Abstract

Berdasarkan data Badan Pusat Statistik (BPS) pada September 2014-Maret 2015, diketahui bahwa telah terjadi kenaikan signifikan jumlah masyarakat penduduk miskin dari kalangan petani. Bertambahnya tingkat kemiskinan di kalangan petani akan mempengaruhi tingkat kesehatan para petani tersebut. Kajian ini ingin melihat hubungan timbal balik antara pendapatan petani yang diukur dengan nilai tukar petani (NTP), dengan angka harapan hidup di Sumatera Utara. Data yang digunakan dalam kajian ini adalah data nilai tukar petani dan data angka harapan hidup penduduk di Sumatera dari tahun 1998-2013. Hasil uji kausalitas Granger menunjukkan bahwa angka harapan hidup mempunyai hubungan kausal dengan tingkat pendapatan petani. Hal ini menyokong pendapat yang menyatakan bahwa perbaikan kondisi ekonomi akan menyebabkan perbaikan kesehatan, tidak sepenuhnya benar. Namun, hal sebaliknya lah yang benar, yaitu kondisi yang sehat akan menyebabkan perbaikan ekonomi. Dengan demikian kebijakan pembangunan kesehatan bagi petani amatlah penting agar mereka dapat meningkatkan pendapatannya.Kata Kunci: Nilai Tukar Petani, Jangka Hayat dan Uji Kausalitas Granger

Abstract

Determination of tariffs with traditional methods is considered unable to overcome various difficulties in determining rates in hospitals. Activity-Based Costing (ABC) is one method of calculating rates that are considered efficient. The purpose of this study was to determine the ratio of unit cost to INA-CBG's rates in class III acute appendicitis patients at Bhayangkara Medan Secondary Hospital in 2018. This study is a quantitative descriptive study using the ABC method, where this study calculates the unit cost of acute appendicitis. Samples were taken based on total sampling technique, that is, all patients with class III acute appendicitis in Bhayangkara Hospital Medan. Data was collected using primary data in the form of direct interviews from the field and secondary data in the form of patient medical records, as well as costs incurred from all activities that happened. Analysis of the data used in this study is a non-statistical quantitative analysis test. The results of the study show that the unit cost is smaller than the INA-CBG's rate with a difference of Rp. 1,699,448.00. It is recommended that the Head of the Bhayangakara Level II Hospital in Medan analyze the unit cost of each disease using the ABC method in order to have efficient tariffs.

Abstract

This study deals with patients' perceptions of food service quality at Royal Prima Hospital Medan. Someone's perception in evaluating the quality of service will greatly affect customer satisfaction. From someone's perception, it will produce a sense of satisfaction or dissatisfaction with the service. And this will greatly affect the desire or willingness to use or use the service. This type of qualitative research will be used is a case study approach with in-depth interviews with informants. The case study approach focuses intensively on one particular object that studies it as a case. Based on HTA assessment that the patient's perception of nutritional service at Royal Prima Hospital is still in accordance with what is expected of the patient's wishes. It can be concluded that the method of serving food at the hospital is good in its implementation, where the waitress in serving food greets the patient by mentioning the patient's identity to adjust whether the diet given is right for the intended patient. Therefore, the assessment based on HTA has met the MOH standards.

Abstract

Regency in North Sumatra Province has 213 hospital units consisting of 187 general hospitals and 26 specialized hospitals, although there are many health services but many seek treatment abroad, such as to Penang, Malaysia. In 2015 there were 600,000 patients taking treatment to Penang. The amount of government foreign exchange issued to foreign hospitals is as much as 600 million dollars in one year, if this is not immediately handled it will have a negative impact on the country's economy. The purpose of this study is to explore the experience of Karo Ethnics who seek treatment in Penang. This type of research is a qualitative research with a phenomenological approach. Participants numbered eight people. The themes found in this study are three, namely (1) getting a financial comparison, (2) choosing health services, (3) getting satisfying health services. In conclusion, participants had less than satisfactory experiences, so Karo Ethnics tried to find and choose satisfying health services. It is recommended to health services in Indonesia to improve health services to the community.

