The purpose of this research was to investigate the correlation of coderâs comprehension, audit implementation, medical record the completeness, SOP implementation, as well as facilities and infrastructure completeness with the inaccuracy of outpatient diagnosis coding in General Surgery and Neurosurgery Polyclinics, Hospital X, Badung Regency. This research was quantitative analytic research with cross-sectional study design. A total of 8 coders were sampled and as many as 200 medical records were selected for the determination of coding inaccuracy. The results showed that most medical records are inaccurately coded. The bivariate test using spearmans who indicated that there was a correlation between the coderâs comprehension (sig. = 0.000), audit implementation (sig. = 0.003), medical record completeness (sig. = 0.000), SOP implementation (sig. = 0.004), as well as facilities and infrastructure completeness (sig. = 0.000) with the inaccuracy of outpatient disease diagnosis coding.

ABSTRAK?Seiring dengan proses penuaan, kemampuan gen yang mengontrol viabilitas sel mengalami penurunan, sejalan dengan meningkatnya aktivitas gen yang menyebabkan kematiansel dan berujung pada patogenesis? penyakit. Dengan memanipulasi aktivasi dan non-aktivasi gen yang terlibat dalam proses penuaan, maka kesehatan akan meningkat dan proses penuaandapat dihambat bahkan dikembalikan. Saat ini telah ditemukan teknologi CRISPR/Cas-9 yangmemungkinkan manipulasi pada tingkat genetik. CRISPR sebenarnya merupakan bagian prosesfisiologis pada bakteri, yang merupakan salah satu sistem imunitas bakteri. Namun saat iniCRISPR telah banyak dibuktikan dan diaplikasikan untuk reprogram DNA pada sel mamaliadan dapat mengembalikan perubahan genetik yang tidak diinginkan.Kata kunci: Penuaan, CRISPR/Cas-9, anti-aging medicine, manipulasi genetik ?ABSTRACTAs all living thing undergo aging, genes that maintain cellular viability and vitality are downregulated, in line with upregulated expression of genes that promote cells senescence anddisease-related pathogenesis. By manipulating the initiation and inhibition of aging-relatedgene expression, the youthful health may be systemically restored. Recently, a new technologycalled CRISPR is found to be usefull for manipulating gene and rapidly transform senescentcells to regain youthful function and structure. CRISPR is originally developed in nature by bacteria as a way to destroy the DNA of viruses which part of their immune system. A naturalversion of CRISPR has been adapted by scientists to enable the reprogramming of cellular DNAto rid cells of unfavorable genetic changes. ABSTRACT The process of recording the diagnosis of viral infection in children in the polyclinic of Wangaya Hospital is still using manual method. The development of information technologyincreasingly advanced to the health world. With the application of technology in the world ofhealth will be able to facilitate in performing services to patients. However, with the developmentof technology, a system is needed to facilitate medical staff to provide medical services to patients.To answer those needs, an interactive system was built to help record the diagnosis of viraldisease in children. This interactive system is designed based on Android using the programminglanguage PHP and uses a database of MYSQL. The workings of this interactive system is byasking questions such as symptoms or complaints experienced by the patient, then this system willmake the process of analysis based on the percentage likelihood of a diagnosis of the patient'sillness. This interactive system can help record the diagnosis of viral infection. In addition toshowing the results of diagnosis, the system can also display the definition of each viral infectiousdisease in children, which has been inputted into the database application.Keywords: Interactive System, Android, Diagnosis of Virus Infections on Children ABSTRAK Dalam melakukan pencatatan diagnosis infeksis virus pada anak di poliklinik anak di RSUDWangaya masih menggunakan cara manual. Perkembangan teknologi informasi semakinberkembang hingga ke dunia kesehatan. Dengan adanya teknologi dalam dunia kesehatan akanmempermudah dalam melakukan pelayanan kepada pasien. Dengan perkembangan teknologi maka sangat di perlukan suatu sistem yang akan mempermudah petugas medis dalam memberikanpelayanan medis kepada pasien.Sistem interaktif dibangun bertujuan untuk membantu pencatatandiagnosis penyakit virus pada