Abstract

Background: Ventricular septal rupture (VSR) is a fatal mechanical complication following acute myocardial infarction (AMI). The definitive treatment remains surgical repair, however several aspects are still debatable, including the timing of surgery. Patient’s refusal of treatment and lack of medical facilities put other challenges in management of VSR. Case presentation: A-48-year-old male with a history of diabetes mellitus and hypertension came with a late-presentation anterior AMI, and refused the reperfusion therapy. By the third day, he developed VSR and cardiogenic shock. The patient was also against any referral plan for further therapy. Hemodynamic status was optimally controlled with vasopressor and inotropic agents. The patient was discharged with a grade II-III New York Heart Association (NYHA) on the tenth day. In follow up evaluation a week later, the patient presented limb swelling and functional NYHA class III-IV. The patient agreed to received percutaneous coronary intervention (PCI). Drug eluting stent (DES) in left anterior descending (LAD) coronary artery was implanted successfully without defect closure. Follow-up after 12-months revealed no signs or symptoms of decompensated heart failure. Keywords: ventricular septal rupture; percutaneous coronary intervention; acute myocardial infarction

Abstract

Hypertension remains a serious health problem it is dubbed as “the silent killer”. Pathophysiology of hypertension involve alteration of vascular structure. The use of anti hypertension leads to many side effects, and many people tend to choose natural therapy. The study aimed to investigate the effect of lumbrokinase on angiogenesis as a hypertension therapy agent. In true experimental design, 22 males wistar rat were divided into control group and treatment group (Lumbrokinase 0.5 gr/KgBW/day). The results showed that the systolic blood pressure decrease after therapy with lumbrokinase from 187 mmHg to 159 mmHg whereas diastolic blood pressure consistant at 80 mmHg after induction of hypertension as well as therapy of lumbrokinase. An examination of cardiac histopathology, there was an increasing number of blood vessels in the treatment group, 10.55± 1.44 compared to 7.45± 1.21 in control group. It can be concluded that lumbrokinase had an angiogenesis effect and decrease systolic blood pressure in hypertensive rat model. Abstract

Stage V chronic kidney disease (CKD) patients need renal replacement therapy. Hemodialysis is main choice for renal replacement therapy and correlates with changes of left ventricular function. Left ventricular diastolic function is one parameter of left ventricular function. The aim of this study was to analyze the effect of hemodialysis towards left ventricular diastolic function in stage V CKD patients at RSD dr. Soebandi Jember. This study included 30 patients (17 female, age 48,20±8,02 years) and underwent echocardiography 30 min before and after a hemodialysis session. Sample characteristics were divided into sex, age, duration of hemodialysis, blood pressure before and after hemodialysis. Diastolic function was measured with E/E’ ratio. Diastolic function criteria of 16 samples (53,5%) were normal (E/E’ ratio <10) after doing a hemodialysis session. Mean ratio E/E’ was significantly reduced (13,09±4,55 vs 11,6±5,04, p<0,05). In conclusion, hemodialysis could increase left ventricular diastolic function in stage V CKD patients at RSD dr. Soebandi Jember.

Abstract

Hypertension is a state where there is an increase of blood pressure above normal threshold,120/80 mmHg. The phatophysiology of hypertension involves the changes of vascular structure. Therefore, the beraprost drug was created. It is a vasodilator and antitrombolytic which is able to induce angiogenesis. The study aimed to investigate the effect of Beraprost on angiogenesis as a hypertension therapy agent. In true experimental design, 22 males wistar rat were divided into control group and treatment group (Beraprost 100mcg/KgBW/day). The results showed that the systolic blood pressure decrease after therapy with Beraprost from 180 mmHg to 143 mmHg whereas diastolic blood pressure consistant at 80 mmHg after induction of hypertension as well as therapy of Beraprost. An examination of cardiac histopathology, there was an increasing number of blood vessels in the treatment group, 16.637.29 compared to 7.45 1.21 in control group. It can be concluded that Beraprost had an angiogenesis effect and decrease systolic blood pressure in hypertensive rat model. Keyword : Hypertension, Beraprost, vascular structure, angiogenesis.

