Abstract

Scabies is a parasitic infestation of the skin, that is affecting on the low income and crowded community in many tropical countries, especially developing countries such as Indonesia. Scabies infestation increases the incidence of secondary pyoderma include impetigo, folliculitis, cellulitis, ecthyma, abscess. Secondary pyoderma is a skin infection disease mainly caused by group A Streptococcus (GAS) and Staphylococcus aureus (SA). Pyoderma is a risk factor for the glomerulonephritis infection, rheumatic diseases, which significantly increases morbidity and mortality, causing the government burden. The aim of this study is determining the etiology and correlation of pyoderma infection in scabies patient. To determine sosio-demographic included sex, age in pediatric patients in primary schools (SD) in the district of Kertapati Palembang. The study design was cross sectional, and study samples were new scabies patients in the elementary school (age 6-14-year-old) with or without pyoderma. Clinical findings included history, physical examination and diagnostic procedure, which was investigation of skin scraping specimen material (SSB = skin surface biopsy) in confirmation with dermoscopic polar examination (DS) to show Sarcoptes scabiei mites. Microbiological examination with Gram stain identified the etiology of pyoderma. Results of this study shows that there was a significance relationship between scabies infestation and pyoderma in children in elementary school. Staphylococcus aureus dan GAS are the most common caused of pyoderma in pediatric patients with scabies. Conclusion is there is a significant correlation between scabies and pyoderma. There is a need to provide scabies and pyoderma medication at primary care health center as well as counseling for prevention in Palembang area with crowded population periodically.

Abstract

Background: Basal cell carcinoma (BCC) is the most common skin malignancy found in nonmelanoma skin cancer. There are multiple factors contribute to risks for BCC but the underlying mechanism is genetic alteration. The Sonic hedgehog (SHH) pathway is implicated in the etiology of the most common BCC. We reported BCC nodular type in the tip of nasal region with increase SHH. Case Illustration: 50 years old man, presented with hyperpigmented tumor on the nasal tip which easily bleed since 6 months ago. Dermatological examination: the tip of nasal region: a solitaire irregular immobile hyperpigmented tumor, with the size of 2x1x0,3 cm, pearly border with rubbery consistency. Dermoscopy examination found multiple blue-gray globule and telangiectasia feature. Histopathology examination found nodule and micronodule with palisade feature in the periphery and “slit like retraction” in central. Immunohistochemistry examination found more than 50% cell show SHH expression. Wide surgical excision was done with tumor margin 3 mm. Discussion: The hedgehog (Hh) pathway known have important role in embryonic development, that responsible for patterning development organ and tisuue. SHH is one protein ligand that most study and well known in Hh pathways.Conclusion: Activating SHH pathway can resulting in unrestricted SMO activity that causes uncontrollable cell proliferation and eventually carcinogenesis. Increased expression of SHH can be used as a marker in aggresivity of BCC. Keywords: clinical manifestation, histopathology, immunohistochemistry, Sonic hedgehog pathway

Abstract

Diabetes mellitus (DM) is a chronic and complex disease that affects various parts of the body. It can lead to multiple systemic complications and also cutaneous manifestation. Diabetic foot ulcer (DFU) is one of the most devastating complications of DM in dermatology. The main etiology is an increase in plasma glucose, risk factors, or comorbidities due to DM itself. Neglected DFU can lead to further complications, including high amputation and mortality rates; thus, the healing of ulcers is the main objective of the treatment. Management is divided into the standard of care and adjuvant therapies. This study aims to optimize DFU management, so it can provide proper treatment and prevent complications.

Abstract

Pruritus uremik yang merupakan sensasi yang mengarahkan keinginan untuk menggaruk, yang sering terjadi pada pasien gagal ginjal kronik yang menjalani hemodialisis.Pruritus uremik sering merupakan masalah utama bagi pasien dengan penyakit ginjal stadium akhir  dan sering terjadi pada 10-85% pasien hemodialisis.Kondisi ini lebih umum pada lelaki yang tidak bekerja.Penelitian ini bertujuan mengetahui hubungan karakteristik klinis pruritus uremik pada pasien gagal ginjal kronik yang mendapat hemodialisis. Diambel sampel 92 orang pasien gagal ginjal kronik yang mendapat hemodialisis dari tahun 2014-2016 dengan metode consecutive sampling. Data diperoleh dari rekam medik pasien pruritus uremik di RSUP Palembang tahun 2014-2016. Diambil sampel 90 orang pasien pruritus uremik yang mendapat hemodialisis dari tahun 2014-2016 dengan metode consecutive sampling. Data diperoleh dari rekam medik pasien PU di RSUP Palembang tahun 2014-2016. Analisis data dilakukan dengaan menggunakan tabel distribusi frekuensi dan uji statistik chi-square test dan Odd ratio serta korelasi rank spearman. Pasien PU paling banyak merasakan gatal sedang yaitu 41 pasien (45.6%), di dominasi oleh laki-laki yaitu 59 pasien (65.6%), paling banyak terjadi pada usia 59 sampai 66 tahun yaitu 22 pasien (24.4%), paling banyak melakukan cuci darah pada tahun 2015 yaitu 39 pasien (43.3%), dan semua pasien mendapat durasi hemodialisis 1 sampai 5 tahun dengan frekuensi 2 kali seminggu.Hasil uji statistik pada penelitian ini menunjukkan tidak terdapat hubungan tingkat keparahan PU dengan jenis kelamin sedangkan nilai korelasi yang diperoleh adalah 0.018 yang tergolong korelasi sangat lemah dan tidak signifikan secara statistic

