Abstract

Aspek ketepatan waktu pengembalian berkas rekam medis rawat inap harus sesuai dengan SPO yang ditetapkan oleh rumah sakit Estomihi yaitu 2x24 jam setelah pasien pulang dan sudah dalam keadaan lengkap. Oleh karena itu perlu adanya evaluasi faktor penyebab keterlambatan waktu pengembalian berkas rekam medis rawat inap untuk meningkatkan mutu pelayanan rumah sakit. Penelitian ini bertujuan untuk mengetahui penyebab ketidaktepatan waktu pengembalian berkas rekam medis rawat inap, jenis penelitian ini yaitu deskriptif, teknik pengumpulan data menggunakan observasi, kuisioner dan wawancara. Dari hasil penelitian yang dilakukan diketahui bahwa jumlah ketidaktepatan waktu pengembalian berkas rekam medis sebesar 63 atau 72,41% berkas rekam medis rawat inap dan jumlah berkas yang tepat waktu sebesar 24 atau 27,59%. Faktor penyebab keterlambatan waktu pengembalian berkas rekam medis rawat inap yaitu disebabkan oleh dokter yang terlambat mengisi kelengkapan berkas rekam medis, belum adanya petugas khusus pengembalian berkas rekam medis rawat inap, serta jarak Instalasi rawat inap ke Instalasi rekam medis yang cukup jauh sehingga mengakibatkan keterlambatan pengembalian berkas rekam medis rawat inap ke Instalasi rekam medis. Saran sebaiknya pihak rumah sakit melakukan sosialisasi dan mengingatkan kembali dokter dan profesi medis yang lain saat rapat tentang ketepatan pengembalian berkas rekam medis agar lebih disiplin dalam melengkapi berkas rekam medis sesuai dengan kebijakan SPO sehingga pengembalian berkas rekam medis tidak mengalami keterlambatan.

Abstract

Abstract Form design is the activity of designing a form based on the transaction needs of service activities and the preparation or preparation of organizational reports. The surgery report is a surgical procedure for patients. To find out the design of the operation report form at the Rasyida Kidney Special Hospital in 2018. This type of research is a qualitative descriptive study. This research was conducted at the Rasyida Kidney Hospital in Medan, the population and sample used in this study were 1 Operation Report form used at the Rasyida Kidney Hospital in Medan, using the observation method. Based on the results of research on the design of the operation report form in Medan Rasyida Kidney Hospital, the anatomic aspects have not included a complete form heading and there is no clear introduction. From the physical aspect it is not appropriate because it still uses Kuwarto paper with a weight of 70 grams. While the content aspect is appropriate because it has been adapted to the 2008 Minister of Health Regulation. Based on the results of the study it can be suggested that the paper material should be replaced with A4 paper of at least 80 grams to reduce the risk of tearing and the need to add an explanation about filling out the operating report.

Abstract

The Social Security Organizing Agency (BPJS) is an institution formed to organize a Social Security Program in Indonesia based on Law Number 40 of 2004 and Law Number 24 of 2011. Indonesian General Imelda Worker Hospital is a private hospital in the city of Medan who have collaborated with BPJS Based on the initial survey of researchers on 30 medical records documents 13 files (43%) are in accordance with claim requirements and 17 files (56%) are incomplete. Data collection in this study is a questionnaire method. This research was conducted using quantitative research methods with cross sectional approach. The population used in this study were all medical record officers in the medical record unit in the IPI General Hospital Medan as many as 31 officers. Based on the results of a statistical test research conducted using the Chi Square (X2) computer test application with a significance of 95% (p <0.05) it can be concluded that the Hospital conducts an evaluation for medical records officers so that the performance of officers is as expected. Increase evaluation for the completeness of medical record documents used in BPJS claims.

Abstract

The Imelda Workers General Hospital of Indonesia does not yet have specific routines regarding the release of medical record information to third parties. however, they have used regular procedures for providing information and borrowing medical records in general, This study aims to determine the number of requests for medical record information. This type of research is descriptive, namely the research method carried out with the aim of making an objective state. The population was 4 people and the sample size was 4 people as the total sampling with the research methodology using quantitative methods. How to collect data by interview and observation. The results showed that the services of the Imelda Hospital for Indonesian Workers in Medan according to legalized death certificates amounted to 34 percent (0.56%), claims for raharja services amounted to 56 percent (0.92%), audits and insurance claims were 69 percent (1.14%) , research or education as much as 54 percent (0.89), post mortem as much as 28 percent (0.46%), BPJS as much as 5,790 percent (99.00%). Based on the results of the study, it is known that knowing the data on the number of requests for medical record information in January-June 2020 is 6,224 with a percentage (99.97%). As well as the absence of a special SPO on procedures and utilization of medical record information of deceased patients, however, they have used the permanent procedure of providing information and borrowing medical records in general. It is recommended that hospitals and medical record officers provide regular training or coaching to officers. And to health workers, especially medical records, in order to maintain the confidentiality of patient medical record information and carry out the established procedures properly.