Abstract

Erythrocyte sedimentation rate (ESR) is non-specific blood tests that can be used as a marker of chronic inflammation. Some proteins that affect the erythrocyte sedimentation rate are fibrinogen, lipoprotein, and immunoglobulins. These proteins elevate in patients with atherosclerosis and acute coronary syndrome. This study aimed to determine the relationship between erythrocyte sedimentation rate and the incidence of acute coronary syndrome and stable angina at dr. Soebandi General Hospital Jember. This analytic observasional study using cross-sectional design.This study used 40 samples, divided into four groups; stable angina, UA, NSTEMI, and STEMI. The average value of ESR were 12,1mm/h, 20,7mm/h, 28,9mm/h, and 25,3mm/h for stable angina, UA, NSTEMI, and STEMI, respectively. Kolmogorov-Smirnov statistical test showed significant difference between groups(p=0.001). It indicated that there was relationship between erythrocyte sedimentation rate and the incidence of acute coronary syndrome and stable angina in dr. Soebandi General Hospital Jember. Abstract

Acute Coronary Syndrome (ACS) is a syndrome that occurs because of blockage of the coronary artery causing clinical symptoms due to unfulfilled oxygen supply. Atherosclerosis is suspected as the main cause of ACS. Inflammation is one pathophysiology of atherosclerosis characterized by leukocytosis. NSTEMI is one spectrum of ACS that diagnosed by anamnesis, ECG, and cardiac marker such as troponin. SGOT is also released into the circulation when there is myocardial damage. Studies has reported that SGOT increased from the normal range during a myocardial infarction. The purpose of this study was to determine the relationship between levels of SGOT and leukocytes in patients with NSTEMI. The sample included 10 patients with NSTEMI in July-December 2016 from ICCU RSD dr. Soebandi Jember. The data was collected from medical record. The result showed that the average of SGOT increased from the normal range, while the average levels of leukocytes were in normal range. Pearson correlation test showed that there was no significant relationship between the two variables with p=0.083>0.05. In conclusion, there was no relationship between the levels of SGOT and leukocytes in patients with NSTEMI at ICCU RSD dr. Soebandi Jember. Keywords: NSTEMI, SGOT, Leukocyte

Abstract

Abstrak Perkembangan penanganan penyakit jantung koroner (PJK) mengalami kemajuan pesat. Pada era percutaneus coronary intervention (PCI) mortalitas dan morbiditas PJK menurun dengan penempatan stent pada area stenosis. Namun, manajemen ini masih menyisakan masalah baru berupa stent trombosis (ST) akut sekitar 0,5-2,2% dari total kasus dengan mortalitas 45%. Beberapa faktor seperti prosedur intervensi, pasien dan karakteristik lesi serta resistensi platelet menjadi pemicu kejadian ST. Kemajuan industri stent, perkembangan prosedur intervensi, dan preparasi pasien dapat menekan kejadian ST, namun resistensi antiplatelet sebagai pemicu ST pada masing-masing individu belum dapat diprediksi.

Abstract

Background: The mortality rate caused by myocardial infarct (MI) escalates in the presence of ventricular septal rupture (VSR). This article aimed to analyze clinical factors that might be valuable as predictors of VSR. Case Illustration: We found and presented six documented post-infarction VSR out of 1,613 MI cases between January 2015 to December 2019. Conclusion: Based on the analysis, clinicians should be aware of these factors as they were potential to be the predictors: advanced age, first MI, anterior location of the infarct, infranodal conduction disorder, no reperfusion treatment, high systolic blood pressure on admission, and no smoking history.

Abstract

Chronic kidney disease (CKD) stage V attacks other organs such as heart that can cause cardiovascular disease (CVD). One of them is CVD caused by CKD stage V that is the kinetic segment of left ventricle. Cardiovascular disease occurs due to excess fluid and buildup of uremic toxin. Chronic kidney disease stage V can not be cured so it requires therapy that is hemodialysis (HD). Hemodialysis can reduce excess fluid, uremic toxin, and maintain electrolyte balance in stage V stage. Improved fluid and decreased uremic toxin can reduce the burden of the heart and strain so that blood supply to certain myocardial areas increases and the left ventricular kinetic segment increases. The purpose of this study is to determine the effect of HD on left ventricular kinetic changes in CKD stage V patients in dr. Soebandi hospital in Jember. The research design used was quasi-experimental one group pretest-postest design. The sample was 30 patients. Respondents were determined by nonprobability sampling technique with purposive sampling method. The data used are primary data and secondary data. Data analysis using Wilcoxon test. Based on the results of the study, the distribution of the sample based on the age of the most in the range of 45-54 years, based on the sex obtained more women, based on the duration of hemodialysis obtained the period of most hemodialysis> 24 months and based on blood pressure before and after hemodialysis obtained patients with hypertension. Hypothesis test using Wilcoxon test got p-value (0.000). From the results of the analysis can be concluded that there is influence of hemodialysis on kinetic segment of left ventricle in patient of CKD stage V in dr. Soebandi hospital in Jember.