Abstract

Keputihan (leukorea, flour albus, vaginal discharge) adalah sekret yang berlebihan dari vagina selain darah haid, dan tidak disebabkan neoplasma atau penyakit sistemik. Keputihan dapat bersifat fisiologis (normal) dan patologis (abnormal).. Pengetahuan dan perawatan yang baik dalam menjaga kebersihan organ reproduksi dapat memelihara kesehatan reproduksi. Kurangnya pengetahuan, sikap, dan perilaku dalam perawatan vagina diyakini berpengaruh terhadap kejadian keputihan patologis. Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan, sikap, dan perilaku perawatan vagina terhadap kejadian keputihan patologis pada mahasiswi Program Studi Pendidikan Dokter Fakultas Kedokteran Universitas Sriwijaya.Penelitian ini adalah penelitian analitik observasional dengan pendekatan cross-sectional. Sampel penelitian ini adalah semua mahasiswi Program Studi Pendidikan Dokter Fakultas Kedokteran Universitas Sriwijaya angkatan 2015-2018.Dari 599 responden, faktor berpengaruh bermakna terhadap terjadinya keputihan patologis adalah pengetahuan (p= 0,044), sikap (p= 0,041) dan perilaku (p= 0,000) sesuai dengan hasil multivariat.pengetahuan, sikap, dan perawatan vagina merupakan faktor yang berpengaruh terhadap kejadian keputihan patologis

Abstract

Background: Uremic pruritus (UP) is the most common symptoms in patients with chronic renal failure (CRF) undergoing hemodialysis (HD). It is a chronic itch sensation of the skin, with the most frequent clinical manifestation being xerosis, which is difficult to treat. There is a significant relationship between UP and moderate to severe uremic xerosis (UX) in CRF patients undergoing HD. This condition could affect the quality of life and increase the risk of morbidity and mortality. The causes of UP with xerosis are still unclear, however it is evident that there is a disruption of skin barrier function. A 20% urea with the base materials cream consisting sodium pidolat sodium lactate (NaPCA) and vegetable oils that act as a natural moisturizing factor (NMF) can improve the skin barrier function by increasing skin hydration, reducing transepidermal water loss (TEWL) will result in the improvement of  UP with UX.

Abstract

Background: Seborrhoeic dermatitis (SD) is a chronic papulosquamous inflammatory disease which resistant to medical treatment. Various treatment such as topical corticosteroid, antifungal and calcineurin inhibitor has been widely practiced and gives varying results Objective: Our objective was to compare the efficacy of Sebclaira® and topical hydrocortisone 2,5% for management of moderate SD on the face Methods: A randomised clinical trial, controlled, double blind study was performed for four weeks. We assessed the efficacy and side effects of these topical treatment. The severity of SD was evaluated using Seborrhea Area Severity Index-Facial (SASI-F) score. The severity of pruritus was evaluated using Visual Analogue Scale. Demographic characteristics, baseline SASI-F and VAS were recorded in the medical record. Results: A 34 patients (14 males, 20 females) with moderate SD on the face completed the four weeks study. The mean of SASI T2 and VAS score of the Sebclaira® group was significantly lower than Hydrocortisone 2.5% group with p = 0.000 and p = 0.000 respectively. Tolerance between Sebclaira® and Hydrocortisone 2.5% showed insignificant results (p = 1.000) Conclusions: The longer application of Sebclaira® was significantly more effective to improve moderate SD

Abstract

Irritant contact dermatitis (ICD) is an inflammatory epidermal disorder associated with physical or immunological provocation that results in impaired skin barrier. The prevalence of ICD is found to be 17-30% in healthcare workers compared to general population. The first-line management strategies of ICD is based on prevention. Intensive hand cleansing is one of the causes of ICD. Hand hygiene is general term that refers to any action of hand cleansing. Hand hygiene practices include hand washing and hand rubbing with antiseptics. Hand hygiene is the most effective preventive measure in preventing infection control, especially in the era of coronavirus (COVID-19) pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) since December 2019. This disease is transmitted through inhalation or contact with infected droplets. Prevention and mitigation actions are key in controlling infection.

