Abstract

Introduction: A community-based exercise was implemented with the specific theme “Awareness raising among Family Welfare Program and Youth Organization group members of the importance of Family Nutrition Literacy and of nutrition education and mastication improvement through provision of removable dentures” in Ketapang sub-district, Kademangan district, Probolinggo. Aim: This community-based service was intended to inform the public of the importance of nutritional consumption to ensure a well-nourished family and enhanced mastication resulting from the use of removable dentures. This event was held in the middle of the COVID-19 pandemic. Methods: Two teams managing innovative and promotion were created. The role of the former was to conceptualize activities, while that of the latter was to directly inform the target community using a combination of printed and electronic media. These teams consisted of lecturers and 2nd year residents of the Department of Prosthodontics, Faculty of Dentistry, Universitas Airlangga. Results: The event involved the use of video conferencing through which residents delivered online counselling session to Family Welfare Program, Youth Organization group, and health cadre mem-bers who had already gathered at Ketapang Health Center, while maintaining physical distancing. Several residents worked on site alongside health cadres from the Ketapang Health Center. Given the ongoing pandemic, the teams decided to donate certain props: four full dentures, four removable partial dentures and four fixed partial dentures. Conclusion: Online counseling of the Ketapang Health Center cadres was expected to increase their awareness and comprehension of nutrition education and improvement in the mastication function of dentures through the use of teaching aids.

Abstract

Dental implants have been used for several decades. Patients of all ages have chosen dental implants to replace a single tooth or several teeth or to support partial or full dentures. This paper reports two cases of patients treated with dental implant as alternative to replace the missing teeth and connected with natural tooth as abutments in a fixed restoration with distal cantilever bridge. The underlining reasons that we decided to make such kind fixed prostheses are because of clinically imposible to put the implant on certain area and the patients asked for prostheses as optimum as possible, so the mastication function could return to the homeostasis condition. The benefit of these treatments are that prostheses could be made as optimum as possible with a more economic price, so the patients feel quite satisfied. The result shows that a few years after the treatments finished there is no any disadvantageous effect of connecting teeth to implants as abutments in fixed partial dentures and there is no sign of a harmful effect to the opposing teeth either.

Abstract

Ectodermal dysplasia is a rare hereditary disorder with a characteristic physiognomy. The ectodermal dysplasia constitutes a group of hereditary disorders whose clinical manifestation can be defects in ectodermal structures. The case of a 11-year-old child with hypohidrotic ectodermal dysplasia and partial anodontia is presented. Affected children require extensive dental treatment to restore appearance and help the development of a positive self image. Partial denture was provided to encourage a normal psychological development and to improve the function of the stomatognatic system. It is important for the patient and the dentist to understand that continued monitoring for dental problems is necessary. This paper had an objective to relate and discuss a case of hypohidrotic ectodermal dysplasia, with the approach of the influence of an esthetic rehabilitation and functional alternative in the improvement of the quality of life.

Abstract

A resilient denture liner is placed in the part of the hollow obturator base that contacts to post hemimaxillectomy mucosa. Replacing the resilient denture liner can makes the hollow obturator has an intimate contact with the mucosa, so it can prevents the mouth liquid enter to the cavum nasi and sinus, also eliminates painful because of using the hollow obturator. Resilient denture liner is a soft and resilient material that applied to the fitting surface of a denture in order to allow a more distribution of load. A case was reported about using the hollow obturator with resilient denture liner on post hemimaxillectomy to overcome these problems.