Abstract

The choice of referral hospital from Hospital of Subulussalam City based on a preliminary survey is varies. Inpatients are referred to Class A hospitals are on average 65%, Class B hospitals are 30%, and Class C hospitals are 5%, while outpatients are referred to Class A hospitals are on average 49%, Class B hospitals are 28%, class C hospitals are 23%. This type of research is qualitative with a phenomenological approach obtained by conducting in-depth interviews with 13 informants consisting of 3 specialist doctors, 3 general practitioners, 3 patients, 3 patient's family and 1 BPJS verifier in Hospital of Subulussalam City. The study was conducted from February to October 2018. The results showed that during the referral hospital decision-making process there was an interaction between the doctor, the patient and the patient's family, the final decision was on the patient and bearing down the consequences if the hospital's choice was not recommended by the doctor. The determinants of decision making for referral hospitals consist of perceived quality in the form of referral hospitals accepting patients referred from Hospital of Subulussalam City, emergency conditions for patients by choosing the closest hospital, facilities in the form of supporting tests and complete medical devices and additional facilities, the availability of specialist doctors and subspecialty according to the patient's condition, geographical constraints due to the long distance to the referral hospital, references from doctors to patients and vice versa, learning in the form of doctor and patient experience in the previous referral process, not all doctors consider peer relations to be a determinant of choosing a referral hospital due to the BPJS system, social factors in the form of a family in the area of ​​a referral hospital, the same religion and culture as a patient in a referral hospital area make the patient feel comfortable. Suggestions in this study is the management of Hospital of Subulussalam City to confirm and coordinate the referral hospital and make a referral MOU.

Abstract

This research deals with the relationship between motivation factors and unsafe action on passenger ship crews in Tanjung Pinang. This type of research is an analytical survey using cross sectional design aimed at explaining a situation by analyzing the relationship between individual factors and work factors with unsafe action on the crew of passenger ships in Tanjung Pinang City. The study was conducted at the port of Sri Bintan Pura in Tanjung Pinang City. The population in this study is the crew of passenger ships with the position of Captain, Mualim, KKM (Head of Engine Room), and Oiler in the port of Sri Bintan Pura Tanjungpinang City with the destination of Tanjungpinang to Batam. Based on the results of the study indicate that there is a significant relationship between motivation and unsafe action. In the multivariate test, it is known that motivation variable has an influence on unsafe action with p value = 0.019 and exp (B) value of 8.577, which means that the motivation variable in the low category will cause unsafe action to be high by 8.577 times compared to the high motivation category. It can be conclude that the majority of respondents classified as young, have a high level of education, a new tenure, have a high income, good knowledge of occupational health and safety, high motivation, and unsafe action. Based on the bivariate analysis obtained levels of education, years of service, knowledge, motivation, related to unsafe action.

Abstract

The Indonesian government has developed a Health Insurance program National Social Security Administering Bodies (BPJS) Health. One of the ways to pay for health services for hospitals in Era JKN is the Indonesian Case Base Groups (INA-CBG's) payment system, namely the amount of claim payment by BPJS Health to the hospital for the package services based on diagnosis of disease and procedures. This study aims to analyze hospital real rates and INA CBG rates on the action of sectio caesarea and to find out the efforts made hospitals to cover INA-CBG's shortfall in claims costs. Types of research Qualitative descriptive conducted by in-depth interviews. Research sites at the hospital. Imelda Indonesian Worker having his address at Jalan Bilal No. 24 Kelurahan Pulo Brayan Darat I, East Medan District. The research was carried out since months January 2019 until finished. The population in this study is claim data delivery of inpatients who performed sectio casearea at month March and April as many as 143 cases The samples in this study were all population of 143 cases. The results showed that the difference in real rates hospital and INA-CBG rates on Sectio Caesaria at RSU. Imelda Worker Indonesia Medan in 2019 amounted to 73% of the real hospital rates exceeding the tariff INA-CBG's with a total difference in costs reaching IDR 199,661,028. and 39% real hospital fee less than INA-CBG's package rate. Efforts are conducted by the RSU. Imelda Indonesian workers to cover the shortage of costs INA-CBG's claims include using students who come from schools Imelda's own foundation, set the Sectio Caesare service package on general patients and providing supporting facilities. Suggested to RSU. Imelda Indonesian workers evaluate the financial risks received from services Sectio Caesare and evaluating the distribution of INA-CBG's claim costs for services Sectio Caesare is up to standard.

