Critical Medical and Surgical Nursing Journal (CMSNJ) (p-ISSN: 2355-1992), (e-ISSN: 2656-470X) was established at the end of 2012 under Universitas Airlangga. The CMSNJ was prepared in response to the development of nursing/health science and technology to contribute to the development of sustainable quality nursing/health resources. Abstract

Introduction:.Anxiety arises from physical, psychological and other pressure of threats. Anxiety may occure in patients who are planned for surgery, from mild untill severe anxiety, that also involving changes in vital signs. One of the techniques used to reduce the anxiety is sensory motor body therapy. Nevertheless, this therapy has not been proven yet reduce an anxiety of elective surgery patients. Therefore, this study was aimed to investigate the effect of sensory motor body therapy on anxiety level and vital sign of pra operation patients.Method:The research was`used a quasy experiment (pre and post with control group). Sample on this study were 26 respondents which divided into 2 groups, the control and treatment group. The independent variable was sensory motor body therapy whereas the dependent variables were blood pressure, heart rate and respiration rate. The data were collected using Zung self-rating anxiety scale (SAS/ SRAS) and vital signs observation and were then tested using Wilcoxon Signed Rank Test, Mann Whitney Test, Paired t-Test and Independent t-Test with significant level of p<0.05.Result:Sensory motor body therapy has been an effect in anxiety level (p=0.001), systole pressure (p=0.020) and respiration rate (p=0.001). But, it has not been an effect in diastole pressure (p=0.316) and heart rate (p=0.481) because of most patients were in the normal boundary.Discussion:Based on the reseach results, we can conclude that sensory motor body therapy in pra operation have a significant role in controling patients anxiety level, systole pressure and respiration rate. For further study, next researcher can improve a new method of sensory motor body therapy on anxiety in pra operation patient.

Abstract

Introduction: Development of cancer and the side effect of chemotherapy could lead to Cancer Related Fatigue (CRF) in breast cancer patient that getting worse due to inactivation. Hence, exercise was needed. This aimed of this was to analyze the effect of Walking Exercise Program (WEP) to Cancer Related Fatigue.Methode: This study was used quasy experimental design and the population was breast cancer patient undergoing chemotherapy in RSUD Ibnu Sina Gresik in June 2012. The 10 respondents who met inclusion criteria were taken with matching allocation technique and divided into control and case group. CRF were collected by a subjective instrument called Brief Fatigue Inventory (BFI), then analyzed using Wilcoxon signed rank test and Mann Whitney test with signification value was α≤0,05.Result: Result of Wilcoxon signed rank test showed that CRF scale in pre-test was different compared to CRF in post-test of case group (p=0.043). Mann Whitney test showed the comparison of post-test scores in both of groups was different (p=0.009). It can be concluded that WEP has effect to CRF.Discussion: The result showed that WEP could decrease CRF in breast cancer patient undergoing chemotheraphy in RSUD. Ibnu Sina Gresik. It is recommended to apply WEP as one of fatigue management in breast cancer patient undergoing chemotherapy. Further study should use WEP and psychosocial therapy to manage CRF in each fatigue dimension to reduce more scale of fatigue in patient.

Abstract

Introduction: Diabetes Mellitus (DM) is one of the incurable cronic diseases. The patient’s dietary obedience is a principal component in diabetes treatment success. One of the efforts to increase dietary obedience behavior in type 2 Diabetes Mellitus patients is using a Diabetes Self Management Education (DSME) principles approach. Diabetes Self Management Education (DSME) is the ongoing process of facilitating knowledge, skill and ability necessary for diabetes self care. The purpose of this study was to analyze the influence of DSME principles approach toward the dietary obedience behavior that consists of knowledge, attitude, and practice aspects in patients with type 2 Diabetes Mellitus at Puskesmas Kebonsari Surabaya.Methods: A quasy experimental design was used in this study with 24 people as the sample which taken by using purposive sampling and divided into two groups (treatment and control group). A structured questionnaire and daily meal note for seven days were used to collect the data. The data were analyzed by using Wilcoxon signed rank test and Mann Whitney U test with significant level α≤0.05. Result: The results showed that there was an influence of DSME principles approach after being applied to the treatment group and control group toward the dietary knowledge with the value of p=0.004 and p=0.083 respectively, the attitude of dietary obedience with p=0.025 and p=0.083 and the dietary obedience practice with p=0.002 and p=0.564.Discussion: The study showed that DSME approach had a significant influence toward the dietary obedience behavior in type 2 Diabetes Mellitus patients. DSME principles approach used learning based on patient empowerment and behavior strategies. Ongoing support also contributed in maintaining progress achieved so that resulting dietary obedience behavior in type 2 Diabetes Mellitus patients. Larger respondents, more proper instruments and longer duration of study are needed to get a better results of the study concerning diabetic patient’s obedience in the future.

Abstract

Introduction: There are many drugs in the effort to cure hypertension for the old that in fact have bad influence because they cause dependence and are risky with misestimated doses. Communication back massage and back massage are two of complementary therapies recommended to lower blood pressure due to each relaxation effects are good. This study aims to clarify the difference of communication back maassage and back massage to reduce blood pressure. Method: This study was used quasy experimental design. The total samples in this study were 14 old patients who got hypertension. They were divided into 2 groups, one of the groups was treated with communication back massage and the other one was only treated with back massage. The independent variable was communication back massage and back massage. The dependent variable was blood pressure, which was observed for 5 days. Data were collected using aneroid sphygmomanometer and observation paper then analysis was implemented using Paired t-test with level significance α≤0.05 and Independent t-test with level significance α≤0.05. Result: The result showed that blood pressure decrease in significantly both in group one and group two (p=0.04 in group one and p=0.046 in group two). Between the group one and the group two, there was no difference on blood pressure reduction (p=0.315). Discussion: Communication back massage and back massage have a same effect on blood pressure reduction in old patient who get hipetension. The possible explanation for these findings is that communication back massage and back massage have a relaxation effect that can decrease norepinefrin, decrease ADH and increase endorphine. The decrease of norepinephrine can widen blood vessel. The decrease of ADH can reduce excessive fluid in the blood circulation. The increase of endhorphine can reduce sympathetic nerve activity. Therefore, blood pressure level will be decreased.

