Abstract

Background: Acne vulgaris is a common skin condition that causes blackheads, whiteheads, and pimples. It is caused by a number of things, like too much oil, clogged pores, and inflammation. Topical combination creams are an alternative treatment for acne. They can help reduce inflammation, kill bacteria that cause acne, and stop the skin from making too much oil. Topical combination creams like retinoids, antibiotics, and steroids are all mixed together in a single cream or gel. This study talks about the effectiveness and side effects of using combination creams (tretinoin 0.05%, clindamycin 3%, and dexamethasone 0.05%) to treat acne in the short and long term (local and systemic). Methods: This study is a survey that was done at the Sukma clinic with data from 2022 patients who were diagnosed with acne vulgaris. The survey was done on Google Forms, which is a website. In this study, different factors were looked at, such as demographics, efficacy (like reducing acne severity, making the skin brighter, getting rid of blackheads on the face, getting rid of acne scars, improving skin texture, hiding scars, minimizing facial pores, getting rid of wrinkles and dark spots), local adverse events (like burning, itching, stinging, eruptive papules, hypopigmentation, hyperpigmentation, erythema, scaling, and other), and systemic adverse events. Results: The results of this study show that a combination of tretinoin 0.05%, clindamycin 3%, and dexamethasone 0.05% is very effective and has minimal side effects. Local symptoms only show up during the first week of taking the drug (the "sensitization phase"), and then they tend to get less common over time. The only exceptions are hyperpigmentation and hypopigmentation, which show up later because they are caused by healing acne lesions. Conclusion: There was no direct link between the use of anti-acne combination cream drugs and the number of systemic side effects. This is because the patient had often had this happen before (constipation, GERD, and others).

Abstract

Hyperlipidemia is a metabolic disorder that indicates an increase in lipid levels in the body. Obesity, particularly central obesity, is correlated with hyperlipidemia and metabolic syndrome. Complications of hyperlipidemia such as coronary artery disease, type 2 diabetes, and high blood pressure can be fatal if left untreated. Education and early detection of hyperlipidemia are crucial to prevent serious complications and maintain good health. This community service involved 50 elderly at Santa Anna Elderly Care Facility. All respondents participated in a series of activities, including education through posters, physical examinations, and diagnostic tests. Results of the diagnostic tests indicated 2 respondents had LDL levels ? 135 mg/dL, 6 respondents had HDL levels < 40 mg/dL, and 12 respondents had total cholesterol levels ? 200 mg/dL. The results of the education activities are expected to increase public awareness, all of which can reduce the overall healthcare burden when applied correctly and properly.

Abstract

World Health Organization (WHO) approximates 1.28 billion individual suffers hypertension, with 46% of them does not realize it, and only 1 of 5 hypertensive individual has their blood pressure controlled. Hypertension prevalence is still relatively high in the elderly population who live in nursing home. This is one of the main health adversities that often happens and should not be underestimated due to its risk of complications. Uncontrolled blood pressure is one of the main risk factor to the global disease burden and total of global death, contributing up to 9,4 millions of deaths (8,5% of the global total) annually. This activity aims to increase the knowledge of the community about hypertension in elderly who lives in nursing home to increase the quality of life and reducing the cost burden of therapies and treatments. We use PDCA activity method to ensure the program could run smoothly and efficiently. This activity included 34 elderly respondents of the St Fransiscus Community with the mean age of 4 (11,8%) years. We found that 4 (11,8%) of the respondents have uncontrolled blood pressure. With the implementation of this program, it is hoped that the public awareness of elderly hypertension and its prevention is increased, so that there will be increase of quality of life for the elderly in the future and relieving the economical burden.

Abstract

Obesity, characterised by excessive or abnormal fat accumulation, poses significant health risks. The prevalence of obesity in Indonesia's adult population increased from 14.8% in 2013 to 21.8% in 2018, with central obesity rising from 26.6% to 31%. Contributing factors include genetics, lifestyle, psychological stress, and socioeconomic status. Although the Body Mass Index (BMI) is a standard measurement for assessing obesity, BMI does not accurately reflect the composition or distribution of body fat. Therefore, additional anthropometric measurements such as waist and hip circumference are essential. This activity uses the PDCA (Plan-Do-Check-Action) methodology to screen for obesity in adults at Kalam Kudus Middle School, West Jakarta. Of the 129 participants, 26 people (20.16%) were overweight and 35 people (27.14%) were obese. Waist circumference indicated central obesity in 61 women (47.29%) and 19 men (14.73%). Hip circumference showed that 9 men and women (6.98%) had a high risk of metabolic disease. Anthropometric measurements provide important insights into obesity and its health implications, emphasising the need for regular screening and lifestyle modification.

Abstract

Aneurysmal Subarachnoid Hemorrhage (aSAH) is one of the challenging neurologic emergencies with a high mortality rate along with various permanent disabilities. In order to provide the patient with the most appropriate and accurate treatment, as well as to prevent further complications, a multidiscipline approach is required. This study aimed to review the various mechanisms, indications, management, and sedation of mechanical ventilation in aSAH, along with a review of prone positioning and acute respiratory distress management in aSAH. Although the main injured organ is the brain, aSAH also affects the respiratory system through various mechanisms. The usage of mechanical ventilation plays an important part in brain oxygenation and perfusion and helps prevent related complications. Levels of oxygen and carbon dioxide in the blood might play some roles in aSAH patients. No significant difference was found in using various sedative regimens. Prone positioning is indeed beneficial for the oxygenation of aSAH patients, provided that continuous monitoring is done. Blood glucose and calcium levels might be able to help predict the outcome of aSAH patients. Mechanical ventilation plays an important part in aSAH management. Clinicians must be aware of the impact of mechanical ventilation on neurological organs and the cardiopulmonary system. Balancing between oxygenation, ventilation, and sedation must be in line with aSAH condition. Several prognostic factors and tools can help predict aSAH mortality that might be able to help the clinician tailor aSAH management to their patient's needs.

Abstract

Obesity, characterized by excessive accumulation of body fat, increases susceptibility to chronic disease and adversely impacts overall quality of life. Elderly people often exhibit higher fat mass and lower muscle mass and bone density, influenced by reduced physical activity, hormonal changes, and metabolic adjustments. Visceral fat accumulation is of particular concern because of its association with metabolic syndrome, insulin resistance, diabetes, hypertension, and cardiovascular disease. This activity uses the PDCA (Plan-Do-Check-Act) framework to screen for obesity in the elderly using Body Mass Index (BMI) and waist circumference measurements. Activities were held at St. Fransiskus Assisi, Jakarta involving 36 participants. The study revealed that 16 participants (44.5%) and 30 people (83.3%) were obese and centrally obese, respectively. Early detection and intervention through anthropometric measurements are very important in managing health risks related to obesity in the elderly, so as to improve their quality of life and reduce morbidity and mortality.