Abstract

Background: Bone regeneration studies involving the use of chitosan–hydroxyapatite (Ch-HA) scaffold seeded with human amnion mesenchymal stem cells (hAMSCs) have largely incorporated tissue engineering experiments. However, at the time of writing, the results of such investigations remain unclear. Purpose: The aim of this study was to determine the osteogenic differentiation of the scaffold Ch-HA that is seeded with hAMSCs in the regeneration of calvaria bone defect. Methods: Ch-HA scaffold of 5 mm diameter and 2 mm height was created by lyophilisation and desalination method. hAMSCs were cultured in hypoxia environment (5% oxygen, 10% carbon dioxide, 15% nitrogen) and seeded on the scaffold. Twenty male Wistar rat subjects (8 – 10 weeks, 200 - 250 grams) were randomly divided into two groups: control and hydroxyapatite scaffold (HAS). Defects (similar size to scaffold size) were created in the calvaria bone of the all-group subjects, but a scaffold was subsequently implanted only in the treatment group members. Control group left without treatment. After observation lasting 1 and 8 weeks, the subjects were examined histologically and immunohistochemically. Statistical analysis was done using ANOVA test. Results: Angiogenesis; expression of vascular endothelial growth factor; bone morphogenetic protein; RunX-2; alkaline phosphatase; type-1 collagen; osteocalcin and the area of new trabecular bone were all significantly greater in the HAS group compared to the control group. Conclusion: The three-dimensional Ch-HA scaffold seeded with hypoxic hAMSCs induced bone remodeling in calvaria defect according to the expression of the osteogenic and angiogenic marker.

Abstract

Background: After tooth extraction, the socket leaves a defect on the alveolar bone. The administration of shell crab-derived hydroxyapatite maintains bone dimensions that are important for achieving successful prosthodontic treatment. Purpose: The aim of the study was to determine the osteogenic properties, such as the number of osteoclasts, osteoblasts and osteocytes, after the application of hydroxyapatite-based shell crab in the post-extraction sockets of Wistar rats. Methods: There were two groups: the control group (K) and the treatment group (T). Wistar rats were randomly divided into control and treatment groups. After tooth extraction, hydroxyapatite gel derived from Portunus pelagicus shells was applied to the tooth sockets of Wistar rats. Observations and calculations of osteoclasts, osteoblasts and osteocytes were carried out on the 14th and 28th days under a light microscope with 400 times magnification. Statistical analysis was performed using one-way ANOVA. Results: There was a significant difference (p<0.05) between the K14 and P14 groups, K28 and P28 groups, K14 and K28 groups, and P14 and P28 groups. The results indicated that there were significant differences between groups of variables. Conclusion: The application of shell crab-derived hydroxyapatite (Portunus pelagicus) was able to decrease the number of osteoclasts and increase the number of osteoblasts and osteocytes.

Abstract

Background: Socket preservation using bone graft is one way to minimise resorption and maximise the bone formation process. Tumor necrosis factor-α (TNF-α) is an inflammatory cytokine that affects bone regenerating osteoblast activity, while osterix (Osx) is an osteoblast-specific transcription factor that activates gene receptors during pre-osteoblast differentiation. The hydroxyapatite (HA) scaffold from crab shells (Portunus pelagicus) has osteoconduction properties. Purpose: To analyse the decrease of TNF-α expression and the increase of Osx expression and the correlation between these two in the post-extraction socket after the transplantation of a crab shell HA scaffold. Methods: The lower left incisors of Cavia cobaya (n = 24) were extracted and divided into four groups: the first and second groups were control groups on Day 7 and Day 14 (K7 and K14), the third and fourth groups were treatment groups (P7 and P14). The statistical analysis used was a multivariate analysis of variance (MANOVA) with a significance level of 0.05. Results: A MANOVA test showed that the use of crab shell HA scaffolds led to a significant difference (p < 0.05) in TNF-α expression (p = 0.01) and Osx expression (p = 0.01). A Pearson correlation test result showed a strong inverse correlation between TNF-α and Osx expressions (p = 0.00 and r = -0.78). Conclusion: The transplantation of HA scaffolds from crab shells can decrease TNF-α expression but increase Osx expression in the post-extraction socket of C. cobayas. Furthermore, an inverse correlation was found between TNF-α and Osx.