Abstract

Introduction: Coping mechanism was an individual effort to deal with a stressor. Diabetes mellitus was a degenerative disease that could not be cured and could become a stressor for the patients. If this stressor unhandled it can make the condition of patients become worse.Methods: The object of this research was to know the coping mechanism of diabetic person. Design used in this research was quasy experimental pre post test design. This research was using purposive sampling with five people as the samples. The recording result of interview would be transcribed as the data. Data would be classified depend on the criteria inclusion and would be analyzed by reduction, data serving, and data conclution to get the good result.Result and Analysis: The results showed that interviews with partisipants indicated they have accepted their condition by making efforts to deal with negative feelings experienced through self control, taking a positive appraisal and accepting the responsibility. They tended able to do a good self-management to deal with diabetes mellitus by performing the routine control, dietary changes, looking for information about diabetes mellitus and exercise regularly. Partisipants were also getting support from family, friends, and health professionals.Discussion: coping mechanisms were performed by diabetic person include emotional focused coping to deal with negative emotions and problem focused coping to deal with diabetes itself through management regimen task.

Abstract

Introduction: Based on the latest research, exercise had significant effect to decrease blood glucose. The aimed of this study was to investigate the effect of isometric exercise model on blood glucose in Diabetic Mice (Mus musculus).Methodes: This research was used a true experimental study with randomize post-test only control group design. The sample was male mice (Mus musculus) in 20-27 grams weight. 27 male mice were divided into three groups; the first group was normal group (control) that injected by NaCl as placebo, the second group was diabetic mice and the third group was diabetic mice with exercise. Both second group and third group were injected by streptozotocin (STZ) to induce Diabetes Mellitus. After 2x24 hours injected, all groups were given oral glucose (Dextrose-40% or D40). One hour after they given by D40, the third group was treated by isometric exercise used treadmill for 23.31 minutes with 21 cm/second speed and 0° elevation angle. Then the blood samples of all groups were taken to measure the blood glucose level. Data were analyzed by One Kolmogorov-Smirnov test, ANOVA test, and Least Significance Difference (LSD) with significance level of α<0.05.Result: The result of One Kolmogorov-Smirnov test showed that there was p=0.94 in normal group, p=0.50 in diabetic group, and p=0.19 in diabetic with exercise group. The result of ANOVA test was p=0.08. The result of LSD for blood glucose revealed was p=0.23 for diabetic group and diabetic with exercise group. The researcher had an assumption that probably it happened because of in second group there were two mice that had normal blood glucose levels and in the third group the initial blood glucose levels were unknown, hence they probably could had normal blood glucose levels too. Discussion: It can be concluded that there is no effect of isometric exercise to decrease blood glucose. Further research should investigate more about the frequency and intensity of isometric exercise that could has effect to decrease blood glucose.

Abstract

Introduction: Cough is one of clinical manifestations of respiratory tract infection. Technique of effective cough helps remove pleghm more easily during cough. However, the optimum implementation of effective cough has not been performed well. Changing the behavior of practicing effective cough to students needs a lot effort. One of it is by giving health education using the demonstration and lecture. The aimed of this study was to analyze the effectiveness of health education the knowledge, attitudes and actions of practicing effective cough between the demonstration method and lecture to students. Method: A quasy experiment research design was used in this study. The sample was students of SDN Grogol Sidoarjo and there were 44 students. Purposive sampling was used in this study. Data were obtained using questionnaires and standard operational procedures.Result: The effectiveness of health education between the demonstration used and lecture were analyzed using Mann Whitney U Test. The results showed that demonstration and lecture were effective to change knowledge, attitudes and actions of students with p=0.004, p=0.003, and p=0.003 respectively. Discussion: The author concluded that the demonstrations and lecture can enhance the ability to perform effective cough practice. However, the demonstration is more effective to be applied to students in order to improve their ability to practice effective cough because demonstration directly exhibit something that’s followed by students in order to the knowledge or skill is easier to br memorized.

Abstract

Introduction: Diabetes Mellitus is a chronic disease that cannot be cured so that compliance is required to manage the disease. Patient compliance is in accordance with the provisions of the behavior of health workers. The aimed of this study was to analyze the effect of peer group support on the compliance of type 2 Diabetes mellitus disease management.Methods: This research was used a quasy experiment pre post test designed. The selection of sample using a purposive sampling and was obtained 16 respondents from 77 population. The independent variable was a peer group support that carried out for two weeks while dependent variable was dietary compliance, exercise and drugs consumption which data was collected by observation sheet. The data were analyzed using Wilcoxon Signed Rank Test and Mann Whitney U Test with significantvalue of ≤α 0.05.Result: The results showed that there was increase of dietary compliance (p=0.012) but there was no effect of peer group support for dietary compliance (p=0.14), the increase of exercise compliance (p=0.012) and there was effect of peer group support for exercisecompliance (p=0.004), and the increase of drugs consumption compliance (p=0.027) and there was effect of peer group support for drugs consumption compliance (p=0.048).Discussion: It can be concluded that peer group support had good effect to improve dietary compliance, exercise, and drugs consumption so that blood sugar could be controlled. Larger respondents, more proper instruments and longer duration of study will be needed to get a better result of the further research.
