Abstract

About 40%of heart disease patients do not realize they have coronary heart disease (CHD). Based on the lifestyle of people in Aceh, there are several factors that can increase the CHD risk, such as smoking, coffee, fatty foods, stress, and lack of physical activity. Such a lifestyle has the potential to increase the risk of CHD, as evidenced statistical papers reportinghigh levels of CHD cases in Aceh. This study was aiamed toreview the patients lifestyle with coronary heart disease at Zainoel Abidin Hospital, Banda Aceh, Indonesia. This research is quantitative, with a case-control design.In April 2017, 206 cases of CHD and 206 non-PJK cases were analyzed, using patients medical record and a lifestyle questionnaire, in Zainoel Abidin Hospital. The result of bivariate analysis through a chi-square test showed that lifestyle factors related to CHD included diet, physical activity, smoking, coffee, and stress control (p <0,05). The results of the multivariate analysis with logistic regression showed that smoking is a lifestyle that remains a consistent as a risk factor for CHD. In addition, after interacting with other risk factors, stress control and coffee drinking habits become a protective factor of CHD. Smokers are twice risky to have CHD than nonsmokers. The influence of lifestyle on CHD is also determined by age and income factors. The research implications of this study offer various parties the opportunity to pursue an active involvement in preventive efforts against CHD.Keywords: Determinant, lifestyle, coronary heart disease, Zainoel Abidin Hospital

Abstract

Background: HIV and AIDS is a global public health problem. Until the end of 2016, there were more than 36.7 million people living with HIV. HIV & AIDS counseling is a confidential communication between clients and counselors aimed at increasing the ability to deal with stress and make decisions regarding HIV & AIDS. The purpose of this study was to determine the effect of the ability to pay and the environmental factor on consultation health service utilization at Budi Kemuliaan hospital, Batam.Subjects and Method: This was a cross-sectional study was conducted at Budi Kemuliaan Hospital, Batam, from May 8 to June 18, 2018. A sample of 76 HIV patients was selected by systematic random sampling. The dependent variable was the use of consulting service. The independent variables were risk of illness and environment. The data were collected using questionnaires and analyzed by a multiple logistic regression.Results: The use of consultation health service was influenced by the risk of illness (OR = 6.82; p = 0.011) and supportive environment (OR= 4.59; p= 0.028).Conclusions: The use of consultation services is influenced by risk factors for illness and the environment.Keywords: consultation service, the risk of illness, environment, HIV/AIDS patientCorrespondence:Dawiyah Siregar. Abstract

Background: Strategic purchasing plays an important role in the cost structure and competitiveness of small and large businesses. Managing Board of Social Insu-rance (BPJS) has implemented strategic purchasing of health services in the National Health Insurance (JKN) scheme. Strategic purchasing is important to provide quality products and services at low costs. Strategic purchasing of health service has five critical criteria: (1) what is to purchase; (2) to whom it is for; (3) whom it is from; (4) what is the price; (5) how to purchase it. This study aimed to assess BPJS barrier to strategic purchasing of primary care service at public and private health facilities in Subulussalam, Aceh.Subjects and Method: This was a qualitative study conducted in Subulussalam, Aceh, from February to July 2015. A total of 12 key informants for this study, including BPJS administrators and hospital managers. The theme was strategic purchasing of health service. The data were collected by in-depth interview, direct observation, and document review. The data was analyzed by thematic analysis.Results: BPJS administered the same standards of strategic purchasing health service from the public and private health providers. However, there were some unresolved problems: (1) BPJS faced difficulty to select primary care providers independently; (2) Illegal use of other members JKN card to get health service; (3) Redundant claims from the primary care providers; (4) Claim of inpatient days exceeding the maximum allowable limit; (5) Delay in non-capitations reimbur-sement payment to primary care provider; (6) Inequitable distribution of health manpower across health facilities.Conclusion: BPJS has implemented strategic purchasing of primary care services with the same standards applied to public and private primary care providers. However, there remain some problems of purchasing health services that call for immediate corrective actions, including routine monitoring, explanation of delay, advocacy, equitable distribution of health manpower.Keywords: strategic purchasing, primary care service, health facility, BPJS.Correspondence: Satria Darma. Abstract

The basic principle of a hospital is fulfilling the needs and demand of patients who hope that their health problems can be settled in a hospital. They expect ready, quick, responsive, and comfortable services. Patient satisfaction can be influenced by the health care quality of health care providers who sympathize, respect, and respond to their needs. The research used a qualitative method. The data were gathered by conducting participant observation, and in-depth interviews. The research subjects were 4 informants, IGD (emergency room) personnel and  IGD patients in Prima Inti Medika hospital. The result of the research, in the pre-service, showed that nurses’ response was quick, complaints were responded well, the process of inpatient and outpatient was good, nurses’ cooperation was good, and the personnel was accredited; but, the facility was inadequate and incentives were lower than the government’s standard. From the patients’ perspectives, it was found that pre-service was in accordance with SOP, the process of doctors’and pharmacy’s service was good, but nurses’ service and IGD facility were unsatisfactory, nurses’ cooperation was good, but their competence and quality were inadequate.

