepileptic drugs treatment, and smoking status from the subjects. Both instrument’s mean scores were analyzed for the correlation using Spearman correlation test. Result: This study showing that the subjects average age were 32.9 years old, female (55.4%), and low educated (62.5%). According to the clinical characteristics most subjects were not-depressed (67%), poor quality-controlled seizure (55.4%), idiopathic etiology (66.9%), generalized seizure (71.4%), and having monotherapy (85.7%). Most of the subjects were not a smoker (76.8%). In this study both the mean score of MMSE and MoCA-Ina were 25.3 and 21.3. MMSE and MoCA-Ina score mean have a strong correlation (r=0.766; p<0.001). Conclusion: MMSE and MoCA-Ina score mean on patients with epilepsy have a strong correlation. Therefore, both instruments could be used to evaluate cognitive function on patients with epilepsy

Abstract

Executive dysfunction is a consequence of type 2 diabetes mellitus decreasing the quality of life of the patients. Currently, its prevalence is still scarce. Early detection of this executive dysfunction is beneficial in improving the clinical outcomes of the patients. This study aims to investigate the prevalence of executive dysfunction among type 2 diabetes mellitus patients in Mataram. This case-control study involving 106 subjects divided into two groups, type 2 diabetes subjects (n=53) and healthy subjects (n=53) in outpatient setting. The data collected from both groups were age, gender, years of education, TMT-B score, and executive function status. The data collected only from type 2 diabetes mellitus group were age, gender, years of education, duration of diabetes, treatments, smoking, hypertension, coronary heart disease, body mass index, and dyslipidemia. There was significant difference in the frequency of executive dysfunction between type 2 diabetes mellitus and healthy subject groups (p<0.05). The prevalence of type 2 diabetes mellitus-associated executive dysfunction was 49.1%. Gender was the only characteristics associated with the prevalence of type 2 diabetes mellitus-associated executive dysfunction (OR=0.25; 95%CI=0.07-0.88, p<0.05). There was a high prevalence of type 2 diabetes mellitus-associated cognitive impairment and it was associated with female gender.
