Abstract

Background. Coronary artery disease (CAD) is caused by narrowing of the coronary arteries. In the event of acute myocardial infarction (AMI), there will be an increase in heart markers. Several studies have examined the relationship between Platelet Distribution Width (PDW) and AMI, but there is no specific correlation with troponin as the most sensitive and specific cardiac biomarker for AMI. This study aims to see the correlation PDW values with troponin I in patients with AMI. Method. This study was a cross-sectional method to determine the correlation of PDW variable values ​​with troponin I in patients with AMI. In this study, the sample consisted of 64 people. Results. There is a moderate correlation between PDW and troponin I (r2= 0.72, p <0.001). There is a strong correlation with positive direction between PDW and Troponin I in patients with AMI based on the onset of chest pain (<6 hours and > 6 hours : r2 = 0.647 p <0.001and r2 = 0.756, p <0.001). Conclusion. Platelet Distribution Width (PDW) is strongly correlated with Troponin I in patients with AMI, and based on chest pain onset also has a significantly positive correlation.

Abstract

Background: Restless Legs Syndrome (RLS) is one of the most common sleep disturbance problems in dialysis patients. In this study, we would like to explore some of bone and mineral marker disorder especially fibroblast growth factor-23 (FGF-23)and its correlation with RLS in regular hemodialysis patients. Methods: This is a cross sectional study design involving 71 respondents with chronic kidney disease that already had regular hemodialysis at Haji Adam Malik Hospital Medan. Status of RLS in this patients were being evaluated according to the International Restless Leg Syndrome Study Group (IRLSG) by interviewing and laboratory examination of FGF-23, hemoglobin (Hb), serum iron (SI), total iron binding capacity (TIBC), transferrin saturation (TSAT), serum ferritin, calcium (Ca) and phosphate (P) level. Results: Of all study subjects, 26 respondents (34.6%) diagnosed with RLS. Bivariate analysis result showed that there is a relationship between FGF-23, hemoglobin, phosphate, and transferrin saturation level with RLS. Logistic regression analysis used to see the most dominant factor of all. Conclusion This study conclude that increase in FGF-23 levels can increase the risk of RLS. However, FGF-23 is not the most dominant risk factor for RLS in regular hemodialysis patients.

Abstract

COVID-19 is caused by a novel coronavirus known as SARS-CoV-2, a single chain RNA virus with a particle size of 120-160 nm. Covid-19 can affect almost all age groups, despite, the elderly and people who have records of chronic disease (co-morbid) have the risk to be affected more often and with worse complications from this disease. Diabetes mellitus (DM) is one of the risk factors for enhancing the severity of COVID-19 infection. A 26 years old man, was referred to the hospital with symptoms of shortness of breath, fever, and cough. The patient has no history of comorbidities, but his father has newly type-2 diabetes mellitus (T2DM). The patient was examined, with ad random blood sugar of 220 mg/dL and a chest radiograph suggesting bronchopneumonia. The patient was then examined for fasting and postprandial glucose, HbA1c, and diagnosed with DM, the patient's throat swab showed a positive COVID-19 result. Patients were then treated with antiviral, antibiotic, regulated insulin, and his blood sugar was monitored. The patient's blood sugar is stable and the patient discharge after 34 days of hospitalization.

Abstract

Background Coronary Heart Disease (CHD) is one of the leading causes of death in developed and developing countries.  Syzygium polyanthum leaf (daun salam) are reported to have anti-inflammatory, antioxidant, antidiabetic, antimicrobial, antihypertensive, antitumor, antidiarrheal, acetylcholinesterase inhibition, and lipase inhibitory activity. The purpose of this study was to prove the effect of bay leaf extract (Syzygium polyanthum) on serum interleukin (IL)-10 levels in the heart of Wistar Strain Mice Model of Acute Coronary Syndrome (ACS). Methods This is an experimental study to compare the IL-10 level in 32 ACS mice which were divided into 8 groups. Serum IL-10 values were collected and analyzed according to the corresponding group with and without bay leaf extract administration, on day 1,4,7, and 14. Result: There is an IL-10 value increasing with the duration of examination following the onset of ACS induction. The highest average IL-10 value was found on the 14th day, in the two groups. On the same day, it was seen that the treatment group had a higher IL-10 value compared to the control group (p<0.001). The result showed that the concentration of IL-10 was significantly greater among the treatment group, which  reveal the potential benefit of bay leaf extract in patients with ACS. Conclusion: There was found a significant difference of serum IL-10 between Wistar Strain Mice Model of ACS that was given S. polyanthum leaf extracts. S. polyanthum leaf extract has quite extensive benefits in the cardiovascular system.