anak. Sistem interaktif ini di rancang berbasis Android denganmenggunakan bahasa pemrograman PHP dan menggunakan database dari MYSQL. Cara kerjasistem interaktif ini adalah dengan memberikan pertanyaan berupa gejala-gejala atau keluhan yangdi alami oleh pasien, lalu sistem ini akan melakukan proses analisis berdasarkan presentasekemungkinan diagnosis dari penyakit pasien tersebut. Sistem interaktif ini akan dapat membantupencatatan hasil diagnosis penyakit infeksi virus. Selain menampilkan hasil diagnosis sistem inijuga dapat menampilkan definisi dari setiap penyakit infeksi virus pada anak yang telah diinputkedalam database aplikasi. ABSTRACTRumah Sakit Umum Daerah (RSUD) Wangaya of Denpasar City has not used tracer yet toprocess and take the medical record. The lending book or expedition book that has function recordsubject, quantity and time of medical record movement, is not available in the medical recordinstalation. According the earlier records that was hold in Rumah Sakit Umum Daerah (RSUD)Wangaya of Denpasar city on May-June 2016, it has been knowing that 22.3% medical record hasbeen missfiled. It consist 867 from 3.950 medical record. That case cause dropout of medical record.Rumah Sakit Umum Daerah (RSUD) Wangaya of Denpasar City needs an integrated applicationand it can be used to coordinate medical record distribution between medical record departmentstaff and polyclinic staff. The goal of this research is to design and develop a medical record filetracer application based on hypertext preprocessor to help staff to trace the medical record. Thisapplication is developed with PHP programming language and MySQL as the database server.Development of this system used System Development Life Cycle (SDLC) method. The MedicalRecord File Tracer Application Based On Hypertext Preprocessor is hoped to help the medicalrecord department staff and polycilinic staff to trace and monitors the medical record patient.Keyword : Medical record, file tracer, php, filing, medical record drop out.ABSTRAKRumah Sakit Umum Daerah (RSUD) Wangaya Kota Denpasar belum menggunakan tracerdalam proses pengambilan rekam medis. Buku peminjaman atau buku ekspedisi yang berfungsiuntuk mencatat siapa dan berapa jumlah rekam medis yang dipinjam serta tanggal rekam medisdipinjam tidak tersedia di instalasi rekam medis. Berdasarkan studi pendahuluan yang dilakukan diRumah Sakit Umum Daerah (RSUD) Wangaya Kota Denpasar pada bulan Mei -Juni 2016 diketahuijumlah kejadian missfile sebanyak 867 rekam medis dari 3.950 rekam medis dengan tingkatpersentase mencapai 22.3%. Hal ini menyebabkan terjadinya dropout rekam medis.Rumah SakitUmum Daerah (RSUD) Wangaya Kota Denpasar membutuhkan aplikasi yang terintegrasi dan dapatdigunakan untuk koordinasi distribusi rekam medis antara petugas rekam medis dan petugas poli.Tujuan penelitian ini adalah menghasilkan rancang bangun aplikasi tracer medical record fileberbasis hypertext preprocessor (PHP) untuk mempermudah petugas melakukan proses pelacakandan monitoring rekam medis. ABSTRACTRecording and processing data of mother and child in health book (KIA) at Sumbersari PublicHealth Center is still done manually by handwriting. Manual KIA book recording has an impacton the quality of information contained in KIA books that are not presented optimally. Thisresulted in inhibition of health services and complete immunization to mothers and children, aswell as delays in the process of reporting by health personnel. In connection with this, the currentstudy was conducted to design and implement the application of maternal and child healthrecording data based on android system. This application design method uses system developmentlife cycle which consists of planning, analysis, design, implementation and usage. The design ofthis application using logic modeling by creating context diagrams, data flow diagrams, and entityrelationship diagrams. The application is made with android operating system version 6.0.1, anduses java based eclipse, with Angular JS as a web-based framework. This application has theability or features to record patient demographic data, maternal and child health developmentchecks, medications and vaccines, and periodically display charts and tables of maternal and childhealth. This android based application can be used by midwives and integrated health servicecadres with online system.Keywords: Maternal and Child Development, Android.ABSTRAKKegiatan pencatatan dan pengolahan data buku