Abstract

Selain perempuan, anak merupakan korban tersering dalam domestic violence. Sebagian besar merupakan korban child neglect, diikuti physical abuse dan sexual abuse. Penganiayaan yang dialami saat kecil membuat anak rentan mengalami attachment yang insecure, low self esteem, regulasi emosi yang tidak baik, sense of self yang buruk, kesulitan untuk percaya pada orang lain, penolakan dari teman sebaya, dan pencapaian prestasi pendidikan yang buruk. Psikopatologi yang muncul rentan membuat anak mengalami gangguan jiwa seperti Post Traumatic Stress Disorder (PTSD), penyalahgunaan zat, gangguan tingkah laku dan depresi. Terdapat beberapa penanganan untuk anak-anak korban domestic violence antara lain: Play therapy, Parent-Child Interaction Therapy (PCIT), Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), Group counseling.

Abstract

Consultation-Liaison Psychiatry is sub specialistic in psychiatric as a connector in clinical service team, teaching and research in borderline between psychiatric and medical medicine/surgery science. Consultation-Liaison Psychiatry is the connector to all diagnoses, therapeutic, research and educational service which being done by psychiatrist in general hospital and as a bridge between psychiatrist and other specialists. Burn injury is one of the leading causes of death and disability in the world. Global Burden Disease’s study reported approximately 265.000 deaths happened every year. According to WHO, 238.000 peo- ple died due to burn injury in 2000 and 95% of the death happened in developing and low income countries. Burn injury can be a remarkable traumatic, psychologic, social and economic experience. Physiologic changes can cause symptoms, both organic and psychiatric, starting from anxiety, delirium to psychosis. Psychiatric relation team can contribute substantially in order to manage this problem.

Abstract

Infertilitas adalah suatu kondisi dimana perempuan dan laki-laki berhubungan seksual secara rutin minimal 1 tahun atau lebih tanpa menjadi hamil.Kondisi ini dapat menimbulkan berbagai reaksi emosi yang dapat mempengaruhi hubungan suami istri.Ada beberapa penanganan yang dapat dilakukan, baik secara fisik maupun psikologis.Secara fisik dapat dilakukan In Vitro Fertilization (IVF).Prosedur IVF ternyata menimbulkan beban psikologis pada pasangan, sehingga diperlukan intervensi psikologis, yang salah satunya dengan couple therapy.Couple therapy bertujuan untuk membantu pasangan memperoleh pengertian yang lebih baik tentang diri mereka sendiri dan pasangannya, serta menolong pasangan untuk mengungkapkan reaksi emosi mereka dengan komunikasi yang baik dan penerimaan terhadap kondisi pasangannya.Ada beberapa pendekatan couple therapy yang dapat dilakukan, diantaranya Emotionally Focused Therapy (EFT) dan Solution Focused Therapy (SFT).Diharapkan dengan dilakukannya intervensi couple therapy sebelum, selama dan setelah prosedur IVF ini, dapat meningkatkan keberhasilan kehamilan sekaligus mencegah kekecewaan dan masalah psikologis akibat kegagalan prosedur IVF.

Abstract

Parental Alienation Syndrome (PAS) is a process in which one parent (Aligned Parent) teaches his children to reject or antago- nize other parents (Rejected Parent) which results in disruption to the relationship between children and parents. PAS can be a central issue in child custody disputes and is a form of emotional abuse to children that can disrupt the process of growth and development of children and cause mental disorders in the children's future. Although there are differences of opinion about PAS, the impact can already be seen in children who are in the PAS situation. In severe PAS conditions, disorders in children can occur in the dimensions of behavior, emotions, and cognitive. In the long run, someone who has been exposed to PAS in childhood has a greater likelihood of experiencing depression, anxiety, and decreased quality of life in the future. Given the mag- nitude of the impact caused by PAS, it is necessary to do the management carried out simultaneously by mental health practition- ers, legal professionals and the court. Therapeutic interventions that can be carried out include Multi Model Family Intervention (MMFI), Family Reflections Reunification Program (FRRP), Overcoming Barriers Family Camp (OBFC), Parallel Group Ther- apy, and Family Bridges Workshop. There is no conclusion which intervention is the best. Ultimately, the goal of family therapy is to achieve and maintain healthy parent-child relationships.

Abstract

Kegawatdaruratan di bidang psikiatri terutama agitasi menimbulkan cara baru untuk mengatasi gangguan perilaku. Agitasi adalah kumpulan ketidaksesuaian perilaku yang beresiko pada keamanan pasien dan orang yang merawat. Neuroleptisasi cepat adalah terapi yang segera dapat menenangkan pasien dengan menggunakan neuroleptik dosis tinggi dan berulang. Pemberian intramuskular dan pemberian oral sama efektifnya. Haloperidol lebih efektif dengan efek merugikan yang rendah. Antipsikotik generasi kedua mampu mengatasi gejala dengan baik meskipun perlu perhatian pada efek samping. Golongan benzodiazepin mampu menurunkan agitasi dan mengurangi penggunaan antipsikotik pada terapi kombinasi. Penggunaan antipsikotik generasi kedua mempunyai kemampuan yang kurang lebih sama dengan antipsikotik generasi pertama dalam mengatasi agitasi pada pasien dengan demensia.

Abstract

Depression is an important mental health problem in developed societies, and as many as 50%-90% of patients with depression complain of sleep disorders. In the elderly, sleep disorders comorbid depressive disorders are getting worse. Late-life Depression (LLD) is often not detected because the symptoms are less specific than younger age. It even provides an independent risk factor impact on suicide ideas and behaviour. Causative risk factors of sleep disorders in Late-life Depression are biological and non-biological factors that form the basic of pharmacological and non- pharmacological management.