Abstract

Background. Inflammation plays an important role in the initiation of atherosclerosis from the beginning of plaque to rupture cause Acute Coronary Syndrome (ACS). Neutrophil Lymphocyte Ratio (NLR) indicator of systemic inflammation in ACS. Risk stratification was needed for assessment and selection of initial invasive strategies and find the best strategy in ACS. The Global Registry of Acute Coronary Events (GRACE) scores recommended risk stratification of ACS. Aims of the study to determine the association and cut-off value NLR with risk stratification GRACE score. Method. This study is analytical with a cross-sectional retrospective design. Data were analyzed after distribution test, then mean difference and correlation test was using the SPPS program where p <0.05 was considered statistically significant. Results. This study showed significantly higher NLR value in the high risk stratification and intermediate-risk compared to low risk stratification (7.9 ± 2.7 vs 3.6 ± 1.7; p=0.001) (5.2 ± 2.3 vs 3.6 ± 1.7; p=0.018). Significant correlation between NLR ​​with GRACE scores (r=0.570; p<0.001). Significant AUC values ​​were obtained (0.782, p <0.001, IK95% 0.674-0.89), and cut-off values NLR 4 ​​with sensitivity (78.8%) and specificity (70.3%) on the GRACE score. Conclusion. The significant association between NLR ​​with GRACE risk score in ACS.

Abstract

Background. Red cell distribution width (RDW) is a coefficient of variation in red blood cells that can decrease erythropoesis or increase the destructiveness of red blood cells. The objectives of research to determine the relationship of RDW as an inflammatory marker with renal function and hematological parameters in patients undergoing regular hemodialysis Method; Cross sectional research on 20 patients undergoing regular hemodialysis > 3 months in RSUP H Adam Malik Medan. Vital sign, antropometry and venous blood retrieval are performed shortly before hemodialysis. RDW measurement comes from the red blood cell distribution curve in hematological analysis and is an indicator of variation in red blood cell size. Result: out of 20 subjek studies, there were 13 men (65.0%) and 7 (35.0%) women The majority of subjects had comorbid diabetes mellitus 14 (70.0%), hypertension 4 (20.0%). The average length of time patients underwent hemodialysis was 24.45 ± 20.98 months. There is a significant correlation between RBW and creatinine, Hb, and neutropil (r: 0.519, p:0,019*; r: 0.497, p: 0.026*;r: 0.464, p: 0.039*, respectively) Conclusion: There is a significant relationship between RBW and creatinine, Hb and neutropphils in patients undergoing regular hemodialysis > 3 months.

Abstract

Background: Insulin Resistance (IR) causes a metabolic disorder including hyperglycemia, high blood pressure, and dyslipidemia. The purpose of the study was to assess the relationship of Triglycerides and Glucose Index (Tyg) decline relationship with lipid profile after bay leaf extract treatment in dyslipidemia patients. Method: Fifteen dyslipidemia patients were administered 2x200 mg salam leaf extract capsules for 30 days. Criteria dyslipidemia is defined as one of the following: total cholesterol level ≥ 240 mg/dL, triglyceride levels ≥150 mg/dL, LDL cholesterol levels ≥140 mg/dL, HDL cholesterol levels <40 mg/dL, or the use of lipid-lowering drugs. Blood samples lipid profile for biochemical tests were drawn in the morning after fasting 12 h. Anthropometric BMI and waist circumference measures were taken using standard procedures. The TG/HDL-C ratio was calculated by dividing the serum concentration of TG by HDL-C measured in mg/dL. TyG index was calculated based on formula: Ln [TG (mg/dL) x FG (mg/dL). Result: After 30 days of salam leaf extract treatment, there was a significant correlation of TyG reduction with parameters TC, LDL-C, TG, TG/HD ratio (p: 0.025*, p: 0.001*, p: 0.013*, sequentially) Conclusion: There is a significant correlation between Tyg reduction with TC, TG, and TG/HDL, which leads to a decrease in cardiovascular risk in dyslipidemia patients.