Abstract

Background: Edentulousness resulting from extraction of teeth often leads to psychological problem and social isolation of the patient. The patient has to adapt to the situation with respect to speech, chewing, swallowing, and so forth. A smooth transition from dentulousness to edentulousness is the result of immediate over denture with a few retained teeth. Purpose: to report overdenture prevents alveolar ridge bone absorption, stabilizes a denture, and makes the typical sensory function to be looked closer to natural teeth. Case: A 79-year-old female complaint of missing multiple teeth and desire to replace them. Extraoral examination and temporomandibular joint movement were normal. Five teeth (12,11,21,22,34) exhibited grade II mobility with diagnosis apical chronic periodontitis. Radiographic examination is an overview of diffuse radiolucent on the apical on the teeth 17,13,12,11,21,22,34. Case Management: First, make a diagnostic impressions of the maxillary and mandibular with irreversible hydrocolloid then poured in dental stone. After that, the final impressions were taken and poured in dental stone, casts mounted on articulators. Bite registration was done after using bite rim and bite registration material. The next step is setting up the artificial teeth then tried to the patient. Five teeth (12,11,21,22,34) were trimmed from the casts on the functional model. The patients tooth 12, 11, 21,22 and 34 were reduced to a dome shape only 2 mm above gingival margin then restored with GIC on the surface. The denture was then placed immediately after reduced. Discussion: The selected abutments teeth were reduced to a dome shape only 2 mm above gingival margin. This will create adequate space for the overlying artificial denture tooth and denture base. Also, it reduces the lateral stresses and lever action of the tooth. The retained tooth roots, used for overdenture, transfers occlusal forces to the alveolar bone through the periodontal ligament and maintain alveolar ridge morphology. Conclusion: Overdenture can be used to prevents alveolar ridge bone absorption, stabilizes a denture, and makes the typical sensory function to be looked closer to natural teeth.

Abstract

Background: Treating missing tooth with denture, in some cases, still leave the patient unfulfilled. Good mastication and retention are the main considerations especially for those who require more retentions. In some cases, combination of partial and fixed denture the best approach to achieve better functional and retention results. Purpose: To report compound denture procedures with good mastication and retention as the main considerations. Case: A 43-year-old male patient reported a complaint of chewing difficulty due to missing right upper front teeth caused by work accident and extracted posterior. Patient wanted to wear partial denture to regain good mastication. Case management: Compound denture was chosen to optimize the remaining teeth for better functional and aesthetic. Zirconia all ceramic fixed dentures were fabricated on 13, 14, 15 with an occlusal rest on 13 and 15. Pontic on the 14 used ridge lap design. Discussion: Compound denture is a combination of removable and fixed denture where a minor connector of a removable denture should involve a fixed denture. Fixed denture shall be fabricated first and then removable denture. Conclusion: The definitive restoration of this case was compound denture, which is a combination of partial denture metal frame and fixed denture to restore the good mastication and retention.

Abstract

Background: Bone resorption due to tooth extraction leads to unpredictable bone volume for future prosthetics. Crab shells were promoted as a solution to prevent bone resorption, along with an effort to reduce biological waste. Purpose: This study aimed to analyze the expression of bone sialoprotein (BSP) and dentine matrix protein-1 (DMP1) in the wound healing process in tooth-extraction sockets after applying a crab shell-derived hydroxyapatite scaffold. Methods: The subjects (28 Cavia cobaya) were divided into control and treatment groups. The control group was left untreated, while the treatment group received a hydroxyapatite scaffold of Portunus pelagicus shell in the tooth socket. The expression of BSP and DMP1 was determined by immunohistochemical staining on days 7 and 14. One-way analysis of variance and Tukey’s honest significance difference test were used to find the groups with the most significant difference. Results: The highest mean expression of BSP and DMP1 was in the day 14 treatment group, while the lowest was in the day 7 control group. Conclusion: Administering hydroxyapatite scaffold derived from the Portunus pelagicus shell to the post-extraction sockets increased the expression of both BSP and DMP1.