Abstract

A B S T R A C TBackground: Xerosis skin and pruritus is the most commonly found in elderly withsymptoms mild to severe recurrent pruritus, the skin looks rough, scaly, fissured orcracked caused defects in the epidermal barrier function, increase co-morbidity aschronic infections and ulcers that are difficult to heal can decrease the quality of life.Virgin Coconut Oil (VCO) has been traditionally used for people in tropical regionas a moisturizer and anti-bacterial effect. In recent study that essential plant oilmixture have effect synergistically as anti inflammatory, anti pruritus, promotingwound healing may act promoting barrier skin and therapeutic. Objective: Efficacyand safety of plant oil mixtures as therapeutic and maintenance moisturizer.forXerosis and pruritus in elderly Method:A randomized controlled trial was involved61 elderly from nursing home and geriatric clinic in Moh. Hoesin General Hospital.They were randomized into 2 groups: plant oil mixtures (n=30) and Virgin Coconutoil (VCO) (n=31), applied on the volar legs twice a day for 4 weeks.Outcomes weremeasured at baseline and weekly, week 2 and 4.To measure of clinical dry scoreused ODSS; pruritus severity and side effect used VAS; for skin hydration usedcorneometer CM825and for sebum level used sebumeter SM 810. Results:Clinicaldry score, pruritus severity of plants oil mixtures group were significantly decreasedthan VCO at week 2 and 4 (p 0.000). Skin hydration and sebum level of plants oilmixture group were significantly increased than VCO at week 2 and 4 (p0.000).Clinical dry score, pruritus severity, skin hydration, andskin sebum levels ofplant oil mixtures were significantly better than VCO at week 4 (p 0.000). There isno side effects on both of plant oil mixtures and VCO groups Conclusion: Plant oilmixtures add pureCelin oil showed better efficacy as a therapeutin and maintanancemoisturizer for xerosis and pruritus in elderly. Both of plant oil mixtures and VCOcan be tolerated and safe to use.

Abstract

A B S T R A C TIntroduction: Basal cell carcinoma (KSB) is a non-melanoma skin cancer (KKNM),which is most commonly found compared to other skin cancers. KSB originates fromstem cells in the bulk of hair follicles or inter-follicular epidermis, through the Sonichadgehog (SHH) activation pathway, an increase in Sonic hadgehog (SHH) proteinexpression, involving Patches protein (PTCH), smothened protein (SMO), in the formof increased protein transcription activation Glia (GLI) in the nucleus, binds to DNAto initiate tumor-aggressive growth and tissue. Objective: to determine therelationship between Sonic hadgehog (SHH) expression and non-aggressive andaggressive basal cell carcinoma. Methods: The study was carried out in anobservational laboratory with 35 primary KSB patients, the tissue was taken usingelliptic biopsy technique, made paraffin block specimens for histopathologicalexamination of the subtype of KSB consisting of 20 non-aggressive KSB patients,namely nodular and superficial KSB; 15 patients with aggressive KSB werepigmented KSB; Infiltrates KSB, micronodular KSB, metypical KSB (basosquamousKSB) and SHH immunohistochemical (CPI) examination using SHH antibodies, inthe Anatomy Pathology section, FK Unsri / RSMH Palembang. The characteristics ofKSB patients were recorded, namely sex, age, occupation based on the length ofexposure to BC, namely exposure <3 hours / day, exposure 3-6 hours / day,exposure ≥ 6 hours / day. The data were processed using the Statistical AnalysisSoftware Package (SPSS) version 20.0 (IBM Corporation), tested with Pearsoncorrelation test and chi square test and presented in the form of diagrams, andnarrative tables. Results: Pearson's test showed a significant correlation betweenthe clinical features of KSB and the histopathologic features of non-aggressive andaggressive types of KSB (p 0.020), there was a significant relationship between thesubtypes of histopathologic features of KSB with non-aggressive and aggressivetypes of KSB (p 0.000), there was a significant relationship between strong SHHexpression and BCC aggressive compared to non-aggressive KSB, p 0.000 (p <05α),and r = 732 Conclusion: There is a relationship between SHH expression and KSBaggressiveness. The increase in strong SHH expression shows the aggressiveness ofKSB, SHH expression can be used as a biological gene target both as a prognosticindicator and can be used as a target for treatment of aggressive KSB, especially inthe elderly.
