Abstract

The insertion of an intrauterine device (IUD) is installed immediately after delivery has been recommended by the WHO, as one method of contraception is safe and effective for interim and prevent missed opportunity (unmet need). IUD insertion after childbirth can avoid the discomfort that usually occurs during the interval insertion, and lochia can obscure any bleeding from the insertion. However, post-partum IUD insertion has disadvantages as well. The risk of the possibility of spontaneous expulsion is very high. This study is an experimental study with the method of post-test control group design to determine differences in IUD expulsion rate tied and not tied when installed during caesarean section at RSUP. Dr.M.Djamil in Padang, and Military Hospital Reksodiwiryo Padang and Painan District Hospital. There were no significant differences between trancaesarean IUD insertion methods that are not tied or tied (P> 0.05). The percentage of expulsion is not tied 11.4% higher compared to 0% tied expulsion. Statistically, were not significant differences as obtained P value> 0.05.Keywords: IUD, tied, trancaesarean

Abstract

Background : Uterus bicornis subseptum is a deformity of the uterus such as the shape of the heart, the fundus appears to be curved inward, and has a barrier or wall inside to the uterine cavity. This abnormality occurs due to failure of the fusion of the left and right mullerian ducts. If pregnant, women who have this form of the uterus will usually experience a location abnormality, in which the fetus is often transverse or breech. However, women who have this disorder sometimes find it difficult to get pregnant so they experience infertility. In addition, there is usually a disturbance in the formation of the kidneys, but in this patient there was no abnormality in the urinary tract. In cases where the patient does not have children and a bicornu uterine abnormality is found, uterine repair is performed and it is hoped that with the uterine anatomy returning to normal, the patient can become pregnant. Objective : Discuss the management of cases of congenital abnormalities of the internal genital organs in patients with a bicornu uterus.Method : Case Report.Case : Reported a patient aged 25 years with primary infertility 4 years and a bicornu uterus. In patients, chromotubation was performed first, after chromotubation, the patient was performed metrosplasty to remove the bulkhead in the uterus and to repair the uterus. During the post-op follow-up at the hospital the patient did not show any signs of infection or acute abdomen, then the patient was discharged on day 5. The patient was advised to go to the OBGYN polyclinic regularly for further follow-up to the success of his uterine repair. Assessment can be done in several ways including transvaginal ultrasound to assess whether there is adhesions to the endometrium from the results of uterine repair, HSG, SIS or hysteroscopy can also be done in assessing the success of metroplasty in this patient. Conclusion : After metrosplasty in a patient with a bicornu uterus, further follow-up is required to determine the success of this procedure by performing a follow-up ultrasound. Keywords: Bicornu uterus, metroplasty

Abstract

Premature rupture of membranes is the most common complication of pregnancy. The incidence of premature rupture of membranes in pregnancy ranged from 6% to 10%, and 20% of these cases occur before 37 weeks gestation. The incidence of premature rupture of membranes in Indonesia ranges from 4.5% to 7.6% of all pregnancies. This research was conducted to determine the cross-sectional differences in the blood plasma levels of vitamin C in term pregnancy premature rupture of membranes with blood plasma levels of vitamin C in term pregnancy without premature rupture of membranes in M. Jam- il Padang hospital, Achmad Muchtar Bukittinggi hospital, and Pariaman Hospital. There are significant differences in vitamin C blood plasma levels in term pregnancy with premature rupture of membranes and term pregnancy without premature rupture of membranes ( P < 0.05). Mean levels of vitamin C in blood plasma at term pregnancy with premature rupture of membranes lower than in the blood plasma levels of vitamin C in term pregnancy without premature rupture of membranes. Keywords: Premature rupture of membrane in aterm, blood plasma levels of vitamin C

Abstract

Background : Surgical wound infection is a problem that is often found today in surgery. The Indonesian Ministry of Health reported that in 2011, 55.1% of surgical wound infections were found in government hospitals. This is in line with WHO data in 2010, where the sectio caesarean rate in Indonesia was recorded at 6%. Skin preparation can be used to prevent surgical wound infection by reducing the number of germs on the skin prior to incision. Objective : To see the relationship between skin preparation and post sectio caesarea surgical wound infection at Achmad Mochtar Bukittinggi Hospital. Methods : This study was an analytical study using a cross sectional study design by looking at the subject's medical records according to the time and place of the study. Samples were all medical records of mothers giving birth by sectio caesarea in the Obstetrics and Gynecology Section of Achmad Mochtar Bukittinggi Hospital in the period 1 January 2017 to 31 December 2017. Samples were taken from populations that met the inclusion and exclusion criteria. The sample was taken using simple random sampling technique which was taken from the medical records of Achmad Mochtar Hospital Bukittinggi. Statistical analysis to assess significance used the chi-square test. Results : The sectio caesarean group that carried out skin preparation without using chlorhexidine gluconate had the highest number (59.8%), with post sectio caesarean wound infection of 70.7%.Conclusion : There was a significant relationship between skin preparation and post sectio caesarean surgical wound infection (p <0.021) at Achmad Mochtar Bukittinggi Hospital. Keywords: Sectio caesarea, skin preparation, surgical wound infection

Abstract

Premature rupture of membranes (PROM) is defined as rupture of membranes before contraction (his). PROM is the case of 5-10% of all deliveries. 70% of cases of PROM occur in term pregnancies. So that PROM in aterm pregnancies is suggested to induce labor to reduce the incidence risk of mother and fetus complications. This study aimed to compare the effectiveness of cervical ripening by using Misoprostol 25 µg peroral and without using Misoprostol 25 µg at term premature rupture of the membrane with Bishop Score ≤ 4. This study was conducted at the Departement of Obstetrics and Gynecology, Dr. M. Djamil Central General Hospital, and Reksodiwiryo Hospital Padang from November 2013 – August 2014 Simple Random Sampling design. The subject selected based on inclusion criteria, then assess the state of the cervix and laboratory examination and nonstress test, then recapitulated and processed data is computerized using the computer software and statistical test. Significant differences increase bishop scores on 25 mcg misoprostol orally with those not given to premature rupture of membranes at term with less than bishop scores or equal to (≤) 4. The p-value evidence this was 0.000 <0.05. And there is no side effects in this study. Keywords: misoprostol effectivity, premature rupture of membrane

Abstract

Management of preeclampsia is limited to blood pressure control and symptom relief in combination with close maternal/ fetal surveillance. Until recently, very little attention has been paid to preeclampsia's psychological consequences, especially maternal anxiety. Anxiety disorders can be identified using questionnaires instrument determining anxiety level, the Taylor Minnesota Anxiety Scale (TMAS) questionnaires is one among them. This study aimed to determine the TMAS maternal anxiety score difference between preeclampsia and normal pregnancy. This was an analytical study with a cross-sectional design conducted from March-August 2014 in 40 subjects, consisting of 20 women with preeclampsia and 20 women with normal pregnancy who met the inclusion and exclusion criteria. Anxiety was measured with the TMAS questionnaire. Statistical analysis to determine the TMAS maternal anxiety score difference between preeclampsia and normal pregnancy was t-test. In preeclampsia group, the mean of TMAS maternal anxiety score was 30,45 ± 9,65. While in the normal pregnancy group, the mean of TMAS maternal anxiety score was 18,50 ± 7,62. There was a significant TMAS maternal anxiety score difference between preeclampsia and normal pregnancy (p = 0,000).Keywords: TMAS maternal anxiety score, preeclampsia
