Abstract

Diabetes mellitus was a metabolic disease characterized by hyperglycemia that occured due to the inability of the pancreas to secrete insulin, impaired insulin action, or both. According to Riskesdas data (2018), the prevalence of diabetes mellitus in Jember district was in third place. The increased prevalence was caused by one of them, namely the result of inappropriate drug use. The rationality of the drug could be seen from the accuracy of the type of drug, dosages, patient, dosage of each drug. The purpose of this study was to determine the rationality profile of drugs in patients with diabetes mellitus. This type of research was an observational study with a cross-sectional design. The data collection technique used random sampling, with the number of samples in the study being 43 samples. The sample in this study was medical record data of diabetes mellitus patients. The instruments used in the study were patient medical records, hospital formularies and data recapitulation sheets. Data was processed descriptively displayed in the form of frequency and percentage. The results showed that the accuracy of the drug was 95,35%, the accuracy of the dosage was 100%, the accuracy of the patient was 100%, the accuracy of the drug indication was 95,35%, so it was concluded that the rationality of the drug was 97,67%. Drugs were given based on indications, doses, drugs, and patient conditions. Physician provided drug therapy according to hospital procedures that refered to the hospital formulary.

Abstract

Renal failure was a clinical condition characterized by irreversible decline in kidney function. Renal failure was divided into chronic and acute renal failure. Hypertension was a major factor triggering the occurrence of kidney disease and kidney failure. Data released by WHO shows that 50% of people with kidney failure, both acute and chronic, reach 50%. Based on Riskesdas, in 2018 the prevalence of chronic kidney failure in Indonesia was 0.38%. East Java Province was 0.3%, while the highest prevalence based on age was 65-74 years old at 8.23%. Effectiveness could provide information about how much a drug could achieve the desired effect in clinical practice. The purpose of this study was to compare the effectiveness of used captopril and valsartan in hypertensive patients with kidney failure at hospital. Methods: This research uses a numerical comparative analytical research type with an observational and cross-sectional design. The research was conducted at. Citra Husada Hospital in March-April 2023, using patient medical record data sources, and a sample of 64 patients. Data sources were analyzed using the Chi square test. Data is displayed in frequency and percentage form. Results: The results of this study showed that the average valued of blood pressure before administration of captopril was 183. 65 mmhg/99. The average value of blood pressure after administration of captopril was 171. 00 mmhg/91. 778 mmhg. The average value of blood pressure before administration of valsartan was 165. 87 mmhg/92. 81 mmhg. The average value of blood pressure after administration of valsartan was 151. 15 mmhg / 81. 09 mmhg. From the results of the studies, it was found that there was no difference between the comparison of the effectiveness of used captopril and valsartan in hypertensive patients with kidney failure at hospital. Conclusion: The conclusion of this study shows that there was no comparison of the effectiveness of used captopril and valsartan in hypertensive patients with kidney failure, both of which had the same effectiveness in lowering blood pressure

Abstract

Hypertension was a condition where systolic blood pressure was more than equal to 140 mmHg and diastolic was more than equal to 90 mmHg after two separate measurements and was one of the main risk factors for kidney failure because it can cause damage to blood vessels in the kidneys so that reduces the ability of the kidneys to filter blood properly. Hypertension was a disease with a high incidence rate worldwide and was a cause of death, around 1.13 billion people in the world have hypertension and its complications, therefore the aim of this study was to determine the effect of using antihypertensive drugs in hypertensive patients with kidney failure in hospital outpatients, jember regency. In a study using a cross-sectional design, the population in this study were all hypertensive patients with kidney failure who were outpatients in hospital outpatients, jember regency in the period December 2022-March 2023. The number of samples used probability sampling with the proportionate stratified random sampling technique as many as 100 patients in hospital outpatients, jember regency. The results of this study indicate that there was an effect of using antihypertensive drugs in hypertensive patients with kidney failure. Based on the output of "Test Stalistics", it is known that Alsymp.Sig. (2-tailed) has a value of 0.002. The calrenal value is 0.002 less than <0.05, malkal was concluded as "Hypothesis accepted". Alrtinyal aldal antihypertensive drug use paldal hypertensive patients with renal failure in hospital outpatients, jember regency. The blood pressure before the use of antihypertensive paldal antihypertensive medication with renal failure, that was, paldal paldal hypertension with gallstone saltu, while the blood pressure after the use of antihypertensive paldal pallid hypertensive patients with renal failure decreases in dual staging hypertension. From a statistical analysis using the Wilcoxon Maltch Palirs Test, it was discovered that Halsil might actually have an effect on the use of antihypertensive drugs in hypertensive patients with kidney failure in hospital outpatients, jember regency.