kesehatan ibu dan anak (KIA) di puskesmasSumbersari dilakukan secara manual dengan menggunakan tulisan tangan. Pencatatan buku KIAsecara manual berdampak terhadap kualitas informasi yang dimuat dalam buku KIA yang tidaktersaji secara optimal. Hal tersebut mengakibatkan terhambatnya pelayanan kesehatan danimunisasi lengkap kepada ibu maupun anak, serta tertundanya proses pembuatan laporan olehtenaga kesehatan. Sehubungan dengan hal ini, penelitian ini dilakukan untuk merancang danmengimplementasikan aplikasi pencatatan perkembangan kesehatan ibu dan anak berbasisandroid. Metode perancangan aplikasi ini menggunakan siklus hidup pengembangan sistem yangterdiri atas tahap perencanaan, analisis, desain, implementasi dan penggunaan. Perancanganaplikasi ini menggunakan pemodelan lojik dengan membuat diagram konteks, data flow diagram,dan entity relationship diagram. Aplikasi dibuat dengan sistem operasi android versi 6.0.1, danmenggunakan eclipse berbasis java, dengan Angular JS sebagai framework berbasis web. Aplikasiini memiliki kemampuan atau fitur untuk mencatat data demografi pasien, hasil pemeriksaanperkembangan kesehatan ibu dan anak, obat dan vaksin, serta menampilkan grafik dan tabelperkembangan kesehatan ibu dan anak secara berkala. Aplikasi ini berbasis android, yang dapatdigunakan oleh bidan dan kader posyandu secara online.Kata Kunci : Perekembangan Ibu dan Anak, Android.

ABSTRACTThe medical record officer at Bright Smiles Bali Clinic is still having difficulty in the process ofmanaging the data of outpatients because the clinic still using manual system that has not beencomputerized. So it is necessary to design a web-based electronic medical record information systemthat can help medical record officer in patient data management process. The method used in thedesign of this system is the system development life cycle (SDLC) which consists of the planning,analysis, design, implementation, and usage phases. The purpose and benefits of this informationsystem design is to produce a web based outpatient medical record information system at BrightSmiles Bali Clinic.This information system facilitate medical record officer in carrying out patientdata management covering patient registration process, recording of patient medical record, doctorsdata recording, code search on ICD 9 CM, code search on ICD 10. Makadiperlukan sebuah sistem informasi rekam medis elektronik berbasis web yang dapat membantupetugas rekam medis dalam proses pengelolaan data pasien tersebut. Metode perancangan sistem inimenggunakan siklus hidup pengembangan sistem (Systems Development Life Cycle-SDLC) yangterdiri dari tahap perencanaan, analisis, desain, implementasi, dan penggunaan. Sedangkan tujuan danmanfaat dari perancangan sistem informasi ini yaitu menghasilkan sistem informasi rekam mediselektronik rawat jalan berbasis web di Klinik Gigi Bright Smiles Bali. Dengan adanya sistem informasiini, dapat memudahkan petugas rekam medis dalam pengelolaan data pasien yang meliputi prosespendaftaran pasien, pencatatan rekam medis pasien rawat jalan, pencatatan data dokter, pencarian kodeICD 9 CM, pencarian kode ICD 10. Selain itu sistem informasi ini menghasilkan berbagai laporanlaporanserta informasi rekam medis pasien yang dibutuhkan pihak manajemen untuk pengambilankeputusan.Kata Kunci : Sistem Informasi, Rekam Medis Elektronik, Rawat Jalan, Web.

The development of the world of information technology is so fast and widespread throughout the world, so this will have an impact on the activities of institutions, especially those that use information technology facilities so that it will increase the efficiency and effectiveness of the performance of government, private and individual institutions including health institutions. Based on the results of observations made by the author, the number of patient visits at the Mangusada Hospital, Badung Regency reached 4,185 patients, the average medical record of patients without cover per day was 135 medical records. The number of medical records of patients without a cover who are controlled again per day is 55 medical records. Searching for medical records of patients without cover at the Mangusada Hospital, Badung Regency is still done manually, namely by sorting out the medical records one by one. The system built using the SDLC method in the development of a web-based system. Based on the results of the usability test conducted, the system is classified as very good for implementation.

