Abstract

Tension-Type Headache (TTH) headache is a type of primary headache which mostly complained by patients. The pain often ignored due to low mortality rates. Inadequate management makes the pain becoming chronic and causing a high disability rate. Many factors involved in transforming infrequent tension-type headache to chronic tension-type headache. A deeper understanding of the immune system's role in the pathophysiological process and modulation of chronic pain could be a potential target for developing therapies in managing chronic pain.Keywords: chronic tension-type headache, immunology aspect, pathophysiology

Abstract

Headache is one of the most common neurological complaints. The InternationalHeadache Society (IHS) classification divides headaches into a primary and secondaryheadache. The cause or type of headache can be determined from the history of thedisease and physical examination. Neurophysiological tests are used in establishing adifferential diagnosis of headache. There are several neurophysiological methods usedin the diagnosis of headaches, one of them is electroencephalography. This study aimsto look at the most frequent EEG picture in cases of a primary headache without focalneurological deficits in the Neurology Polyclinic of Dr. M. Djamil Padang Hospital fromJanuary 2019 to December 2019. This research used a descriptive method. The studypopulation was all patients who underwent an EEG examination in the IntegratedDiagnostic Installation (IDT) of Dr. M. Djamil Padang Hospital from January 2019 toDecember 2019. Subjects were patients who had a chief complaint of headache whohad been subjected to an EEG examination. Statistical analyzes were performedcomputerized using IBM SPSS Statistics version 23.0 for Windows. From this study,the most common chronic headache is migraine, and the most common EEG feature isnormal. EEG can be used as a modality for investigating chronic headaches withoutfocal neurological deficits.

Abstract

Introduction : Anxiety is one of the most non-motorized symptoms in patients with Parkinson's which greatly affects the quality of life, but in clinical practice it is often neglected. Anxiety in patients with Parkinson's can accelerate motor deterioration / disability and also increase mortality. Methods: This cross-sectional study was conducted at the Neurology polyclinic Dr. M Djamil Padang from July to December 2020 in patients who had been diagnosed with Parkinson's Disease (PD) by excluding secondary Parkinson's and a history of stroke. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS). The research subjects were 60 people. Univariate analysis to present baseline characteristics and bivariate tests assessed factors associated with anxiety and the relationship between variables. A p value <0.05 was considered statistically significant. Results: Most of the subjects were male (55%) with a mean age of 58.05 ± 9.7 years and disease duration of 6.35 ± 5.29 years. By examining the Hamilton Anxiety Rating Scale (HARS), it was obtained 38.3% of Parkinson's sufferers with anxiety. There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety (p <0.05) and there is no significant relationship between age and sex with the incidence of anxiety in patients with Parkinson's. Conclusion: There is a significant relationship between marital status, duration of illness and degree of disease with the incidence of anxiety in patients with Parkinson's and there is no relationship between age and sex with the incidence of anxiety in patients with Parkinson's.

Abstract

Anxiety is one of the most common non-motor symptom in Parkinson's disesase which greatly affects oatients’ quality of life, however it’s often neglected in clinical practice. The purpose of this study was to determine the rate and association between anxiety and Parkinson's disease using Hamilton Anxiety Rating Scale (HAR). This was a cross sectional study conducted at Neurology Polyclinic of Dr. M. Djamil Padang General Hospital. Sample was taken by consecutive sampling method. There were 60 patients diagnosed with Parkinson's disease who met inclusion criteria. Male (55%) patients were more common than female (45%) and most patients aged more than 50 years (63%). Based on the HARS assessment obtained 23 patients with anxiety and 37 patients without anxiety. There were significant associations among duration of illness, staging, and marital status with anxiety in Parkinson’s disease.

Abstract

Contraception is a very important issue in women with epilepsy, since very broad questions taken in applying it to prevent unplanned pregnancy. The choice of contraceptive methods must be related to how the mechanism of action of antiepileptic drugs (AEDs), the contraceptive itself, and both interactions. Hormonal contraception can induce metabolism of AEDs and lead to increasing of seizure frequency, and in the other hand AEDs may decrease the blood levels of hormonal contraception. Therefore, physician must consider the type of contraception being used before prescribing AEDs. Additionally, they should emphasize the interaction between them in counseling, to assure that women with epilepsy have time to decide which type will be best use for them. This challenging matter needs better recommendation for practice, both neurologist and gynecologist.Keywords: antiepileptic drug, contraception, women with epilepsy

Abstract

Migraine is a common headache characterized by unilateral throbbing-like headache and pulsating in nature and sometimes associated with aura. Migraine is a disabling disorder that among adults is more prevalent among women than men. It is primary headache that is often found in pregnancy. Migraine is a common disorder in women of childbearing age, and usually requires pharmacological treatment. Migraine can be considered an important risk factor for hypertensive and vascular diseases during pregnancy. Migraine therapy in pregnancy is very challenging, it must taken into a consideration about the health of the mother and fetus.  Several effective antimigraine medications are reasonably safe for use by pregnant and breastfeeding women. Prophylactic theraphy should be given to patient with recurrent migraine attacks in pregnancy. Nonpharmacological strategies are always first-line treatment options for mild migraine, and should also be used complementarily whenever pharmacological treatment is required. Women with migraine should be offered periconceptional counselling to promote a safe and healthy pregnancy. Keywords: migraine; pregnancy; therapy

Abstract

Menstrual Migraine is divided into 2 subtypes: Menstrually Related Migraine (MRM) and Pure Menstrual Migraines (PMM). In PMM symptoms do not occur outside the menstrual cycle while MRM, symptoms can occur at other times apart from the menstrual cycle. The occurrence of menstrual migraines is related to the female hormones cycle in the form of the decrease in estrogen levels which usually occurs a week before the onset of menstruation. The mechanism is unclear, but it is thought that a decrease in estrogen levels can trigger decrease in serotonin levels, causing cranial vasodilation and sensitization of the trigeminal nerve.  Keywords: menstrual migraine, hormones

Abstract

Corona virus disease 2019 (COVID-19) was declared as global pandemic and caused devastating crisis in society. Despite of the growing pandemic, high quality medical services toward gynaecologic oncology patients must continue without overlooking the safety of medical staffs. Reducing risk is crucial and achieved by limiting high risk situations. The decision to perform or postpone surgery should be made based on the type and stage of the disease, medical condition of the patient, area census of COVID-19 cases, COVID-associated risks, and available logistic support including adjuvant treatment services. There are several recommendation for gynaecology cancer treatment published by several countries. However, those guidelines cannot be applied to every country across the globe because of the different situations of COVID-19 therefore we proposed guidelines for Indonesia. Surgical management for confiermed COVID-19 case should be postponed for at least 15 days for nonemergency cases. Surgery must be performed immediately for emergency cases such as Haemorrhage with unstable vital status refractory to transfusion, viscus perforation, signs of bowel obstruction refractory to conservative treatment, closed loop bowel or large bowel obstruction, hydatiform mole for live saving procedure.  Keywords: COVID-19, gynaecology cancer, surgical management