Abstract

Human Resources (HR) is needed so that the availability of qualified medical personnel and in accordance with the needs of medical personnel needed to ensure the implementation of health development in order to improve the highest level of public health. The purpose of this research is to know the planning of human resources (HR) needs of the medical record unit at Permata Bunda Medan Hospital. This type of research is descriptive. The method used in this study is the method of observation and interview. The population in this study was a medical record unit of 9 peoples. Samples were taken by total sampling. Available working time based on the amount of effective work totaling 2475 / year, in 1 effective working day is 7 hours. The number of outpatient visits was 43669 patients / year, while the number of inpatient visits was 9284 patients / year. The standard time allowance for officers is 0.01 minutes. Based on the calculation of workforce requirements using the WISN formula that has been carried out in the medical record unit of the Permata Bunda General Hospital in Medan, there were 13 officers, and the number of officers in the medical record unit is 9 people. It is recommended to Permata Bunda General Hospital to add 4 medical record officers with D3 Medical Record education qualifications.

Abstract

Health center accreditation standards are one of the assessments of the medical record file storage system. The medical record file storage system at the Medan Johor Health Center is very important because it helps officers in retrieving and returning files on the medical record file storage rack. The socialization of the medical record file storage system based on this accreditation standard was carried out at the Medan Johor Health Center as a form of updating knowledge and implementing standardized medical record file storage. The purpose of this community service is to help the Medan Johor Health Center in preparing all the files and necessities to follow the Second Edition of the Public Health Center accreditation standard, version 2021 regarding the medical record file storage system at the Medan Johor Health Center. This community service method is to provide socialization of the medical record file storage system in accordance with the second edition of the Puskesmas accreditation instrument, version 2021. The results of community service are expected that medical record officers are able to apply procedures for retrieval and return of medical record files according to the accreditation standards of the storage system which will help medical record officer in the future when the accreditation period for the Medan Johor Health Center will arrive

Abstract

Rekam medis adalah catatan atau dokumen yang berisikan identitas, anamnesa, penentuan fisik, laboratorium, diagnosa dan tindakan medis yang diberikan kepada pasien dan pengobatan baik rawat jalan, rawat inap mauapun pelayanan gawat darurat. Dokumen rekam medis harus didukung dengan keamanan dan kerahasiaan di ruangan peyimpanan berkas rekam medis itu sendiri. Metode yang digunakan dalam pengabdian masyarakat ini adalah dengan memberikan sosialisasi/penyuluhan secara langsung tentang faktor penyebab kerusakan fisik dokumen rekam medis di Puskesmas Pulo Brayan dan demonstrasi serta pendampingan tentang kerusakan fisik dokumen rekam medis di ruang penyimpanan. Hasil dari kegiatan pengabdian kepada masyarakat ini adalah bertambahnya pengetahuan dan keterampilan petugas penyimpanan dalam mengelola dokumen rekam medis untuk meminimalisir terjadinya kerusakan fisik dokumen rekam medis. Kegiatan ini berhasil dilaksanakan dengan baik berkat kerjasama yang baik serta dukungan dari semua pihak baik tim pelaksana, mahasiswa, petugas dan Kepala Puskesmas Pulo Brayan serta Dinas Kesehatan Kota Medan.