According to observations made on data from December 2021 to January 2022 at RSU Tabanan Medical Record Installation, out of 300 inpatients' medical records, , the damage caused by physical factors amounted to 90 (30%), chemical factors amounted to 58 (19.3%) and biological factors amounted to 88 (29.3%). The aim of this study was to identify the physical, chemical, and biological sources and forms of damage to medical records. The research sample for this study consisted of 300 inpatient medical records, 1 head of the Medical Record Installation, 7 filling officers, and a qualitative-descriptive research methodology. The study's findings reveal the physical, chemical, and biological factors such as a cramped space and fully stocked medical record storage shelves, roof leaks that encourage mold growth, a humid environment, and the presence of rats, and cats that wander into the medical record storage room that lead to damage to inpatients' medical records. Medical record covers that are ripped, disconnected, and yellow to brown in color as a result of mildew and dust are three examples of medical record degradation caused by these three variables. Based on the study's findings, it can be said that physical, chemical, and biological variables cause damage to inpatients' medical records, with the forms of damage such as being torn, loose, and yellow to brown in color.

ABSTRACTA summary of the patient's entry and exit is the initial sheet of a medical record document containing information about the patient's identity, how to receive patients, and a summary of patient data. From a preliminary study at the Ari Canti Hospital in Gianyar in January-March 2018, the researchers found in January the total number of incompleteness was 99(12,7%), in February incompleteness totaled 81(10,2%), in March incompleteness totaled 97(10,8%). This study aimed to determine the influence of patient identity , evidence of patient data records and authentication of legal aspects in Ari Canti Gianyar Hospital. The research design used was quantitative analytic descriptive with cross sectional approach. The number of samples used was 267 medical records. Data obtained by observation and checklist. The analysis used was univariate test and bivariate test with chi square. Based on the results of chi square, the variables that influence the legal aspects were the identity of the patient with p = 0.003, evidence of data records of patients with p = 0.000 and authentication with the value of p = 0.001.From the results of the research obtained, it can be concluded that there is an influence of patient identity, evidence of patient data records, and authentication toward the legal aspects in Ari Canti Gianyar Hospital.Keywords : Summary of Entry and Exit, Patient Identity, Evidence of Patient Data Records, Authentication and Legal AspectsABSTRAKRingkasan masuk dan keluar pasien adalah lembaran awal dokumen rekam medis yang berisikan informasi tentang identitas pasien, cara penerimaan pasien, ringkasan data pasien. Dari studi pendahuluan di Rumah Sakit Ari Canti Gianyar pada bulan Januari-Maret 2018, peneliti menemukan pada bulan Januari ketidakengkapan berjumlah 99(12,7%), pada bulan Februari ketidaklengkapan berjumlah 81(10,2), pada bulan Maret ketidaklengkapan berjumlah 97(10,8). Penelitian ini bertujuan untuk mengetaui pengaruh identitas pasien, bukti rekaman data pasien dan autentifikasi terhadap aspek hukum di Rumah Sakit Ari Canti Gianyar. Rancangan penelitian yang digunakan adalah deskriptif analitik kuantitatif dengan jenis penelitian cross sectional. Jumlah sampel yang digunakan sebanyak 267 rekam medis. Data diperoleh dengan cara observasi dan chec klist. Analisis yang digunakan yaitu uji univariat dan uji bivariat dengan chi square. Penelitian ini menunjukkan bahwa variabel yang berpengaruh terhadap aspek hukum adalah identitas pasien dengan nilai p=0,003, bukti rekaman data pasien dengan nilai p=0,000 dan autentifikasi dengan niai p=0,001. Kesimpulan dalam penelitian ini bahwa terdapat pengaruh identitas pasien, bukti rekaman data pasien, autentifikasi terhadap aspek hukum di Rumah Sakit Ari Canti Gianyar.Kata kunci : Ringkasan Masuk Dan Keluar, Identitas Pasien, Bukti Rekaman Data Pasien, Autentifikasi dan Aspek Hukum
