Abstract

Background: Indonesia is a developing country with a high maternal mortality rate (MMR) and perinatal mortality, the third highest in ASEAN and the second highest in the South East Asian Nation Regional Organization. In pregnancy hypertension has been proven that oxidants, especially if increased fat peroxide will damage endothelial cells called endothelial dysfunction. Vasospasm that occurs also induces platelet integration and endothelial damage which adds to the contribution in maintaining platelet dysfunction and triggering the use of platelets. Thrombocytopenia is the most important sign of the severity of preeclampsia. Objective: to determine the comparison of platelet levels in patients with preeclampsia with eclampsia.Methode: This study was observational cross sectional method. The sample in this study was a total sampling, using purposive sampling of patients with preeclampsia and eclampsia at the Wiyung Sejahtera Hospital in January-December 2019. The instrument used was using secondary data of medical records of patients. Results: Patients with preeclampsia have a minimum platelet level of 301,000/mm3, a maximum of 415,000/mm3, and an average of 351,733.33/mm3 with a standard deviation of 33,552.66/mm3. Patients with eclampsia have a minimum platelet level of 122,000/mm3, a maximum of 281,000/mm3, and an average of 209,200/mm3 with a standard deviation of 42,465.45/mm3. There are significant differences in platelet levels of patients with preeclampsia with eclampsia(p <0.05)..Conclusion: there is a significant difference between the platelet levels of patients with preeclampsia and eclampsia.

Abstract

Background: Persistent Pulmonary Hypertension of the Newborn (PPHN) is a condition of the failure to decrease pulmonary artery pressure, so that blood pressure in the lungs remains high, as in a fetus. PPHN cases are 2-6 of 1000 live births and responsible for 10% of confirmed deaths in the neonatal intensive care unit (NICU). Sildenafil is vasodilator that can be used to reduce pulmonary artery systolic pressure (PASP), morbidity, and mortality in PPHN infants. Objective: To analyze the effect of sildenafil on decrease the severity of pulmonary artery systolic pressure in PPHN infants.Method: This is an analytic observational study with cross-sectional design. The samples were collected using consecutive sampling on 30 PPHN infant patients in 2018. The data were collected from medical records of PPHN infant patients, then assessing the severity value of PASP which obtained from echocardiography result. The severity of PASP is classified into normal (PASP≤35 mmHg), mild (PASP36-45 mmHg), moderate (PASP 45-60 mmHg), and severe (PASP>60 mmHg)Results: This study showed that 27 PPHN infant patients have decreased PASP level to normal, previously from mild 7 infants (23.3%), moderate 16 infants (53.3%), and severe 4 infants (13%), after being given oral sildenafil. There were 1 (3.3%) infant patient remained severes, 1 (3.3%) patient was decreased from severe to moderate, and 1 patient (3.3%) was decreased from severe to mild. The p value <0.05 was obtained, so oral sildenafil significantly decreased the PASP severity in PPHN infants patients. Conclusion: There is a beneficial effect of oral sildenafil on decreasing the severity of pulmonary artery systolic pressure in PPHN infants at the NICU of Siti Khodijah Muhammadiyah Hospital Sepanjang.

Abstract

Background: Across all age ranges, one in a thousand people is diagnosed with ileus annually. Ileus is categorized into two types, obstructive ileus, and paralytic ileus. Obstructive ileus is a condition where intestinal passage disorder occurs as a result of a mechanical blockage. There are 7,024 obstructive ileus without hernia in Indonesia and 7,059 paralytic ileus cases in 2004.Objective: To understand the comparison of characteristics of obstructive ileus patients.Method: This analytic descriptive research use the cross-sectional method. Samples are diagnosed with ileus based on the laparotomy result from RSUD Dr. Soegiri, Lamongan. The sampling method is total sampling based on 2015-2019 data. The samples were also using secondary data from medical records, from December 2019 to January 2020.Result: Respondents are mostly male with ages 65 years and over, less than ten days treatment duration, 39 minutes in the average length of partial bowel obstruction surgery duration, 42 minutes in the average length of total bowel obstruction surgery duration, less than two days treatment duration before surgery, percentage of ICU admission as high as 51.5 %, and blood transfusion percentage on 6.1%.  There are significant differences in the characteristics of ileus patient’s age (p-value 0,021).Conclusion: Based on this study, it can be concluded that there are differences based on age in the characteristics of partial bowel obstruction and total bowel obstruction on ileus patients, whereas gender, treatment duration, treatment duration before surgery, and surgery duration did not deliver any differences.

Abstract

The leaves of Binahong (Anredera cordifolia (Ten) Steenis) have been widely studied to have benefits and uses in various medical purposes, one of which is in the wound healing process. The existence of several active compounds contained in the leaves of the Binahong tree plant have an important role in wound healing, such as flavonoids, terpenoids, alkaloids, saponins, ascorbic acid and citrate compounds, these compounds are the targeted for further process of this natural product. The eminence of the extraction method that is used repeatedly and consistently to obtain pure compounds from Binahong leaves to be isolated is the one of topics discussed in this literature study. The perspective of implementation from liquid-solid extraction followed by liquid-liquid extraction using chloroform as a solvent is discussed comprehensively in this article. The identification of the isolation of compounds are able to conducted by implemented a thin layer chromatography (TLC) techniques, and for the further structural determination by UV, MS, FTIR, NMR spectroscopy and separation techniques using GC-MS. Respectively, the components are successfully to be identified and reported in previous research, for the following criteria that have been completed, some isolated active compound are purposed as the candidate for the wound healing agent. For the further application it is reasonable to implied these matters for some pharmaceutical products.

Abstract

Background: Exclusive breastfeeding is giving breast milk exclusively from newborns to 6 months of age without giving any food. Exclusive breastfeeding is essential for optimal baby growth and development, contains various essential nutrients for babies. In 2018 the coverage of exclusive breastfeeding in Indonesia still reached 65.16%, and the coverage of breastfeeding in East Java itself was still below the target of 76.98%. The low level of exclusive breastfeeding is still a problem because it can cause the baby's immune system to become lower, indigestion and illness risk in the baby is more generous to target. Objective: To determine the relationship between mothers' level of knowledge about breastfeeding and exclusive breastfeeding at Kenjeran Primary Health Center. Methods: This observational analytic study with a cross-sectional study design. The sampling technique was using the purposive sampling technique. Respondents in this study were mothers who had babies 6-24 months. The data collection process was directly asked through a questionnaire and analyzed using the contingency coefficient test. Results: This study involved 51 mothers who gave exclusive breastfeeding and 51 mothers who did not. The results showed that most of the respondents who breastfed and did not have the right knowledge level. Based on the contingency coefficient test results, it was found that there was no relationship between the level of mother's knowledge about breast milk to give exclusive breastfeeding at the Kenjeran Primary Health Center (the value of p = 0.251).Conclusion: There is no relationship between mothers' level of knowledge about breastfeeding and exclusive breastfeeding at the Kenjeran Primary Health Center.