Abstract

Analisa Kuantitatif adalah telaah /review bagian tertentu dari isi rekam medis dengan maksud menemukan kekurangan, khususnya yang berkaitan dengan pendokumentasian rekam medis. Klaim BPJS Kesehatan adalah pengajuan biaya perawatan pasien peserta BPJS oleh pihak rumah sakit kepada pihak BPJS Kesehatan yang dilakukan secara kolektif dan ditagihkan kepada pihak BPJS Kesehatan setiap bulannya. BPJS Kesehatan akan melakukan persetujuan klaim dan melakukan pembayaran untuk berkas yang layak, namun untuk berkas yang tidak layak klaim atau pending (unclaimed) harus dikembalikan. Permasalahan yang ditemukan di RSU Imelda Pekerja Indonesia disamping perbedaan sistem tarif yang disebutkan diatas adalah masalah ketidaklengkapan dan ketidaktepatan pencatatan pada rekam medis yang menyebabkan terkendalanya proses klaim dan rendahnya tarif INA-CBG's. Kegiatan pengabdian dilakukan dengan mensosialisasikan tentang kelengkapan dan ketepatan pengisian rekam medis disesuaikan dengan persyaratan pengajuan klaim manfaat pelayanan kesehatan dalam Peraturan BPJS Kesehatan Nomor 7 tahun 2018. Sosialisasi diberikan kepada petugas administrasi klaim BPJS dan petugas rekam medis yang dilakukan dengan menggunakan metode diskusi, ceramah dan tanya jawab. Peserta sosialisasi adalah petugas administrasi klaim BPJS sebanyak 5 orang dan petugas rekam medis sebanyak 13 orang. Kegiatan yang dilaksanakan adalah pemaparan materi tentang persyaratan pengajuan klaim sesuai Peraturan BPJS Kesehatan Nomor 7 Tahun 2018 yang difokuskan pada kelengkapan administrasi umum, kelengkapan khusus dan kelengkapan pendukung pengajuan klaim. Sosialisasi ulang Standar Prosedur Operasional (SPO) yang sudah ada dan memberikan masukan untuk perbaikan (revisi) SPO dan membuat lembar ceklist kelengkapan rekam medis yang disesuaikan dengan persyaratan pengajuan klaim manfaat pelayanan kesehatan di FKRTL dalam Peraturan BPJS Kesehatan Nomor 7 Tahun 2018. Hasil kegiatan pengabdian kepada masyarakat ini yaitu bertambahnya pengetahuan petugas tentang kelengkapan dan ketepatan pengisian rekam medis sesuai dengan Peraturan BPJS Kesehatan Nomor 7 Tahun 2018.Kegiatan ini berhasil dilaksanakan dengan baik berkat kerjasama yang baik serta dukungan dari semua pihak baik tim pelaksana, petugas administrasi klaim BPJS, pegawai rekam medis, dan Direktur RSU Imelda Pekerja Indonesia Medan.

Abstract

BPJS Health claims are the submission of hospital patient care costs by the hospital to the BPJS Health party which is carried out collectively and billed by the hospital to the BPJS Health every month. Based on an initial survey conducted at the Indonesian Workers' Imelda RSU, it was one that caused the pending claim file (unclaimed), namely the incompleteness of recording Medical Records. As for the pending claim (unclaimed) it is found more in hospitalizations, and based on information that researchers found it is known that the number of inpatients at Indonesian Workers' Hospital in 2021 had 8,004 patients. Inpatient file data submitted in the last month, April 2022, was 472 files. A total of 371 files (78.6%) were successfully claimed, 100 files (21.2%) were pending claims, and 1 file (0.2%) which failed purif. The method used in this study is quantitative descriptive with a cross section approach, and by univariate data analysis. Research results show factors that cause pending claims in BPJS participant patients in Indonesian Workers' Hospital are coding mismatches there are as many as 25 medical records (50%), incompleteness of administrative data of 10 medical records (20%), and incompleteness and or inaccuracy in filling medical records of 15 medical records (30%). Suggestions put forward are making regulations in the form of policies, rules, or SOPs, as well as adding training and attending seminars related to coding and enforcement of diagnoses.

Abstract

Tujuan pengabdian ini untuk mengetahui efisiensi penggunaan tempat tidur berdasarkan tipe kelas menggunakan grafik Barber Johnson. Jenis pengabdian ini adalah Mix Methods. Data primer pada pengabdian ini adalah hasil observasi peneliti dan data sekunder data yang diperoleh dari unit kerja rekam medis dan SIRS RSU Imelda Pekerja Indonesia Medan. Pengumpulan data dalam pengabdian ini dilakukan dengan menggunakan teknik wawancara dan observasi. Hasil pengabdian adalah nilai BOR belum memenuhi standar Barber Johnson yaitu ruang rawat inap kelas I (48%), kelas II (38%) dan kelas III (60%). Nilai LOS sudah memenuhi standar nilai ideal Barber Johnson yaitu ruang rawat inap kelas I (4,2 hari), kelas II (4,5 hari) dan kelas III (4,2 hari). Nilai TOI masih melebihi nilai standar Barber Johnson yaitu ruang rawat inap kelas I (7,7 hari), kelas II (8,7 hari), kelas III (3,3 hari). Nilai BTO pada ruang rawat inap kelas I (28 kali) dan kelas II (25,5 kali) masih belum sesuai dengan standar Barber Johnson, sedangkan kelas III (43,5 kali) sudah sesuai dengan standar nilai ideal Barber Johnson. titik pertemuan pada grafik Barber Johnson ruang rawat inap kelas I, kelas II dan kelas III masih berada diluar daerah efisien sehingga indikator ruang rawat inap RSU Imelda Pekerja Indonesia masih belum efisien.