Abstract

Background: Increased Lp(a) accelerates atherosclerosis by means of cholesterol deposits in tunica. The purpose of the study was to assess the relationship of decreased Lp(a) with lipid profile in patients with dyslipidemia given bay leaf extract [syzygium polyanthum (Wight) Walp] treatment. Method: Thirty subjects were divided into 2 groups: group I received salam leaf extract therapy 2 x 200 mg and group II received salam leaf extract therapy 2 x 300 mg per day capsules for 30 days randomly selected and double blind. Examination of Lp(a), total cholesterol, LDL-C, HDL-C and TG before and after research. The manufacture of bay leaf extract is done by maseration, the extraction process using ethanol 70%, then the extract is inserted in the capsule. Result: In groups I and II there were significant differences in variable Lp(a) and lipid profiles after treatment except HDL-C. There is a significant correlation between Lp(a) and LDL at salam leaf extract therapy doses of 2x200 mg per day. Conclusion: Decrease in Lp(a) is significantly correlated with decreased LDL-C. Salam leaf extract therapy can predict decreased cardiovascular risk in patients with dyslipidemia.

Abstract

Abstract: Pituitary adenomas are the most common type and benign tumours arising from hormone expressing cells in the anterior pituitary gland. They account for 10% to 15% of all intracranial masses. Pituitary adenomas present clinically in three ways: syndromes of hormone hypersecretion or deficiency; neurologic manifestations from mass effect of an expanding gland; or an incidental finding on imaging done for an unrelated issue. A 49-years old man, was reffered to the hospital with symptoms of chronic headache and blurred vision. General physical examination revealed no abnormalities. History of previous illness and medication were unremarkable. MRI brain showed hyperintense lesion in suprasellar/selllar area. Hormone assay reciprocally revealed hypocortisolism and hyperprolactinemia. Abstrak: Adenoma hipofisis merupakan tumor jinak tersering dari sel penghasil hormon di kelenjar hipofisis anterior, berkisar 10%-15% dari seluruh massa intrakranial. Adenoma hipofisis secara klinis bermanifestasi dalam tiga cara: sindrom hipersekresi atau defisiensi hormon; manifestasi neurologis dari efek massa yang membesar; atau temuan secara tidak sengaja pada modalitas pencitraan. Seorang laki-laki berumur 49 tahun dirujuk ke rumah sakit dengan kkeluhan sakit kepala kronik dan pandangan kabur. Pemeriksaan fisik secara umum dalam batas normal. Riwayat penyakit sebelumnya atau pengobatan juga tidak spesifik. MRI otak menunjukkan lesi hiperintens pada area suprasellar/sellar. Temuan pemeriksaan hormon juga sejalan menunjukkan hipokortisolisme dan hiperprolaktinemia.

Abstract

ABSTRACT: Background: Fournier gangrene (FG) is a rare but life-threatening necrotizing fasciitis of the external genitalia, perineal area, or perianal caused by a combination of aerobic and anaerobic bacterial infection. It has a higher incidence in men with risk factors including diabetes, HIV, alcoholism, and other immunocompromised states Case presentation: A man, aged 62, was admitted to the hospital complaining of ulcers on his testicles. The patient feels that his testicles are getting swollen, painful, and red. Ulcers appear black, accompanied by pus and accompanied by a foul smell. The patient was hematoligic examination, radiologic and blood cultur and diagnosed with Fournier's gangrene on scrotal area and T2DM. The patient is subjected to immediate debridement and is given broad-spectrum antibiotic therapy, administration of insulin, and monitoring of blood sugar. Conclusion: We reported a case of Fournier gangrene being comprehensive management, causing the prognosis to good improvement Keyword: Fournier gangrene, T2DM ABSTRAK: Latar belakang: Fournier Gangrene (FG) merupakan suatu fasciitis nekrotikans langka namun mengancam nyawa pada genitalia eksterna, daerah perineal, atau perianal yang disebabkan oleh gabungan infeksi bakteri aerob dan anaerobik. Penyakit ini memiliki insiden yang lebih tinggi pada laki-laki dengan faktor risiko termasuk diabetes, HIV, alkoholisme, dan keadaan imunokompromise lainnya. Presentasi Kasus: Seorang pria, berusia 62 tahun, dirujuk ke rumah sakit dengan keluhan luka borok pada buah zakarnya. Pasien merasakan buah zakarnya semakin membengkak, nyeri dan memerah. Luka borok tampak menghitam, disertai nanah dan disertai bau busuk. Pasien dilakukan pemeriksaan darah dan radiologi di diagnosis dengan Fournier gangrene scrotal area dan DMT2. Selanjunya pasien dilakukan debridemen segera, dan diberikan terapi antibiotik spektrum luas, pemberian insulin, dan dilakukan pemantauan gula darah Kesimpulan: Dilaporkan seorang kasus Fournier gangrene yang ditangani secara komprehensif, menyebabkan perbaikan prognosa.