Abstract

Rekam medis merupakan salah satu unsur utama didalam sistem pelayanan kesehatan Rumah Sakit yang baik dan bermutu. Ketidaklengkapan dokumen rekam medis menjadi salah satu masalah yang masih belum teratasi sampai saat ini. Pada survei awal yang dilakukan di ruang rawat inap Rumah Sakit Permata Bunda, masih didapati ketidaklengkapan dokumen rekam medis oleh dokter spesialis. Tujuan penelitian adalah untuk mengetahui pengaruh komitmen dokter spesialis terhadap pengisian rekam medis di ruang rawat inap Rumah Sakit Permata Bunda Kota Medan. Jenis penelitian ini adalah survei explanatory research dengan desain cross sectional. Lokus penelitian ini di Rumah Sakit Permata Bunda Kota Medan, pada bulan Juli tahun 2018 sampai dengan selesai. Keseluruhan jumlah populasi juga merupakan sampel penelitian yaitu sebanyak 41 dokter spesialis. Metode analisis data menggunakan regresi logistik. Hasil penelitian menunjukkan variabel komitmen dokter berpengaruh secara signifikan terhadap pengisian rekam medis. Semakin berkomitmen seorang dokter spesialis maka akan meningkatkan pengisian rekam medis dibandingkan dengan dokter spesialis yang tidak berkomitmen. Disarankan kepada manajemen rumah sakit agar memberikan reward kepada dokter spesialis yang meluangkan waktu lebih dalam mengisi rekam medis. Kata Kunci: Komitmen, Dokter Spesialis, Rekam Medis

Abstract

Kualitas pelayanan merupakan tingkat kesempurnaan pelayanan kesehatan yang memuaskan pelanggan yang diberikan sesuai dengan standard dan etika profesi. Kepuasan pelanggan merupakan salah satu aspek yang dipengaruhi oleh kualitas pelayanan. Salah satu sarana kesehatan yang dinilai turut adil dalam pelayanan kesehatan adalah Rumah Sakit. Penelitian ini bertujuan untuk menganalisis pengaruh kualitas pelayanan terhadap kepuasaan pasien di rawat jalan penyakit dalam RSUD Dr. RM Djoelham Binjai. Populasi dalam penelitian ini adalah seluruh pasien yang berobat ke rawat jalan penyakit dalam RSUD Dr. RM Djoelham Binjai dengan sampel sebanyak 57 pasien, yang diambil secara accidential, dengan kriteria merupakan pasien yang berobat ke rawat jalan penyakit dalam RSUD. Dr. RM. Djoelham Binjai. Hasil penelitian menunjukkan bahwa secara statistik kualitas pelayanan meliputi kehandalan (0,008), jaminan (0,030), bukti fisik (0,001), perhatian (0,026), dan daya tanggap (0.001), berpengaruh signifikan terhadap kepuasan pasien rawat jalan di rawat jalan penyakit dalam RSUD Dr. RM Djoelham Binjai. Kualitas pelayanan RSUD Dr. RM Djoelham Binjai pada dasarnya telah terakreditasi paripurna. Namun untuk selanjutnya agar kualitas semakin baik lagi pihak manajemen rumah sakit harus meningkatkan standard pelayanan ke tahap internasional yang diikuti dengan langkah-lankah monitoring dan evaluasi setiap tahunnya. Abstract

Dalam asuhan keperawatan, perawat harus memiliki kemampuan dan motivasi untuk memberikan layanan terbaik. Dengan kemampuan dan motivasi, perawat akan terdorong untuk melakukan pekerjaannya dengan baik. Survei awal yang dilakukan di Rumah Sakit Permata Bunda, menemukan bahwa lebih dari 45% perawat tidak senang dengan gaya kepemimpinan kepala ruangan yang didominasi oleh metode otoriter. Selain itu, ditemukan bahwa 27 (90%) dari 30 perawatan masih memiliki lowongan. Sampel dan populasi penelitian ini adalah seluruh perawat yang bekerja di bangsal rawat inap Rumah Sakit Permata Bunda Kota Medan yaitu sebanyak 106 orang. Metode analisis data dalam penelitian ini menggunakan analisis regresi logistik. Hasil analisis multivariat menunjukkan bahwa variabel kompetensi supervisor memiliki nilai p = 0,056 dan OR 3,201, yang berarti bahwa kompetensi superviso memiliki pengaruh terhadap motivasi kerja perawat dan kompetensi pengawas yang baik akan meningkatkan motivasi kerja perawat sebesar 3,2 kali. Saran kepada pimpinan rumah sakit untuk meningkatkan kualitas pengawasan untuk meningkatkan motivasi kerja perawat yang akan mempengaruhi peningkatan kualitas kerja dan kualitas pelayanan.Kata Kunci: Motivasi Kerja, Kompetensi Supervisor, dan Penghargaan
