Abstract

Kondisi hiperglikemia kronik pada diabetes melitus dapat mengganggu fungsi reproduksi, salah satunya adalah penurunan kadar testosteron total. Latihan fisik teratur merupakan salah satu pendekatan nonfarmakologi untuk menurunkan kadar glukosa darah dan memperbaiki kadar testosteron total pada diabetes melitus. Penelitian bertujuan untuk menentukan pengaruh latihan fisik teratur terhadap kadar glukosa darah puasa (GDP) dan hubungannya dengan kadar testosteron total pada tikus diabetes melitus. Penelitian eksperimental murni rancangan pre-postest with control group design dilakukan di Laboratorium Fisiologi, Fakultas Kedokteran dan Laboratorium Riset, Fakultas Kedokteran Hewan Universitas Syiah Kuala, Banda Aceh, periode Juni?Oktober 2013. Lima belas tikus putih jantan Sprague-Dawley dibagi tiga kelompok: (I) kontrol, (II) kelompok diabetes melitus, dan (III) kelompok diabetes melitus yang diberi latihan fisik. Latihan fisik dilakukan pada intensitas ringan-sedang selama 9 minggu sesuai protokol Souza. Kadar GDP diukur setiap 3 minggu selama latihan fisik dan perubahannya dibandingkan antarkelompok. Kadar testosteron total diukur setelah 9 minggu menjalani latihan fisik. Kadar testosteron total rata-rata tikus diabetes melitus yang diberi latihan fisik berbeda dibanding dengan tanpa latihan fisik (p=0,032). Terdapat korelasi negatif antara perubahan kadar GDP dan kadar testosteron total (p=0,007; korelasi Pearson - 0,661). Simpulan, latihan fisik teratur dengan intensitas ringan-sedang dapat menurunkan kadar glukosa darah dan memperbaiki kadar testosteron total pada tikus diabetes. A regular physical exercise is one of the non-pharmacological approaches to reduce blood glucose level and improve total testosterone level in diabetes mellitus. The study was aimed to determine the effects of regular physical exercise on fasting blood glucose (FBG) levels and its relationship to total testosterone levels in diabetic rats. Pre-postest experimental study with control group design, was conducted during June?October 2013 at Physiology Laboratory of Medical Faculty and Research Laboratory of Veterinary Medicine, Syiah Kuala University, Banda Aceh. Fifteen male Sprague-Dawley rats were divided into three groups: (I) control, (II) sedentary diabetic, and (III) exercise diabetic. The physical exercise was performed at low-moderate intensity for nine weeks according to Souza?s protocol. The levels of blood glucose were measured every three weeks during the regular physical exercise; the changes of blood glucose level were compared between groups, whereas the levels of total testosterone were measured after nine weeks of regular physical exercise. The mean of total testosterone level was different (p=0.032) between the group with physical exercise and without physical exercise. There was a negative correlation between changes in FBG levels and total testosterone levels (p=0.007, Pearson correlation -0.661). In conclusion, regular physical exercise with low-moderate intensity reduces blood glucose levels and improves total testosterone levels in diabetic rats. Abstract

Abstrak. Streptozotocin (STZ) dapat menginduksi terjadinya diabetes melalui kerusakan sel beta pankreas. Penelitian sebelumnya menyatakan bahwa pemberian STZ dosis rendah secara bertahap dapat menimbulkan perbaikan spontan kadar gula darah pada tikus diabetes. Penelitian ini bertujuan untuk mengamati perubahan kadar glukosa darah puasa (GDP) setelah pemberian tunggal STZ dosis rendah (35 mg/Kg i.p) selama 12 minggu. Enam belas ekor tikus putih jantan Spraguen-Dawley dengan rerata berat badan 214,40 ± 8,25 g, dibagi dua kelompok yaitu kelompok kontrol dan tikus yang diinduksi STZ dosis rendah. Tikus kontrol hanya diberikan larutan NaCl fisiologis secara intraperitoneal. Kadar GDP diukur setiap 2 minggu selama 12 minggu pasca induksi STZ dan dibandingkan kedua kelompok. Pada akhir minggu ke-12 kadar GDP masih menunjukkan hiperglikemia (172,77 ± 14,57) dan berbeda bermakna dibandingkan kadar GDP hari ke-3 pasca induksi (p = 0,011). Data kedua kelompok pada akhir minggu ke-12 pasca induksi juga memperlihatkan perbedaan bermakna (p = 0,004) yang menunjukkan belum terjadi perbaikan spontan kadar GDP. Pemberian tunggal STZ dosis rendah (35 mg/Kg i.p) dapat meningkatkan kadar GDP dan belum terjadi perbaikan spontan kadar GDP yang diamati setelah 12 minggu pasca induksi (p = 0,004). Abstract. Streptozotocin can induces diabetes through the damage of the pancreatic islet B cells. In a previous study was conducted that multiple low doses of streptozotocin (STZ) can induce spontaneous restoration of normoglycemia in diabetic rats. This study was designed to investigate the fasting blood glucose (FBG) disturbances after the low dose streptozotocin administration (35 mg/kg i.p) for 12 weeks. Sixteen male Sprague-Dawly rats weighing 214,40 ± 8,25 g were divided into two groups, control and single low dose streptozotocin induced diabetic rats (35 mg/Kg i.p). Control rats were given only physiological saline i.p. Fasting blood glucose level was measured every 2 weeks for 12 weeks after induction and compared to other groups. At the end of twelfth week, FBG levels still showed the hyperglycemia (172.77 ± 14.57) and was different significantly than third day post-induction (p = 0.011). FBG levels of both groups was also different significantly (p = 0.004) which indicated no spontaneous restoration. Study found that single low dose streptozotocin (35 mg/Kg i.p) may increase of fasting blood glucose and  no spontaneous restoration of normoglycemia were observed after 12 weeks (p = 0,004).