Abstract

Background: HIV has become a problem in the international community. Rapid transmission of HIV becomes a threatfor the entire global community in order to prevent an increasingly widespread. One of the factors that causes HIV is unhealthy sexual behavior. This study aimed to determine the effect of changing partners and condom use on HIV incidence in men having sex with Men (MSM) in the Teladan Health Center in Medan.Subjects and Method: This was case-control study conducted at Teladan Health Center, in May 2018. A sample of 96 study subjects was selected for this study. The dependent variable was the incidence of HIV. The independent variables were multiple sex partners and condom use. HIV status was obtained from medical record. The other variables were measured by questionnaire. The data were analyzed by a multiple logistic regression.Results: Multiple sexual partners (OR= 2.93; 95% CI= 1.25 to 6.86; p= 0.022) and absence of condom use (OR= 2.83; 95% CI= 1.23 to 6.52; p= 0.024) increased the risk of HIV among men having sex with men.Conclusions: The occurrence of HIV in MSM is probably 2.9 times due changing partner actions and 2.8 times due to the inconsistent condom use.Keywords: HIV, multiple, sexual partners, condom use, men having sex with menCorrespondence:Evi Erlinda Wati. Abstract

Background: The maintenance phase in methadone maintenance therapy required a long time un­til the patient was declared recovered, so that it often caused saturation and results in high drop­out rates. Therapy retention or survival of patients in the process of therapy was one indicator of the success of this methadone therapy. This study aims to determine the effect of patient charac­te­ris­tics and methadone maintenance therapy dose on the retention of methadone therapy at H. Adam Malik General Hospital Medan.Subjects and Method: This study used a cross sectional design and was carried out at H. Adam Malik General Hospital in Medan. The number of samples in this study was the total population of 63 patients in the maintenance phase January 2017 to January 2018. The dependent variable was the retention of methadone therapy and the independent variables included age, education level, ma­rital status, work status, domicile, and average maintenance dose even. The data in this study were obtained from the medical record data of maintenance phase methadone patients and the data analysis was done using multiple logistic regression tests.Results: The results showed that the average maintenance dose had an effect on retention of me­tha­done therapy (OR = 5.94, 95% CI 1.06 to 33.18, p = 0.042).Conclusions: The maintenance dose of ≥60 mg affects the retention of methadone therapy 6 times higher than the dose of <60 mg.Keywords: Therapy retention, patient characteristics, maintenance dosesCorrespondence: Dian Natalya Simangunsong, Master’s Program in Public Health of Uni­ver­si­tas Sumatera Utara, Jl. Abstract

Secara global sekitar 17 juta kematian pertahun akibat penyakit kardiovaskular, hampir sepertiga dari total penduduk dunia. Dari data tersebut, komplikasi dari hipertensi mengakibatkan 9,4 juta kematian setiap tahun. Prevalensi hipertensi di Indonesian yang didapat melalui pengukuran tekanan darah  pada penduduk umur 18 tahun  ke atas yaitu sebesar 25,8%. Prevalensi hipertensi di Sumatera utara berdasarkan diagnosis oleh tenaga kesehatan sebesar 6,8%. 31,7% atau 1 dari 3 orang mengalami hipertensi. Sekitar 75% penderita hipertensi tidak mengetahui bahwa dirinya menderita hipertensi. Mereka baru menyadari jika telah terjadi komplikasi. Di Indonesia, ancaman hipertensi tidak boleh diabaikan. Hal ini dapat dibuktikan dengan jumlah penderita hipertensi yang setiap waktu semakin bertambah. Penelitian ini bertujuan untuk melihat pengaruh aktivitas fisik  terhadap kejadian hipertensi pada laki-laki dewasa awal (18-40 tahun) di wilayah Puskesmas Bromo Medan tahun 2017. Jenis penelitian adalah ini adalah studi analitik observasional dengan menggunakan desain case control. Sampel penelitian sebanyak 44 kasus dan 44 kontrol yang ditetapkan secara consecutive sempling. Pengumpulan data dengan menggunakan kuesioner. Analisis data dilakukan secara univariat dan bivariat dengan menggunakan uji chi-square.  Berdasarkan hasil penelitian menunjukkan terdapat pengaruh aktivitas fisik terhadap kejadian hipertensi pada laki-laki dewasa awal (18-40 tahun) dengan nilai  p= 0,010 OR= 3,095 (95%CI: 1,292-7,417). Disimpulkan bahwa laki laki dewasa awal (18-40 tahun) yang beraktivitas fisik ringan memiliki perkiraan risiko 3 kali akan terkena hipertensi dibandingkan dengan yang beraktivitas fisik sedang dan berat.