Abstract

Hypertension an increase in blood pressure, including systolic and diastolic blood pressure into two types, namely essential hypertension, the most common being essential and secondary hypertension. The prevalence of hypertension in Indonesia using measurement methods in samples aged ≥18 years 34.1%, while the prevalence of hypertension cases in East Java province 36.3%. In pharmacological therapy the administration of drugs includes thiazide diuretics, adrenergic blockers, Angiotensin Converting Enzyme Inhibitors (ACE-inhibitors), angiotensin - II - blockers, in non-pharmacological therapy, namely changes in diet and lifestyle by consuming balanced foods according to needs, avoiding saturated fat, maintaining body weight, limiting salt consumption, the aim of knowing changes in blood pressure in hypertensive patients with kidney failure when administering the drug valsartan at Hospital Citra Husada. This research used a numerical comparative analytical research type with a retrospective design, the research was conducted in March, April 2023 at Citra Husada Hospital, the source of research data was from medical records. The sample obtained was 32 hypertensive patients with kidney failure who met the inclusion criteria, namely all genders aged 18 years and over, and outpatients at Hospital on a single drug (valsartan). Sampling used a total sampling technique, using Wilcoxon test data analysis to analyze changes in blood pressure. The average value of systolic blood pressure before administering the drug valsartan was 165.87 mmHg with a standard deviation of 27.06. The average systolic value after administering the drug valsartan was 151.15 mmHg with a standard deviation of 26.06, with a difference value of 14.72 mmHg. The average value of diastolic blood pressure before administering the drug valsartan was found to be 92.81 mmHg with a standard deviation of 20.16. The average diastolic value after administering the drug valsartan was found to be 81.09 mmHg with a standard deviation of 18.8, with a difference value of 11.72 mmHg. Conclusion: There are marked changes in blood pressure after administering the drug valsartan, valsartan can be recommended as an antihypertensive with kidney failure according to the inclusion criteria in the study.

Abstract

Diabetes melitus (DM) is a group of diseases with characteristic hyperglycemia that occurs due to impaired insulin secretion, insulin action, or both. The International Diabetes Federation (IDF) notes that Indonesia has the country with the fifth most significant number of diabetics in the world, with 19.5 million Indonesians aged 20-79 years suffering from the disease. Therefore this study aims to determine the profile of the use of a combination of two classes of oral hypoglycemic drugs in type 2 DM patients at Citra Husada Hospital in 2022. This research is a descriptive study with retrospective data collection. The population in this study were all outpatients diagnosed with Type 2 DM at Citra Husada Hospital in 2022. Source of medical record data. The number of samples using a population of 100 patients in the outpatient installation at Citra Husada Hospital. This study used a total sampling technique which is a sampling technique that uses all members of the population. Data analysis used univariate analysis in the form of proportions. Research results show that 100 samples are included in the inclusion criteria. Results of the study Most of the patient characteristics were female (61%), age range 56-65 years (39%), and hypertension co-morbidities (33%). The results of a combination study of two classes of oral anti-diabetes drugs, namely the sulfonylurea group (glimepiride at a dose of 3 mg, the drug frequency is taken once a day every morning with the thiazolidinedione group (pioglitazone at a dose of 30 mg, the drug frequency is taken once a day during the day) (35%). The most common used of combinations of two classes of oral anti-diabetes drugs in patients Diabetes melitus at Citra Husada Hospital are sulfonylureas and thiazolidinediones

Abstract

Diabetes mellitus is a non-communicable disease which is a concern a national and global public health problem. Indonesia was ranks sixth in the world with a prevalence of 8,6% of the total population and it is estimated that in 2025 it will increase to 12,4 million sufferers. This research was conducted at the one of public health center. The purpose of this study was to determine changes at blood glucose levels of diabetes mellitus patients on administration of a combination of metformin and glibenclamide drugs. This research used a type of comparative, comparatively paired sample analytical research, with the design cohort retrospective. The instrument used in this research was a patient's medical record. The population in this study were all adults diabetes mellitus patients that over 18 years old at the period of March-Mei 2020.Samples were calculated using the total sampling technique during the research period. The Data was processed using SPSS version 18 and analyzed use Wilcoxon test.The results showed that random blood glucose levels before the use of a combination metformin and glibenclamide with an average of 292,80 mg/dl and after using combination metformin and glibenclamide drugs was had decrease of -76.1mg/dl. Conclusion: A combination of metformin and glibenclamide drugs provided changes in blood glucose levels decrease in diabetes mellitus patients, so it can be used as an antihyperglycemic recommendation according to the classification of patients in research.

Abstract

Pre-eclampsia was a class of hypertension in pregnancy. Based on WHO in 2015 the prevalence of preeclampsia in developed countries was around 1.3-6% while in developing countries it was around 1.8-18%. Antihypertensive therapy in pre-eclampsia patients was calcium channel blockers. Nifedipine was a dihydropyridine calcium channel blocking agent used as a first-line antihypertensive treatment for pre-eclampsia. The used of hypertension drugs in pregnancy can be said to be successful and effective when blood pressure reaches the target of <140/90 mmHg. The purposed of this study was to analyze the profile of the effectiveness of using nifedipine in pre-eclampsia patients at Hospital. This research was a quantitative research with a retrospective descriptive design. The total sample was 91 patients. The data source used medical record data from pre-eclampsia patients at Hospital in the 2022 period. The sampling used was simple random sampling technique and data was displayed in the form of frequency and percentage. The results of this studied indicate that the used of the drug nifedipine 10 mg (13%) nifedipine 3x10 mg (87%), the average blood pressure before 161.37/101.26 mmHg, the average blood pressure after 132.97/85.60 mmHg and the effectiveness of using nifedipine was 100% effective. Thus it could be concluded that the profile of the effectiveness of using nifedipine was said to be effective because it could reduce blood pressure. Nifedipine was recommended in pre-eclampsia patients to lower blood pressure