Abstract

Abstrak. Menopause merupakan suatu keadaan yang ditandai dengan tidak adanya periode menstruasi selama 12 bulan terakhir akibat berhentinya fungsi ovarium. Wanita pascamenopause mungkin lebih rentan terhadap kehilangan memori dan kognitif daripada wanita premenopause akibat kekurangan estrogen. Kadar brain-derived neurotrophic factor (BDNF) plasma juga menurun dengan bertambahnya usia. Hal ini terkait dengan penyusutan hippocampus dan defisit memori pada wanita pascamenopause. Bukti klinis menunjukkan bahwa, penurunan kadar estrogen dan BDNF di otak menimbulkan perubahan dalam memori, kognisi, suasana hati dan perilaku. Estrogen dan protein BDNF dapat membantu mempertahankan memori verbal dan meningkatkan kapasitas untuk belajar hal baru pada wanita pascamenopause. Interaksi estrogen dan BDNF juga dapat meningkatkan plastisitas sinaptik di otak, pertumbuhan sel neuron, dan neurogenesis hippocampal. Latihan fisik teratur atau aerobik dapat membantu mengendalikan sejumlah masalah fisik dan psikologis pada wanita pascamenopause, dan meningkatkan kesehatan otak serta plastisitas sinaps melalui peningkatan estrogen dan kadar BDNF plasma. Latihan aerobik intensitas sedang dapat memperbaiki dan meningkatkan sekresi estrogen dan ekspresi BDNF pada fase postmenopause. Kesimpulannya, latihan fisik teratur dapat berperan sebagai salah satu faktor protektif kuat dan penting untuk meningkatkan kesehatan otak, memori dan fungsi kognitif pada wanita pascamenopause. Abstract. Menopause is a state of an absence of menstrual periods for 12 months caused by function of the ovaries ceases. Postmenopausal women may be more vulnerable to memory and cognitive loss than premenopausal women because of deficiency in estrogens. Circulating levels of brain-derived neurotrophic factor (BDNF) also decline with advancing age. It would be related to shrinkage of the hippocampus and memory deficits in postmenopausal women. Clinical evidences show that, decreased level of estrogen and BDNF in the brain gives rise to modifications in memory, cognition, mood and behaviour. Estrogen and BDNF can help to maintain verbal memory and enhance the capacity for new learning in postmenopausal women. Interaction of estrogen and BDNF also enhances synaptic plasticity in the brain, neurite growth, and hippocampal neurogenesis. Aerobic or regular physical exercise may help control a number of physical and psychological problems in postmenopausal women, and improve brain health and sinaps plasticity via enhancing estrogen and plasma BDNF levels. Moderate intensity aerobic can improve and increase estrogen secretion and BDNF expression in postmenopause phase. In conclusion, regular physical exercise could represent an important and potent protective factor for improving brain health, memory and cognitive function in postmenopausal women.

Abstract

The incidence of type 2 diabetes mellitus (DM) has been predicted to increase until 2045 in the world. Furthermore, long-term treatment and lifestyle factors affect the quality of life. This study aims to determine the models that can be used to predict the quality-of-life domains in prediabetes patients by using Artificial Intelligent (AI) devices. This is a cross-sectional design in which the inclusion criteria were individuals of age above 18 years and has never been diagnosed with diabetes mellitus (both type 1 DM and type 2 DM), fasted for at least 8 hours, and are willing to sign an informed consent after having received an explanation. Participants were asked to fill out two questionnaires, namely the Indonesian version of the Finnish Diabetes Risk Score (FINDRISC) and the EuroQoL-5 Dimensions-5 Level (EQ-5D-5L). The AI application uses Orange® machine learning with three models used in predictive analysis, such as Logistic Regression, Neural Network, and SVM. In addition, the model was evaluated using the sensitivity, precision, and accuracy of the AU-ROC parameters. The results showed that the neural network model based on the AUC value, precision, accuracy, and also the ROC analysis, was the best for predicting the utility index of domains in the EQ-5D-5L questionnaire, based on demographic data and the FINDRISC questionnaire.