Abstract

Background: Self-harming behavior (SHB) is an iceberg phenomenon commonly occurred in both normal population and patients with mental illness. This behavior showed an increase in trend and may develop into suicidal behavior if not treated properly. Therefore an effort in screening and prevention of self-harm is necessary to prevent early deaths caused by suicide. Currently Indonesian version of self-harm screening instrument has not been developed yet.Purpose: Conducting validity and reliability test of Indonesian version of self-harm questionnaire.Method: This research was a descriptive analytic study with cross-sectional design. We recruited 110 psychiatric outpatients from Bantul Hospital Yogyakarta (n=110). SHB was measured using Indonesian version of Self Harm Inventory (SHI). Internal validation was tested using Aiken's Validity Index (AVI) and Pearson Product Moment Correlation (PPMC). Reliability was tested using Cronbach Alpha (CA). Statistical significance was defined as p<0,05.Result: The respondents consisted of 57 men (51.8%) and 53 women (48.2%). Twenty four respondents had done more than 5 items of SHB. AVI value ranged from 0.83 to 0.97 which considered good. PPMC obtained 21 valid items with correlation coefficient >0.25 (p <0.05). Item number 22 was excluded because coefficient value was <0.25. Rresult of Keizer Meyers Oklin was 0,686 and Barlett Test of Spherecity 0,000 (p <0,05). CA was 0.831 which means very good.Conclusion: Indonesian version of Self-Harm Inventory is a valid and reliable screening tool against self-harm behavior in clinical setting.

Abstract

Secara global, bunuh diri merupakan penyebab kematian nomor tiga di dunia dengan kecenderungan peningkatan pada kelompok anak dan remaja. Tingkat bunuh diri bervariasi mulai dari ide bunuh diri, ancaman bunuh diri, percobaan bunuh diri dan tindakan bunuh diri. Faktor risiko bunuh diri pada anak dan remaja mencakup gangguan psikiatri, stresor psikososial, faktor kognitif dan faktor biologi. Selain itu bunuh diri pada anak dan remaja juga dipengaruhi oleh perkembangan kognitif, pemahaman mengenai kosep kematian, faktor afektif dan peran kelekatan. Banyak penelitian yang telah mengembangkan alat penapisan bunuh diri seperti Ask Suicide Screening Question(ASQ) dan Risk for Suicide Quessionare (RSQ), dan lainnya yang dapat digunakan sebagai langkah preventif untuk mengurangi dan membantu anak dan remaja yang berisiko untuk melakukan bunuh diri. Pengetahuan dan pemahaman (psikodinamika) yang baik serta komprehensif tentang bunuh diri pada anak dan remaja akan sangat membantu dalam melakukan prevensi dan intervensi yang tepat dalam penanganan kasus ini.

Abstract

Objektif : Penelitian ini mengamati intimacy dan kepuasan pernikahan pasien kanker serviks beserta suaminya untuk menekankan pentingnya perspektif psikososial pada pasien kanker. Metode : Observasional kualitatif, serial kasus, total dan purposive sampling pada pasien kanker serviks stadium 0 sampai 3 berusia 20-50 tahun dan belum diterapi. Dilakukan wawancara mendalam, penilaian dengan kuesioner skala Personal Assessment of Intimacy, adaptasi Revised Dyadic Adjustment Scale dan follow up 3-6 bulan. Sample dari Klinik Onkologi Kandungan RSUD Dr. Soetomo tahun 2016. Analisis data kualitatif disajikan dalam narasi dan tabel. Hasil : 5 pasangan subyek, usia 39 – 58 tahun. Satu pasien stadium 3B menyalahkan kanker sebagai penyebab ketidakpuasan pernikahannya. Disrupsi intimacy ditemukan pada 4 pasangan terutama aspek seksual dan rekreasional. Satu pasangan mampu mempertahankan wellbeing sebagai individu maupun pasangan meskipun aktivitas intim aspek tertentu berubah. Faktor-faktor lain yang didiskusikan mencakup aspek budaya, lingkungan, kepribadian, stigma serta persepsi akan kanker dan pernikahan.Simpulan : Intimacy yang dimaknai sebagai kedekatan, dalam perkembangannya selama menikah dapat makin memperkuat komitmen, mempengaruhi kepuasan serta pertahanan relasi terutama selama masa sulit,termasuk adanya kanker. Kualitas intimacy berperan pada manajemen stres diantaranya membantu kenyamanan pasien selama adaptasi. Kanker mempengaruhi pasien dan pasangan sehingga lebih baik dikelola sebagai "penyakit pasangan".

Abstract

Treatment-resistant depression occurs in about 20% of all Major Deppresion Disorder patients. In addition to the high cost of treatment to be borne, the high functional disability rate, the suicide rate triggered by the disorder is also quite large. Various efforts were made to overcome this, including dose optimization and duration of treatment, substitution of drug selection, combination therapy and augmentation using non-antidepressant drugs and bilateral electroconvulsion therapy. Current pharmacological options according to some experts are no more efficacious than the 1950s. Clearly, a novel therapeutic approach to treatment - resistant depression disorders is urgently needed. Over the last few decades, there has been a renewed interest in focal neuromodulation as a treatment approach for neuropsychiatric conditions. The neuromodulation-based interventions discussed include Transcranial Magnetic Stimulation (TMS) and Transcranial Direct Current Stimulation (tDCS), which are non invasive intervention therapy and Vagus Nerve Stimulation (VNS) and Deep Brain Stimulation (DBS), which are invasive interventional therapies. This literature review proves that, although today only TMS and VNS have been approved for use by the Food and Drug Administration (FDA) in the United States, but neuromodilation-based intervention therapy has proven to be promising as a more effective and efficient resistant depression therapy in the future.