Abstract

Menurut data WHO, 171 juta penderita Diabetes Mellitus dan akan meningkat dua kali, 366 juta pada tahun 2030. Prevalensi Diabetes Mellitus di Indonesia mencapai jumlah 8.426.000 yang diproyeksikan mencapai 21.257.000 pada tahun 2030. Artinya, terjadi kenaikan tiga kali lipat dalam waktu 30 tahun. Diabetes Mellitus merupakan salah satu masalah kesehatan yang besar didunia, Di Provinsi Nanggroe Aceh Darussalam data pada tahun 2017 dari bulan Januari sampai April, terdapat Pra Lansia (45-59) yang mengalami DM sebanyak 2.512 orang. Sedangkan di BLUD RSUD Meuraxa Kota Banda Aceh jumlah Pra Lansia (45-59) yang mengalami Diabetes Mellitus tipe I dan II adalah 484 orang. Aceh masuk dalam daftar sembilan besar daerah Indonesia yang penduduknya banyak menderita penyakit DM. Diabetes Mellitus (DM) merupakan penyakit metabolik yang ditandai dengan peningkatan kadar gula darah akibat gangguan pada sekresi insulin, kerja atau keduanya. Adapun tujuan penelitian adalah untuk menganalisis pengaruh kebiasaan makan, aktivitas fisik, dan riwayat keturunan  terhadap kejadian Diabetes Mellitus pada Pra Lansia di BLUD RSUD Meuraxa Kota Banda Aceh tahun 2017.Jenis penelitian adalah analitik observasional desain case control. Sampel penelitian sebanyak 61 kasus dan 61 kontrol. Pengumpulan data dengan kuesioner. Analisis data dilakukan secara bivariat dengan menggunakan uji chi-square. Berdasarkan hasil penelitian menunjukkan terdapat pengaruh kebiasaan makan p=<0,001 OR = 5,067 (95%CI 2,324-11.048).Disimpulkan bahwa lansia yang mempunyai kebiasaan makan lebih mempunyai risiko 5 kali akan menderita diabetes mellitus dibandingkan dengan lansia yang mempunyai kebiasaan makan cukup. Kata kunci: Diabetes Mellitus, kebiasaan makan

Abstract

Background: Dengue hemorrhagic fever (DHF) is one of the most important public health problems in tropical developing countries. It also has major economic and societal consequences. This study aimed to assess the effectiveness of larva monitoring training on knowledge of dengue hemorrhagic fever control among primary school students in Sibolga, North Sumatera.Subjects and Method: This was a quasi-experiment before and after with no controlled design. The study was conducted in elementary school in Sibolga, North Sumatera, in 2018. A sample of 30 students was selected for this study and received training on DHF for four days. The training employed leaflet and video. The dependent variable was know­ledge. The independent variable was training on DHF. The data were collected by questionnaire and tested by t-test.Results: Knowledge of DHF among students increased by mean= 26.67 (SD= 14.51) after training, and it was statistically significant (p= 0.037).Conclusion: Knowledge of DHF among students increases after training, and it is statistically significant.Keywords: training, knowledge, dengue hemorrhagic fever.Correspondence: Wandi Damanik. Abstract

Background: Diabetes mellitus (DM) is currently one of the major health problems in the world because it has an impact on declining productivity and quality of human resources. This disease also causes sufferers to experience a state of disability, loss of productivity, and become a burden for individuals, families and communities that can cause emotional distress in people with diabetes mellitus. The purpose of this study was to determine the effect of emotional distress on self-care behavior of people with type 2 diabetes mellitus at the Matsum Health Center.Subjects and Method: This study used a cross sectional design conducted at Matsum Health Center in Medan. The number of samples was 49 people and taken with non-probability sampling technique by quota sampling. The dependent variable was self-care behavior. The independent variable was emotional distress. Data were collected by questionnaires and analyzed using Chi square test.Results: Low emotional distress increases self-care behavior in patients with type 2 diabetes mellitus (OR= 11.33; 95% CI with p = 0.004).Conclusion: Low emotional distress increases self-care behavior in patients with type 2 diabetes mellitus.Keywords: Emotional distress, self-care behavior, diabetes.Correspondence: Susy Hariaty Situmorang.
